Abstract
The invention relates to medicine, in particular to otosurgery and auricular reconstructive surgery, and can be used for treating conductive, mixed, or unilateral hearing loss by implanting an audotory system with peri-implant protection in patients with thin soft tissues, postoperative scars, or multiple previous surgical interventions.Summary of the invention consists in that a computed tomography of the temporal bone of the side requiring implantation is performed in the preoperative period, then using 3D reconstructions, the bone thickness, the position of the sigmoid sinus, and thedura materare assessed, with landmarks established in the superior retromastoid region. General anesthesia is performed, and the patient is placed in dorsal decubitus, with the head rotated contralateral to the ear required for implantation of the auditory system, after which a curved or linear retroauricular incision is made, the soft tissues are dissected down to the periosteum, and a wide periosteal flap is raised with exposure of the mastoid region, the position of the auditory implant is marked using a template for positioning the auditory implant, then a cavity is milled in the bone with a depth corresponding to the thickness of the aforementioned implant, which is introduced into the formed cavity and fixed with monocortical screws through the holes of the titanium anchor plate, and the auditory implant cable is placed without tension in the grooves prepared in the bone or subperiosteum, then the periosteum is refixed, after which a musculocutaneous vascular pedicle flap from the temporal muscle is prepared in the posterior cervical region, the surgical field is prepared by removing necrotic edges of the skin, and the prepared flap is rotated and placed over the auditory implant and fixed with sutures or anchor devices for mechanical stability.
