Abstract
Description
[0001] The utility model relates to the field of medical technology, namely to devices for artificial lung ventilation devices, in particular devices for liquid artificial lung ventilation devices.
[0002] Respiratory fluid regeneration is one of the key processes that must be ensured during the operation of any closed-loop liquid ventilator. The primary objective of this process is to remove carbon dioxide dissolved in the respiratory fluid during gas exchange in the lungs and to saturate the respiratory fluid with oxygen.
[0003] A device for complete liquid ventilation is known (patent publication US 20020153010, IPC A61M 16 / 00, 24.10.2002 - prototype), allowing for fluid regeneration during liquid artificial ventilation of the lungs and comprising a container for performing gas exchange of the respiratory fluid; a connector for connecting an oxygen source; a connector for connecting a vacuum source; a connector for supplying respiratory fluid for regeneration, connected to the container for performing gas exchange of the respiratory fluid through a first automatic valve below the interface between the gas and liquid phases; a connector for extracting regenerated respiratory fluid, connected to the container for performing gas exchange of the respiratory fluid through a second automatic valve; a control unit for controlling the operation of the automatic valves, connected to the control inputs of the automatic valves. In this device, carbon dioxide removal and oxygenation occur simultaneously by bubbling oxygen into the breathing fluid in a gas exchange chamber. A disadvantage of this device is that effective carbon dioxide removal requires a high oxygen flow rate and a large oxygenator volume, which in turn requires the use of a large amount of breathing fluid. Furthermore, this device cannot be used to prevent decompression sickness, for example, using the method described in patent RU 2738015, IPC A61M 16 / 00, December 7, 2020, since operating under hyperbaric conditions will result in supersaturation of the fluid with oxygen. To ensure carbon dioxide removal, the device must be supplied with an excess of oxygen, which is unacceptable in hyperbaric conditions, as it will cause oxygen toxicity.
[0004] The objective of this utility model is to create a fluid regeneration device for a liquid artificial lung ventilation apparatus that ensures the effective removal of carbon dioxide without a large consumption of oxygen and the use of a large amount of respiratory fluid, removes metabolically indifferent gases from the respiratory fluid and dosedly saturates the respiratory fluid with oxygen.
[0005] The technical result of the proposed utility model is a reduction in oxygen consumption and the volume of respiratory fluid during the procedure of liquid artificial ventilation of the lungs.
[0006] The solution to the problem is achieved in that a device for regenerating fluid for a liquid artificial lung ventilation apparatus, comprising a container for carrying out gas exchange of respiratory fluid; a connector for connecting an oxygen source; a connector for connecting a vacuum source; a connector for supplying respiratory fluid for regeneration, connected to the container for carrying out gas exchange of respiratory fluid through a first automatic valve; a connector for extracting regenerated respiratory fluid, connected to the container for carrying out gas exchange of respiratory fluid through a second automatic valve below the interface between the gas and liquid phases;a control unit for controlling the operation of automatic valves connected to the control inputs of the automatic valves, a third automatic valve, through which a connector for connecting an oxygen source is connected to a container for implementing gas exchange of the respiratory fluid to stop the oxygen supply during the removal of dissolved gases from the respiratory fluid by vacuuming; a fourth automatic valve, through which a connector for connecting a vacuum source is connected to a container for implementing gas exchange of the respiratory fluid to stop the vacuuming during the saturation of the respiratory fluid with oxygen; a connector for connecting a compressed air source connected through a fifth automatic valve to a container for implementing gas exchange of the respiratory fluid above the interface between the gas and liquid phases to control the pressure during the extraction of regenerated respiratory fluid;a connector for supplying breathing fluid for regeneration, connected to a container for carrying out gas exchange of the breathing fluid through the first automatic valve below the interface between the gas and liquid phases to minimize the loss of breathing fluid due to evaporation when passing the breathing fluid through the gas phase.
[0007] The proposed utility model is explained by a drawing. Fig. 1 shows a basic diagram of the device. The device comprises hydropneumatic pipelines for transporting gaseous and liquid media, indicated by solid lines, solid arrows indicate the flow directions, switching lines indicated by dashed lines, a container for implementing gas exchange of respiratory fluid 1, partially filled with respiratory fluid, which is separated from the gas phase by the interface between the gas and liquid phases 2, a connector for supplying respiratory fluid for regeneration 3, a first automatic valve 4, a connector for extracting regenerated respiratory fluid 5, a second automatic valve 6, a connector for connecting an oxygen source 7, a third automatic valve 8, a connector for connecting a vacuum source 9, a fourth automatic valve 10, a connector for connecting a compressed air source 11, a fifth automatic valve 12, a control unit 13.The device works as follows.
