Description

[0001] The present invention relates to a pad for orthopedic aids, comprising a pad base body with at least two paired expansion areas.

[0002] Furthermore, the invention relates to an orthopedic aid, in particular an orthosis or bandage, which has at least one such pad.

[0003] Pelottes, which are used for various prophylactic and therapeutic applications, are already known from the prior art, such as from the patent applications DE 27 22 563 C2, EP 0 598 291 A1 and EP 0 600 218 A2.

[0004] Many patients with knee osteoarthritis suffer from retropatellar osteoarthritis, experiencing pain when pressure is applied to the patella and then to the trochlear groove. Even moderate but sustained pressure can cause pain, such as when wearing tight jeans. The dominant muscles around the large arthritic joints are often painfully tight and shortened, and painful trigger points frequently develop on the muscle surface. In knee osteoarthritis, the tissue, the joint space, and the retropatellar region must be relaxed. Physiotherapy employs specific, targeted massage techniques for this purpose. Additionally or alternatively, bandages and orthoses with pads are used. These pads, with their raised pressure elements such as nubs, provide a massage effect when pressure is applied to the muscle, releasing contractures and eliciting a sensorimotor response from the muscle.Pads with such elevations, especially nubs for massage, are known not only for the patella but also for other areas of the body and are described, for example, in DE 42 38 610 A1 and DE 10 2012 105 626 A1.

[0005] The nubs act on the muscles, for example, the vastus medialis muscle, in terms of sensorimotor function. At rest, the pressure / compression is solely due to the tension of the knitted fabric of the bandage or orthosis. When the muscle contracts, the increased volume of the muscle leads to a pressure increase, generating a sensorimotor response that relieves pain. During movement, such as walking with knee and hip flexion, the nubs also increase the pressure under load.

[0006] DE 20 2022 100 765 U1, which discloses the preamble of claim 1, discloses pelottes with raised areas and grooves.

[0007] A disadvantage of state-of-the-art pads, such as patellar pads, is that movement can cause the pad to slip. For example, depending on the stretch resistance of the teripatellar pad, which is often made of silicone, flexion of a patellar pad can exert a significant distal pull on the upper part of the bandage. This leads to slippage and, in patients with arthritis, to uncomfortable pressure on the patella. Simultaneously, this distal pull reduces the effective vertical pressure of the nubs, partly because the nubs bend, thus significantly weakening the desired sensorimotor response of the vastus medialis muscle.

[0008] The invention is therefore based on the objective of creating a pad that, despite its advantageous massage function, reduces slippage of the pad during movement.

[0009] The invention is therefore also based on the objective of creating a pad that, despite its advantageous massage function and effect, is less bulky and ensures comfortable wear even during longer periods of use.

[0010] The present invention solves the underlying technical problem by means of a pad according to claim 1.

[0011] The present invention solves the underlying technical problem by means of a pad comprising a pad base body, wherein the pad base body has a front and a back, wherein the front of the pad base body has at least one projection, wherein the pad base body has at least two paired expansion areas, wherein the two expansion areas divide the pad base body into a first pad base body area and a second pad base body area, and wherein the at least two expansion areas each comprise at least one groove in the pad base body, wherein the pad base body has a recess, wherein the pad base body has an outer surface and an inner surface facing the recess, wherein the at least one groove begins on the outer surface and does not extend to the inner surface or begins on the inner surface and does not extend to the outer surface.

[0012] The at least one groove is therefore an expansion groove which advantageously allows expansion in the expansion area of the pad base body, in particular between the first pad base body area and the second pad base body area.

[0013] The paired arrangement of the expansion zones advantageously enables uniform expansion. Preferably, in this paired arrangement, the first expansion zone is positioned at one edge of the pad base, and the second expansion zone is positioned at the opposite edge. This paired arrangement is particularly advantageous when the pad base has a recess. Such recesses are present, for example, in patellar pads or elbow pads. These recesses are generally known to those skilled in the art and serve to accommodate the patella or elbow, so that the pad base encompasses the patella or elbow.

