Description

E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) DESCRIPTION Attached to a patent application for an INDUSTRIAL INVENTION having by title “Rectoscope for inspection of a cavity” ******* The present invention relates to a rectoscope for the inspection of a cavity. The present invention finds advantageous application in the field of diagnosis of pathologies relating to human cavities and orifices, natural or artificial 5 surgically, diagnosis which is performed by visual inspection of the tissues delimiting cavities and orifices. The present invention finds advantageous application in the sector of the anorectal inspection. In other words, the rectoscope is in use inserted into an anorectal cavity of 10 an individual. In the state of the art, inspection devices comprising a retractor body and an introducer body: the retractor body is hollow and It presents a tubular development between a distal opening for access visually and operationally to the mucous membranes delimiting the cavity, and a 15 proximal opening, accessible by the operator at the time of use; the the introducer body also has a tubular development, between a distal end of substantially ogival shape and one end proximal, graspable by the operator. The introducer body is configured to be coupled to the body 20 spreader for sliding the introducer body inside the body spreader, until the ogival end of the introducer body protrudes from the distal opening of the spreader body, obtaining external walls designed to facilitate the insertion of the inspection device into the patient's cavity. 25 For the specific case of rectoscopes, the need may arise to remove the introducer body if the spreader body encounters resistance E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) upon insertion of the device due to the patient's anal contractions. In this case, in fact, it is planned to insert air inside the body spreader in order to dilate the anal walls thus promoting the dilation of the anal canal and the consequent insertion of the body 5 spreader. In order to prevent air from being introduced into the spreader body leak from the proximal opening and / or that body fluids or materials biologicals may leak from the proximal opening during insertion of the spreader body, the use of a lid or other device is foreseen 10 similar in order to occlude the proximal opening. In this way, when air is introduced into the spreader body, the air flow will be caused to exit from the distal opening and any fluids will be blocked by the lid itself. However, there is a risk that, after the opening of the 15 cover for the purpose of inserting inspection and / or other tools, of the body fluid may leak from the proximal opening due to the different pressures between the inside of the spreader body or cavity and from the outside. In other words, there is a risk that fluid may leak out and invest the surgeon with consequent inconveniences both from a medical point of view 20 sanitary and hygienic. The technical task of the present invention is therefore to to provide a rectoscope that is capable of overcoming the drawbacks emerging from the prior art. The aim of the present invention is therefore to provide 25 provision of a rectoscope to prevent unwanted fluid leaks even after inserting the rectoscope into the cavity. A further aim of the present invention is therefore that of make available a rectoscope that allows for the creation of inspection operations and / or other types of operations preventing inconvenience 30 by the surgeon. E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) The specified technical task and the specified purposes are essentially achieved by a rectoscope including the technical characteristics set forth in one or more of the attached claims. The dependent claims correspond to possible embodiments of the invention. 5 In particular, the specified technical task and the specified purposes are essentially achieved by an inspection rectoscope of a cavity of an individual. According to one aspect of the present invention, the inspection rectoscope of a cavity is insertable into an anorectal cavity of the individual. 10 The rectoscope comprises a retractor body, having a prevalent development along a longitudinal axis, between a proximal end and an extremity distal. The proximal end and the distal end are connected by a side wall and define, together with the side wall, a volume internal. 