Eyelid Bag and Tear Trough Surgical Assist System Eye Bag and Tear Trough Surgical Assistance System Technical Field
[0001] This work relates to a surgical assistance system, and more particularly to a surgical assistance system for under-eye bags and tear troughs. Prior Technology
[0002] Eye bags and tear troughs are extremely common and highly distressing cosmetic issues in modern medical aesthetics. Eye bags refer to the bulging of the orbital fat pads due to the gradual loosening of the orbital septum fascia with age, resulting in a noticeable bulge in the lower eyelids, making people appear tired, aged, and lacking energy. Tear troughs, on the other hand, refer to the sunken lines extending from the lower edge of the eye socket to the cheekbone, formed by bone structure depressions, insufficient subcutaneous fat volume, or excessive traction of the tear trough ligaments on the skin. When both occur together, they often create a height difference below the eye socket, with the bulge above and the depression below, causing shadow accumulation and making the person appear extremely old, seriously affecting their appearance, self-confidence, and quality of life. Therefore, simultaneous correction surgery for eye bags and tear troughs has become one of the most in-demand procedures in oculoplastic surgery and medical aesthetics. However, existing surgical methods have many fundamental technical defects, and to date, there is no ideal solution that can simultaneously ensure safety, aesthetics, and durability. Further research and improvement are urgently needed.
[0003] Currently, the mainstream surgical approach commonly used in the industry involves directly removing the fat from the eyelids. The surgeon enters the orbital fat chamber through either the lateral approach (outer eyelid skin) or the medial approach (inner eyelid conjunctiva), directly removing the bulging orbital fat to eliminate the appearance of lower eyelid bulge. However, while this type of excision can eliminate eyelid bulge in the short term, it does not improve tear trough depressions at all. In fact, excessive removal of orbital fat reduces the volume of the orbital supporting structures, further deepening existing tear trough depressions and worsening the overall facial appearance after surgery. Instead, it makes the eyes appear sunken and tired, resulting in an unsatisfactory aesthetic outcome. Furthermore, the surgical stimulation and postoperative scarring of the lower eyelid skin and orbicularis oculi muscle may accelerate sagging and drooping of the skin around the eyes, leading to poor long-term results. Another modified surgical method involves re-implanting the removed orbital fat into the tear trough area to address both eye bags and tear troughs simultaneously. However, because the fat is completely separated from the original vascular structure, the survival rate of this ex vivo fat transplantation method is greatly affected by multiple factors, including the blood supply to the recipient area, the amount of fat transplanted, and the patient's individual constitution, resulting in an unstable survival rate. Furthermore, the outcomes are often unpredictable and difficult to determine precisely. Clinically, uneven fat absorption, formation of localized lumps, unnatural contours, or recurrence of depressions are common postoperative complications. When the results are unsatisfactory, secondary surgery is frequently required, significantly increasing the patient's medical risks, psychological burden, and expenses. In addition, current surgical procedures lack auxiliary tools that can systematically provide objective and quantitative imaging data. Surgeons cannot obtain complete quantitative information on the volume of bulging eye bags, the depth of tear trough depressions, and the degree of skin laxity around the eyes before surgery. This results in a lack of sufficient data foundation for surgical planning, limiting the predictability of surgical results and impacting patient well-being. Patients' preoperative understanding of the expected surgical results is limited by the lack of visual presentation tools, resulting in poor doctor-patient communication efficiency. Furthermore, traditional anesthesia methods, such as general anesthesia or deep intravenous sedation, cause patients to lose their senses during surgery and are unable to immediately report any abnormal sensations to the surgeon, increasing uncontrollable factors during the surgical procedure. Therefore, developing an integrated eyelid bag and tear trough surgery assistance system that can integrate objective preoperative imaging data provision, intelligent surgical plan output, fat repositioning technology, and real-time intraoperative perception assurance is a core problem that urgently needs to be solved in this field. Summary of the Invention
[0004] The main purpose of this invention is to provide an auxiliary system for blepharoplasty (eyelid bag and tear trough surgery). It uses a camera module to capture high-resolution images of the patient's periorbital area and provides objective and quantitative image analysis data through the image analysis unit of an electronic device. This helps surgeons to fully grasp detailed information on the volume of blepharoplasty, the depth of tear trough depression, and the distribution of orbital fat, providing surgeons with a more complete data foundation for preoperative planning and effectively improving the accuracy, consistency, and repeatability of surgical plan formulation.
