[0006] The present disclosure belongs to the field of medicine and relates to a drug combination comprising a kinesin family member 18A (KIF18A) inhibitor and a tubulin inhibitor, a combination product or a pharmaceutical composition comprising the drug combination, and the use of the drug combination, the combination product or the pharmaceutical composition for treating tumors. Background Art
[0007] Cancer is often characterized by unregulated cell proliferation. Damage to one or more genes in the cell proliferation pathway can cause the loss of normal regulation of cell proliferation. These dysregulated genes can encode various tumor suppressors or oncogene proteins, leading to unchecked cell cycle progression and cell proliferation. Various kinases and kinesins have been identified as playing a key role in the cell cycle, mitosis regulation and progression of normal and cancer cells.
[0008] Kinesins are molecular motors that play a vital role in cell division and the transport of intracellular vesicles and organelles. Mitotic kinesins play a role in multiple processes, including spindle assembly, chromosome segregation, and centrosome separation. Human kinesins are classified into 14 subfamilies based on sequence homology within the so-called "motor domain," whose ATPase activity drives unidirectional movement along microtubules. The non-motor domains of these proteins are responsible for attaching to their "cargo," which can be a variety of membranous organelles, signal transduction scaffolds, and chromosomes. Kinesins utilize the energy from ATP hydrolysis to move their "cargo" along polarized microtubules. For this reason, kinesins are often referred to as "plus-end" or "minus-end"-directed motor proteins. The KIF18A gene belongs to the kinesin-8 subfamily and is a plus-end-directed motor protein. KIF18A is believed to influence the movement of the plus ends of centromeric microtubules to control correct chromosome positioning and maintain spindle tension. Ablation of human KIF18A in HeLa cervical cancer cells results in longer spindles, increased chromosome oscillations during metaphase, and activation of the spindle assembly checkpoint (SAC). KIF18A is overexpressed in multiple types of cancer. In cancer cells with chromosomal instability (CIN), KIF18A gene deletion, knockout, or inhibition affects the mitotic spindle. In particular, it has been found that inhibition of KIF18A can activate the SAC, induce mitotic arrest, and ultimately lead to mitotic catastrophe in cancer cells or mitotic slippage during interphase, resulting in apoptosis of cancer cells. Therefore, KIF18A may be a viable target for cancer therapy.
[0009] The spindle assembly checkpoint is a surveillance mechanism that prevents centromeres from exiting mitosis in the presence of unattached or incorrectly attached chromosomes. This prevents the addition or deletion of entire chromosomes and their deleterious effects on cell physiology, and promotes accurate chromosome segregation during mitosis. Microtubule dynamics are crucial for successful mitosis, particularly for the proper function of the spindle during mitosis. Microtubule inhibitors disrupt microtubule dynamics, arresting the cell cycle in the G2 / M phase and ultimately leading to apoptosis. Microtubule inhibitors can be divided into two categories: microtubule stabilizers and microtubule destabilizers. Microtubule stabilizers, such as taxanes and epothilones, act by promoting microtubule polymerization and increasing the mass of microtubule polymers in cells. Microtubule destabilizers, such as vinca alkaloids, reduce the mass of microtubule polymers by depolymerizing microtubules or inhibiting their polymerization. Microtubule inhibitors have been used to treat a variety of solid tumors and hematological malignancies. However, intrinsic or acquired drug resistance limits the use of microtubule inhibitors. Tautomers generally exist in equilibrium, and attempts to isolate a single tautomer usually result in a mixture whose physical and chemical properties are consistent with a mixture of compounds. The position of equilibrium depends on the chemical properties within the molecule. For example, in many aliphatic aldehydes and ketones such as acetaldehyde, the keto form predominates, while in phenols, the enol form predominates. The present disclosure encompasses all tautomeric forms of the compounds.