15 According to one aspect of the present invention, the proximal end identifies a proximal opening, coaxial with the longitudinal axis and connected to a gripping element, accessible or graspable by a operator. According to one aspect of the present invention, the distal end identifies 20 a distal opening, for access to the tissues delimiting the cavity from the internal volume. The retractor body includes a cover at the proximal end mobile between a first position and a second position. In the first position, the lid defines a closure (preferably 25 a hermetic seal) of the proximal opening. In other words, the first The lid position allows for fluid introduction without return. through the proximal opening of the retractor body. In the second position, the lid is moved away from the opening proximal so as to make the internal volume of the body accessible 30 spreader. In other words, in the second position of the lid is it is possible to access the internal volume in order to introduce instruments E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) for inspection and / or medical purposes of various kinds. According to one aspect of the present invention, the spreader body It also includes, at the proximal end, a cannula designed to facilitate, when the lid is in the first position, a communication of air 5 from the outside of the spreader body to the internal volume. According to one aspect of the present invention, the rectoscope comprises a pump or other analogous instrument connected or connectable to the cannula and configured for generate an airflow which, when the lid is in the first position, allows to generate a pushing action through the distal opening which 10 allows the cavity to expand to facilitate insertion of the spreader body. According to one aspect of the present invention, the rectoscope comprises also an introducer body, having a gripping portion. The grip portion, accessible or graspable by the operator, is connected 15 to a portion of the insertion presenting a long elongated development the longitudinal axis between an insertion end, of shape substantially ogival, distal to the grip portion, and a intermediate portion proximal to the grasping portion. The insertion portion is slidably insertable into, and disinsertable from, 20 from, internal volume through the proximal opening when the lid is in second position. According to one aspect of the present invention, the lid comprises at least one window made of breakable material and breakable in use, when the lid is in the first position, to allow access 25 controlled to the internal volume of the spreader body. Preferably, the lid comprises at least two windows, arranged in opposing portions of the lid with respect to a central portion of the lid itself. In other words, windows (or breakable windows) allow the insertion of 30 small medical instruments that, breaking the windows themselves during their insertion, it prevents the occurrence of fluid backflow E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) unwanted. Preferably, the at least one window is designed as a sealed hole using a film or film. Preferably, at least one window is delimited or made 5 by means of weakening lines defined on the lid itself. According to one aspect of the present invention, the spreader body is made of transparent material. According to one aspect of the present invention, the lid is made of transparent material. Therefore, even the at least one window (i.e. the 10 windows) is made of transparent material. Further features and advantages of the present invention will appear. clearer from the indicative, and therefore non-limiting, description of an embodiment of a rectoscope. This description will be presented below with reference to the united 15 drawings, provided for indicative purposes only and therefore not limiting, in which: - Figure 1 is a schematic perspective view of a rectoscope object of the present invention in a first configuration of use; - Figure 2 is a schematic perspective view of a rectoscope object of the present invention in a second configuration of 20 use; - Figure 3 is a schematic cross-sectional view of a rectoscope object of the present invention in a third configuration of use; - Figure 4 is a schematic cross-sectional view of a rectoscope object of the present invention according to a further form of 25 realization; - Figure 5 is a schematic side view of a rectoscope object of the present invention according to a further embodiment; - Figure 6 is a schematic perspective view of a rectoscope of the present invention according to a further embodiment. 30 With reference to the attached figures, with 1 it was overall a rectoscope was indicated. E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) The rectoscope 1 is in use inserted into a cavity of an individual. In In particular, the rectoscope 1 can be inserted into the anorectal cavity of a individual. The rectoscope 1 is a device for the inspection of the above mentioned cavity by 5 part of an operator. The rectoscope 1 can therefore be used for inspection of a patient's rectal or anorectal cavity. The terms “distal” and “proximal” will be used in the description. These terms are, in this context, to be referred to with respect to the operator: the term “proximal” refers to the parts furthest from the patient (i.e. 10 those closest to the