[0005] Another objective of this invention is to provide an auxiliary system for eyelid bag and tear trough surgery. The system uses an electronic device's skin assessment unit to quantitatively assess the degree of skin laxity around the patient's eyes. Based on the assessment results, the surgical suggestion unit outputs suggestions for external or internal incision approaches, providing the surgeon with data-supported surgical approach references. This makes the formulation of surgical plans more individualized and precise, and also helps to facilitate thorough communication and explanation with the patient before surgery.
[0006] Another objective of this invention is to provide an auxiliary system for eyelid bag and tear trough surgery. By administering local anesthesia to the patient through a local anesthesia module, the patient can remain conscious throughout the entire procedure. The patient can immediately report any discomfort, pressure, or abnormal sensations to the surgeon, who can then adjust the direction of the procedure and the depth of anesthesia accordingly. Compared to general anesthesia, this significantly improves the safety of the surgery, helps accelerate postoperative recovery, and shortens the patient's observation time.
[0007] Another objective of this invention is to provide an auxiliary system for eyelid bag and tear trough surgery. By using the ligament release device of the fat transfer instrument group to release the tear trough ligament and create sufficient fat placement space, the fat transfer instrument then flips and lays the bulging eyelid bag fat flat in the tear trough area while maintaining the connection between the eyelid bag fat and the original blood vessel tissue. This achieves the core technology of fat transfer preservation. Since the fat always maintains a living state with blood supply, there is no problem of unstable survival rate of ex vivo fat transplantation. The transferred fat can survive stably in the tear trough area, filling the depression with a long-lasting and natural effect, while eliminating the appearance of bulging eyelid bags, achieving the dual aesthetic benefits of simultaneous improvement of eyelid bags and tear troughs.
[0008] A further objective of this invention is to provide an auxiliary system for eyelid bag and tear trough surgery. By using a fat transfer instrument group with an eyelid tightening auxiliary instrument, the skin around the eyes is appropriately lifted and tightened while fat transfer is completed. This allows patients to simultaneously achieve triple aesthetic improvement in terms of eyelid bag elimination, tear trough filling, and skin tightening around the eyes. The overall aesthetics and patient satisfaction are significantly better than traditional single surgical methods.
[0009] The preferred implementation of this invention aims to provide an auxiliary system for blepharoplasty (eyelid bag and tear trough surgery). The system uses an electronic display unit to visually present image analysis results and surgical recommendations, allowing surgeons to have thorough preoperative communication with patients. This enables patients to have a clear understanding of the surgical recommendations and expected results. Furthermore, the system allows for postoperative imaging of the patient's periocular area to be objectively compared with preoperative images, providing a complete preoperative and postoperative record as an important basis for tracking surgical effectiveness.
[0010] To achieve the above objectives, this invention provides an auxiliary system for blepharoplasty (eyelid bag and tear trough surgery), comprising: a camera module for capturing images of a patient's periocular area; an electronic device connected to the camera module for analyzing the patient's eyelid bag and tear trough conditions based on the periocular images, and outputting a surgical suggestion plan based on the analysis results, the surgical suggestion plan including a suggested surgical approach and a suggested fat relocation; a local anesthesia module connected to the electronic device for administering local anesthesia to the patient according to the surgical suggestion plan, so that the patient can maintain their senses during the surgery; and a fat relocation instrument group connected to the electronic device for relocating and spreading the patient's eyelid bag fat to the patient's tear trough area according to the surgical suggestion plan, while preserving the eyelid bag fat without removing it.