[0227] Example 2: Preparation of Antibody-Drug Conjugate Ladiratuzumab-MMAE
[0228] 2.1 Construction and production of anti-human LIV-1 monoclonal antibodies
[0229] The sequence of the anti-human LIV-1 monoclonal antibody Ladiratuzumab is shown in Table 1 below (antibody sequence source US20200165335A). The nucleic acid sequences encoding the antibody VH and VL were recombined into the expression vector pTT5 with a signal peptide and heavy chain constant region (sequence) / light chain constant region sequence to obtain a recombinant plasmid expressing Ladiratuzumab. After sequencing verification, the plasmid was extracted. The plasmid and transfection reagent PEI (Polysciences, catalog number: 24765-1) were added to OPTI-MEM (Gibco, catalog number: 11058021) and mixed well. After standing for 15 minutes, the cells were added to Expi293 cells (manufacturer: Thermofisher, catalog number: A14527) and cultured in a shaker at 5% CO2, 120 rpm, and 37°C. On the second day of transfection, OPM-293 ProFeed (Shanghai Aopuma, Catalog No.: F081918-001) and 6 g / L glucose (Manufacturer: Sigma, Catalog No.: G7528) were added. On the sixth day of transfection, the cell supernatant was collected.
[0230] The sequence information of the anti-human LIV-1 antibody Ladiratuzumab is as follows:
[0231] 2.2 Antibody Purification
[0232] Purify the above-mentioned antibodies from cell culture supernatant using Protein A affinity chromatography (GE, Catalog No. 17549802). Wash the Protein A affinity column with 6M guanidine hydrochloride for 3-5 column volumes, followed by 3-5 column volumes with purified water. Equilibrate the column with a buffer system such as 1× PBS (pH 7.4) for 3-5 column volumes. Load the cell supernatant for binding at a low flow rate, controlling the flow rate to achieve a retention time of approximately 1 minute or longer. After binding, wash the column with 1× PBS (pH 7.4) for 3-5 column volumes until UV absorbance returns to baseline. Elute the sample with 0.02M citrate buffer, pH 3.5. Collect the elution peak under UV monitoring. Adjust the pH of the eluted product to 5-6 using 1M Tris-HCl (pH 8.0, Sinopharm, Catalog No. 30188336) for storage. The eluted product can be subjected to solvent exchange using methods well known to those skilled in the art, such as ultrafiltration and concentration using ultrafiltration tubes and solvent exchange into the desired buffer system, or using molecular exclusion chromatography, such as G-25 desalting, to replace the desired buffer system, or using a high-resolution molecular exclusion column such as Superdex 200 to remove aggregates from the eluted product to improve sample purity. After purification, proteins that meet purity requirements are sterile filtered using a 0.22 μm filter (Millipore, Catalog No. SLGVR13SL). After passing SEC-HPLC purity testing, the samples are aliquoted and stored at -80°C until ready for use.
[0233] 2.3 Preparation of Antibody-Drug Conjugates
[0234] 2.3.1 Sources of Drug-Linker Compounds
[0235] The structure of drug-linker 1 is shown below, which was purchased from MedChemExpress (MCE) with the product number HY-15575.
[0236] 2.3.2 Antibody and Drug-Linker Conjugation
[0237] The prepared antibody was dialyzed into 20 mM phosphate buffer, 150 mM NaCl, and 1 mM EDTA (pH 6.5). A 2.4-fold volume of 10 mM tris(2-carboxyethyl)phosphine solution (TCEP, Thermo Scientific #77720) was added to the antibody solution, and the mixture was reduced on a thermostatic metal shaker at 37°C for 3 hours. A 10-fold molar equivalent of the aforementioned drug-linker 1 compound (purchased from MCE, Cat. HY-15575) (dissolved in DMSO to 20 mg / ml) was added to the reaction system, and the reaction mixture was conjugated at 25°C for 12 hours. The reaction product was desalted on a G25 column and exchanged into phosphate-buffered saline (PBS) buffer to remove unreacted free small molecule toxins, yielding the antibody-drug conjugate. The ADC product was analyzed for purity and DAR value using SEC and LC-MS.
