[0003] FIELD OF THEINVENTION The present invention relates to the treatment of subjects with cancer whose tumors lack at least one activating EGFR mutation. [Background technology]
[0004] The individual roles of both epidermal growth factor receptor (EGFR) and the receptor tyrosine kinase mesenchymal-epithelial transition factor (c-Met) in cancer are well established, making these targets attractive for combination therapy. Both receptors signal through the same survival and anti-apoptotic pathways (ERK and AKT), and thus joint inhibition of this pair may limit the potential activation of compensatory pathways, thereby improving overall efficacy.
[0005] Molecular segmentation of advanced non-small cell lung cancer (NSCLC) based on oncogenic driver mutations has improved overall survival and quality of life for patients with treatable driver mutations.
[0006] Amivantamab is a bispecific antibody targeting EGFR and c-MET. Its clinical activity has been explored across a range of EGFR activating mutations in clinical trials, but has not been evaluated for the treatment of lung cancers that are EGFR positive but lack EGFR activating mutations. [0075] "Specific binding" or "specifically binds" or "specific binding" or "binds" refers to an antibody binding to an antigen or an epitope within an antigen with higher affinity than other antigens. Typically, antibodies bind to an antigen with an equilibrium dissociation constant (K D ) is the K for binding to non-specific antigens (e.g., BSA, casein) D At least 100 times smaller than about 5 × 10 -8 M or less, for example, about 1×10 -9 M or less, approximately 1×10 -10 M or less, approximately 1×10 -11 M or less, approximately 1×10 -12 K below M D The dissociation constant can be measured using known protocols. However, an antibody that binds to an antigen or an epitope within an antigen may have cross-reactivity to other related antigens, e.g., the same antigen (homolog) from other species, such as humans or monkeys, e.g., Macaca fascicularis (cynomolgus monkey, cyno) or Pan troglodytes (chimp). A monospecific antibody binds to one antigen or one epitope, whereas a bispecific antibody binds to two different antigens or two different epitopes.
[0076] "Antibody" has a broad meaning and includes monoclonal antibodies, including murine, human, humanized, and chimeric monoclonal antibodies, antigen-binding fragments, multispecific antibodies, such as bispecific, trispecific, tetraspecific, dimeric, tetrameric, or multimeric antibodies, single-chain antibodies, domain antibodies, and immunoglobulin molecules, including any other modified form of an immunoglobulin molecule that contains an antigen-binding site of the required specificity. A "full-length antibody" is composed of two heavy chains (HC) and two light chains (LC), and multimers thereof (e.g., IgM), interconnected by disulfide bonds. Each heavy chain is composed of a heavy chain variable region (VH) and a heavy chain constant region (consisting of domains CH1, hinge, CH2, and CH3). Each light chain is composed of a light chain variable region (VL) and a light chain constant region (CL). The VH and VL regions are further divided into regions of hypervariability called complementarity determining regions (CDRs) that are interspersed with framework regions (FRs). Each VH and VL is composed of three CDR and four FR segments, arranged from the amino terminus to the carboxy terminus in the following order: FR1, CDR1, FR2, CDR2, FR3, CDR3, and FR4.
[0077] "Complementarity determining region (CDR)" is the region of an antibody that binds to an antigen. CDRs can be defined using various descriptions, such as Kabat (Wu et al. (1970) J Exp Med 132:211-50) (Kabat et al., Sequences of Proteins of Immunological Interest, 5th Ed. Public Health Service, National Institutes of Health, Bethesda, Md., 1991), Chothia (Chothia et al. (1987) J Mol Biol 196:901-17), IMGT (Lefranc et al. (2003) Dev Comp Immunol 27:55-77), and AbM (Martin and Thornton (1996) J Bmol Biol 263:800-15). Correspondence between the various descriptions and numbering of the variable regions has been described (see, for example, Lefranc et al. (2003) Dev Comp Immunol 27:55-77; Honegger and Pluckthun, (2001) J Mol Biol 309:657-70; and the International ImMunoGeneTics (IMGT) database; Web resources, http: / / www_imgt_org). Available programs such as abYsis by UCL Business PLC can be used to delineate CDRs. As used herein, the terms "CDR", "HCDR1", "HCDR2", "HCDR3", "LCDR1", "LCDR2" and "LCDR3" include CDRs defined by any of the methods of Kabat, Chothia, IMGT or AbM as described above, unless otherwise expressly stated herein.
