Description

[0001] The utility model relates to the field of medicine, in particular to control units for surgical equipment that combine the functions of mechanical processing of tissues and vacuum removal of products of intracorporeal cutting of biological tissues.

[0002] The device is intended for use in systems for minimally invasive surgery, where compact placement of power and control units is required.

[0003] The utility model solves the problem of integrating a vacuum pump, a controller that simultaneously controls the processes of intracorporeal cutting of biological tissues and their aspiration, a transformer and a power supply in a single protected housing.

[0004] The proposed solution allows for optimization of the internal layout by rigidly fixing the elements to the base of the case.

[0005] The prior art discloses an OTLOGICAL ASPIRATOR (patent CN 209019153 U, published: 06 / 25 / 2019), which is a medical equipment control unit with a vacuum pump installed inside, connected to a control controller and equipped with a system of sensors and switches.

[0006] The aforementioned analog has a drawback: its controller only controls the aspiration process, not the intracorporeal cutting modes of biological tissue, which are performed using the morcellator control unit. As a result, such a controller cannot automatically synchronize the morcellator's intracorporeal cutting modes with the vacuum pump operating modes, which in turn prevents optimization of the intracorporeal cutting speed of the morcellator's cutting attachments and the tissue extraction rate.

[0007] Furthermore, a DEVICE FOR AUXILIARY SUCTION OF A RADIOFREQUENCY RAZOR (patent US 20240115286 A1, published 11.04.2024) is known from the prior art, from which an electrosurgical system is known, including: an electrosurgical generator of the radio frequency range, a vacuum pump and an electrosurgical instrument in the form of a rotating shaver (cutting attachments). This electrosurgical system is characterized in that during use, the radiofrequency electrosurgical generator delivers a radiofrequency coagulation or ablation signal through radiofrequency electrical connections to the active electrode, and the vacuum pump provides suction through a central suction channel connecting a suction hole located inside the electrode to the vacuum pump.

[0008] The aforementioned analog has the following drawbacks: the controller used only controls the aspiration process, not the intracorporeal cutting modes of biological tissue performed using a rotary shaver. Furthermore, the vacuum pump itself is installed separately, as an independent module, rather than in the shaver control unit. As a result, this controller is unable to automatically synchronize the shaver's intracorporeal cutting modes with the vacuum pump's operating modes. This, in turn, prevents automation of the tissue separation rate following shaving (intracorporeal cutting of biological tissue) and the removal rate of such tissue. In addition, placing equipment in the form of several modules significantly increases the space occupied by such equipment and complicates the process of preparation for surgery, due to the need for additional steps to connect and configure the modules.

[0009] This analogue is the closest in technical essence to the declared technical solution and is selected as a prototype.

[0010] The technical result of the utility model is to provide the ability to simultaneously coordinate the control of the modes of intracorporeal cutting of biological tissues by cutting nozzles and a vacuum pump.

[0011] The technical result is achieved due to the fact that the control unit of the shaver-morcellator with integrated aspiration, containing a housing, an interface for connecting the handle of the shaver-morcellator, a vacuum pump, a control controller, an isolating transformer and a power supply unit, characterized in that the vacuum pump and the control controller are integrated inside a single housing, wherein the control controller is configured to simultaneously control the modes of intracorporeal cutting of biological tissues by cutting attachments of the handle of the shaver-morcellator and the vacuum pump.

[0012] In particular, the case is a shock-resistant rectangular case, the elements of which are made of D16T aviation aluminum, steel with a protective coating and organic glass.

[0013] In particular, the vacuum pump is made in a plastic casing and installed inside the casing, ensuring a gap to its internal walls of at least 15 mm.

[0014] In particular, the control unit contains a display configured to display the speed of intracorporeal cutting of biological tissues by the cutting attachments of the shaver-morcellator handle, the connection status of the shaver-morcellator handle, pedals, and the achievement of the negative pressure in the system required for operation.