[0014] According to the invention, the base of the pelotte has a recess.

[0015] In a preferred embodiment, the pad base has at least two expansion zones, which are separated in the pad base by the recess. Preferably, the two paired expansion zones are located next to the recess, for example, to the left and right of the recess or above and below it. Preferably, the recess has a diameter of at least 3 cm, and is therefore, in particular, a recess that can encompass the patella, the elbow (i.e., the head of the ulna), or the head of the ulna or radius at the wrist. The size and shape of such recesses are known to those skilled in the art. Such recesses usually surround bony or cartilaginous protrusions on the body, so that the size of the recesses is determined by these protrusions.

[0016] A pad, in particular a patellar pad, elbow pad or wrist pad with exactly two stretching zones, is preferred.

[0017] In a particularly preferred embodiment, the at least one groove is located on the front of the pad base body. According to the invention, the pad base body has an outer surface and an inner surface facing the recess, wherein the at least one groove begins on the outer surface and does not extend to the inner surface, or begins on the inner surface and does not extend to the outer surface.

[0018] Therefore, the groove, or expansion groove, is not a continuous channel with two lateral openings, but rather a cut that has only one lateral opening or no lateral opening at all.

[0019] Preferably, the at least one groove of an expansion area, i.e., the expansion groove, has either only one lateral opening or no lateral opening at all. Preferably, the at least one groove of an expansion area, i.e., the expansion groove, has only one lateral opening.

[0020] Preferably, an expansion area consists of at least one groove, for example one groove, two grooves, three grooves, four grooves, five grooves or more grooves.

[0021] Preferably, the two paired expansion zones have the same number of grooves and / or grooves of the same shape and size.

[0022] The number, width, and depth of the grooves, i.e., expansion grooves, influence the elasticity.

[0023] Preferably, the groove, i.e., the expansion groove, has a width of at least 0.1 millimeters. Preferably, the groove, i.e., the expansion groove, has a width of at least 0.3 millimeters. Preferably, the groove, i.e., the expansion groove, has a width of 0.1 millimeters to 100 millimeters, particularly 0.3 millimeters to 50 millimeters. It has been shown that this achieves the aforementioned properties in a particularly advantageous manner. Preferably, the groove, i.e., the expansion groove, has a depth of 0.1 millimeters to 10 millimeters, particularly 0.5 millimeters to 3 millimeters. It has been shown that this achieves the aforementioned properties in a particularly advantageous manner.

[0024] According to a preferred embodiment of the invention, the groove, i.e., the expansion groove, has a constant depth or at least two different depths in its longitudinal extent.

[0025] According to a preferred embodiment of the invention, the groove, i.e., the expansion groove, has a depth that corresponds to at least 10% of the height of the base body at the edge of the groove. The groove can be designed similarly to a "record groove". Thus, its width and depth can be variable.

[0026] In a preferred embodiment, the at least one raised area is a pressure element for the pad. In another preferred embodiment, the at least one raised area consists of knobs and / or ridges. Suitable raised areas are known to those skilled in the art. The pad can also have several raised areas, in particular different raised areas, which are positioned differently on the front of the pad.

[0027] The raised areas can be made of the same material as the pad base or of a different material. Preferably, the raised areas are made of a different material.

[0028] In a preferred embodiment, the protrusions are harder than the pad base body.

[0029] In this variant, the mobility and thus the massage function of the pad is also determined by the choice of materials. Specifically, the base and / or the raised section, particularly the pad's pressure element, are made of a viscoelastic material that allows for advantageous deformation of the pad during use, resulting in the desired massage effect. The materials are specifically selected to differ in their stiffness or elasticity.

[0030] According to a preferred first embodiment, the raised section, in particular the pressure pad element, is made of a material that is harder than the material of the base body. This allows the raised sections to transmit a higher pressure to the area being treated. However, the harder material reduces the mobility of the raised sections.

