Abstract

A wrist or finger mounted wearable device and method for determining a state or activity level of the heart and the autonomic nervous system, such as irregular heart beats, Atrial Fibrillation and the respiratory rate and the parasymphatetic- and symphatetic nervous system influence on heart beats. The wearable device having at least a pair of electrocardiograms (ECG) measurement electrode pads configured to perform hand-to-hand ECG measurements. The device or method including monitoring of the pair of ECG measurement pads at a first time period to arrive at a first reading of hand-to-hand ECG, and at a second time period to arrive at a second reading of hand-to-hand ECG. The device or method analyzing the first and second readings of ECG to arrive at various indications of the state of a user. The test protocols for establishing the necessary biomarkers and analytics to test and monitor heart rhytm problems, parasymphatetic immune system changes, stress and mental health. The method can also contain machine learning and artificial intelligence models to personalize the health analytics and monitoring.

DEVICES, METHODS, AND TEST PROTOCOLS FOR MEASURING, ANALYZING, AND ESTABLISHING CARDIAC- AND GENERAL HEALTH BIOMARKERS WITH NON-INVASIVE SINGLE LEAD HAND-TO-HAND ELECTROCARDIOGRAM TESTSBACKGROUND

[0001] The vagus nerve is the main part of the body's parasympathetic nervous system. It is the longest and most complex of the 12 pairs of cranial nerves. It transmits information to and from the brain to organs in the body such as the gut and heart. The parasympathetic nervous system is also known as the ‘rest and digest’ system. The ‘opposite’ of this system is the sympathetic nervous system which is known as the ‘fight and flight’ system. It is activated when the body is in stress such as during disease progress.

[0002] The autonomic nervous system is the main nervous system involved in the body’s stress response. The parasympathetic nervous system is a very important part of the autonomic nervous system which enables, for instance, relaxation. The vagus nerve is also the most important nerve for immune system responses and it is considered to be very important in the treatment of autoimmune diseases, depression, anxiety, and other diseases that require lowering or raising the immune system responses.

[0003] Recent advances in immunology reveal a significant pathogenic role for inflammation in the development and progression of inflammations in cardiac-, neurological- or respiratory systems. The worldwide CO VID-19 epidemic revealed a lack of digital health monitoring and early detection systems for such diseases.

[0004] The heart's rhythm and pulse are primarily regulated by neurological signals that originate from the autonomic nervous system (ANS) and the heart's intrinsic conduction system. The Autonomic Nervous System consists of: A) The Sympathetic Nervous System which increases heart rate, contractility, and conduction velocity through the release of neurotransmitters like norepinephrine, which act on beta-adrenergic receptors in the heart. B) The Parasympathetic Nervous System which decreases heart rate and conduction velocity through the vagus nerve, which for instance can act on the heart by releasing acetylcholine, a neurotransmitter that acts on muscarinic receptors in the heart. The heart's intrinsic system isC) The Sinoatrial (SA) Node, located in the right atrium. It is known as the heart's natural pacemaker. It generates electrical impulses that initiate each heartbeat, setting the pace for heart rate. The rate at which these impulses are generated can be influenced by the ANS. D) The Atrioventricular (AV) node, located at the junction between the atria and ventricles, receives impulses from the SA node and delays them slightly before passing them on to the ventricles. This delay ensures that the atria contract and empty their blood into the ventricles before the ventricles contract. E) The His-Purkinje System which conducts impulses from the AV node throughout the ventricles, causing them to contract in a coordinated manner for efficient blood ejection.

[0005] The parasympathetic nervous system (Vagus nerve) is one of the main contributors affecting pulse, heart rhythm, and adjusting heart rate variability (HRV) to optimize oxygenation and homeostasis. It is also known to contribute to irregular heartbeat patterns such as Atrial Fibrillation and Ectopic heartbeats.SUMMARY OF THE INVENTION

[0006] The invention is defined by the features of the independent claims. Some specific embodiments are defined in the dependent claims.

[0007] According to the first aspect of the present invention, there is provided a wrist or fingermounted wearable device for determining electrocardiogram (ECG) electrical heart signals which are used for instance to determine the activity level of the autonomic nervous system and respiratory rate. These are used to determine the activity of the vagus nerve. The wearable device comprises at least a pair of electrocardiogram (ECG) measurement electrode pads configured to perform hand-to-hand ECG measurements, at least one processing core, and at least one memory including computer program code, with at least one memory and the computer program code being configured to, with the at least one processing core, cause the wearable device to: monitor the pair of ECG measurement pads during a first test period to arrive at a first reading of hand-to-hand ECG, following the first test period, provide for instructions for the user to perform tasks such as to move body posture or to breathe in a specified fashion for a specified period. The software application may then advise to execute a second test period, monitor the pair of ECG measurement pads during the second test period while providing instructions to perform for instance controlled breathing, to arrive at a second reading of hand-to-hand ECG, analyze the first and second readings of ECG to arrive at indications of at least one of: a minimum, maximum and average pulse, a size of pulse changes, a smoothness of pulse changes, the pattern of ECG measured as PQRST pulse wavecomponents, the amplitude of PQRST, the relationship and distances between each PQRST wave points, heart rate variability, smoothness of pulse variability, a respiratory rate, depth of respiration, and a smoothness of respiration.

