Abstract
Description
Technical Field
[0001] This invention belongs to the field of medical assistive devices, specifically a bronchial tube closure device. It utilizes integrated innovation to solve the above-mentioned clinical problems by incorporating a balloon imaging monitoring module and a bendable design to expand its applicability. This allows for flexible adjustment of the angle to the required position during intubation and real-time monitoring of the balloon closure status, significantly reducing the risk of surgical interruption. Prior Technology
[0002] Note that a endotracheal tube is a tube inserted into the trachea. Its main purpose is to establish and maintain a clear airway and ensure adequate exchange of oxygen and carbon dioxide, thereby ensuring sufficient oxygenation and ventilation for the patient. It is inserted through the mouth or nose, travels along the trachea to the bronchi, and then supplies air. However, in the past, during intubation, medical personnel had to ensure the endotracheal tube was accurately inserted into the designated position. Therefore, an imaging monitor was used to observe and ensure the correct insertion position and depth. However, patient movement or violent shaking during transport could cause the endotracheal tube to be pulled out or even dislodged.
[0003] Therefore, the so-called endobronchial blocker has appeared on the market. It is similar to the endobronchial tube in terms of its setting method. However, because the endobronchial blocker has an inflatable cuff at the front end for fixing to the bronchial wall, it can only be placed inside the endobronchial tube to reach the set position in the bronchus. After reaching the set position, it is inflated to fix it to the inner wall of the bronchus. However, during thoracic surgery, the endobronchial blocker cuff often shifts due to surgical pulling, causing the interruption of the single-lung breathing state and requiring readjustment of the blocker position. Therefore, it is still necessary to use it in conjunction with an imaging bronchoscope. The endobronchial blocker has a bend at the front end as a guide for insertion, allowing medical personnel to rotate and change the direction. However, the existing endobronchial blocker has two major drawbacks: (1) It does not have an imaging bronchoscope function. If it is necessary to confirm the insertion position or reposition it during surgery, a small bronchoscope must be placed for confirmation. However, the narrow space inside the trachea often causes inconvenience in operation. According to clinical statistics, more than 90% of physicians believe that this problem exists; (2) The existing products are made of plastic and cannot be rotated flexibly. Readjustment during the operation often causes the operation to be interrupted. Therefore, the medical community urgently needs to improve these defects. Summary of the Invention
[0004] One objective of this invention is to provide an endobronchial tube closure device that utilizes an operating box, an extension tube, a closure cuff, an inflation module, and a bend module. The operating box, in conjunction with the extension tube and the bend module, controls the movement and orientation of the closure cuff during placement. Furthermore, the cuff imaging monitoring module provides internal illumination and image capture to confirm the correct positioning of the extension tube and the closure cuff within the bronchus. The inflation module supplies air to the closure cuff, causing it to expand and lock securely within the bronchus. During the surgical procedure, the cuff imaging monitoring module continuously monitors the closure status of the closure cuff, preventing problems and minimizing surgical interruptions, significantly improving the accuracy, stability, and safety of closure device placement.
[0005] To achieve the above objectives, the present invention provides an endobronchial tube closure device for insertion into the bronchus for medical procedures. It comprises: a control box, which internally forms a first receiving space and a second receiving space, such that a communication hole is formed between the first receiving space and the second receiving space; the first receiving space forms an operating hole relative to the top of the control box, and a connection hole relative to the bottom of the control box; An extension tube is connected to the second accommodating space, and the extension tube has a first channel and at least one second channel inside, and the extension tube has a first end and a second end, and a mounting part is provided on the outer surface of the extension tube adjacent to the first end; a locking airbag is provided on the surface of the first end, so that the locking airbag is connected to the first channel, and the locking airbag remains in a contracted state during movement; an inflation module has an inflation tube connected to the first channel of the second end, for supplying gas to the locking airbag to inflate its volume; and a bending module includes a pulling means and a control handle, and the pulling means is connected from the inside of the extension tube to the first end and is linked with the control handle, so that the extension tube can be bent or straightened by operating the control handle. In specific implementation, the elbow module 15 includes two pull ropes 1511, a control handle 152, and a turntable 1512. Each pull rope 1511 is respectively inserted into each second channel 122, with one end fixed to the first end 123 and the other end passing through the connecting hole 113 and connected to... Within the two mounting holes 15121 of the turntable 1512, the control handle 152 is linked to the turntable 1512. By operating the control handle 152, the two pull ropes 1511 can be extended and retracted respectively, so that the extension tube 12 can achieve the effect of bending or straightening. Furthermore, the two mounting holes 15121 are located at two opposite corners of the turntable 1512, and the two opposite corners are 180 degrees apart, and each opposite corner is 90 degrees from the entry point of the pull rope 1511. In addition, the operating hole 114 is linearly opened, and the control handle 152 swings along the extension direction of the operating hole 114.