[0031] According to a second embodiment of the invention, the projection, in particular the pad pressure element, is made of a material that is softer than the material of the base body, so that it has higher elasticity and reduced stiffness compared to the base body. This affords the pressure projections a high degree of mobility, which in particular causes a chaotic movement of several pressure projections of the pad pressure element.

[0032] In a preferred embodiment, the material of the raised section, in particular the pad pressure element, has a Shore hardness of at least 40 Shore 00, preferably at least 6 Shore A and at most 20 Shore A. In a preferred embodiment, the material of the raised section, in particular the pad pressure element, has a Shore hardness of at least 20 Shore A and at most 80 Shore A. In a preferred embodiment, the material of the raised section, in particular the at least one pad pressure element, has a hardness of at least 20 Shore A and at most 60 Shore A. In a preferred embodiment, the material of the raised section, in particular the at least one pad pressure element, has a hardness of at least 20 Shore A, more preferably at least 25 Shore A. In a preferred embodiment, the material of the raised section, in particular the at least one pad pressure element, has a hardness of at most 50 Shore A.In a preferred embodiment, the material of the raised section, in particular of at least one pad pressure element, has a hardness of at most 49 Shore A, more preferably at most 45 Shore A. More preferably, the material of the raised section, in particular of the at least one pad pressure element, has a hardness of at least 35 to at most 45 Shore A. More preferably, the material of the raised section, in particular of the at least one pad pressure element, has a hardness of approximately 40 Shore A.

[0033] In a preferred embodiment, the material of the pad base body has a Shore hardness of at least 20 Shore 00 and at most 60 Shore 00 and / or the material of the protrusion, in particular of the pad pressure element, has a Shore hardness of at least 40 Shore 00, preferably at least 6 Shore A and at most 20 Shore A.

[0034] Preferably, the at least one raised area is shaped like a knob, cone, cylinder, pyramid, cone, and / or mushroom. Depending on the intended application, a different shape may be more suitable, and if there are multiple raised areas, they can also have different shapes. The raised areas and the inventive design of the pad ensure that the movable raised areas exert alternating pressure, similar to that exerted by a therapist during a massage, so that the pressure effect on sensitive nerves, lymphatic vessels, and blood vessels lasts for a long time. This is particularly advantageous when the pad is designed as a knee pad, where a corresponding massage effect is applied to the base of the meniscus and the Hoffa's fat pads.

[0035] Preferably, one or at least one of the several projections is oriented in its longitudinal direction perpendicular to the outer surface of the pad base or inclined to the outer surface of the base. By orienting the projection at an angle to the outer surface, a direction of deformation is predetermined for the projection during treatment. If the projection is loaded against the inclination, it acts particularly stiff and hard; if it is loaded in the direction of the inclination, it yields more quickly or with less force. This allows the pressure distribution to be optimally adjusted to the individual patient, depending on the application. If the projection extends perpendicular to the outer surface of the base, it can be deformed equally, or with the same force, in all directions transverse to its longitudinal direction.

[0036] In a preferred embodiment, particularly as a patellar pad, the pad base body has at least one, preferably two wings, wherein the at least one wing has at least one elevation, preferably bumps and / or ridges.

[0037] Preferably, the pad is a knee pad, an ankle pad, a back pad, an elbow pad, a pad for the arm area or a pad for the abdominal area.

[0038] Preferably, the pad according to the invention is a pad for the kneecap, a pad for the elbow, or a pad for the wrist. More preferably, the pad according to the invention is a pad for the kneecap, i.e., a patellar pad.

