Description

[Technical Field]

[0001] Related Application Information The present application claims priority based on U.S. Provisional Application No. 63 / 518,508 filed on August 9, 2023 and U.S. Provisional Application No. 63 / 596,929 filed on November 7, 2023, and the entire contents of these provisional applications are incorporated herein by reference.

[0002] The devices and methods disclosed herein relate to a surgical suction device capable of reducing clogging during surgery. [Background Art]

[0003] Surgical suction devices, generally referred to as suction wands, suction tips, or suction devices, allow physicians to remove fluid, tissue, and / or other tissue debris from a surgical site inside or on the surface of a patient's body. A surgical suction device may comprise a tip and / or a shaft that is inserted into a surgical site, wound, or other body site. The shaft has a generally elongated shape and may comprise a handle or grip section for holding the suction device and further for coupling a vacuum source to the device. Such a vacuum source can be a tube connected to a suction pump that provides suction at the tip.

[0004] Suction during surgery is generally necessary to remove tissue debris and unnecessary blood from the surgical field that may cause (or have already caused) conditions such as cerebral hemorrhage, deep vein thrombosis, stroke, and embolism. Conventional devices often clog frequently, necessitating removal of the suction device from the surgical site. In such cases, the suction device is either replaced, or the clog is cleared using another device or a wire. In any case, clogging of the suction device during surgery results in longer operative time, increased costs due to the use of additional devices, and / or increased risk of injury to the patient in surgeries with strict time constraints. [Summary of the Invention] [Problems that the invention aims to solve]

[0005] Therefore, there is a need for suction devices that can efficiently capture and remove unwanted substances from the target area. [Means for solving the problem]

[0006] This specification discloses a medical device for applying suction force from a suction source to a target area during a medical procedure. The medical device may comprise a handle and an elongated shaft extending from the handle. The elongated shaft may comprise a longitudinal axis, a proximal end, a distal end, and a central lumen extending to a distal tip having an opening. The medical device may comprise a plurality of cutting elements located within the central lumen, which extend from the inner surface of the elongated shaft and are inclined with respect to the longitudinal axis such that the edges of each cutting element adjacent to the proximal end are tapered, and these cutting elements are spaced apart from each other both along the longitudinal axis and in the circumferential direction with respect to the inner circumference of the elongated shaft.

[0007] When the suction source draws material through the opening, the material engages with multiple cutting elements, thereby allowing the material to be gradually cut while being drawn towards the proximal end of the elongated shaft.

[0008] At least two of the multiple cutting elements may be spaced 90 degrees apart from each other in the circumferential direction. Multiple cutting elements may be spaced apart in the circumferential direction over a range of at least 360 degrees. Adjacent cutting elements along the longitudinal axis may be spaced apart longitudinally at a variable distance along the elongated shaft. The elongated shaft may be rigid. Multiple cutting elements may be configured to bend when engaging with material. Multiple cutting elements may be made of an elastic material. Multiple cutting elements may be made of Nitinol®. The handle may be detachable from the elongated shaft. The handle may be disposable.

[0009] Multiple cutting elements may comprise multiple rows of tines extending longitudinally along an elongated shaft. These rows of tines may be spaced 90 degrees apart circumferentially along the elongated shaft. Each of the multiple cutting elements may have a length exceeding half the diameter of the elongated shaft. The tips or edges of each of the multiple cutting elements may overlap radially.

[0010] The handle may be configured to include a suction opening for controlling the suction force from the suction source. The handle may further include a rotatable barb connected to the suction tube. The suction tube may connect the suction source and the central lumen of the elongated shaft. The rotatable barb and the suction tube may be connected via a rotatable suction connector. The perfusion tube may be positioned concentrically outside the central lumen.

[0011] In some embodiments, a method is disclosed for applying suction force from a suction source to a target area during a medical procedure. This method may include the step of introducing a medical device to the target area. When the suction source draws material through the opening, the material engages with the cutting elements, thereby allowing the material to be gradually cut while being drawn toward the proximal end of the elongated shaft.