[0078] Immunoglobulins can be assigned to five major classes, IgA, IgD, IgE, IgG, and IgM, depending on the amino acid sequence of the heavy chain constant domain. IgA and IgG are further subdivided into isotypes IgA1, IgA2, IgG1, IgG2, IgG3, and IgG4. [0179] Other publicly available bispecific anti-EGFR / c-Met antibodies may be used in the methods of the disclosure, so long as they exhibit similar characteristics when compared to amivantamab, as described in U.S. Patent No. 9,593,164. Bispecific anti-EGFR / c-Met antibodies that may be used in the methods of the disclosure may also be generated by combining publicly available EGFR-binding VH / VL domains and c-Met-binding VH / VL domains and testing the resulting bispecific antibody for its characteristics as described in U.S. Patent No. 9,593,164.
[0180] The bispecific anti-EGFR / c-Met antibodies used in the methods of the present disclosure can be generated using Fab arm exchange (or half molecule exchange) between two monospecific bivalent antibodies, for example, by introducing substitutions in the heavy chain CH3 interface in each half molecule to favor the formation of heterodimers of two antibody half molecules with different specificities, in a cell-free environment in vitro or using co-expression. The Fab arm exchange reaction is the result of a disulfide bond isomerization reaction and dissociation-association of the CH3 domains. The heavy chain disulfide bonds in the hinge region of the parent monospecific antibodies are reduced. The resulting free cysteine of one of the parent monospecific antibodies forms an intra-heavy chain disulfide bond with a cysteine residue of the second parent monospecific antibody molecule, and at the same time, the CH3 domain of the parent antibody is released and reformed by dissociation-association. The CH3 domain of the Fab arm may be engineered to favor heterodimer formation over homodimer formation. The resulting product is a bispecific antibody with two Fab arms or half molecules that each bind a different epitope, i.e., an epitope in EGFR and an epitope in c-Met. For example, bispecific antibodies of the invention can be generated using the techniques described in WO 2011 / 131746. For IgG1 antibodies, the mutation F405L in one heavy chain and K409R in the other heavy chain can be used. For IgG2 antibodies, wild type IgG2 and IgG2 antibodies with F405L and R409K substitutions may be used. For IgG4 antibodies, wild type IgG4 and IgG4 antibodies with F405L and R409K substitutions may be used. To generate a bispecific antibody, a first monospecific bivalent antibody and a second monospecific bivalent antibody are engineered to have the aforementioned mutations in the Fc region, and the antibodies are incubated together under sufficient reducing conditions to allow the cysteines in the hinge region to undergo disulfide bond isomerization, thereby generating the bispecific antibody by Fab arm exchange. Incubation conditions may optimally be returned to non-reducing conditions.Exemplary reducing agents that may be used are 2-mercaptoethylamine (2-MEA), dithiothreitol (DTT), dithioerythritol (DTE), glutathione, tris(2-carboxyethyl)phosphine (TCEP), L-cysteine, and β-mercaptoethanol. For example, incubation at a temperature of at least 20° C., in the presence of at least 25 mM 2-MEA or in the presence of at least 0.5 mM dithiothreitol, at a pH of 5-8, e.g., pH 7.0 or pH 7.4, for at least 90 minutes may be used.