[0011] Preferably, the camera module includes an image sensing unit and a light source unit. The image sensing unit is used to capture the periocular image, and the light source unit is electrically connected to the image sensing unit. The light source unit is used to provide auxiliary illumination to improve the clarity of the periocular image, so that the electronic device can accurately analyze the condition of the eye bags and the tear trough.
[0012] Preferably, the electronic device includes an image analysis unit, a skin assessment unit, a surgical suggestion unit, and a display unit. The image analysis unit is used to perform digital image processing on the periocular image to analyze the condition of the eye bags and the tear troughs. The surgical suggestion unit is connected to the image analysis unit and the skin assessment unit and is used to output a surgical suggestion plan based on the condition of the eye bags, the condition of the tear troughs, and the degree of skin laxity around the eyes. The display unit is connected to the surgical suggestion unit and is used to display the surgical suggestion plan.
[0013] Preferably, the surgical suggestion unit is further used to output an external or internal surgical approach suggestion based on the degree of skin laxity around the eyes, so as to provide the surgeon with data reference for the surgical approach.
[0014] Preferably, the surgical suggestion unit is further used to output the external approach suggestion when the degree of skin laxity around the eyes reaches a preset laxity threshold, and to output the internal approach suggestion when the preset laxity threshold is not reached.
[0015] Preferably, the local anesthesia module includes an anesthetic storage unit and an injection unit. The anesthetic storage unit is used to store a local anesthetic, and the injection unit is connected to the anesthetic storage unit. The injection unit is used to inject the local anesthetic into the target area of the patient's periocular anesthesia, so that the patient can perceive the operation and react in real time during the operation.
[0016] Preferably, the fat relocation device assembly includes a ligament release device, a fat relocation device, and a periorbital tightening auxiliary device. The ligament release device is used to release the patient's tear trough ligament to create a fat-accepting space. The fat relocation device is used to flip and flatten the eye bag fat towards the tear trough area while maintaining blood supply. The periorbital tightening auxiliary device is used to perform a lifting treatment on the patient's periorbital skin while relocating the eye bag fat.
[0017] Using the aforementioned technologies, this novel eyelid bag and tear trough surgery assistance system has at least the following effects: First, the camera module, combined with the image analysis function of the electronic device, provides the surgeon with complete quantitative data on the periorbital area, giving the surgical plan a more robust data foundation; Second, the skin assessment unit's approach suggestion output function provides the surgeon with data-driven surgical approach references, helping to improve the individualization of the surgical plan; Third, the real-time perception guarantee mechanism of the local anesthesia module significantly improves surgical safety while ensuring patient comfort; Fourth, the core technology of fat repositioning ensures the long-lasting and stable effect of fat repositioning, and simultaneously solves both eyelid bags and tear troughs, achieving a dual improvement effect that is difficult to achieve with traditional surgery; Fifth, the combined use of periorbital tightening auxiliary instruments further enhances the overall tightness of the periorbital area, resulting in a more comprehensive and outstanding overall aesthetic effect. [0019] Figure 2 is a schematic diagram showing the appearance of the periocular area before and after the application of the surgical assistance system for eye bags and tear troughs created in this invention. Implementation
[0020] To enable those skilled in the art to understand and implement the content of this invention, appropriate embodiments are described below with illustrations. Equivalent substitutions and modifications based on the content of this invention are all included within the scope of this invention. Furthermore, it is stated that the quantifiers "a" or "an" used throughout this invention are for the general meaning of expressing the scope of this invention and should be interpreted as including one or at least one, and a single concept also includes plural cases, unless otherwise clearly intended in this invention.
[0021] Please refer to Figure 1, which is a schematic diagram of the system architecture of the proposed eyelid and tear trough surgery assistance system. The proposed eyelid and tear trough surgery assistance system includes a camera module 10, an electronic device 20, a local anesthesia module 30, and a fat transfer instrument group 40. The camera module 10 is connected to the electronic device 20 to transmit the periocular images captured by the camera module 10 to the electronic device 20 in real time for subsequent analysis and processing. The local anesthesia module 30 and the fat transfer instrument group 40 are both connected to the electronic device 20 to perform corresponding surgical operations according to the surgical suggestions output by the electronic device 20. The proposed eyelid and tear trough surgery assistance system, through the integrated and coordinated operation of the above modules, constitutes a complete and systematic eyelid and tear trough surgery assistance process, which helps to improve the standardization of the surgical process.