operator), while the term “distal” refers to the parts closest to the patient (i.e. those furthest from the operator). The rectoscope 1 comprises a retractor body 2 having a development prevalent along a longitudinal axis “X”. In particular, the body retractor 2 extends between a proximal end 21 and an extremity 15 distal 22. In other words, the retractor body 2 has a development elongated along the longitudinal axis “X” defining two ends, that proximal 21 and distal 22. The proximal end 21 and the distal end 22 are connected by a side wall 23 and define, together with the side wall 23, a volume 20 internal “V”. In other words, the side wall 23 delimits and identifies, together with the proximal end 21 and the distal end 22, the volume internal “V”. Preferably, the rectoscope 1 (i.e. at least the retractor body 2) is made of transparent polymer material, in order to allow the 25 view of the surrounding tissues and mucosa. Preferably, the spreader body 2 develops with a shape substantially tubular. Preferably, the spreader body 2 is develops with a divergent path from the proximal end 21 at the distal end 22. In other words, the retractor body 2 develops 30 with a convergent course from the distal end 22 to the extremity proximal 21. Advantageously, this shape of the retractor body 2 E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) makes it easier to insert the spreader body 2 itself into the cavity to explore. The proximal end 21 identifies a proximal opening “PO”, coaxial with the longitudinal axis “X”. 5 Similarly, the distal end 22 identifies a distal opening “DO”, preferably coaxial with the longitudinal axis “X”, configured for allow access from the internal volume “V” to the tissues and mucous membranes delimiting the cavity. The proximal opening “PO” and the distal opening “DO” have the shape 10 essentially circular. The proximal end 21 is connected to a gripping element 24, accessible or graspable by an operator. In other words, the element of socket 24 develops transversely with respect to the longitudinal axis “X”. Preferably, the rectoscope 1 comprises illuminating means 3 for 15 generate a light beam directed from the proximal end 21 towards the distal end 22. The illuminating means 3 are preferably defined by an LED light. Preferably, the spreader body 2 has a cavity 25 obtained inside the same and at the proximal end 21. 20 The illuminating means 3 are internally contained in the cavity 25 for generate the light beam directly inside the spreader body 2. In other words, the illuminating means 3 are capable of generating a beam bright which, thanks to the material with which the spreader body is made 2 itself (usually a transparent or low-opacity material) is able 25 to illuminate the internal volume “V” or the distal opening “DO”. In other words, in the vicinity of the proximal opening “PO” or in proximity of the proximal end 21, the cavity 25 is defined within which the illuminating means 3 are inserted. The cavity 25 has a at least partially elongated conformation and develops according to a 30 incident axis, at least in the vicinity of the distal opening “DO”, with the axis longitudinal “X”. In this way, the illuminating means 3 are oriented in E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) so as to illuminate the internal volume “V” or the distal opening “DO”. Therefore, in this embodiment of the rectoscope 1, the illuminating means 3 placed in cavity 25 define an indirect light source of the distal opening “DO” or of the internal volume “V”. 5 Preferably, the gripping element 24 comprises a first element 24a, integral with the spreader body 2, and a second element 24b can be coupled to the first element 24a. Preferably, the gripping element 24 is made in the form of a handle. Even more preferably the handle is defined by the first element 10 24a and from the second element 24b. Preferably, the second element 24b is in turn defined by a first half-shell “S1” and a second half-shell “S2”. With particular reference to the first element 24a, the latter includes a seat 50, extending across the entire extension of the first element 24a, 15 to accommodate the second element 24b. The seat 50 is interrupted by a window 51 suitable for receiving a corresponding protrusion, integral with the second element 24b, to facilitate the connection and the stability of the coupling between the first and second element 24a, 24b. 20 Preferably, the second element 24b has a termination 24c suitable for receiving and retaining the illuminating media 3. In other words, the illuminating means 3 are placed inside the cavity 25 by means of the second element 24b. The retractor body 2 comprises in the proximal end 21 a 25 cover 4 movable between a first position (figures 1 and 2) and a second position (figure 3). In the first position, the lid 4 defines a closure (preferably a hermetic seal) of the proximal opening “PO”. In in other words, in the first position the cover 4 prevents access to the 30 internal volume “V”. Therefore, in the first position the lid 4 is transverse or perpendicular to the longitudinal axis “X”. In the first E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) position, the cover 4 is preferably locked by interlocking at the proximal end 21. In the second position, the cover 4 is moved away from the opening proximal “PO” so as to make the internal volume “V” of the 5 spreader body 2. Preferably, the cover 4 can be removed entirely from the proximal end 21. Preferably, the lid 4 can be at least partially separated from the proximal end 21. 