[0012] In some embodiments, systems are disclosed for capturing and cutting material in a target area during medical procedures. The system may comprise a handle and an elongated shaft extending from the handle. The elongated shaft may comprise a longitudinal axis, a proximal end, a distal end, and a central lumen. The system may comprise a plurality of cutting elements located within the central lumen, these cutting elements extending from the inner surface of the elongated shaft. The system may comprise a distal tip coupled to the distal end of the elongated shaft. The distal tip may comprise a suction window. The system may comprise a suction source configured to apply suction force to draw material into the central lumen through the suction window. The system may comprise an umbrella portion located at the distal end of the elongated shaft. This umbrella portion may be axially slidable to engage with material drawn in through the suction window.

[0013] When the suction source draws material through the opening, the material engages with multiple cutting elements, thereby gradually cutting the material while being drawn toward the proximal end of the elongated shaft. The multiple cutting elements may be inclined with respect to the longitudinal axis such that the edges of each cutting element adjacent to the proximal end are tapered. The multiple cutting elements can be curved tines that are spaced apart from each other in the circumferential direction with respect to the inner circumference of the elongated shaft. [0015] FIGS. 1A and 1B respectively show a perspective view and a side view of a medical suction device 100 according to an embodiment of the present invention. The device 100 may include an elongated shaft 102 having a central lumen therein. The elongated shaft 102 may be formed of a cylindrical structure made of metal, polymer, composite material, or a combination thereof. The device 100 may be introduced to a desired target region by a conventional method.

[0016] The suction device 100 may include a rotary luer coupling 104 having a cam surface that enables rotation of the shaft 102 relative to the handle 106. The suction device 100 may include any number of curved surfaces 108 and finger rests 110 to assist in the operation of the device and to improve the ergonomic characteristics of the suction device 100. These surfaces 108, the finger rests 110, and / or other surfaces of the handle 106 may include any number of raised areas 112 to assist in gripping the suction device 100.

[0017] The suction device 100 may further include a control window 114 configured to house a control device for operating suction from a suction source (not shown). The suction device 100 may also include a barb connector 116 that connects to a suction tube.

[0018] The suction device 100 may include a ball joint 118 for housing a barb connector 116. This ball joint 118 has a ball surface to allow articulation of the barb connector 116 relative to the handle 106 of the suction device 100. The ball joint 118 may include an O-ring to reduce leakage during surgery. This ball joint 118 may be capable of allowing rotational movement of about 10 to about 20 degrees (e.g., about 15 degrees).

[0019] In other embodiments, the ball joint 118 can be tilted so that the suction tube is substantially perpendicular to the longitudinal axis of the handle 106, thereby enabling rotation of approximately 180 degrees.

[0020] The shaft 102 may be inclined to improve visibility over its distal end. As will be described below, the shaft 102 may include a plurality of cutting elements configured to help prevent blockage of the shaft 102.

[0021] Figure 2 shows an example of the distal end of a shaft 102 for use with a suction device 100. In this example, a portion of the shaft wall can be bent inward to form multiple cutting elements 200. The shaft 102 can be laser-cut such that a portion of the wall is bent toward the center of the lumen. The shaft 102 may be fitted with an outer layer or sleeve (e.g., a heat-shrinkable jacket) to allow vacuum to be drawn inside the lumen. The shaft may have an opening 202 at its distal end, through which tissue fragments or material can enter the central lumen 204 as the distal end advances to the target region.

[0022] These multiple cutting elements 200 are located within the central lumen 204 and may extend from the inner surface of the elongated shaft 102. The multiple cutting elements 200 can be inclined with respect to the longitudinal axis such that the edges 206 of one or more cutting elements 200 are oriented toward the distal end of the elongated shaft 102. In other examples, the cutting elements 200 can extend substantially perpendicular to the longitudinal axis of the shaft 102 such that the edges 206 extend normal to the longitudinal axis. The edges 206 of each cutting element 200 adjacent to the proximal end of the shaft 102 may be tapered.

[0023] Multiple cutting elements 200 may be spaced apart in the longitudinal direction along the longitudinal axis. In some examples, multiple cutting elements 200 may be spaced apart at a constant interval along the shaft 102. In some examples, multiple cutting elements 200 may be spaced apart at a variable interval along the shaft 102.