[0022] The camera module 10 is used to capture images of a patient's periorbital area. These images include complete information about the patient's lower eyelid, under-eye bag bulge area, tear trough depression area, and periorbital skin. The camera module 10 includes an image sensing unit 101 and a light source unit 102. The image sensing unit 101 is used to capture these periorbital images. The image sensing unit 101 can be a high-resolution digital camera, a 3D structured light scanner, a stereoscopic camera, an infrared image sensor, a time-of-flight rangefinder, or a combination thereof, to perform multi-angle, multi-dimensional precision image capture of the patient's periorbital area. The high-resolution digital camera can capture two-dimensional planar images, providing detailed information on skin texture, pigment distribution, and contours. The 3D structured light scanner can generate accurate 3D point cloud data, providing stereoscopic measurement data of under-eye bag bulge volume and tear trough depression depth, thus improving the accuracy of subsequent image analysis. The light source unit 102 and the image sensor... The sensing unit 101 is electrically connected, and the light source unit 102 is used to provide auxiliary illumination to improve the clarity of the periocular image. The light source unit 102 can be a cool white light source, a ring light source, a polarized light source, a near-infrared light source, or a combination thereof. Among them, the polarized light source can effectively eliminate the specular reflection on the surface of the periocular skin and highlight the three-dimensional contour of the bulging eye bags. The ring light source can provide uniform, shadow-free frontal illumination to ensure the consistency of image color reproduction, so that the periocular image captured by the image sensing unit 101 has sufficient multi-layered detail information, so that the image analysis unit 201 of the electronic device 20 can provide accurate analysis data on the state of eye bags and tear troughs. In actual operation, the camera module 10 can be designed as a portable handheld device, a fixed shooting stand, or integrated into a head-mounted device to meet the usage needs of different clinic environments and ensure a standardized procedure for each image capture, so that pre-operative and post-operative images are comparable.
[0023] The electronic device 20 is connected to the camera module 10. The electronic device 20 is used to analyze the condition of one of the patient's eye bags and one of the tear troughs based on the periorbital image, and outputs a surgical suggestion plan based on the analysis results for the surgeon to refer to in order to formulate the most suitable surgical plan. The electronic device 20 can be a device with data processing capabilities such as a tablet computer, desktop computer, laptop computer, smartphone or dedicated medical workstation. The electronic device 20 includes an image analysis unit 201, a skin assessment unit 202, a surgical suggestion unit 203 and a display unit 204. The units are interconnected and work together to complete the complete process of preoperative data analysis and surgical suggestion output.
[0024] The image analysis unit 201 is used to perform digital image processing on the periocular images captured by the camera module 10. The image analysis unit 201 can use techniques such as image correction, contour extraction, three-dimensional reconstruction, depth measurement, and image segmentation to perform multi-dimensional analysis of the periocular images. Specifically, the image analysis unit 201 can use image segmentation technology to distinguish the boundary between the bulging area of the eye bags and the sunken area of the tear trough, use three-dimensional reconstruction technology to convert the two-dimensional image into a three-dimensional model, use depth measurement technology to quantitatively calculate the height of the bulging eye bags and the depth of the sunken tear trough, and use volume estimation algorithms to calculate the total volume of orbital fat and the individual volume of fat chambers in each region, so as to generate complete periocular quantitative data for the surgeon's reference. In addition, the image analysis unit 201 can also use a deep learning model to automatically identify and classify the periocular images, providing standardized quantitative scores, so that the surgeon can clearly understand the overall condition of the patient's periocular area, which serves as an important data reference for formulating a surgical plan.