[0181] Bispecific anti-EGFR / c-Met antibodies for use in the methods of the disclosure can also be generated using designs such as knobs-in-holes (Genentech), CrossMAb (Roche) and electrostatically-matched (Chugai, Amgen, NovoNordisk, Oncomed), LUZ-Y (Genentech), Strand Exchange Engineered Domain body (SEEDbody) (EMD Serono), and Biclonic (Merus).
[0182] In the "knobs-in-holes" approach (see, for example, WO 2006 / 028936), selected amino acids that form the interface of the CH3 domain of human IgG can be mutated at positions that affect CH3 domain interactions to promote heterodimer formation. Amino acids with small side chains (holes) are introduced into the heavy chain of an antibody that specifically binds to a first antigen, and amino acids with large side chains (knobs) are introduced into the heavy chain of an antibody that specifically binds to a second antigen. After co-expression of the two antibodies, heterodimers are formed as a result of the preferential interaction of heavy chains with "holes" and heavy chains with "knobs". Exemplary CH3 substitution pairs that form knobs and holes are T366Y / F405A, T366W / F405W, F405W / Y407A, T394W / Y407T, T394S / Y407A, T366W / T394S, F405W / T394S, and T366W / T366S_L368A_Y407V (expressed as modification position in the first CH3 domain of the first heavy chain / modification position in the second CH3 domain of the second heavy chain).
[0183] In addition to utilizing a "knobs-in-holes" approach to promote Fab arm exchange, CrossMAb technology utilizes CH1 / CL domain swapping in one of the half-arms to ensure correct light chain pairing of the resulting bispecific antibody (see, e.g., U.S. Pat. No. 8,242,247).
[0184] Other crossover techniques may be used to generate full length bispecific antibodies of the invention by swapping variable or constant, or both, domains in one or both arms between the heavy and light chains of the bispecific antibody or within the heavy chain, including, for example, VH-CH1 and VL-CL, VH and VL, CH3 and CL, and CH3 and CH1 as described in WO 2009 / 080254, WO 2009 / 080251, WO 2009 / 018386, and WO 2009 / 080252.
[0185] Other approaches, such as promoting heavy chain heterodimerization using electrostatic interactions by substituting positively charged residues on one CH3 surface and negatively charged residues on the second CH3 surface, may be used as described in U.S. Patent Application Publication No. 2010 / 0015133, U.S. Patent Application Publication No. 2009 / 0182127, U.S. Patent Application Publication No. 2010 / 028637, or U.S. Patent Application Publication No. 2011 / 0123532. In another approach, heterodimerization can be achieved by the following substitutions: L351Y_F405A_Y407V / T394W, T366I_K392M_T394W / F405A_Y407V, T366L_K392M_T394W / F405A_Y407V, L351Y ... A / T366A_K409F, L351Y_Y407A / T366V_K409F, Y407A / T366A_K409F, or T350V_L351Y_F405A_Y407V / T350V_T366L_K392L_T394W (represented as modification position in the first CH3 domain of the first heavy chain / modification position in the second CH3 domain of the second heavy chain).
[0186] SEEDbody technology may be utilized to generate bispecific antibodies of the invention. SEEDbodies have selected IgG residues in their constant domains substituted with IgA residues to promote heterodimerization, as described in US Patent Application Publication No. 20070287170.
[0187] Mutations are typically made at the DNA level, using standard methods, on molecules such as the constant domain of an antibody.

[0189] The invention will now be described with reference to the following specific, non-limiting examples.
[0190] Example 1. Characterization of non-small cell lung cancer (NSCLC) patient-derived xenograft (PDX) tumors expressing EGFR lacking activating mutations. EGFR and MET protein levels, as determined by immunohistochemistry (IHC); signaling, as determined by proximity ligation assay (PLA); and tumor associated macrophage (TAM) content were assessed in 39 NSCLC patient-derived xenograft (PDX) models with EGFR lacking activating mutations. Absence of activating mutations in EGFR was determined by whole exome sequencing (WES). Thirty-nine formalin fixed and paraffin embedded (FFPE) NSCLC PDX tumors with EGFR lacking activating mutations were obtained from Charles River Laboratory (CRL, Freiburg, Germany). Tumors included 19 adenocarcinoma and 20 epidermoid NSCLC. Correlations between receptor protein levels and receptor signaling were assessed for both EGFR and MET in these models using IHC and PLA.