10 Preferably, the lid 4 comprises a coupled base 40 or defined at the proximal end 21 and a flap portion 41 movable between the first and second position with respect to the base 40 by means of a hinge coupling 42. In other words, the hinge coupling 42 defines an axis of rotation for the flap portion 41 with respect to the 15 base 40. In one embodiment, such as that of Figure 6, the hinge coupling 42 has a presenting guide 42a C-section and a rotation pin 42b in use that can be inserted into the guide 42a. In particular, the rotation pin 42b extends between two ends, in which 20 a first end 42c defines a pin insertion portion of rotation 42b within the guide 42a and a second end 42d defines a limit stop portion of the rotation pin 42b during its insertion within guide 42a. As shown in the attached figures, the first end 42c has 25 a diameter smaller than the diameter of the second end 42d in so as to allow the rotation pin 42b to be inserted into the guide 42a. The pivot pin 42b also has connecting arms 41a between the rotation pin 42b itself and the tilt portion 41. In particular, the 30 two arms 41a are associated with the two ends of the rotation pin 42b. When inserting the rotation pin 42b into the guide 42a, the E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) 41a connecting arms do not interfere with insertion into the guide 42a and indeed at least the one associated with the second end 42d is in able to also slide within guide 42a. Once the tilt portion 41 has been folded down, the connecting arms 41a 5 will prevent unwanted removal of the cover 4 by going to interfere with the sliding of the rotation pin 42b. Preferably, the flap portion 41 may be provided with a tooth interlocking 43 which, in the first position, can fit into the cavity 25 or in other portion of the proximal end 21 suitable for the task. 10 The spreader body 2 preferably comprises, at the end proximal 21 a cannula 32 suitable to facilitate, when the lid 4 is in the first position, a communication of air from outside the body spreader 2 to the internal volume “V”. Preferably, the rectoscope 1 comprises a pump or other 15 similar instrument (not shown) connected or connectable to the cannula 32 and configured to generate an airflow that, when the lid 4 is in the first position, it allows you to generate a pushing action through the distal opening “DO” which allows the dilation of the cavity so as to facilitate insertion of the retractor body 2. In other words, the 20 rectoscope 1 comprises a communicating pumping device in use with cannula 32 and configured to generate an internal air flow to the internal volume “V” suitable for defining the thrust action through the distal opening “DO”. The cover 4 or at least one window 5 prevents, when the 25 lid 4 is in the first position, an air leak from the volume internal “V” outside the spreader body 2 through the opening proximal “PO”. Preferably, the rectoscope 1 further comprises an introducer body 60, having a grip portion 61, accessible or graspable 30 by the operator, connected to an insertion portion featuring a elongated development along the longitudinal axis “X” between one end of E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) insertion 62, of substantially ogival shape, distal to the grip portion 61, and an intermediate portion proximal to the socket portion 61. The insertion portion is slidably insertable into, and disinsertable from, 5 from, internal volume “V” through the proximal opening “PO” when the lid 4 is in the second position. In use, the introducer body 60 is used in a first phase of insertion of the rectoscope 1 into the anal cavity and, subsequently, removed and closed the cover 4 so as to block the opening 10 proximal “PO” so that air can be safely pumped through the cannula 32 so as to generate the aforementioned pushing action through the distal opening “D” which allows dilation of the anorectal cavity in so as to allow the correct insertion, along its entire length, of the rectoscope 2 or retractor body 2. 15 With reference to the attached figures, the lid 4 (i.e. the portion a flap 41) of the rectoscope 1 comprises at least one window 5 (or window breakable 5) made of breakable material. Window 5 is in use breakable, when the lid 4 is in the first position, to allow a controlled access to the internal volume “V” of the spreader body 2. 