[0025] The skin assessment unit 202 is used to assess the degree of skin laxity around the eyes of one of the patients based on the periocular images, and quantifies the assessment results into a skin laxity index. The skin assessment unit 202 can use image texture analysis technology to assess the density and depth of fine lines and wrinkles on the skin surface, use a skin elasticity model to assess the tension characteristics of the periocular skin, and use an aging degree scoring model to comprehensively calculate the skin aging index. After weighted integration of the above multiple assessment results, a quantified skin laxity index is output, which is used by the surgical suggestion unit 203 as the data basis for outputting surgical approach suggestions. The higher the value of the skin laxity index, the more obvious the degree of skin laxity around the eyes, and the more suitable it is to refer to the external opening approach for simultaneous skin lifting treatment.
[0026] The surgical suggestion unit 203 is connected to the image analysis unit 201 and the skin assessment unit 202. Based on the data on the state of eye bags and tear troughs provided by the image analysis unit 201 and the degree of skin laxity around the eyes output by the skin assessment unit 202, the surgical suggestion unit 203 outputs a surgical suggestion plan. This surgical suggestion plan includes a surgical approach suggestion and a fat relocation suggestion, for the surgeon to refer to when formulating a surgical plan. In a preferred embodiment, the surgical suggestion unit 203 is further used to output an external approach suggestion when the degree of skin laxity around the eyes output by the skin assessment unit 202 reaches a preset laxity threshold, and to output an internal approach suggestion when the degree of skin laxity around the eyes does not reach the preset laxity threshold. The external approach refers to the surgeon making an incision through the outer skin of the lower eyelid, suitable for cases with significant skin laxity around the eyes and the presence of excess skin. For patients with ptosis, the advantage is that the surgeon can operate under a direct surgical field and simultaneously remove excess loose skin to lift and tighten the lower eyelid skin. The medial approach refers to the surgeon making an incision inside the lower eyelid conjunctiva, which is suitable for patients with good skin elasticity and mild skin laxity around the eyes. Its advantage is that no surgical incision scars are left on the skin surface, the postoperative recovery is faster, and the appearance is more natural. The surgical suggestion unit 203 uses the quantified skin laxity index as the data basis for the approach suggestion, providing the surgeon with valuable information on the surgical approach, which helps the surgeon to formulate the most suitable surgical plan based on the individual patient's condition. In addition, the fat relocation suggestion is based on the data on the distribution of fat in the eye bags and the shape of the tear trough provided by the image analysis unit 201, providing specific suggestions on the amount of fat to be relocated, the direction of relocation, and the target coverage area, so that the surgeon has a clearer data reference basis when performing fat relocation.
[0027] Display unit 204 is connected to surgical suggestion unit 203. Display unit 204 is used to display the surgical suggestion plan for reference by the surgeon and patient. Display unit 204 can be a liquid crystal display, organic light-emitting diode display, touch screen, projection device or head-mounted display device, etc. The content presented by display unit 204 may include a three-dimensional stereoscopic visualization of the patient's periocular images, quantitative values of the severity of eye bags and the depth of tear trough depression, skin laxity index, surgical approach suggestions, estimated fat relocation amount and simulated preview of surgical results. Through the visualization of display unit 204, the surgeon can have a thorough preoperative communication with the patient. The patient can clearly understand the objective data status of their periocular problems and the data basis of the surgical suggestion plan, and have a more realistic understanding and reasonable expectations of the expected surgical results, which greatly improves the efficiency of doctor-patient communication. In addition, the periocular images captured after surgery can be compared side by side with the preoperative images through display unit 204, providing an objective and quantitative record of postoperative effectiveness tracking, which serves as an important basis for long-term follow-up.
