[0001] The present invention relates to a surface-modified artificial skull fragment for stable engraftment during cranioplasty and a method for manufacturing the same. Background Technology

[0003] Cranioplasty is a surgical procedure that restores the skull to its original shape by inserting artificial bone into the skull defect area following a craniotomy. An emergency craniotomy is performed to control malignant cerebral edema, and if the patient survives, cranioplasty is performed to restore the skull defect area.
[0004] Previously, cryopreserved autologous skulls were used, but their use has declined due to concerns regarding the risk of infection and auto-absorption. As an alternative, surgeries are now performed using materials other than bone. While bone cement was previously used, it has low strength and can result in significant cosmetic variations depending on the surgeon.
[0005] Recently, 3D printers are being used for fabrication, primarily using metal (titanium) or PEEK (Polyether Ether Ketone) materials. Once manufactured, titanium cannot be deformed during surgery. Additionally, issues such as metal allergies continue to be raised. On the other hand, PEEK has very low biocompatibility and allows for deformation, such as additional trimming as needed during surgery, enabling more efficient operation.
[0006] However, in the case of PEEK material, there is a problem in that the scalp does not engraft properly intermittently, and cerebrospinal fluid (CSF) collection occurs due to cerebrospinal fluid leakage (Fig. 1). This is believed to be caused by the fact that when PEEK is manufactured, the surface is made smooth, which hinders the engraftment of the scalp.
[0007] Treatment for this requires opening a new bone fragment and suturing the cerebrospinal fluid fistula or dural defect. However, there are cases where the dura mater is completely dissolved (Fig. 2), and there are also risks associated with general anesthesia. [0038] A 61-year-old female patient visited the emergency room of the Regional Trauma Center of our hospital due to a fall. The patient was in a state of reduced consciousness, unable to open her eyes or speak. An emergency head CT scan revealed acute subdural hemorrhage and acute epidural hemorrhage. Consequently, emergency craniotomy and hematoma removal were performed. Due to the presence of severe cerebral edema, the skull could not be reattached and was removed, after which the skin was sutured. Following the surgery, the patient was placed in the neurosurgery intensive care unit to attempt to lower intracranial pressure using medication. Thirty days after the surgery, the patient regained consciousness and underwent rehabilitation therapy while some cognitive, speech, and motor impairments remained. To address the intracranial pressure reduction caused by the skull defect, a patient-specific artificial bone fragment was fabricated using newly performed CT images. Surgery was performed on January 13, 2025 (Fig. 9) using an artificial skull fragment manufactured according to an embodiment of the present invention, rather than a conventional smooth surface. The patient was discharged after surgery with no cerebrospinal fluid leakage and proper engraftment of the scalp, and is currently undergoing outpatient follow-up without any particular issues.