[0008] According to a second aspect of the present invention, there are defined sets of testing procedures which are used to reach more indepht insight into the test persons health. These are called ‘ Vagus test protocols ’ which compromise for instance:a) The ‘Basic test’ protocol which is one at least 30 second time-period ECG recording where the user is performing normal, subconsciouly determined breathing,b) The ‘Controlled breathing test’ protocol which is at least one 30-second time-period ECG recording where the user is breathing in a predetermined fashion, such as the system instructing the user to breathe at a particular rate, in particular, to breathe at a frequency of 0.1 Hzc) The ‘ Vagus POTS ECG test’ where the user is first performing the ECG recording while sitting and then during the 2ndECG recording, stands up and leaning arms / hands on a surface which is at the level of the chest.d) The ‘Valsava maneuver Vagus test’ where the user performs a ‘Valsava maneuver’ between the 1stand the 2ndtest.e) The ‘ Vagus Cold Immersion test ’ where the user, as intervention is placing a body part (normally the right foot) in ice cold water during the 2ndtest phase to induce stress-related changes in the heart rhytm patterns.f) The ‘Digestion intervention test’ where the user is consuming pharmacological products, foodstuffs, drinks, or other digestible or nicotine products between the 1stand the 2ndtest.

[0009] The third and fourth claims determine the uniqueness of how the system is establishing biomarkers, analytics, and artificial intelligence-based monitoring for the health of the user. There are methods of determining the state of cardiac health and general health for an individual by monitoring primarily the PQRST during base- and controlled breathing of an individual via hand-to-hand electrocardiograph. These analytics methods are then applied to the different test protocols which enable a wide range of health findings.BRIEF DESCRIPTION OF THE DRAWINGS

[0026] Heart rate variability (HRV) provides for a method to evaluate vagal tone or vagus nerve activity. However, HRV has a major statistically originating flaw when used in short-term measurements. Since the heart beats only on average once per second during a resting heart rate - the statistical accuracy when measuring only 20 or 30 heartbeats (25-second test), is on average low. However, methods and devices according to embodiments of the present invention monitor amplitude changes and electrocardiograms (ECG) PQRST-peaks and internal patterns to arrive at a metric that can reveal consistent early indicators for the onset of disease. Further, the amplitude changes and electrocardiogram’s (ECG) PQRST-peaks, internal patterns, and irregular heartbeats and their amplitude and patterns act as consistent early indicators for the onset of disease.

[0027] Certain embodiments of the present invention provide for a Vagus Test as a novel standardized test protocol enabling disease, immune system and vagus nerve measurement and assessment. In recording and analysis of more than 75 000 hand-to-hand ECGs, embodiments of the present invention have been proven to detect changes in the cardiac -respiratory synchronization (RSAsync) patterns when the users have become sick or have mentally derived issues which affect the vagus nerve. These changes in RSAsync can be used for early indicators of disease, infection and continuous monitoring of disease. Such changes can also be used to monitor the effects of medication.

[0028] RSAsync according to certain embodiments of the present invention is a measurement of cardiac / respiratory synchronization. RSAsync patterns can provide for reliable and consistent indication when users have become sick or have mentally derived issues which affect the vagus nerve. Heart rate and breathing are generally considered to be synchronized by the oxygenation effect, whereby heart rate increases at inspiration and decreases at expiration. It has been shown that when the immune or nervous systems in general are disrupted, this synchronization is altered or disappears. These changes in RSAsync can be used for early indicators of disease, infection and continuous monitoring of disease- and for instance medication effects.

[0029] When studying the respiratory and RSAsync effects in hand-to-hand ECG during a controlled breathing test period, a systematic correlation between the user’s cardiac health and the smoothness of the pulse curve has been proven. Similarly, there is a systematic correlationbetween the smoothness of the respiratory data curve and the user’s reported respiratory health problems. A mathematical model to establish this smoothness is provided per certain embodiments of the invention and may be references as Cardiac Sinus Smoothness (CSS) and Respiratory Sinus Smoothness (RSS).