[0015] In particular, it contains at least one handle-button, configured with the possibility of turning on the control unit of the shaver-morcellator with integrated aspiration 1 when it is pressed for the first time and turning off the control unit of the shaver-morcellator with integrated aspiration 1 when such handle-button 11 is pressed again, as well as with the possibility of turning clockwise or counterclockwise and regulating the speed of intracorporeal cutting of biological tissues by cutting attachments of the connected handle of the shaver-morcellator when such handle-button 11 is turned clockwise or counterclockwise.

[0016] In particular, the control controller 9 contains presets for the operating modes of the vacuum pump 5 depending on the selected speed of intracorporeal cutting of biological tissues by the cutting attachments of the connected shaver-morcellator handle.

[0017] In particular, it further comprises a stainless steel pressure sensor and a polymer material muffler, connected to a vacuum pump through an internal piping system.

[0018] In particular, the internal piping system is made of chemically resistant silicone with a wall thickness of 2 to 4 mm and is located with a bending radius of at least 20 mm.

[0019] In particular, a power supply unit connected to the control controller is additionally placed inside the case.

[0022] Fig. 2 - Internal layout of elements in the control unit of the shaver-morcellator with integrated aspiration.

[0023] Fig. 3 - Stand for installing the control unit of the shaver-morcellator with integrated aspiration.

[0024] The drawings indicate:

[0025] 1 - shaver-morcellator control unit with integrated aspiration, 2 - housing, 3 - isolation transformer, 4 - power supply, 5 - vacuum pump, 6 - pressure sensor, 7 - muffler, 8 - electromagnetic valve, 9 - control controller, 10 - shaver-morcellator handle connection interface, 11 - button handle.

[0026] Technical experts know that a typical shaver-morcellator consists of two key components: the shaver-morcellator handle and the cutting attachments. The shaver-morcellator handle is designed to be connected to the shaver-morcellator control unit and contains the components and elements responsible for driving the moving part of the cutting attachment. A typical cutting attachment consists of an internal hollow moving part and an external fixed part (blades). During operation of a typical shaver-morcellator, biological tissue is drawn into the cavity of the moving part of the attachment through aspiration. Rotation of the moving part of the attachment removes the tissue, which is then removed from the patient through aspiration. The handle of the shaver-morcellator and the cutting attachment, including the movable and fixed parts of the cutting attachment, are not included in the control unit of the shaver-morcellator with integrated aspiration 1.

[0027] The control unit of the shaver-morcellator with integrated aspiration 1 is made consisting of a housing 2. Housing 2 is shock-resistant, rectangular, with overall dimensions ranging from 300 × 200 × 150 mm to 400 × 300 × 200 mm, while the elements of housing 2 are made of D16T aircraft aluminum, steel with a protective coating, and organic glass, and the housing 2 itself is located outside the device as a supporting shell. Isolating transformer 3 is rigidly installed inside housing 2, connected to cable assemblies via terminal connections and is designed to provide galvanic isolation of the working circuits from the power supply network. Power supply 4 is made of a rectangular shape, fixed inside housing 2 and connected to control controller 9 with a specified dimensional tolerance of placement.Vacuum pump 5 is housed in a plastic housing and mounted within housing 2 with a minimum clearance of 15 mm to the inner walls to prevent vibration transmission. It is connected to the internal piping system (not shown in the drawings) to ensure a tight, airtight fit. Pressure sensor 6 is made of stainless steel and is connected to the internal piping system (not shown in the drawings) on one side of vacuum pump 5 and positioned perpendicular to the air flow vector. Silencer 7 is cylindrical and made of polymer material and is connected to the outlet of vacuum pump 4 through the internal piping system (not shown in the drawings). The electromagnetic valve 8 is made of an electrical alloy and a copper winding, mounted on the housing 2, and is connected to the pipeline system (not shown in the drawings), interfaced with cable assemblies (not shown in the drawings) via a plug connector with a mechanical lock and is configured to turn on or off the vacuum in the aspiration system. The control controller 9, which includes a screen (not shown in the drawings), a control board (not shown in the drawings), a control panel (not shown in the drawings), an interface for connecting the handle of the shaver-morcellator 10 and at least one handle-button 11, is configured to simultaneously control the modes of intracorporeal cutting of biological tissues of the cutting attachments, through the interface for connecting the handle of the shaver-morcellator 10 and the vacuum pump 5, is rigidly fixed on one of the walls of the housing 2, in such a way that at least a part of the screen (not shown in the drawings) is brought out of the housing 2.Moreover, at least one knob-button 11 is configured to turn on the control unit of the shaver-morcellator with integrated aspiration 1 upon its initial pressing and to turn off the control unit of the shaver-morcellator with integrated aspiration 1 upon a second pressing of such knob-button 11, as well as to be capable of being rotated clockwise or counterclockwise and to regulate the speed of intracorporeal cutting of biological tissues by turning such knob-button 11 clockwise or counterclockwise. In a particular embodiment, the control controller 9 may contain preset operating modes of the vacuum pump 5 depending on the selected cutting speed of biological tissues by the cutting attachments. The internal piping system (not shown in the drawings) is made of chemically resistant silicone with an increased wall thickness of 2 to 4 mm. It is located between the pneumatic components within Housing 1 and is designed with a bending radius of at least 20 mm to prevent kinks and eliminate the possibility of collapse. Cable assemblies (not shown in the drawings) are made of multi-strand copper wires in PVC insulation with a cross-section of 0.2 to 1.5 square millimeters. They are located along the internal walls of Housing 1, secured with polymer cable ties, and connected to the power connector and power switch.