[0024] Multiple cutting elements 200 can be offset from each other in the circumferential direction with respect to the inner circumference of the elongated shaft 102. For example, as can be seen from Figure 4B, adjacent cutting elements 200 can be offset from each other by 90 degrees (i.e., the first cutting element extends from the inner surface of the shaft 102 at an angular position of 90 degrees from the position of the second cutting element). Thus, material drawn in through the central lumen can be gradually cut as the material moves proximal due to the suction. This configuration allows the cutting elements 200 to cut the material until it is small enough to be drawn in through the exit port of the handle.

[0025] These multiple cutting elements 200 can be made of an elastic material (e.g., a nickel-titanium alloy). This allows the cutting elements 200 to bend when they engage with any material passing through them. Thus, the combination of the suction energy in the central lumen and the engagement of the material itself with the cutting elements 200 can generate a vibrating force that assists in cutting the material. The shaft 102 can be made of a rigid material to enable the bending of the cutting elements 200. The outer diameter of the shaft 102 may be heat-shrinkable to withstand the suction force applied in the central lumen 204.

[0026] In some examples, the multiple cutting elements 200 may consist of multiple rows of cutting elements extending longitudinally along the elongated shaft 102. The multiple rows of cutting elements 200 may be spaced 90 degrees apart from each other in the circumferential direction along the elongated shaft 102. In some examples, the multiple rows of cutting elements 200 may be spaced at a variable angle apart from each other in the circumferential direction along the elongated shaft 102.

[0027] In some examples, the shaft 102 may function as an insert for the outer tube or outer shaft of the suction device 100, providing a sealed surface for vacuuming the inside of the insert.

[0028] In some examples, the cutting element 200 may be fabricated by forming two semitubular bodies having an internal structure, and these semitubular bodies may be welded or joined together to form a suction-capable airtight pipe.

[0029] Figures 3A and 3B show partial cross-sectional views of another example of a shaft 102 of a suction device 100, comprising a plurality of fixed cutting elements 200, blades, or tines within the shaft 102. As shown in Figure 3A, the cutting elements 200 may be positioned in a longitudinal row and inclined such that the edges of cutting elements 200 adjacent to the proximal end are tapered toward the proximal end of the device. These rows of cutting elements 200 can be spaced apart circumferentially along the inner surface of the shaft 102 (for example, spaced about 90 degrees apart from each other), but variable spacing may be used.

[0030] In some examples, the cutting elements 200 may be positioned in a helical direction and / or other shapes so as to be able to cut or alter the shape of material that may clog the suction device 100. In some examples, the cutting elements 200 may be oriented proximal or distal and may extend over short, long, or variable distances within the lumen of the shaft 102. Furthermore, the cutting elements 200 may be asymmetrical and / or have a variable pitch. The number of cutting elements 200 may be variable, and the position of the cutting elements 200 may be set to any initial distance from the distal end of the shaft 102.

[0031] In some examples, the shaft 102 may function as an insert for the outer tube of the suction device 100, providing a sealed surface for vacuuming the inside of the insert.

[0032] Figure 4A shows an example of a shaft 400 in which cutting elements 404 are formed by bending a portion of the wall into the lumen 402 of the shaft 400, thereby creating spaced-out cutting elements 404 in the axial direction. In the cross-sectional view shown in Figure 4B, the cutting elements 404 can be relatively linear and can extend over a length of more than half the diameter of the shaft 400, but not over the entire lumen 402. In this configuration, adjacent cutting elements 404 can be offset from each other by approximately 90 degrees (i.e., the first cutting element extends from the inner surface of the shaft 400 at an angular position of 90 degrees from the second cutting element). The cutting elements 404 can be spaced out by a length L in the axial direction along the shaft 400. As can be seen from Figure 4B, the tips or edges 206 of each of the multiple cutting elements 404 can overlap radially. The example shown in Figure 4B illustrates that the cutting elements 404 are spaced apart circumferentially throughout the entire 360 degrees inside the lumen 402.

[0033] In the example shown in Figure 4C, the cutting element 404 may be bent in an arc shape within the lumen of the shaft 400. In this example, the cutting element 404 can extend over a portion of the diameter of the lumen 402. Such cutting elements can be multiple curved tines spaced axially along the elongated shaft 400 and offset circumferentially with respect to the cross-section of the elongated shaft. In this example, three cutting elements 404 are spaced circumferentially over a 180-degree range of the lumen 402. Some embodiments of the device may include any number of cutting elements spaced apart from one another over the entire or partial circumference of the device.