[0028] The local anesthesia module 30 is connected to the electronic device 20. The local anesthesia module 30 is used to administer local anesthesia to the patient according to the surgical proposal output by the electronic device 20, so that the patient can maintain their senses during the surgery for immediate response. The local anesthesia module 30 includes an anesthetic storage unit 301 and an injection unit 302. The anesthetic storage unit 301 stores a local anesthetic, which can be lidocaine, bupivacaine, ropivacaine, or a mixture thereof with adrenaline, etc. Different local anesthetics have different onset times, durations of action, and vasoconstrictive effects. The surgeon can select an appropriate formulation based on the expected surgery duration and the patient's individual needs. The injection unit 302 is connected to the anesthetic storage unit 301 and is used to precisely inject the local anesthetic into the patient. The target area for anesthesia around the eyes includes the subcutaneous layer of the lower eyelid, the orbicularis oculi muscle layer, and the area surrounding the preorbital fat compartment. By applying the anesthesia in layers with precision, sufficient anesthesia can be ensured in the surgical area. The core advantage of this technique using local anesthesia is that the patient remains awake and has full sensory awareness throughout the entire procedure. The patient can report any pressure, pulling, or discomfort around the eyes to the surgeon at any time. The surgeon can obtain immediate subjective feedback from the patient and adjust the force and direction of the surgical procedure accordingly, or add more local anesthetic as needed. This real-time sensory feedback mechanism constitutes an important guarantee for the safety of the procedure. Compared with general anesthesia, it not only eliminates the risks of intubation and related postoperative side effects, but also significantly shortens the postoperative recovery time, allowing the patient to leave the hospital on their own shortly after the surgery, significantly reducing the overall surgical burden on the patient.
[0029] The fat relocation instrument assembly 40 is connected to the electronic device 20. The fat relocation instrument assembly 40 is used to relocate and flatten the patient's under-eye fat to the tear trough area according to the surgical suggestions output by the electronic device 20, while preserving the under-eye fat without removal. The fat relocation instrument assembly 40 includes an instrument holding bag 400, a ligament release instrument 401, a fat relocation instrument 402, and a periorbital tightening auxiliary instrument 403. The instrument holding bag 400 is used to hold the ligament release instrument 401, the fat relocation instrument 402, and the periorbital tightening auxiliary instrument 403. The instrument holding bag 400 has an unfolded state and a retracted state. In the unfolded state, the instrument holding bag 400 can be laid flat, and the ligament release instrument 401 and the fat relocation instrument 402... The periorbital tightening auxiliary instruments 403 are arranged sequentially in multiple instrument slots of the instrument storage bag 400, allowing the surgeon to easily identify and retrieve each instrument according to the surgical procedure. This effectively improves the efficiency of surgical preparation and reduces the risk of mixing instruments during surgery. In the closed state, the instrument storage bag 400 is secured by a strap to cover all instruments, facilitating carrying, storage, and sterilization. The instrument storage bag 400 can be made of high-temperature and high-pressure sterilization compatible fabric, polymer material, or non-woven fabric to meet the requirements of aseptic surgical procedures. Through the design of the instrument storage bag 400, the fat transfer instrument set 40 created in this invention can be supplied as a complete standardized surgical instrument set, which helps to standardize the preoperative preparation process and ensure consistency in instrument management.
[0030] The ligament release device 401 is used to release the tear trough ligament in this patient, creating a fat-accommodating space before the eye bag fat migrates to the tear trough area, allowing the eye bag fat to lay flat. The tear trough ligament is a fibrous ligament tissue that extends from the orbital periosteum, passes through the orbicularis oculi muscle, and attaches to the dermis of the skin. Its presence not only forms the structural depression appearance of the tear trough area but also constitutes a physical barrier to the downward migration of eye bag fat, preventing fat from spreading flatly to cover the tear trough area. Therefore, before performing eye bag fat migration, the tear trough ligament must first be released from its periosteal attachment point using the ligament release device 401. The ligament release device 401 can be a fine scissor-type release device. Using instruments such as electrocautery probes, ultrasonic scalpels, or laser fiber optic probes, the surgeon uses the ligament release instrument 401 to precisely release the ligaments along the tear trough ligament's direction. This eliminates the indentation caused by the ligament pulling on the skin and creates sufficient anatomical space for the subsequent rotation and spreading of the eye bag fat. This ensures that the repositioned fat can evenly cover the entire tear trough depression area, achieving the ideal filling effect. When performing ligament release, the surgeon should strictly control the release range to avoid over-releasing, which could lead to instability of the periorbital support structure. The design of the ligament release instrument 401 should provide the surgeon with precise force control to ensure that the ligament release operation is completed under the principle of minimally invasive surgery.