[0028] The interface for connecting the handle of the shaver-morcellator 10 is a set of means, rules and methods (devices, buttons, menus, commands, communication protocols, etc.), known to specialists in this field of technology, that ensure communication and exchange of information between the control controller 9 and the handle of the shaver-morcellator 10 connected to the control unit of the shaver-morcellator with integrated aspiration 1. In a particular case of implementation, such an interface for connecting the handle of the shaver-morcellator 10, among other things, may contain connectors for connecting the pedals for controlling the handle of the shaver-morcellator 10, a fitting for a vacuum tube, a connector for connecting the cable of the handle of the shaver-morcellator 10, software for interaction with the handle of the shaver-morcellator 10, including the communication protocols necessary for the correct operation of the handle of the shaver-morcellator 10.

[0029] The technical result is achieved through the comprehensive integration of components and a well-thought-out design that ensures the synchronization of the key elements of the system.

[0030] The design of the housing 2 as impact-resistant, rectangular, using D16T aviation aluminum, steel with a protective coating and organic glass allows for the optimal distribution of the nodes of the control unit of the shaver-morcellator with integrated aspiration 1 inside this housing (including the pipeline system), while ensuring their reliable fastening inside the housing, ensuring the removal of excess heat required for the operation of the device, reducing the weight of the product, ensuring its use in an operating room, significantly reducing the likelihood of damage or failure of the control unit of the shaver-morcellator with integrated aspiration 1 due to various possible mechanical impacts, including vibrations of the vacuum pump 5, and reducing the noise from the product as a whole.

[0031] The presence of an interface for connecting the shaver-morcellator handle 10 in the control unit of the shaver-morcellator with integrated aspiration 1 allows the control controller 9 to receive data on the state of the connected shaver-morcellator handle 10 and transmit control signals to it, ensuring the precise execution of the operator's commands.

