Providing a continuity of care across multiple care settings

Abstract

A method for providing a continuity of care for a patient during transfer between care settings, comprising: a) receiving, from a first care setting, a first evaluation of: i) a plurality of pressure injury risk factors, ii) a plurality of signs of skin damage or pressure injury of at least one body location of said subject, iii) a nutrition level of said patient, and iv) a current intervention; b) performing a first sub-epidermal moisture (SEM) scan of said at least one body location of said subject, c) identifying a delta value based on said first SEM scan; d) assigning a risk level to said subject based on said delta value and said first evaluation; e) identifying an intervention level corresponding to said risk level and performing SEM scans at a predetermined frequency corresponding to said risk level; and f) evaluating said condition of said subject based on a second evaluation of: i) said plurality of pressure injury risk factors, ii) said plurality of physical signs of skin damage or pressure injury of said at least one body location of said subject comprising said SEM scans, iii) a nutrition level of said patient, and iv) said intervention.