[0039] In a preferred embodiment, the pad base body has a recess on its front surface for the at least one projection, in particular the at least one pad pressure element. The contour of this recess corresponds at least substantially to the contour of the at least one projection, preferably the pad pressure element, such that a groove is formed between the base body and the projection, preferably the pad pressure element, at least partially in the circumferential direction of the projection, preferably the pad pressure element. This groove is a spacer groove, not an expansion groove. This spacer groove is therefore an additional groove to the at least one expansion groove. This has the advantage that, through a structurally simple adjustment, the mobility of the projection(s) is increased, and thus the massage effect remains at least the same or is improved for softer, harder, and / or smaller projections. The groove defines a lateral gap between the base body and the pad pressure element. This means that the pad pressure element, and in particular the raised section attached to it, is not in direct lateral contact with the base body. At least one pad pressure element is thus floating within the base body. As a result, during use and under load, the pressure projection is less strongly supported by the base body than before and is therefore more easily deformed. This allows the pad to be made smaller in height, i.e., in relation to the height or length of the pressure projections, while maintaining the same massage effect. Specifically, it is designed that each pressure projection is flush with the side of the base element, i.e., directly at the outer edge of the base element, which defines the outer contour of the pad pressure element. This means that the groove works directly with the respective pad pressure element, or prevents the respective pad pressure element from being directly supported laterally by the material of the base body.

[0040] According to a preferred embodiment of the invention, the groove extends over the entire circumference of the projection, in particular the pad pressure element. This ensures the projection's mobility in the advantageous manner described above, regardless of its design. By forming a circumferential groove, the contour of the recess corresponds to the contour of the pad pressure element or the base element, which also simplifies the manufacturing of the pad.

[0041] Preferably, the spacer groove has a width, i.e., a distance from the base element to the base body, of 0.3 millimeters. It has been shown that this achieves the aforementioned properties in a particularly advantageous manner.

[0042] According to a preferred embodiment of the invention, the groove, i.e., the spacer groove, has a constant depth or at least two different depths along its longitudinal extent. The depth of the groove alters or influences the vertical range in which the base body and the pad pressure element are spaced apart from each other. As the depth of the groove decreases, the base body exerts a greater supporting force on the base element and the respective pressure projection. As the depth increases, the influence and thus the stiffening of the pad pressure element decreases. Therefore, the target function of the pad pressure element can be set or adjusted by both the width and the depth of the groove, viewed in the circumferential direction of the base body.

[0043] According to a preferred embodiment of the invention, the groove, i.e., the spacer groove, has a depth that corresponds to at least 10% of the height of the base body at the edge of the groove. The groove can be designed similarly to a "record groove". Thus, its width and depth can be variable.

[0044] According to a preferred embodiment of the invention, the pad pressure element and the base body are made of the same material. The advantageous mobility of the pressure projections is achieved through the spacer groove. Using the same material reduces manufacturing costs. Preferably, the base body and base element are also formed integrally, resulting in a single-component body. This ensures particularly time- and cost-efficient manufacturing of the pad overall. The mobility of the pressure projections, or of the pad pressure element, is then individually adapted to the requirements by selecting a suitable groove width and / or groove depth for the spacer groove.

[0045] The present invention also relates to an orthopedic aid, for example a bandage or orthosis, comprising a pad according to the invention. The present invention particularly relates to a bandage or orthosis comprising a pad according to the invention, for example a knee bandage or orthosis, an ankle bandage or orthosis, a back bandage or orthosis, an elbow bandage or orthosis, a bandage or orthosis for the arm area, or a bandage or orthosis for the abdominal area.

[0046] Preferably, it is a knee bandage or orthosis, an elbow bandage or orthosis, or a wrist bandage or orthosis. Preferably, it is a knee bandage or orthosis. Preferably, it is a knee bandage.

[0047] The orthosis or bandage according to the invention is characterized by the pad according to the invention. This results in the advantages already mentioned above. Preferably, the orthosis is an elastic knitted orthosis or the bandage is an elastic knitted bandage, in particular a knee orthosis or knee bandage.

[0048] Further advantages and preferred features and combinations of features will become apparent from the foregoing and from the claims. The invention will now be explained in more detail with reference to the drawings. These drawings will show... Figure 1 shows an advantageous pad in a perspective top view; Figure 2 shows a section of an extension area of the pad. Figure 1 Figure 3 shows a section of a preferred embodiment of a pad according to the invention.