[0032] The control controller 9 includes a screen, a control board, a control panel, a connection interface for the shaver-morcellator handle 10, and at least one handle-button 11. It is rigidly fixed inside the housing 2, wherein a part of the screen and the handle-buttons 11 are located outside the housing 2, and the connection interface for the shaver-morcellator handle 10 is accessible through the walls of the housing 2, for example, by means of the presence of corresponding openings (not shown in the drawings) in such walls of the housing 2. This allows the operator to conveniently control the device and simultaneously monitor the operating parameters. In this case, the control controller 9, taking into account the angular position of the handles-buttons 11, coordinates the operation of the surgical instrument (shaver-morcellator) and the vacuum pump 5 through the connection interface for the shaver-morcellator handle 10, ensuring synchronous execution of the operations of intracorporeal cutting of biological tissues and aspiration. This allows the surgeon to simultaneously control the cutting modes of biological tissue and the creation of a vacuum for the removal of excised tissue fragments. The presence of a screen and control panel (not shown in the drawings) in the control controller 9 provides visual and tactile feedback to the surgeon, simplifying parameter adjustments (e.g., the speed of intracorporeal cutting of biological tissue). In certain implementations, the screen included in the control controller 9 may not be equipped with touch functions, which reduces the overall cost of the shaver-morcellator control unit with integrated aspiration 1.The number of knobs and buttons 11 included in the control controller 9 may vary depending on the model of the control unit of the shaver-morcellator with integrated aspiration 1, for example, taking into account how the interface for interaction with the operator is implemented in a particular model, whether the screen is touch-sensitive, whether the control controller 9 contains presets for the operating modes of the vacuum pump 5 depending on the selected speed of intracorporeal cutting of biological tissue, etc. At the same time, it is obvious to a person skilled in the art that in some models of the control unit of the shaver-morcellator with integrated aspiration 1, the functionality provided by the knob-button can be implemented by other technical methods known from the prior art, for example, separate buttons or rotary knobs.

[0033] The presence in the control unit of the shaver-morcellator with integrated aspiration 1 of at least one knob-button 11, configured to turn on the control unit of the shaver-morcellator with integrated aspiration 1 when it is pressed for the first time and to turn off the control unit of the shaver-morcellator with integrated aspiration 1 when such knob-button 11 is pressed again, as well as with the ability to rotate clockwise or counterclockwise and regulate the speed of intracorporeal cutting of biological tissues when such knob-button 11 is turned clockwise or counterclockwise, makes it possible to reduce the size of the control unit of the shaver-morcellator with integrated aspiration 1 as a whole, eliminating, for example, the need to use separate buttons for turning the device on, to facilitate the control of the speed of intracorporeal cutting of biological tissues, and also, for some models of the control unit of the shaver-morcellator with integrated aspiration 1,in which the control controller 9 contains preset operating modes of the vacuum pump 5 depending on the selected speed of intracorporeal cutting of biological tissues, makes it possible to ensure the possibility of simultaneous coordinated control of the modes of intracorporeal cutting of biological tissues of the surgical instrument and the vacuum pump by the operator during the surgical operation.

[0034] The implementation of the vacuum pump 5 in a plastic housing allows for a general lightening of the control unit of the shaver-morcellator with integrated aspiration 1, which has a positive effect on the ergonomics of the device as a whole, since less effort is required for the possible movement of the control unit of the shaver-morcellator with integrated aspiration 1, such as between hospital rooms in preparation for operations, and directly during surgical intervention, if necessary.

[0035] The presence of a gap between the vacuum pump 5 and the walls of the housing 2 reduces the vibration impact on other components of the device, which is important for the accuracy of the operator's work with a surgical instrument connected to the control unit of the shaver-morcellator with integrated aspiration 1.

[0036] Isolation transformer 3 ensures galvanic isolation of the operating circuits from the power supply. This is critical for patient and staff safety, especially in the operating room, as it prevents the risk of electric shock from accidental contact with metal parts of the device or the patient. It also ensures continuous and reliable power to the device components, which is essential for the synchronized operation of the controller 9, vacuum pump 5, and the surgical instrument connected via the shaver-morcellator handle interface 10.

[0037] The cable assemblies provide interference-free transmission of control signals from the control controller 9, through the shaver-morcellator handle connection interface 10, to the surgical instrument connected to the shaver-morcellator control unit with integrated suction 1, and to the vacuum pump 5. The arrangement of the cable assemblies along the housing walls simplifies maintenance and reduces the risk of cable damage during operation.

[0038] The pressure sensor 6 allows you to monitor the level of negative pressure in the system, which allows the controller 9 to adjust the operation of the vacuum pump 5 to maintain the set parameters.

[0039] The electromagnetic valve 8 provides precise vacuum control and maintains a vacuum in the system, due to which the vacuum pump 5 turns on less often, which increases the service life of the vacuum pump 5 and reduces noise in the operating room during surgery.