[0034] Figures 4D and 4E show the length L of the shaft. s Another example of a shaft 400 is shown, comprising cutting elements 404 spaced apart from each other in the axial direction at a variable interval over length L. sThe length can be approximately 10 cm to 20 cm (e.g., 14.5 cm). The cutting element 404 may have a straight edge, a curved edge, or a combination thereof. As can be seen from Figure 4E, the most distal cutting element 404 may be part of the distal tip of the shaft 400. In this example, the bent portion of the cutting element 404 widens in the surface region at the most distal location, thereby allowing it to engage with larger blood clots than would be possible with a simple rounded end.

[0035] In some examples, the cutting element 404 may be oriented linearly in the radial direction. In other examples, the cutting element 404 may be oriented curvilinearly in the radial direction.

[0036] It should be understood that any example of the cutting elements of this disclosure may be used in any example of a shaft. It should also be understood that various orientations and shapes of the cutting elements of this disclosure may be used together in some example of a shaft.

[0037] Figures 5A and 5B show an example of a shaft 500 having an opening 502 distal to it, which has a specific shape to match a cutting element 504. Therefore, the cutting element 504 may have a cross-sectional profile that matches the shape of the opening 502. To connect the cutting element 504 to the opening 502, the cutting element 504 can be bonded or welded into the opening 502. In other examples, the shaft 500 may have multiple cutting elements 504 in multiple openings.

[0038] Figure 5C shows another example of a shaft 500 which may have an angled tip 506 or a tapered portion at the distal end of the shaft 500. This angled tip 506 can increase the surface area of the opening 502 for engaging with the blood clot or material, allowing the suction device to capture more material.

[0039] In some examples, a fixed internal cutting element 508 may be used within the central lumen of the shaft 500. In some examples, the shaft 500 may be equipped with markers and / or bands for tracking the movement of the distal tip of the catheter using X-ray or other non-invasive imaging techniques. In some examples, the marker bands may be made from radiopaque metal or other trackable material and thus laser-cuttable, so that when bent inward, they can form a fixed cutting element 508 within the shaft lumen. Thus, if a blood clot is drawn into the shaft, the clot will pass through this cutting element and be broken down into finer fragments, making aspiration easier and faster.

[0040] In some examples, multiple fixed cutting elements 508 can be laser-cut into the shaft 500, and after the blades are bent into the lumen, this bent portion can form a helical pattern. This helical pattern can provide an impeller function, and thus allow for resizing of tissue or blood clots for faster suction.

[0041] In some examples, multiple fixed cutting elements 508 can be bent inward, allowing the guide wire to still pass through the central lumen and the blade to remain in close proximity to the inner wall of the hypo tube.

[0042] Figure 6 shows a system 600 comprising an elongated shaft 602 and a removable, disposable handle 604. The elongated shaft 602 may be coupled to the handle 604 via an opening in the handle located at a Y-shaped connector (see example in Figure 9A, not shown in Figure 6). This makes the handle 604 removable from the elongated shaft 602 and, if desired, disposable. The shaft 602 may be provided with a rotating handle 608 that allows rotational or sliding motion of the shaft 602 relative to the handle when coupled via the Y-shaped connector.

[0043] The suction source can apply a suction force to the central lumen 204 to pull the material proximal to perform cutting by the cutting element as described herein. Thus, the material pulled proximal can be removed by being sucked towards the proximal end of the handle 604 and passing through the outlet port on the handle 604.

[0044] The handle 604 may include a rotatable suction connector 610 that can be connected to a suction tube connected to a suction source. Rotating this suction connector 610 may facilitate the connection of the suction tube to a barb connector (not shown) within the handle 604 for the user.

[0045] In this example, the cutting elements 200 are tapered toward the proximal end of the shaft 602, while the tips or edges of the cutting elements 200 extend perpendicular to the longitudinal axis of the shaft 602. In other examples, the tips or edges of the cutting elements 200 may extend toward the distal end of the shaft 602.

