Abstract
Description
[0001] The utility model relates to medicine, namely to therapeutic dentistry and endodontics, and is intended to increase the effectiveness of antiseptic treatment of the root canal system of the tooth during the treatment of pulpitis and periodontitis using photodynamic therapy.
[0002] A known attachment for a LED phototherapeutic device (utility model RU No. 202645, A61N5 / 06, March 1, 2021) consists of an optically transparent mounting element and an optical part, which is individually manufactured for each patient using 3D printing technology. The design includes a vestibular plate that follows the curvature of the alveolar process and a palatal plate with bite pads. The device is designed for use with the highly specialized AFS Harmony device. The claimed technical result includes therapy in a closed-mouth environment, targeted light penetration, expanded device functionality, and increased treatment comfort.
[0003] However, this design of the utility model is focused on the treatment of diseases of the mucous membranes of the oral cavity - (periodontitis, gingivitis, etc.) with the treatment of large areas, the design of the device is adapted to cover the anatomical structures of the jaws.
[0004] The technical result achieved by using the claimed utility model is an increase in the efficiency of activation of a photoactive irrigant during photodynamic therapy of the tooth root canal system using a polymerization lamp.
[0005] This technical result is achieved by a custom-made tip for activating a photoactive irrigant, a single-piece structure composed of a matte photopolymer material for 3D printing, allowing for heat sterilization. One end of the custom-made tip follows the shape of the tooth crown and is shaped with a flange, while the other end follows the shape of the light guide of the curing lamp. A guide channel is located inside the custom-made tip, which serves to focus the light.
[0006] The custom-made tip is secured to the light guide of the curing lamp using a tight fit. The precise shape of the curing lamp tip creates sufficient contact pressure, preventing friction from dislodging or removing the custom-made tip during operation. A guide channel runs through the entire custom-made tip to focus the light beam to the desired location. The tip can be custom-made for each patient using additive 3D printing technology from a heat-sterilizable photopolymer material. Printing of the custom-made tip is performed using a customized 3D model of the custom-made tip, based on a preliminary 3D image of the patient's oral cavity.
[0007] The design of the individual nozzle is shown in the images, where Fig. 1 is a top-side view of the individual nozzle, Fig. 2 is a bottom-side view of the individual nozzle, where 1 is the end of the individual nozzle, repeating the shape of the light guide of the polymerization lamp, 2 is the end of the individual nozzle, repeating the shape of the crown of the tooth, 3 is a flange at the end of the individual nozzle repeating the shape of the crown of the tooth, 4 is an internal guide channel.
[0008] The custom-made attachment is a single-piece structure (Fig. 1, Fig. 2) that delivers radiation from the polymerization lamp to the treatment area. One end of the custom-made attachment 1 precisely matches the shape of the polymerization lamp's light guide, while the other end 2 matches the shape of the tooth crown. A flange 3 is provided on the edge of this end of the custom-made attachment for secure attachment and a more secure fit to the tooth crown. A guide channel 4 is located within the custom-made attachment, which serves to focus the light.
[0009] In practice, the individual nozzle is used as follows.
[0010] After standard mechanical preparation, opening of the canal orifices, and intraoral scanning of the jaws using an optical scanner, a custom-made tip is designed using a specialized program and printed on a 3D printer using a matte photopolymer material that can be sterilized using a heat sterilization method. A photoactive irrigant is then injected into the root canal using an endodontic syringe. One end of the tip 1 is fixed to the light guide of the LED polymerization lamp, and the other end of the custom-made tip 2 is fixed to the coronal portion of the tooth. The photoactive irrigant is activated through the internal guide channel 4.
[0011] The device allows to increase the efficiency of photoactive irrigant activation during photodynamic therapy of the tooth root canal system using a polymerization lamp, and also to minimize the risk of thermal damage to surrounding soft tissues.
[0012] This custom-made attachment can be used in conjunction with a polymerization lamp for photodynamic therapy (PDT) treatment of pulpitis and periodontitis. It ensures targeted light penetration to the desired treatment areas, increasing the comfort and effectiveness of the procedures.
