Description

1 / 8 “ULTRASONIC INSTRUMENT FOR ROOT CANAL TREATMENT AND RETREATMENT”. ORDER FIELD

[001] This Utility Model Patent falls within the field of ultrasonic instruments for root canal preparation, preferably those with isthmuses and canals with a flattened or oval anatomical shape, and its use is directed to endodontic treatments and retreatments, especially root canal treatments, in dentistry. GROUNDS FOR THE REQUEST

[002] Most root canals in human teeth have an oval anatomical shape. Studies in this field, using micro-computed tomography, reveal that, in percentage terms, this characteristic reaches 89% in lower molars, 75% in lower incisors, and 54% in upper premolars.

[003] When bacteria invade the root canal and colonize the canal system, they cause periapical diseases. Their removal is essential for the regression of the disease and the restoration of normal periapical tissues.

[004] Most instruments developed for working in the preparation and cleaning of root canals in endodontic treatment have a conical geometry that varies from 0.02 to 0.2 millimeters, from the tip towards the handle, as shown in figure 3. These instruments can be used to work manually, rotating 360 degrees or performing an oscillatory rotary motion by means of motors.

[005] Thus, when an instrument is used during endodontic treatment, it will imprint its conical geometry onto the root canal.

[006] When this occurs, a percentage of the walls of this root canal remain untouched by these instruments. Field studies Petition 870250087746, dated 09 / 29 / 2025, page 7 / 26 Studies showing that approximately 30 to 50% of the canal walls can remain intact are shown in Figure 2 / 8.

[007] Furthermore, some of the dentin shavings or obturation material is carried out through the escape areas of these instruments. However, another part is pushed laterally, impacting this contaminated dentin or obturation material into flattened areas, as has been demonstrated in notable scientific work recently. This accumulation is illustrated in the image shown in Figure 4.

[008] Some research suggests increasing the widening of the canals with larger gauge instruments. However, this can cause significant wear and weakening of the tooth.

[009] In view of this situation, in recent years, robust and compact ultrasonic instruments have been developed for use in conjunction with operating microscopes for endodontic treatments. STATE OF THE ART

[0010] Document US2016113735A refers to an instrument for preparing for post placement. This instrument is designed to work in oval canals. However, unlike the aforementioned document, the present invention does not work with reciprocating motion, but rather by ultrasonic activation. Additionally, unlike the present invention, it is not used to prepare for posts, but rather for cleaning and removing bacterial biofilm and tissue debris in root canal treatments and obturation material in root canal retreatments. Finally, and most importantly, the present invention's main function is to clean the isthmus and flattened areas of the root canal system, and it can be used in conjunction with rotary and / or reciprocating systems in root canal treatments and retreatments. Petition 870250087746, dated 09 / 29 / 2025, page 8 / 26 3 / 8

[0011] Document DE102004036000B4 presents an ultrasonic tool. However, its application is entirely distinct from the application of the present invention. In the cited document, the tool is used to perform a retrograde preparation in the root canal during a surgical procedure, with access achieved through a bony cavity and resection of the apical part of the root. In the present invention, the ultrasonic insert is used during conventional non-surgical endodontic treatment, with access to the root canals achieved through the crown of the tooth and to the root canals.

[0012] Document CN201676027U describes another manual or reciprocating instrument used to file the internal walls of the root canal. Its form and function are entirely distinct from the ultrasonic insert disclosed in the present invention, despite having been developed for oval canals. In this document CN201676027U, another instrument is developed for use with filing kinematics, i.e., wear of the internal walls by means of manual filing movements or activated by means of a reciprocating contra-angle handpiece. In contrast, the ultrasonic instrument disclosed in the present invention acts by cavitation, breaking down the bacterial biofilm, and is also capable of removing obturation material, such as gutta-percha.

[0013] The commercially available “Ellipson” product is an oval ultrasonic insert. However, its function is limited to preparing the root canal to receive an oval fiber post for tooth reconstruction. The Ellipson insert is not indicated for isthmus cleaning or complementary preparation to rotary or reciprocating instruments. Its sole purpose is to prepare the canal to receive a fiber post for reconstruction of the outer coronal portion of the tooth. The Ellipson insert has a single geometry. The inserts described in this document have different geometries that adapt anatomically to the different root canal shapes. The dimensions of the Ellipson insert are Petition 870250087746, dated 09 / 29 / 2025, page 9 / 26 4 / 8 significantly superior to the ultrasonic insert that is the subject of this application. Furthermore, it is advantageously noted that the dimensions of the inserts described in this document were developed to be able to penetrate anatomical isthmuses and recesses, especially to break down bacterial biofilm and remove obturation material. ADVANTAGES OF ORDERING

[0014] Considering the deficiencies of the state of the art, this application proposes an ultrasonic instrument capable of working the root canal space without significantly altering its anatomy clinically, removing bacterial biofilm and obturation material in a minimally invasive way and preserving the tooth structure as much as possible. It applies to both canals with simplified anatomy, the so-called common canals, and to canals with flattened, oval shapes and isthmus areas. Furthermore, this application eliminates the impaction effect of shavings and dentin debris during the removal of disease-causing bacteria and, additionally, enables the removal of gutta-percha and other obturation materials of any kind. BRIEF DESCRIPTION OF THE ORDER

