Abstract
Description
Technology Field
[0001] The present invention relates to a composition for the prevention, improvement, or treatment of diabetic peripheral neuropathic pain comprising a T2A8 compound as an active ingredient. Background Technology
[0002] Diabetes is a disease whose incidence is rapidly increasing worldwide. The World Health Organization (WHO) estimated that there were approximately 180 million patients with diabetes in 2008, and this number is projected to double by 2030. Consequently, the financial and social costs associated with diabetes are gradually increasing, and the mortality rate due to various complications associated with the disease is also rising. However, since strict blood sugar control can prevent the progression of the disease or slow its progression, maintaining stable blood sugar levels through lifestyle changes and appropriate treatment is considered an important approach.
[0003] Diabetic peripheral neuropathy is the most common complication of diabetes and one of the major causes of neuropathy, defined by the signs and symptoms resulting from peripheral nerve dysfunction caused by diabetes.
[0004] Diabetic peripheral neuropathy, which lowers the quality of life and increases mortality in diabetic patients, increases with age and duration of diabetes, and can occur in both type 1 and type 2 diabetic patients, but is more common in type 2 diabetic patients (32.1%) than in type 1 diabetic patients (22.7%).
[0005] Approximately 10% of diabetic patients complain of persistent pain; this pain can be spontaneous or triggered by stimuli, and it can be severe and intractable. Pain caused by diabetic peripheral neuropathy typically worsens at night and is often described as burning, stabbing, throbbing, or constricting. Furthermore, diabetic neuropathy pain is felt more distinctly in the feet than in the hands, and sometimes sudden, intense pain can lead to weight loss or depression.
[0006] Diagnosis of diabetic peripheral neuropathy involves the measurement of muscle strength, touch, temperature, and vibration, as well as the measurement of peripheral cutaneous blood flow and nerve conduction. Currently, three types of treatment are being performed for diabetic peripheral neuropathy: blood glucose control, etiological treatment, and symptomatic treatment. Based on various research results indicating a high incidence of diabetic peripheral neuropathy in diabetic patients with uncontrolled blood glucose, the correlation between hyperglycemia and the progression of neuropathy is well established, and blood glucose control is recognized as a fundamental method for the prevention and treatment of diabetic peripheral neuropathy.
[0007] Symptomatic treatments for relieving pain in diabetic neuropathy include tricyclic antidepressants, anticonvulsants, selective serotonin reuptake inhibitors (SRIs), and topical capsaicin. Tricyclic antidepressants (TCAs) alleviate pain by inhibiting the reuptake of serotonin and norepinephrine in the central nervous system, thereby suppressing pain transmission and enhancing the action of endogenous endorphins. Anticonvulsants and SRIs are used as second-line treatment options for patients for whom TCAs are not suitable. Topical capsaicin is known to be effective in relieving pain when applied topically to the skin; it alleviates pain by increasing the release of substance P from type-C nociceptive fibers at sensory nerve endings and inhibiting its reaccumulation, thereby disrupting the conduction and propagation of peripheral pain signals.
[0008] However, due to the wide variety of etiology and symptoms of diabetic peripheral neuropathy, it is difficult to find a treatment tailored to each individual patient; currently, symptomatic treatments aimed at pain relief and preventing disease progression are primarily employed rather than a fundamental cure.
[0009] Meanwhile, regarding prior art related to diabetic peripheral neuropathic pain, Korean Registered Patent No. 1802089 discloses a "pharmaceutical composition for the prevention or treatment of diabetic neuropathic pain containing GS-E3D as an active ingredient," and Korean Published Patent No. 2007-0010135 discloses a "novel use of a peptide compound for the treatment of pain in painful diabetic neuropathy." However, there has not yet been any disclosure regarding the "composition for the prevention, improvement, or treatment of diabetic peripheral neuropathic pain containing a T2A8 compound as an active ingredient" of the present invention. [0033] The present invention will be described in detail below by way of embodiments. However, the following embodiments are merely illustrative of the present invention, and the scope of the present invention is not limited to the following embodiments.
[0034] [Materials and Methods]
[0035] 1. T2A8 compound
[0036] The T2A8 compound (CAS no. 351898-98-1) used in the present invention was purchased from Riyun Bio Co., Ltd.
[0037] 2. Animal Management
[0038] The animal experiments conducted in this study followed the guidelines established by the Animal Ethics Committee of Gyeongsang National University (GNU-201021-M0079-01). Eight-week-old male C57BL / 6 mice were purchased from Coatec (Animal Breeding Center, Pyeongtaek, Korea). Prior to the experiment, the mice were acclimatized for one week in a specific pathogen-free environment with a 12-hour light-dark cycle. During this period, the mice were allowed to freely consume food and water within the facility.