[0040] The internal piping system is made of chemically resistant silicone with a wall thickness of 2 to 4 mm to prevent the possibility of piping collapse during operation due to a decrease in pressure. Using piping with a wall thickness of less than 2 mm leads to such collapse, which in turn prevents surgical procedures using a surgical instrument connected to the shaver-morcellator control unit with integrated aspiration 1, as aspiration stops, making it impossible to use the surgical instrument to complete the procedure.Manufacturing chemically resistant silicone pipelines with a wall thickness of 4 mm or more is impractical, as it increases the overall cost of the shaver-morcellator control unit with integrated aspiration 1, complicates the placement of the pipeline system within the housing (due to increased bending radii), and increases the overall weight and dimensions of the pipeline system. Placing chemically resistant silicone pipelines with a wall thickness of 2 to 4 mm within the housing 2 with bending radii of at least 20 mm prevents kinks in such pipelines, thereby ensuring the ability to flow the same volume of fluid through each section of such a pipeline per unit of time.

[0041] The key factors in achieving the technical result are that the vacuum pump 5 and the control controller 9 are integrated and securely fixed inside a single housing, while the control controller 9 is designed with the ability to simultaneously control the modes of intracorporeal cutting of biological tissues of the surgical instrument and the vacuum pump 5 - it processes data from sensors (for example, a pressure sensor 6, Hall sensors installed in a surgical instrument connected to the control unit of the shaver-morcellator with integrated aspiration 1, etc.) and personnel commands, generating control signals for the surgical instrument connected through the interface of the shaver-morcellator handle 10 and the vacuum pump 5, which allows for the simultaneous activation of the modes of intracorporeal cutting of biological tissues and aspiration, coordinating the cutting speed of biological tissues with the intensity of the vacuum for the effective removal of excised tissue fragments, and promptly adjusting the parameters during the operation.

[0042] Use the control unit of the shaver-morcellator with integrated aspiration 1 as follows.

[0043] Through the connection interface of the shaver-morcellator handle 10, a surgical instrument and containers for collecting excised biological tissues and waste are connected, the control unit of the shaver-morcellator with integrated aspiration 1 is connected to an external power source, and the control unit of the shaver-morcellator with integrated aspiration 1 is turned on by pressing the handle-button 11. The power supply 5 supplies voltage to the control controller 9 and the vacuum pump 5. After that, the current parameters (for example, the rotation speed of the cutting attachments of the connected surgical instrument, the achievement of the working vacuum level (data from the pressure sensor 6), etc.) are displayed on the screen of the control controller 9.After that, by turning the knobs-buttons 11 clockwise or counterclockwise, they regulate the operating speed of the cutting attachments of the connected surgical instrument and monitor the automatic change in the vacuum level indicators on the screen of the control controller 9, wherein the control controller 9 generates control signals: for the connected surgical instrument - it starts the motor rotating the movable part of the cutting attachment at a given speed; for the vacuum pump 5 - it activates its operation; for the electromagnetic valve 8 - it opens it to create a vacuum. During the operation of the control unit of the shaver-morcellator with integrated aspiration 1, the pressure sensor 6 transmits data on the current vacuum level to the control controller 9. In case of deviation from the set value (e.g. ±0.5 bar), the control controller 9 corrects the operation of the vacuum pump 5 and the electromagnetic valve 8 to stabilize the parameters.Silencer 7 continuously reduces the noise level of vacuum pump 5 (e.g., to 45 dB or less). As needed during the operation, the parameters of intracorporeal tissue cutting and aspiration are adjusted via controller 9. Controller 9 automatically synchronizes the operation of the connected surgical instrument and vacuum pump 5, ensuring the synchronous start and end of intracorporeal tissue cutting and aspiration cycles, maintaining a preset ratio of cutting speed and vacuum intensity, and emergency shutdown in the event of critical parameter deviations (e.g., when the biological waste collection container overflows).