[0015] This application refers to an ultrasonic root canal treatment and retreatment instrument, which acts in the cleaning, shaping and / or enlargement and removal of obturation material such as gutta-percha from the root canal system and, mainly, reaches areas not instrumented by rotary or reciprocating files, in addition to isthmus and anastomosis areas. [0025] Figures 9a, 9b, 9c and 9d are a graphic representation of an optional embodiment of the instrument of the present application, with the parallel tip having two pointed ends and two rounded ends in a rhombus profile. DETAILED DESCRIPTION OF THE ORDER

[0026] This application relates to an ultrasonic root canal treatment and retreatment instrument, which is used for cleaning, shaping and / or enlarging and removing obturation material from the root canal system and, most importantly, reaches non-instrumented areas. Petition 870250087746, dated 09 / 29 / 2025, page 11 / 26 6 / 8 by rotary or reciprocating files, in addition to the isthmus and anastomosis areas.

[0027] For this purpose, the instrument in question comprises a one-piece body, preferably made of titanium or stainless steel and, optionally, coated with diamond or other abrasive.

[0028] The first end of the ultrasonic root canal treatment instrument, which comprises the larger diameter portion, is illustrated and represented with the corresponding numerical references in Figure 1a, in top view, where the following can be observed: the irrigation channel (1), the geometry of the torque wrench (2) and the geometry of the thread fixing (3). Since it has an internal threaded hole, the instrument is coupled with the ultrasonic transducer, preferably in the 22kHz to 38kHz range.

[0029] The second end comprises a tip with a smaller diameter compared to the first end. Preferably, this tip has a geometry that approximates the anatomy of the root canals, as described in figures 2a to 2d, here referred to as a conical tip with a pointed end. Its preferred dimensions are: length ranging from 0.15 to 5 mm. Additionally, for better understanding, this second end can be represented in figure 1b, being formed by the intermediate geometry (4) and the tip geometry (5).

[0030] Preferably, the present application may be presented as a conical or flattened conical tip with the thinner end having or not having a recess and having dimensions in the range of 0.15 to 5 mm. This preferred shape can also be seen in figures 1a, 1b, 1c.

[0031] The aforementioned instrument can be used in conjunction with rotary or reciprocating files. The aforementioned ultrasonic instrument can be used before, between, or after the files. Petition 870250087746, dated 09 / 29 / 2025, page 12 / 26 7 / 8 rotary or reciprocating files. In short, the timing of insert use depends on the best way the user finds to work.

[0032] Optionally, the second end of the instrument disclosed herein may take the forms described below, and shall be further characterized within the scope of this application: - Conical or flattened conical tip with rounded ends and recesses, as shown in figures 5a to 5d, where the preferred dimensions are: 0.15 to 5mm and thickness between 0.2 and 0.6mm; - Flattened tip with a triangular profile, as shown in figures 6a to 6d, where the preferred dimensions are: 0.15 to 15 mm and a thickness between 0.2 and 0.6 mm; - Flattened parallel tip with a pointed end, as shown in figures 7a to 7d, where the preferred dimensions are: 0.15 to 5mm and thickness between 0.2 and 0.6mm; - Parallel tip with pointed ends on all four sides in a diamond profile, as shown in figures 8a to 8d, where the preferred dimensions are: 0.15 to 5 mm and thickness between 0.2 and 0.6 mm, and - Parallel tip with two pointed ends and two rounded ends in a diamond profile, as shown in figures 9a to 9d, where the preferred dimensions are: 0.15 to 5mm and thickness between 0.2 and 0.6mm.

[0033] One of the characteristics of the instrument revealed here is that it does not significantly cut the tooth wall. Its geometry, which follows the anatomy of the canals, allows the removal of bacterial biofilm and obturation material without causing wear that significantly alters the original anatomy of the root canals. The ultrasonic cavitation action within the root canals allows the breakdown of bacterial biofilm without damaging the dental structure. This preserves more tooth structure, removes remnants of obturation material, and at the same time allows a larger area of the walls to be touched. Petition 870250087746, dated 09 / 29 / 2025, page 13 / 26 8 / 8

[0034] Due to the geometry of the present application, which was developed based on the anatomical concepts of root canals, ultrasonic instruments are able to work in oval or long oval canals, where the largest diameter can be 5 times larger than the smallest diameter.

[0035] Furthermore, in cases of retreatment, also called reintervention, it is possible to use the ultrasonic instrument of the present application to remove remaining obturation material from the first endodontic treatment that failed. This is fundamental to increasing the chances of success in retreatment, because if remnants of the previous treatment remain, the chances of success are drastically reduced.

[0036] Those versed in the art will appreciate the knowledge presented here and may reproduce this application in the forms presented and in other variations, covered within the scope of the attached claims. Petition 870250087746, dated 09 / 29 / 2025, page 14 / 26