20 In particular, it is possible to drill window 5 with special tools in order to to be able to inspect the cavity. Advantageously, this solution avoids the occurrence of unwanted fluid returns. The pressure difference that would cause the return of body fluids and / or organic materials which would be to check with the direct opening of the lid 4 it would in fact be reduced, 25 and any fluids / materials would not leak out or in any case their exit would be highly limited. The fracture of window 5 allows the volume to be depressurized internal “V” so that fluids cannot leak out of the window same, as will become clear from the continuation of this description. 30 Furthermore, window 5 is easily breakable using the tools of inspection so as to prevent inconvenience to the individual due to E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) further unwanted thrusts. In other words, window 5 is made in a material resistant to the pressures exerted during the generation of the air flow but easily breakable by inspection tools so as to prevent unwanted thrusts by the rectoscope 1 itself 5 within the individual's anorectal cavity. Preferably, the cover 4 comprises at least one pair of windows 5 opposite each other with respect to a central portion of the lid 4 same. In this way it is possible to insert two simultaneously inspection tools inside the internal volume “V”. 10 Preferably, the lid 4 (i.e. the flap portion 41) comprises a plurality of windows 5 or pairs of windows 5, depending on a extension of the cover 4 itself or of the type of inspections to be carried out perform with specific tools. Preferably, window 5 (or windows 5) has the following shape 15 circular. Preferably, window 5 (or windows 5) has the following shape quadrangular. Preferably, window 5 (i.e. windows 5) can have any shape that, in use, does not obstruct access to inspection tools. 20 Preferably, the at least one window 5 is made in the form of a hole made on the lid 4. The hole, obtained on the cover 4, is preferably aligned with the axis longitudinal “X”. Preferably, the hole develops along its own axis incident with respect to 25 to the longitudinal axis “X” and directed towards the distal opening “DO” so as to facilitate the exit of inspection tools towards the opening distal “DO” itself. In the embodiment where there are two windows 5, both holes are oriented towards the distal opening “DO”. In this embodiment, the hole is occluded by a film or foil. 30 Preferably, the film or foil is made of low-density polyethylene. intensity. Other materials with similar characteristics can be used for the E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) making of the film or film. Preferably, the film or film are made of transparent material, so that a user of the inspection tools can verify that it is safe to puncture the film or film. 5 Preferably, the at least one window is made or delimited by 5a weakening lines such as those shown, for example, in the attached figures. The weakening lines 5a can be obtained on the lid 4 same. 10 The 5a weakening lines can alternatively be obtained on of a plug or other covering element placed to block a hole or specific seat defined on the lid 4. Preferably, the cap or other cover elements can be made of PMMA (polymethyl methacrylate). 15 Preferably, the 5a weakening lines define or are made in the shape of a cross delimiting window 5 itself. Preferably, the pair of windows 5 can both be made in the form of a hole blocked by a film or foil. Preferably, the pair of windows 5 can both be 20 delimited or created by weakening lines 5a. Preferably, one window 5 of the pair of windows 5 may be made in the form of a hole occluded by a film or foil and the other window 5 of the pair can be delimited or made by lines of weakening 5th. 25 Preferably, considering a circular development of the lid 4 (i.e. of the tilting portion 41), the windows 5 are defined laterally to a central portion of the lid 4 itself. Therefore, the windows 5 they are arranged between two semi-circles defining the lid 4 itself. The 5 windows are preferably defined in “central and lateral” portions of the 30 lid 4 if it has different shapes. In this way, any bodily fluids and / or organic materials that E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) were to have deposited towards the proximal opening “PO” they would be contents below the windows 5 and therefore, once they have been drilled, they would not be brought to exit from the proximal opening “PO” through the windows 5 themselves. 