[0238] SEC purity analysis: SEC-HPLC method was used to analyze the protein sample to be tested, to characterize the molecular size homogeneity of the recombinant protein, and to measure the purity of the recombinant protein. The HPLC used in this method is an Agilent 1260, and the chromatographic column is a TSKgel G3000SWXL from Tosoh Bioscience, with a mobile phase of 200mM phosphate buffer pH 7.0 containing 10% isopropanol, a detection temperature of 25°C, a flow velocity of 0.5mL / min, a detection wavelength of 280nm, and a target protein loading of 50μg, with an analysis time of 40 minutes. To SEC-HPLC data, manual integration was used to analyze the chromatogram, and protein purity was calculated according to the area normalization method. The main peak was considered to be a monomer, the chromatographic peak before the main peak was called an aggregate, and the chromatographic peak after the main peak was called a fragmentation body.
[0239] DAR value determination: The DAR value of the ADC molecule was measured using ultra-high performance liquid chromatography-mass spectrometry (UHPLC-MS). First, the ADC molecule to be tested was treated with PNGase F (NEB#P0705L) to remove the N-glycan modification, and then treated with dithiothreitol (DTT, Sigma#646563) and incubated at 37°C for 1 hour to reduce it to light and heavy chains. Then, it was analyzed using a Thermo Vanquish UHPLC-Q Exactive Plus mass spectrometry system. 2 μg of protein was injected onto a Waters ACQUITY Protein BEH molecular exclusion chromatography column. The mobile phase was an aqueous solution containing 0.1% formic acid, 0.05% TFA, and 25% acetonitrile. The flow rate was 0.2 mL / min and the analysis time was 30 min. The mass spectrometer was a Thermo Q Exactive Plus. The main mass spectrometry parameters were as follows: spray voltage 3.8 kV, capillary heating temperature 300°C, sheath gas flow rate 35 arb, parent ion scan range 800-3000, etc. Finally, the mass spectrometry data were analyzed using the Biopharma Finder software. 4.1. Deconvolution processing is performed using the Respect algorithm to calculate the molecular weight information of the light and heavy chain mass spectrum peaks and the mass spectrum response signals of each component, respectively, and thus calculate the DAR value of the ADC sample to be tested.
[0240] The DAR value and SEC purity of the antibody drug conjugate Ladiratuzumab-MMAE are shown in the following table:
[0241] Test Example 1: KIF18A enzyme activity assay
[0242] Based on ADP-Glo TM Kinase Assay: Detect the amount of ADP generated in the reaction to reflect KIF18A enzyme activity.
[0243] 1. Experimental instruments and materials
[0244] The human KIF18A (1-376aa) protease used in the experiment was expressed by Panchao Biotechnology and stored at -80°C (freeze-thaw no more than 5 times).
[0245] Detection kit (ADP-Glo TMThe Kinase Assay was purchased from Promega under the catalog number V9101 and stored at -30°C. The kit detects enzyme activity by measuring the ADP generated during the enzyme reaction. KIF18A enzyme reacts with ATP to generate ADP, and the remaining ATP in the reaction is consumed by the ADP-Glo reagent. The ADP generated during the KIF18A enzyme reaction is reduced to ATP by the detection reagent. ATP is then converted to ATP in the Ultra-Glo TM Under the action of luciferase, it reacts with luciferin to emit light, and the luminescence signal is positively correlated with the KIF18A enzyme activity.

[0299] Test Example 5: In vivo efficacy study of the disclosed KIF18A inhibitor combined with an antibody-drug conjugate in the OVCAR3 model
[0300] Experimental reagents:
[0301] OVCAR3 cell source: ATCC, catalog number HTB-161 TM .
[0302] Experimental methods:
[0303] Animal information: BALB / c nude female mice, 5-6 weeks old, weighing approximately 16-20 g, were purchased from Shanghai Lingchang Biotechnology Co., Ltd. The mice were housed in an SPF-grade environment with individual ventilation in each cage. All animals had free access to standard certified commercial laboratory diet and free drinking water.