[0049] Figure 1Figure 1 shows an advantageous pad 1 for an orthosis or bandage 2, which is not shown in detail here. The illustrated embodiment of the pad is a patellar pad. The pad 1 has a pad base 3, which is essentially planar and made of an elastic, in particular viscoelastic, material. The base 3 has a three-dimensional outer contour with a front surface 4 as the treatment surface and a back surface 5 facing away from the front surface 4. According to the present embodiment, the outer contour of the base 3 is characterized by several wings that project outwards, in particular radially, from a base ring, wherein an oval-shaped opening is also formed in the base ring as a recess 6.The pad has several elevations, i.e. pad pressure elements of different shapes, on the wings and on the base ring, whereby some pad pressure elements 7 are made of the same material as the base body and some pad pressure elements 8a, 8b, 8c are made of a harder material.

[0050] According to the invention, the pad has two paired expansion zones 9, 10, each formed by three grooves 11, 12, 13, 14, 15, 16 cut into the base body 3, which function as expansion grooves. None of the expansion grooves 11, 12, 13, 14, 15, 16 extend from the outer surface 17 of the pad base body 3 to the inner surface 18 of the pad base body facing the recess 6. The paired expansion zones 9, 10 divide the pad base body 3 into a first pad base body area 19 and a second pad base body area 20, wherein, in the case of the patellar pad, the first pad base body area 19 is positioned at the top and the second pad base body area 20 is positioned at the bottom.

[0051] Due to the paired stretch zones 9 and 10, the upper, first pad base area 19 and the lower, second pad base area 20 can move significantly further apart from each other when the knee is flexed than would be the case with a pad without these stretch zones. The positions of the stretch zones 9 and 10 can be described on a clock face as "at 2 o'clock and 10 o'clock." Of course, other positions are also possible, for example, "at 3 o'clock and 9 o'clock" or "at 4 o'clock and 8 o'clock." The essential characteristic, however, is that all stretch zones only perform their desired function when used in pairs.

[0052] Such elasticity could alternatively only be achieved by using a softer base material for the pad itself. However, this would compromise the pad's functions—namely, maintaining the position of the patella and the entire knee brace, and acting as a spacer for the elastic knit fabric spanning the patella—to the required extent. Ideally, the stretch zones, particularly the stretch grooves, are positioned to the right and left of the joint space of the knee joint. This is because, when the knee is flexed, the upper part 19 of the pad must be able to maintain its position relative to the upper end of the patella. Only then is it ensured that the raised elements 1 on the upper part of the pad, especially the pad pressure elements 8a, retain their position. In prior art massage pads, the knobbed area above the oval pad is often displaced so significantly that the knobs on it "bend over" instead of applying massage pressure perpendicular to the body surface as intended. This perpendicular effect of the knobs is advantageous for targeted simulation in therapeutic massage of the vastus medialis muscle. However, if the knobs in this area tilt, the sensorimotor response of the muscle to the stimulus is not as required. The paired expansion zones 9, 10 according to the invention advantageously stabilize the position of the two pad base areas.

[0053] In the patellar pad 1 shown, a lateral wing 21 equipped with pad pressure elements 8a is cast over the main muscle "vastus medialis", which, when pressure is applied to the muscle, generates a massage with release of contractures and a sensorimotor response of the muscle.

[0054] The combination of the stretching areas 9, 10 with the intermittently massaging pad pressure elements 8a incorporated over the vastus medialis muscle according to the invention produces the desired pain relief by relaxing the painfully contracted muscle in the osteoarthritis patient, particularly also by positively influencing the trigger points. This corresponds to the pain-relieving effect that a physiotherapist produces with manual friction massage. Further pain-relieving advantages are the reduced tensile load on the patellar ligament, the reduced shear load on the arthritic patella, and the lower risk of slippage of the bandages.

[0055] The surprising advantages of a combination of elongation areas 9,10 and pad pressure elements 8a can be transferred by the person skilled in the art to other pads such as elbow pads or wrist pads.