[0191] For IHC studies, tissue sections were processed as described by Smith et al. (2015) (Annotation of human cancers with EGFR signaling-associated protein complexes using proximity ligation assays. Matthew A. Smith et.al. 2015, Science Signaling, 8(359):ra4). Briefly, sections were rehydrated and antigens were retrieved. Non-specific binding was blocked by incubation with 1.5% bovine serum albumin (BSA) and incubated overnight in 0.5% BSA in PBST with rabbit antibodies targeting EGFR (Ventana, clone 5B7) or with MET clone D1C2 (Cell Signaling) and phospho-MET clone D26 (Cell Signaling). Slides were washed twice with PBST, incubated with EnVision+anti-rabbit (K400311-2, Agilent) for 1 h, and visualized by DAB (diaminobenzidine). Slides were counterstained with hematoxylin, rehydrated, and hard-mounted. To calculate the H-score, the staining intensity of cells was scored (0, 1+, 2+, 3+) and the percentage of cells at each intensity was determined. The formula [1×(% cells 1+)+2×(% cells 2+)+3×(% cells 3+)] was then used to calculate the H-score, ranging from 0 to 300, for each PDX model.
[0192] Proximity ligation assays (PLA) were performed according to published protocols (Annotation of human cancers with EGFR signaling-associated protein complexes using proximity ligation assays. Matthew A. Smith et.al. 2015, Science Signaling, 8(359):ra4). Briefly, slides containing 5 μm sections of FFPE PDX tumors were rehydrated through xylene and graded alcohols. Heat-induced epitope retrieval was performed in Tris-EDTA (pH 9) in a pressure cooker for 20 min, followed by cooling for 20 min. Nonspecific binding was blocked by incubation with 1.5% bovine serum albumin (BSA) for 30 min at room temperature. Primary antibodies were rabbit antibodies targeting EGFR (clone D38B1, Cell Signaling Technology) or MET clone D1C2 (Cell Signaling) diluted 1:300, and mouse antibodies targeting growth factor receptor-bound protein 2 (GRB2) (clone 81, BD Biosciences) were used in 1.5% BSA in 0.5% phosphate-buffered saline (PBS)-Tween 20 (PBST) overnight. PLA probes were rabbit (-) and mouse (+) and detected with the Duolink™ In Situ PLA Far Red kit (Sigma-Aldrich). Alexa Fluor488-conjugated anti-cytokeratin was used to distinguish epithelial areas (clone AE1 / AE3, eBioscience).
[0193] Confocal images were acquired on a Leica TCS SP5 AOBS (Acousto Optical Beam Splitter) laser scanning confocal microscope through a 40×1.25NA (numerical aperture) Plan-Apochromat oil immersion objective (Leica Microsystems CMS GmbH). Diode (405) and HeNe (647) laser lines were applied to excite the samples, and tunable emission was used to minimize crosstalk between fluorochromes. Z-stack (0.5 μm thick slice) images of each sample were captured with a photomultiplier detector and maximum projections were made using LAS AF software version 2.6 (Leica Microsystems). Further fluorescent images were acquired on a fully automated upright Zeiss AxioImagerZ.1 microscope equipped with a 40×1.25NA oil immersion objective and DAPI and Cy5 filter cubes. Images were generated using an AxioCam MRm CCD (charge-coupled device) camera and the Axiovision version 4.6 software suite (Carl Zeiss Inc.) All tissue-based PLA and AQUA analysis images were acquired using a 20× objective (dry) on an AQUA workstation (PM-2000, HistoRx) equipped with a fully motorized stage and DAPI, Cy3, FITC (fluorescein isothiocyanate), and Cy5 filter cubes.Images were saved as individual channels and exported as a merged RGB TIFF image. (Annotation of human cancers with EGFR signaling-associated protein complexes using proximity ligation assays. Matthew A. Smith et.al. 2015, Science Signaling, 8(359):8(359):ra4; MET-GRB2 Signaling-Associated Complexes Correlate with Oncogenic MET Signaling and Sensitivity to MET Kinase Inhibitors. Matthew A. Smith et.al. 2017, Clin Cancer Res. 2017,23(22):7084-7096).