[0304] Cell Culture: Human ovarian cancer OVCAR3 cells were cultured in RPMI 1640 (cell culture medium) supplemented with 10% fetal bovine serum and 1% penicillin-streptomycin solution in a 37°C, 5% CO2 incubator. Cells were routinely digested and passaged twice weekly using 0.25% trypsin-EDTA. Cells were harvested and counted when cell saturation reached 85%-90% and the required number of cells was reached.
[0305] Cell inoculation: 0.1 mL / (containing 10×10 6 ) OVCAR3 cell suspension was subcutaneously inoculated on the right side of each mouse. On the 20th day after inoculation, the average tumor volume reached 134.5 mm 3 At 4 dpi, randomized stratified groups were assigned based on tumor volume. Four groups (n=6) were administered: vehicle (5% NMP + 15% PEG400 + 80% (20% HP-β-CD), PO, QD), compound 4 (5 mg / kg, PO, QD), ladiratuzumab-MMAE (3 mg / kg, IP, single dose), and ladiratuzumab-MMAE combined with compound 4 (3 mg / kg, IP, single dose + 5 mg / kg, PO, QD).
[0306] Administration: Ladiratuzumab-MMAE was administered at a dose of 3 mg / kg by intraperitoneal injection (IP), a single dose only on the day of grouping; Compound 4 was administered at a dose of 5 mg / kg by PO, once a day (QD) x 21 days.
[0307] Tumor measurements and experimental parameters:
[0308] Tumor diameter was measured twice a week using a vernier caliper. Tumor volume was calculated using the formula: V = 0.5 axb 2 , a and b represent the long diameter and short diameter of the tumor, respectively. The body weight of mice was measured twice a week.
[0309] The tumor inhibition efficacy was evaluated by tumor growth inhibition rate (TGI) (%). TGI (%) = [(1 - (mean tumor volume of a treatment group at the end of drug administration - mean tumor volume of the treatment group at the start of drug administration) / (mean tumor volume of the solvent control group at the end of treatment - mean tumor volume of the solvent control group at the start of treatment)] x 100%.
[0310] The efficacy of compound combinations was evaluated using Kim Jong-kyun's formula. CI = E(A+B) / (EA+EB-EA*EB), where E(A+B) represents the tumor growth inhibition rate (TGI) of compound A and B combined, and EA and EB represent the tumor growth inhibition rates of compound A and B alone. A CI < 0.85 indicates antagonism; a CI between 0.85 and 1.15 indicates simple additive effects; and a CI > 1.15 indicates potentiation.
[0311] Experimental results:
[0312] On day 21 of group-based dosing, once-daily oral administration of 5 mg / kg Compound 4 combined with a single intraperitoneal injection of 3 mg / kg Ladiratuzumab-MMAE (on the day mice were grouped) to tumor-bearing mice showed a significant tumor inhibition effect compared to the vehicle control group (P<0.0001). The efficacy was significantly improved compared to the treatment with Compound 4 (5 mg / kg) or Ladiratuzumab-MMAE (3 mg / kg) alone (P<0.0001). See Table 7 and Figure 42.
[0313] Table 7. Tumor volume and significance analysis of each experimental group of OVCAR3 tumor-bearing mice a Statistical analysis was performed using the two-way anova method with GraphPad Prism 9.0. P < 0.05 was considered to indicate a significant difference (*: p < 0.05; **: p < 0.01; ***: p < 0.001; ****: p < 0.0001).
[0314] Experimental conclusion:
[0315] Compared to the vehicle control group, once-daily oral administration of compound 4 at 5 mg / kg, combined with a single intraperitoneal injection of ladiratuzumab-MMAE at 3 mg / kg, significantly reduced tumor volume in OVCAR3 human ovarian cancer model mice compared to the vehicle control group, and also significantly reduced tumor volume compared to the two single-agent groups. The combination index (CI) calculated by Kim Jung-kyun's formula was 1.43, indicating a synergistic effect.