[0008] The device operating mode is cyclic, one cycle consists of 4 phases.
[0009] The first phase is "Filling with breathing fluid." The control unit 13, using control signals, generates the following valve state configuration: the first automatic valve 4 and the fourth automatic valve 10 are in the "open" state, while the second automatic valve 6, the third automatic valve 8, and the fifth automatic valve 12 are in the "closed" state. The breathing fluid enters the respiratory fluid regeneration fluid supply port 3 and the first automatic valve 4 into the respiratory fluid gas exchange tank 1 due to the vacuum created by pumping the gas phase out of the respiratory fluid gas exchange tank 1 through the fourth automatic valve 10 and the vacuum source connection port 9.
[0010] The second phase "Removal of dissolved gases from the breathing fluid". The control unit 13, using control signals, forms the following configuration of the valve states: the fourth automatic valve 10 is in the "open" state, and the first automatic valve 4, the second automatic valve 6, the third automatic valve 8 and the fifth automatic valve 12 are in the "closed" state. The flow of respiratory fluid through the respiratory fluid supply connector into the respiratory fluid gas exchange tank 1 is stopped, and the gas phase continues to be pumped out of the respiratory fluid gas exchange tank 1 through the fourth automatic valve 10 and the vacuum source connector 9, which leads to the formation of a vacuum in the respiratory fluid gas exchange tank 1. Under the action of the vacuum, the respiratory fluid boils and all dissolved gases are removed from it.
[0011] The third phase is "Oxygenation of the respiratory fluid." Control unit 13 uses control signals to generate the following valve configuration: the third automatic valve 8 is "open," while the first automatic valve 4, the second automatic valve 6, the fourth automatic valve 10, and the fifth automatic valve 12 are "closed." Oxygen flows from the oxygen source connector 7 through the third automatic valve 8 into the respiratory fluid gas exchange tank 1, saturating the respiratory fluid. The amount of dissolved oxygen is regulated by controlling the duration of the third phase.
[0012] The fourth phase "Extraction of regenerated breathing fluid". The control unit 13, using control signals, generates the following valve state configuration: the second automatic valve 6 and the fifth automatic valve 12 are in the "open" state, and the first automatic valve 4, the third automatic valve 8, and the fourth automatic valve 10 are in the "closed" state. Breathing fluid is extracted from the respiratory fluid gas exchange tank 1 through the second automatic valve 6 and the regenerated breathing fluid extraction connector 5 due to the excess air pressure supplied to the respiratory fluid gas exchange tank 1 through the fifth automatic valve 12 and the connector for connecting the compressed air source 11.
[0013] Thus, the fluid regeneration device for a liquid ventilator effectively removes carbon dioxide and metabolically inert gases from the respiratory fluid through the use of vacuum. This eliminates the need to use large volumes of respiratory fluid, as the vacuum efficiency is independent of the volume of respiratory fluid. During the removal of carbon dioxide and metabolically inert gases from the respiratory fluid, the device does not consume oxygen, nor does it consume oxygen during the filling and withdrawal of respiratory fluid. Oxygen consumption occurs only when the respiratory fluid, from which all dissolved gases have been removed, is saturated, significantly reducing overall oxygen consumption throughout the entire cycle.Since the vacuum process removes all dissolved gases, including metabolically indifferent ones, from the respiratory fluid, and oxygen saturation of the respiratory fluid occurs in a measured manner, the device can be used in closed-loop liquid artificial ventilation devices for the prevention of decompression sickness by removing metabolically indifferent gases from the body while under excess pressure (in hyperbaric conditions).
[0014] Implementation example. As a container for carrying out gas exchange of respiratory fluid 1, it is possible to use a clamp-clamp type diopter, stainless steel, DN50, TL050PDCL manufactured by TITAN LOCK (Russia), as the first automatic valve 4, the second automatic valve 6, the third automatic valve 8, the fourth automatic valve 10, and the fifth automatic valve 12, it is possible to use SM55624 valves manufactured by Smart (China), as a connector for supplying respiratory fluid for regeneration 3, a connector for extracting regenerated respiratory fluid 5, a connector for connecting an oxygen source 7, a connector for connecting a vacuum source 9, for connecting a compressed air source 11, it is possible to use a stainless steel detachable connection (AISI304) 1 / 2", BP / BP TL050UFF manufactured by TITAN LOCK (Russia), as a control unit 13, it is possible to use a programmable logic controller PLC-160 manufactured by OVEN (Russia).
[0015] The liquid regeneration device for the liquid artificial lung ventilation apparatus can be widely used in medical practice in the treatment of severe bronchopulmonary pathology, neuroprotection, as well as in the medical support of deep-sea diving.