[0194] PLA scores were determined as previously described (Smith et al. 2015). Briefly, PLA was manually quantified using a scoring criterion based on foci per cell (0, not detectable; 1+, 1–5 foci per cell; 2+, >5–20 foci per cell; 3+, >20 foci per cell) and annotated as “high” (2+–3+ in both cores) or “low” (0–1+).
[0195] The results suggested that a statistically significant correlation between IHC score and PLA score was observed for both EGFR (r=0.63, p<0.0001, see FIG. 1A) and MET (r=0.83, p<0.0001, see FIG. 2A). Spearman's correlation coefficient was used to calculate r values, and two-tailed tests were used to calculate p values (Prism Graphpad v.7.0.0). Specifically, if there were less than 17 XY pairs, a permutation test was used to calculate the p value of the Spearman correlation coefficient. Otherwise, the statistical value

[0197] Example 2. Fc receptor engagement is not required for amivantamab efficacy in NSCLC models with EGFR lacking activating mutations. Next, we tested the efficacy of amivantamab in vivo in an NSCLC model that harbors EGFR lacking activating mutations.
[0198] In vivo studies were performed at Charles River Laboratory (CRL, Freiburg, Germany) in accordance with the policies and procedures of the Janssen Animal Care and Use Committee. Fourteen NSCLC PDX tumors were implanted subcutaneously into the flanks of NMRI nu / nu mice (CRL) until tumors reached 50–200 mm 3 Treatment was administered by intraperitoneal injection of either isotype control, or amivantamab, or EGFR / MET bispecific antibody with silent Fc at 10 mg / kg twice weekly for 3 weeks. The EGFR / MET-silent Fc antibody retains the EGFR and MET arms of amivantamab and has substitutions V234A / G237A / P238S / H268A / V309L / A330S / P331S made to the heavy chain to statistically significantly reduce affinity for Fcγ receptors and Cq1 complement. Tumors were measured with calipers and were calculated using the formula: (length x (width) 2 Tumor volumes were calculated using the formula: {1-[(treatment t -treatment i ) / (Control t - Control i )]} × 100 was used to calculate 7 days after the last dose.
[0199] Results suggested that amivantamab inhibited tumor growth in 13 of the 14 PDX models tested (representative efficacy plots are shown in Figures 2A and 2B). Of these 13 models in which amivantamab showed activity, EGFR / MET-silent Fc antibodies lacking binding to Fc receptors had comparable efficacy (Figures 2A and 2B). LXFA2158, a model with MET gene amplification, was the only PDX model in which amivantamab Fc binding was important for efficacy (Figure 2C). This is consistent with previously reported data (Smruthi Vijayaraghavan et.al. 2020, Molecular Cancer Therapeutics, 19(10):2044-2056) that demonstrated the requirement for amivantamab interaction with Fc in MET amplified and EGFR mutant model systems.
[0200] Example 3. Relationship between receptor expression and signaling and in vivo efficacy of amivantamab. Next, the relationship between receptor expression or signaling and the in vivo efficacy of amivantamab was evaluated.