[0017] The airbag image monitoring module 16 includes an illumination element 161, an image capturing element 162, and a control signal element 163. The illumination element 161 and the image capturing element 162 are electrically connected to the control signal element 163. The illumination element 161 and the image capturing element 162 are spaced apart on the mounting portion 125. The control signal element 163 is located inside the operation box 11, corresponding to the connection hole 115. The airbag image monitoring module 16 has two illumination elements 161 spaced apart, both of which are LED lighting sources. The image capturing element 162 is a complementary metal-oxide-semiconductor (CMOS) chip. For convenient connection and monitoring, the bronchial tube closure device 1 further includes a signal interface 164 electrically connected to the control signal element 163. The signal interface 164 is located on the connection hole 115 for connecting to a monitoring screen 165.
[0018] To enhance safety and convenience during operation, the endobronchial tube closure device 1 of this invention further includes an alarm module 17. The alarm module 17 is located on the surface of the operating box 11 and electrically connected to the image capturing element 162. The alarm module 17 includes an image analysis element 171, an image database 172, at least one light warning device 173, and a buzzer 174. After the image analysis element 171 analyzes the image obtained by the image capturing element 162 and compares it with the outline circumference of the airbag 13 in the image database 172, an abnormality occurs when the exposed circumference of the airbag 13 is greater than one-sixth to one-twelfth, and an alarm is issued through the light warning device 173 and the buzzer 174.
[0019] In addition, this invention provides an air supply tube 18 for the convenience of surgery. It passes through the first channel 121 inside the extension tube 12 along the second accommodating space 112. One end of the air supply tube 18 is located at the first end 123 to form an air supply port 181. The air supply port 181 is separated from the locking airbag 13 so as to supply air over the locking fixation point. Figures 9-11 are schematic diagrams of the monitoring of the closure state of this invention. Figure 9 shows the fully closed state. When the image analysis element 171 analyzes and compares the tissue on the surface of the bronchus 2 and finds that it is tightly bound by the closure airbag 13, the circumference of the exposed outline of the airbag 13 is less than 1 / 24 to 0, indicating that it is blocked by the tissue on the surface of the bronchus 2, making the outline of the airbag 13 almost invisible. Therefore, the outline of the airbag 13 in Figure 9 is represented by a dashed line, so the alarm module 17 will not issue any warning notification. Figure 10 shows the unclosed state. When the image analysis element 171 analyzes and compares the tissue on the surface of the bronchus 2 and finds that the closure airbag 13 has slightly shifted, the airbag is exposed. If the circumference of the airbag 13 is greater than one-twelfth, the alarm module 17 will illuminate one of the indicator lights 173 and emit a low-frequency warning signal with the buzzer 174 to remind medical personnel to pay attention or make adjustments. Figure 11 shows a state of significant deflation or displacement. When the image analysis element 171 analyzes and compares the circumference of the airbag 13 and shows more than one-sixth, the bronchus 2 cannot be completely closed. At this time, the alarm module 17 will illuminate the two indicator lights 173 and emit a high-frequency warning signal with the buzzer 174 to notify medical personnel that the airbag 13 has shifted significantly, deflated, or that the inflation device is damaged, making surgery impossible.
[0020] Accordingly, the endobronchial tube closure device 1 of this invention utilizes the extension tube 12 combined with the closure cuff 13 and the cuff image monitoring module 16 for internal illumination and image acquisition during intubation. The cuff image monitoring module 16 is operated through the operation box 11, and the inflation module 14 provides air to the closure cuff 13 to expand its volume. The cuff image monitoring module 16 further confirms whether the movement and direction of the extension tube 12 are correct, and the elbow module 15 controls the correct position of the extension tube 12 entering the bronchus 2. After the closure cuff 13 is inflated by the inflation module 14, it forms a lock and fixation at the predetermined position of the bronchus 2. The locking status of the closure cuff 13 is monitored at any time to avoid problems and reduce surgical interruptions, thereby improving the accuracy, stability and safety of intubation surgery.