[0031] The fat relocation instrument 402 is used to relocate eyelid fat. After the ligament release instrument 401 releases the tear trough ligament and creates a fat placement space, the surgeon uses the fat relocation instrument 402 to flip and flatten the orbital fat pad downwards into the tear trough area while maintaining its connection with the original vascular pedicle and surrounding tissues, forming a natural, full, and evenly distributed contour. The fat relocation instrument 402 can be a fine tissue separator, fat flipping and fixing device, suture guide device, or miniature needle holder, selected according to the surgical approach and individual anatomical conditions. The core technical principle of fat relocation in this invention is that the eyelid fat remains connected to the original orbital blood vessels after flipping. The connection between the tissue and the fat cells ensures a continuous and sufficient blood supply. Fat cells migrate in a living state with adequate blood supply, resulting in a stable and predictable survival rate. This avoids the problems of fat absorption and lumps caused by insufficient blood supply after ex vivo fat transplantation. The transplanted fat can maintain its position in the tear trough filling area for a long time, providing a lasting and natural effect. Furthermore, since no fat tissue is removed, the overall fat volume of the orbit is preserved, avoiding the long-term situation of orbital hollowing caused by fat removal. After fat migration, the surgeon can use sutures to fix the migrated fat in the appropriate position as needed, ensuring that the fat maintains the expected flat shape during the postoperative tissue healing process.
[0032] The Periocular Tightening Auxiliary Instrument 403 is used to simultaneously relocate under-eye fat and lift the skin around the eyes, thereby improving the appearance of under-eye bags and tear trough depressions. When using an external approach, the surgeon enters through an incision in the lateral aspect of the lower eyelid. While performing the under-eye fat relocation, the Periocular Tightening Auxiliary Instrument 403 can be used to appropriately excise and lift the loose skin of the lower eyelid. The Periocular Tightening Auxiliary Instrument 403 can be a fine surgical scissor, skin retractor, tissue holder, electrocautery device, or ultrasound. The selection of instruments such as the tightening probe depends on the surgeon's operating habits and the degree of skin laxity in the patient. The surgeon should strike a proper balance between eliminating the appearance of skin laxity and avoiding the risk of excessive traction leading to lower eyelid ectropion. With the use of the periorbital tightening auxiliary instrument 403, patients can simultaneously complete the relocation of eye bag fat and tightening and lifting of periorbital skin in the same surgery, achieving a triple aesthetic improvement of eye bag elimination, tear trough filling, and periorbital skin tightening in one procedure. The overall periorbital rejuvenation effect is the most comprehensive, and it has significant advantages in terms of benefits compared to traditional procedures that require multiple sessions.