[0056] To illustrate the expansion zones, in Figure 2An enlarged section of the pad 1 is shown. The expansion area 10 consists of the three expansion grooves 14, 15, 16, which are provided on the front 4 of the pad base body 3. The expansion grooves are all partial, meaning they do not form a channel between the inner surface 18 of the pad base body 3 facing the recess and the outer surface 17 of the pad base body 3. Instead, grooves 14 and 16 are only open towards the outer surface 17 of the pad base body 3, and groove 15 is only open towards the inner surface 18 of the pad base body 3 facing the recess. The expansion area 10 divides the pad base body 3 into two pad base body sections 19, 20. The upper, first pad base body section 19 has the pad pressure elements 8a, which remain better positioned during movement due to the paired expansion areas 10, 11.

[0057] To illustrate the pad pressure elements with spacer groove, see in Figure 3a longitudinal section view of the pad 1 in the area of the pad pressure element 8b, here designated as 110, according to Figure 1 shown.

[0058] The recess 108 thus has a depth which, according to the present embodiment, is at least as great as the height of the inserted base element 111 of the pad pressure element 110. The pressure projections 114 are designed to be so large in height that they protrude from the recess and from the base body 3 of the pad 1. While, according to the present embodiment, the recess 108 has a flat or level bottom, according to a further embodiment – not shown here – the bottom of the recess deviates from a flat plane. This allows the behavior of the pressure projections 114 to be further influenced in use and, for example, a preferred bending direction to be specified.

[0059] The contour or inner contour of the respective recess 108 corresponds at least substantially to the contour or outer contour of the respective pad pressure element 110, in particular that of the respective base element 111. The contour of the respective recess 108 is selected such that, at least in sections, a lateral gap x remains in the circumferential direction of the respective base element 111. This gap x creates a groove, i.e., a spacer groove 120, between the respective pad pressure element 110 and the base body 3. This spacer groove 120 extends at least in sections along the circumference of the respective pad pressure element. The gap x formed by the groove 120 is preferably 0.1 mm to 3 mm, more preferably 0.5 mm to 1.5 mm.

[0060] According to an advantageous embodiment, the spacer groove 120 extends over the entire circumference of the pad pressure element 110. In its longitudinal extent, the groove 120 preferably has a constant depth t.

[0061] The groove 120 preferably has a depth t which corresponds to at least 10% of the height h of the base body 3 at the edge of the groove 120.

[0062] The spacer groove 120 ensures that the deflection behavior and also the deflection direction of the pressure projection 114 are advantageously influenced.

[0063] The pressure projections 114, which are in this case designed as knobs with rounded ends, are preferably made of a harder material than the base body 3. In pads whose pressure projections 114 are harder than the base body 3, the massage effect that can be achieved by the pad 1 or the orthosis or bandage 2 having a pad 1 is greater.

[0064] The depth t and width x of the groove 120, which surrounds the respective pressure projection or the base element 111, determine the mobility of the respective associated pressure projection 114. Because the material of the base body 3 immediately adjacent to the respective pressure projection 114 is thinner or further away, the individual pressure projection 114 can deflect more easily due to the distance x resulting from the groove 120 and can also move more freely within the extended edge region. This applies regardless of whether the respective pressure projection 114 is completely recessed in the depression 108 and, for example, is flush with the outer surface 119 of the base body 3, or whether the respective pressure projection 114 protrudes from the outer surface 119, as shown in the present embodiments.

[0065] The groove 120 helps to make the pad pressure element 110 smaller overall than would be the case with flush mounting on a flat mounting surface without a groove. The pad pressure elements 110 are thus floatingly mounted in the area of the groove 120.

[0066] For certain applications, it can be advantageous if the pressure projections or the pad pressure elements 110 do not protrude from the base body 3 or only protrude slightly, but are highly movable at their respective positions, particularly with regard to their pressure projections 114. According to a further embodiment, the pressure projections 114 thus end, in particular, flush with the outer surface 119 as shown in Figure 3 This is exemplified by a dashed line on the outer surface.