[0044] To confirm the technical result achieved, a prototype shaver-morcellator control unit with integrated aspiration 1 was assembled. The prototype's housing 2 was impact-resistant, rectangular, and measured 330 x 300 x 110 mm. Housing 2 contained a lid, base, and corner posts made of D16T aircraft aluminum, side and rear panels made of steel with a protective coating, and a front panel made of organic glass. An isolating transformer 3 with a transformation ratio of 1:1 and a power of 150 W was installed in Housing 2, providing galvanic isolation of the operating circuits from the 220 V, 50 Hz power supply. Also installed in case 2 was a rectangular power supply unit 4 (120×80×50 mm), which converted the input voltage into stabilized output voltages of 5 V and 24 V to power the control controller 9 and the vacuum pump 5.A vacuum pump 5 in a plastic housing with a capacity of 15 l / min, creating a vacuum of up to 0.20 bar, was also installed in housing 2. The vacuum pump 5 was installed inside housing 2 with a gap of 15 mm to the inner walls. A stainless steel pressure sensor 6 with the following characteristics was also installed in housing 2: measurement range of 0-1 bar, accuracy of ±0.01 bar. Pressure sensor 6 was connected to the internal piping system on one side of vacuum pump 5 and was located perpendicular to the air flow vector. A cylindrical muffler 7 (diameter 40 mm, length 100 mm, material - polymer sound-absorbing composite) was also installed in housing 2. It was connected to the outlet of vacuum pump 5 through the internal piping system. Also installed in housing 2 was a solenoid valve 8 made of an electrical alloy and copper winding with a 50 ms response time, capable of turning the vacuum in the aspiration system on or off. It was mounted on the internal piping system and connected to the cable assemblies via a mechanically latched plug connector. The prototype also contained a controller 9, installed in housing 2, with a 5" non-touch screen, a control board based on an STM32F4 microcontroller, a control panel, a knob / button 11, and an interface for connecting the shaver-morcellator handle, which was secured to the front wall of housing 2 with the screen extending beyond the housing.Furthermore, the screen of the control controller 9 was configured to display the speed of intracorporeal cutting of biological tissue by the cutting attachments of the connected shaver-morcellator handle, the connection status of the shaver-morcellator handle, the pedals, and the achievement of the required negative pressure in the system. The knob-button 11 was designed such that the initial press of it (provided it was connected to a power source) turned the device on, and a second press turned it off. Furthermore, turning the knob-button 11 of the switched-on control unit of the shaver-morcellator with integrated aspiration 1 clockwise increased the speed of intracorporeal cutting of biological tissue, while turning the knob-button 1 counterclockwise decreased the speed of intracorporeal cutting of biological tissue.Controller 9 contained preset operating modes for vacuum pump 5 depending on the selected speed of intracorporeal cutting of biological tissue. As a result, turning knob-button 11 clockwise or counterclockwise, in addition to changing the speed of intracorporeal cutting of biological tissue, led to a simultaneous synchronous automatic change in the aspiration mode. The connection interface for the shaver-morcellator handle 10 consisted of a standard cable connector with protection against pulling out, a nipple for the vacuum tube, and a connector for connecting the control pedals. The internal piping system of the prototype was made of chemically resistant silicone (wall thickness 2.5 mm), with a bending radius of at least 20 mm. Cable assemblies were made of multi-strand copper wires in PVC insulation (cross-section from 0.5 mm² to 1.5 mm). 2), which were secured with polymer ties along the inner walls of the housing. All components of the shaver-morcellator control unit with integrated aspiration 1, located inside housing 2, were securely attached to the bottom and walls of housing 2, for example, using bolts, glue, cable ties, etc.

[0045] During the test operation of the prototype of the shaver-morcellator control unit with integrated aspiration 1, the achievement of the declared technical result was confirmed - providing the ability to simultaneously control the modes of intracorporeal cutting of biological tissues of the surgical instrument and the vacuum pump, and the following additional effects were obtained: reduction of the operation time by 15-20% due to the elimination of the need to switch between the modes of intracorporeal cutting of biological tissues; increased accuracy of intracorporeal cutting of biological tissues due to stable aspiration of tissue fragments; reduced risk of complications due to contamination of the surgical field; ease of control - all settings are available on a single interface (screen and control controller panel 9); safety is ensured - galvanic isolation (isolation transformer 3) eliminates the risk of electric shock.