[0039] 3. Animal Experiment Design
[0040] Mouse models combining a high-fat diet (HFD) and streptozotocin (STZ) are widely used for the study of diabetic peripheral neuropathy. First, mice were fed a high-fat diet (60 kcal%) for 2 weeks, followed by a fast of at least 8 hours, after which STZ (60 mg / kg, Sigma Aldrich, USA) was injected intraperitoneally. One week after the STZ injection, mice with a fasting blood glucose level of 250 mg / dL or higher were considered diabetic and used. They were then fed a high-fat diet for 13 weeks. Mice were randomly assigned to three groups, each consisting of a normal diet group (NFD); a high-fat diet group (HFD); and a high-fat diet and streptozotocin group (HFD + STZ), with 7 mice distributed to each group.
[0041] 4. Oral Glucose Tolerance Test (OGTT)
[0042] Mice were fasted for 8 hours in the morning and then orally administered glucose (2 g / kg). Blood was collected from the tail vein at 0, 30, 60, 90, and 120 minutes after glucose administration. Blood glucose concentrations were measured using a blood glucose meter (Optium Xceed, Abbott Diabetes Care Inc., USA).
[0043] 5. Measurement of insulin and HbA1c levels
[0044] Serum insulin levels were measured using a mouse insulin ELISA kit (Crystal Chem, Inc., IL, USA) after an 8-hour fast in mice according to the manufacturer's instructions. Absorbance was measured using a spectrophotometer (VersaMax TM Measurements were taken at 450 nm and 630 nm using a microplate reader (Molecular Devices, CA, USA). Glycated hemoglobin (HbA1c) levels were measured in whole blood collected from mice on the day of surgery using a mouse HbA1c assay kit (Crystal Chem, Inc.) according to the manufacturer's instructions. VersaMax TM Absorbance was measured at 700 nm using a microplate reader (Molecular Devices).
[0045] 6. Bonfrey Test
[0046] To measure symptoms of mechanical allodynia, experimental animals were stimulated using a calibrated Bonfrey monofilament set (EXACTA Precision and Performance Monofilament, Stoelting Co., Wood Dale, IL, USA), and avoidance responses were observed. Mice were placed in a transparent Plexiglas chamber (10 × 10 × 13 cm) with a metal mesh bottom and allowed to acclimatize for at least 30 minutes. A series of monofilaments (ranging from 0.008 to 300 g) were applied to the surface of the sole of the right hind paw using a force that bent the filament. The buckling weights of the filaments used were 0.008, 0.02, 0.04, 0.07, 0.15, 0.4, 0.6, 1, 1.4, 2, 4, 6, 8, 10, 15, 26, 60, 100, 180, and 300 g. When the mouse lifted its paw, it was recorded as a positive reaction, and the lighter filament was used for the next measurement. If there was no reaction within 10 seconds, a heavier filament was applied to evaluate sensitivity.
[0047] Mechanical sensitivity after oral administration of T2A8 (0.36 mg / kg) was measured at 1, 3, and 5 hours after administration. T2A8 was dissolved in DMSO.
[0048] 7. Statistical Analysis
[0049] Data were expressed as mean ± standard deviation (SD). To analyze differences between groups, a normality test was performed, followed by a one-way analysis of variance using Tukey's post-hoc test. Statistical analysis was performed using OriginPro 2020 software (OriginLab Corp., Northampton, MA, USA), and p-values less than 0.05 were considered statistically significant.
[0050] Example 1. Establishment of a diabetic peripheral neuropathic pain model
[0051] Body weight, blood glucose, feed and water intake, and fat content were measured in a mouse model of diabetic peripheral neuropathic pain induced by high-fat diet (HFD) and streptozotocin (STZ) administration.
[0052] As a result, as disclosed in Figure 1, it was confirmed that blood glucose increased after 30 minutes in the high-fat diet and streptozotocin administration group, blood insulin levels decreased significantly, and glycated hemoglobin levels increased significantly compared to the normal diet group.
[0053] In addition, the results of the pain evaluation for mechanical allodynia confirmed that the threshold value was significantly reduced and pain increased in the high-fat diet and streptozotocin administration group (HFD+STZ) compared to the normal diet group (NFD) (Fig. 2).
[0054] Example 2. Pain relief effect following administration of T2A8 compound in a diabetic peripheral neuropathic pain model
[0055] Pain evaluation following the administration of the T2A8 compound of the present invention was performed in an animal model of diabetic peripheral neuropathic pain induced by a high-fat diet and streptozotocin administration, prepared in Example 1 above. The T2A8 compound was orally administered to the animal model at a dose of 0.3612 mg / kg, and pain was evaluated by measuring mechanical allodynia after 1, 3, and 5 hours.
[0056] As a result, as disclosed in Figure 3, it was confirmed that an analgesic effect was produced by reducing the pain response to mechanical stimulation as the administration time of the T2A8 compound increased.