[0033] Please refer to Figure 2. Figure 2 is a schematic diagram comparing the appearance of the periocular area before and after the application of the blepharoplasty and tear trough surgery auxiliary system of this invention. The upper half of Figure 2 shows the appearance of the patient's periocular area before surgery, and the lower half shows the appearance of the patient's periocular area after surgery. In terms of the preoperative state, as shown in the upper half of Figure 2, obvious bulging of the lower eyelid is visible, and a significant height difference is formed between the bulging area of the eye bag and the lower edge of the eye socket. The tear trough depression line is clearly visible, extending from the inner side of the eye socket to the cheekbone. The skin around the eyes shows varying degrees of laxity and drooping, and the overall appearance of the periocular area presents a tired and aged visual effect. The above-mentioned preoperative appearance features are captured by the image sensing unit 101 of the camera module 10, and the image analysis unit 201 of the electronic device 20 performs objective quantitative analysis on the volume of the bulging eye bag, the depth of the tear trough depression, and the degree of skin laxity around the eyes to provide a complete data basis for the surgeon to formulate a surgical plan. In terms of the postoperative state, as shown in the lower half of Figure 2, the patient's The appearance of the bulging lower eyelid bags has been significantly improved. The originally bulging orbital fat has been flipped and flattened into the tear trough area by the fat transfer instrument 402, and the tear trough depression line has been significantly filled. The height difference under the eye socket has become gentler, and the periorbital contour presents a natural and full appearance. If an external approach is used and the periorbital tightening auxiliary instrument 403 is used for skin lifting, the sagging of the periorbital skin is also improved simultaneously. The overall periorbital appearance has a significant rejuvenation effect. The postoperative periorbital images are also captured by the camera module 10 and compared with the preoperative images shown in the upper part of Figure 2 through the display unit 204 to present the surgical results in an objective and quantitative way. Through the intuitive comparison of the preoperative and postoperative results in Figure 2, the fat-preserving transfer technology used in this eyelid bag and tear trough surgical auxiliary system can simultaneously achieve the triple aesthetic improvement effect of eyelid bag elimination, tear trough filling and periorbital tightening. Compared with the traditional simple eyelid fat removal procedure, it has a more comprehensive and lasting periorbital rejuvenation effect.
[0034] Referring to Figure 1, the complete surgical assistance process of this invention is as follows: During the preoperative assessment stage, the surgeon first activates the camera module 10. Under the auxiliary illumination of the light source unit 102, the image sensing unit 101 performs standardized multi-angle image acquisition of the patient's periocular area. The acquired periocular images are transmitted in real time to the electronic device 20. The image analysis unit 201 of the electronic device 20 performs digital image processing on the periocular images, providing quantitative data on the volume of eye bag bulge, the depth of tear trough depression, and the distribution of orbital fat. At the same time, the skin assessment unit 202 performs quantitative assessment of the degree of skin laxity around the eyes and outputs the skin laxity index. The surgical suggestion unit 203 outputs based on the above data. The surgical plan provides suggestions for an external approach when the skin laxity index reaches a preset laxity threshold, and an internal approach when it does not, along with a fat repositioning suggestion. All analytical data and surgical plans are presented visually by the display unit 204, allowing for thorough preoperative communication between the surgeon and patient. During preoperative preparation and anesthesia administration, the surgeon opens the instrument storage bag 400 to the unfolded state, confirming that the ligament release instrument 401, fat repositioning instrument 402, and periorbital tightening auxiliary instrument 403 are all prepared and arranged in sequence. The surgeon then plans the anesthesia administration points according to the surgical plan, and the anesthetic is administered by the injection unit 302 of the local anesthesia module 30. The local anesthetic in storage unit 301 is injected into the target area of the patient's periocular anesthesia. After confirming that the anesthesia effect is sufficient and the patient still retains sensation, the surgical procedure begins. During the surgical procedure, the surgeon selects an appropriate incision location based on the recommended external or internal approach. First, the ligament release instrument 401 is retrieved from the instrument storage bag 400 and precisely released along the direction of the tear trough ligament to create sufficient fat placement space. Then, the fat transfer instrument 402 is retrieved, and the eye bag fat is flipped and spread evenly into the tear trough depression area while maintaining blood supply. After confirming that the fat evenly covers the tear trough area, sutures are fixed as needed. If an external approach is used and the patient... If the patient's skin around the eyes is significantly loose, the eye tightening auxiliary instrument 403 is further removed, and the lower eyelid skin is appropriately excised and tightened. After the incision is sutured, the surgical procedure is completed. During the postoperative follow-up phase, the camera module 10 is used again to capture images of the patient's eye area after an appropriate time postoperatively. The images are objectively compared and analyzed with the preoperative images through the image analysis unit 201 of the electronic device 20. The preoperative and postoperative comparison images are displayed on the display unit 204, providing a complete quantitative comparison record of the surgical results. This serves as an important basis for subsequent follow-up and necessary adjustments. After the surgery, the instrument storage bag 400 is returned to its closed state and uniformly sent for sterilization for use in the next surgery.