5 Preferably, the lid 4 (i.e. the flap portion 41) is equipped with a connection seat 6. Preferably, the connection seat 6 is defined in a central upper portion of the lid 4 (i.e. of the folding portion 41). Preferably, the 5 windows are defined / made in lateral portions 10 with respect to the connection seat 6. The rectoscope 1 preferably comprises an acquisition device 7 configured to at least acquire images in an external portion at the distal opening “DO”. In other words, the rectoscope 1 defines with the acquisition device 7 an inspection kit for the individual's cavity. 15 The acquisition device 7 is therefore in use connectable to the seat of connection 6. In other words, the acquisition device 7 or a its portion dedicated to image acquisition is shaped so as to fit onto or insert into the connection seat 6. Preferably, the connection seat 6 is equipped with a window 20 transparent or a defining lens (collectively indicated with the numerical reference 6a in figure 3) a passing acquisition axis through the distal opening “DO”. Preferably, the acquisition axis is transverse to the longitudinal axis “X”. Preferably, the connection seat 6, i.e. the transparent window or 25 lens, also allows observation of the anorectal canal with the naked eye so that you don't necessarily have to use the 7 acquisition device. The positioning of the connection seat 6 allows, during a Inserting inspection tools through window 5 (or the windows 5) not to block the acquisition of images with them (or 30 videos) using the 7 capture device. In a further embodiment of the rectoscope 1, the same E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) It also includes a lighting device 8 configured for generate a light beam passing through the proximal opening “PO” and the distal opening “DO”. Preferably, the lighting device 8 is integral with the lighting device 5 acquisition 7 defining an inspection group 100. Preferably, the rectoscope 1 is The at least one connection seat 6 is also configured, in use, to allow the connection of the lighting device 8 to the portion central part of the cover 4. In other words, the connection seat 6 can 10 be shaped so as to allow simultaneous connection of both the acquisition device 7 and the illumination device 8. Preferably, where the lighting device 8 and the acquisition 7 are integral with each other, the connection seat 6 allows the inspection group 100 connection. 15 Alternatively, the connection seat 6 is configured for the maintaining the lighting device 8 in proximity of the central portion of the lid 4. For example, with particular reference to the embodiment in which the inspection group 100 is defined, the portion of inspection group 100 in which the inspection device is defined 20 acquisition 7 is shaped so as to fit or insert into the seat of connection 6 and the lighting device 8 (in use arranged below the acquisition device 7 as shown in the figure 4) consequently finds himself facing the lid 4 towards the opening distal “DO”. In other words, the connection seat 6 is located on the 25 lid 4 so as to define a space (preferably transparent) suitable for the passage of the light beam generated by the device lighting 8. The lighting device 8 therefore defines a light source direct of the distal opening “DO” or of the internal volume “V”. 30 According to one embodiment of the rectoscope 1, the same includes only the lighting means 3. E1.M0056.12.IT.65 Eng. Aldo Paparo (Registered no. 1281 BM) According to a further embodiment of the rectoscope 1, the same includes only the lighting device 8 (whether the same whether or not integral to the acquisition device 7). According to a further embodiment of the rectoscope 1, the same 5 includes both the illuminating means 3 and the lighting device 8. In In this way, it is possible to better illuminate the internal volume “V” so as to facilitate inspection operations. Furthermore, the positioning of the connection seat 6 allows for prevent the same (i.e. the transparent window or the lens) from being able to 10 come into contact (or excessively come into contact) with any fluids and / or organic materials that would occlude the acquisition axis. Preferably, in a further embodiment, the cover 4 (i.e. the folding portion 41) is without the seat and can be equipped with a transparent window or defining lens with which an operator can 15 observe inside the volume “V” or observe beyond the opening distal “DO” if necessary inspect with the naked eye. Advantageously, the rectoscope 1 object of the present invention is in able to overcome the drawbacks that have emerged from the prior art. Advantageously, the present invention prevents leakage 20 unwanted fluid discharge even after insertion of the rectoscope 1 into the cavity. Advantageously, the rectoscope 1 allows for performing operations of inspection and / or other types of operations preventing inconvenience on the part of the surgeon. 25 In other words, the rectoscope 1 object of the present invention allows not having to open cover 4 to carry out inspection operations so as to prevent the aforementioned unwanted leaks of fluids and / or materials organic.