[0201] IHC and PLA assays were performed as described in Example 1. Amivantamab efficacy (%TGI) in PDX tumors with EGFR lacking activating mutations was plotted with respect to EGFR and MET IHC H-scores and PLA scores (see Figures 3A-D). For EGFR, a positive correlation was determined between efficacy and both IHC and PLA (IHC: r=0.397, PLA: r=0.22), but none of the correlations were statistically significant (p>0.05). No positive correlation was observed between tumor efficacy and MET expression or signaling.
[0202] Example 4. Genomic analysis of PDX tumors. Next, the association between expression and mutation status of PDX tumors with EGFR lacking activating mutations and the tumor growth inhibition % obtained in Example 2 was evaluated. The expression data and mutation status of common cancer genes in PDX tumors with EGFR lacking activating mutations used in Example 2 were provided by Charles River Laboratory (Freiburg, Germany) and are wholly owned. Expression of the common cancer genes listed in Figure 4A was determined by RNA-Seq and mutation status was determined by whole exome analysis (Figure 4B). Amivantamab was highly effective in many of these NSCLC PDX models with EGFR lacking activating mutations (Figure 4C), but mutations in the KRAS and PI3K pathways were identified as biomarkers of reduced anti-tumor activity (see Figures 4A-C).
[0203] When assessing EGFR correlation to efficacy, we observed a subset of models in which the observed %TGI tended to fall below the linear fit of the data set, but with relatively high EGFR levels (Figure 3A).Interestingly, this subset of models was found to have mutations in the KRAS or PI3K pathways, suggesting that aberrant signaling in these pathways may reduce the activity of amivantamab.
[0204] Example 5. PDX tumors lacking KRAS and PI3K pathway mutations were associated with increased efficacy of amivantamab. Next, the association of amivantamab efficacy (%TGI) with the combined H-score and PLA-score (see Table 1) was evaluated in a subgroup of PDX tumors from Example 2, where tumors positive for known tumorigenic driver mutations downstream of EGFR and MET, such as KRAS and PI3K, were removed (see Table 2). Correlation coefficients and p-values were calculated using Spearman's correlation coefficient and two-tailed tests (Prism Graphpad 7.00) as described in Example 1.
[0205] Results showed that elimination of tumors with KRAS or PI3K pathway mutations resulted in a statistically significant correlation between amivantamab efficacy and EGFR expression and signaling (IHC+PLA-r=0.88, p=0.0032, see FIG. 5A). Indeed, models with EGFR H-score≧170 and no alternative driver mutations achieved long-term tumor stasis or complete tumor regression upon treatment with amivantamab (representative tumor growth curves are shown in FIG. 2A and FIG. 2B). No correlation between amivantamab efficacy and MET expression and signaling was observed (FIG. 5B).
[0206] [Table 1]
[0207] [Table 2]
[0208] Example 6. Analysis of the association between EGFR ligand expression and amivantamab efficacy in PDX tumors with EGFR lacking activating mutations. Next, the expression of EGFR ligands in PDX tumors with EGFR lacking activating mutations was evaluated in relation to the tumor growth inhibition % obtained in Example 2. The expression of epidermal growth factor (EGF), amphiregulin (AREG), transforming growth factor alpha (TGFα), heparin-binding EGF-like growth factor (HBEGF), betacellulin (BTC), epiregulin (EREG), and epigen (EPGN) in PDX tumors with EGFR lacking activating mutations used in Example 2 was determined using RNA-Seq. These data were provided and fully owned by Charles River Laboratory (Freiburg, Germany). Amphiregulin expression was only available for 11 of the 14 NSCLC models in which in vivo efficacy was tested (no data was available for LXFA2158, LXFA2165, and LXFA2201). A statistically significant correlation (r=0.66, p=0.03) was found between amphiregulin expression and the in vivo efficacy of amivantamab (Figure 6). This association between amphiregulin expression and the efficacy of amivantamab was independent of KRAS and PI3K pathway mutations. Thus, amphiregulin expression can identify patients who are likely to derive clinical benefit from treatment with amivantamab.