[0046] Figure 7 shows a suction device 700 according to one embodiment of the present invention. This suction device 700 may comprise a shaft 702 and an umbrella portion 704 extending from the distal end of the shaft 702. The umbrella portion 704 may have an umbrella head 706 shaped as a flat cap, a dome-shaped cap, or a similar structure. The umbrella portion 704 may extend over the entire circumferential region of the non-traumatic tip portion 708.

[0047] The umbrella portion 704 is coupled to an elastic member 710, which is compressible and slack, allowing the umbrella portion 704 to slide proximal and distally. After material is drawn into the suction device 700, the umbrella portion 704 may be pulled proximal to draw the material into the lumen of the shaft 702. The suction device 700 may have a proximal transition to a wider shaft, through which material can be drawn or sent to an outlet port (not shown) and discharged. The umbrella portion 704 may extend to the distal end of the device 700 toward a non-traumatic tip portion 708 positioned at the distal end of the shaft 702.

[0048] The non-traumatic tip 708 may be provided with a suction window 712 on its side wall. The suction window 712 may have an elliptical, circular, or any other shape that allows material to pass through. The suction window 712 may be positioned in a target area of an anatomical structure so that material can be drawn into the suction device 700 and into the lumen of the shaft 702.

[0049] The umbrella portion 704 can be coupled to a handle (not shown) having an actuator via a mechanical coupling, such as a wire 714 or a similar structure. This actuator (e.g., trigger, ratchet, joystick) can move the umbrella portion 704 proximal, thereby drawing material away from the suction window 712. The wire 714 can be coupled to the umbrella portion 704, which has an opening 716 for inserting the wire 714.

[0050] In some examples, the umbrella portion 704 can be engaged with a cutting mechanism (i.e., the cutting element described above) by being pulled in the proximal direction, thereby breaking or cutting the material into smaller fragments that can be aspirated.

[0051] Figures 8A and 8B show another embodiment of the suction device 800. This suction device 800 includes a distal tip 802 with a suction tube 804, which may be equipped with multiple cutting elements as discussed above. The suction tube 804 may be surrounded by an external perfusion tube 806 positioned concentrically around the suction tube 804. This perfusion tube 806 may be configured to supply perfusion fluid to a target region.

[0052] The suction device 800 may further comprise a vacuum fitting 808 and a fluid fitting 810. The vacuum fitting 808 is in fluid communication with the suction tube 804 and may be equipped with a barb for attachment to an external suction tube coupled to a suction source (not shown). The fluid fitting 810 is in fluid communication with the perfusion tube 806 and may be equipped with a barb for attachment to an external perfusion tube coupled to a perfusion source (not shown). The suction device 800 may further comprise one or more control windows 812, which allow the user to control the suction and / or perfusion of the device 800.

[0053] In some examples, the gap between the suction tube 804 and the perfusion tube 806 allows the suction tube 804 and the perfusion tube 806 to be coupled to an energy source (not shown) and function as a bipolar electrode, thereby enabling bipolar energy delivery to the distal end of the device 800. In such cases, the device 800 may be provided with an external insulating layer 814. Thus, the device may further be provided with a control switch 816 for controlling the application of RF energy.

[0054] Figures 9A and 9B show perspective and side views, respectively, of another embodiment of the suction device 900. As shown, the suction device 900 comprises a shaft 902, which may include a plurality of cutting elements 904 (as described above) configured to help prevent blockage of the shaft 902. In this example, the suction device 900 may have a window 906 at the tip of the shaft 902.

[0055] The suction device 900 may be designed so that the shaft 902 is positioned on the shaft portion 908, allowing the shaft to be rotated relative to the device handle 912 by the shaft handle 910, while enabling vacuum application in the window 906. Furthermore, the suction device 900 may include one or more control windows 920, which allow the user to control the suction and / or perfusion of the device 900.

[0056] The shaft handle 910 and the device handle 912 may have any feature parts, such as curved surfaces and ribs, to improve the operability of the suction device 900. The suction device 900 may have any number of curved surfaces 914 and finger rests 916 to assist in the operation of the device and improve the ergonomic characteristics of the device. The surface 914, finger rests 916, and / or other surfaces of the handle 912 may have any number of protrusions 918 to assist in gripping the suction device 900.

[0057] Additionally, the suction device 900 may be equipped with an integrated barb connector 922 for coupling to a vacuum source.