[0030] PQRST patterns, Ectopics, Irregular Heart Rhytm, Atrial Fibrillation, RSAsync, CSS and RSS can reliably be monitored and measured with the system presented herein comprising at least a ‘hand-to-hand ECG watch or ring which implements the standardized Vagus Test protocol consisting of at least a period of normal resting ECG measurement followed by a controlled breathing phase where the user is instructed how to conduct regular inhales / exhales. For example, regular inhales / exhales each lasting at least 5 seconds. Within at least some embodiments a cloud based analytics system is employed which uses big data processing.

[0031] According to embodiments of the present invention, the electrical potential difference is measured directly between hands as the amplitude of voltage measurements changes when the person inhales and exhales. This was first studied, recognized and documented by Dr. Brody in 1950s. Hence the commonly used ‘Brody effect’ for ECG voltage amplitude changes due to breathing. The reason for these voltage amplitude changes are as follows. During inspiration, there is diaphragmatic contraction in the heart that, because the pericardium is attached to the central tendon of the diaphragm, causes caudal and rotational movement of the heart (and vice versa). This rotational movement in term cause changes in conduction especially in the direction of aVR and aVL electrodes in standard 12-lead ECG measurement. This amplitude change is visible as changes in the R and S amplitude in the hand-to-hand ECG and it is the reason why breathing can be calculated from ECG measurements.

[0032] Embodiments of the present invention enable a new method and test protocol to calculate the synchronization discussed above and its implications with the use of hand-to-hand ECG-derived respiration (EDR) periods. The diagnosis provides explanations for the surprisingly common alternations to this synchronization (coherence) of breathing and heart rate sinus arrhythmia.

CLAIMS:

A wrist or finger-mounted wearable device (100) for determining electrocardiogram (ECG) and a state or activity level of the autonomic nervous system, such as the respiratory rate, heart rate, or other indications of the parasympathetic nervous system, the wearable device and system comprising:- at least a pair of electrocardiograms (ECG) measurement electrode pads (120, 122) configured to perform hand-to-hand ECG measurements,- at least one processing core and at least one memory including computer program code, the at least one memory and the computer program code being configured to, with at least one processing core, cause the wearable device (100) to:- monitor the pair of ECG measurement pads (120, 122) during the test period, while providing instructions to perform controlled breathing, to arrive at a reading of hand-to-hand ECG,- system analyzing the ECG to arrive at indications of at least the following:- Pulse and pulse changes- Cardiac PQRST pattern- Cardiac Arrhythmia detection- Respiratory rate- Vagus test protocols that instruct the user how to perform the tests, with the help of the device, a smartphone, or a computer.2 The wrist or finger-mounted wearable device of any preceding claim, wherein the device is further configured to provide the instructions and enable the user to perform the Vagus test rotocols which compromise at least one of the following:- record an ECG at least for one 30-second time period where the user is performing normal, subconsciously determined breathing ( ‘Vagus base test’).- perform controlled breathing during at least one 30-second time period where the user is breathing in a predetermined fashion, such as the system instructing the user to breathe at a particular rate, in particular, to breathe at a frequency of 0.1 Hz. (‘Vagus controlled breathing test ’)- perform body posture changes, such as instructing the user to perform the 1sttest while sitting down and the 2ndtest while standing up and leaning arms / hands on a surface which is at the level of the chest. This test protocol is called the ‘Vagus POTS ECG Test'.- perform interventions such as the commonly known ‘ Valsava maneuver’ between the 1stand the 2ndtest. This test protocol is called the ‘Vagus Valsava Smartwatch ECG Test'.- perform interventions such as placing a body part in ice-cold water during the 2ndtest phase to induce stress-related changes in the heart rhythm patterns. This test protocol is called the ‘Vagus cold immersion ECG test’.- consumption of pharmacological products, foodstuffs, drinks or other digestible interventions between the 1stand the 2ndtest.- any other combination of the above-mentioned interventions or actions.- more consecutive tests whereby the user performs more than 2 ECG recordings over longer period while performing one or several of the abovementioned interventions or actions.3 A method of determining a state of health for an individual by monitoring any of the above-mentioned Vagus test protocols whereby the method is analyzing at least the hand-to-hand electrocardiogram (ECG), the method comprising the steps of:- processing each ECG recording by the systems cloud-based computer software to establish necessary biomarkers which are required to determine the necessary health analysis - utilizing artificial intelligence systems and platforms to perform the analysis and find individual and common patterns and health diagnostics.- The system is analyzing the data in order to establish new personalal biomarkers representative of the users state of health. These biomarkers being PQRST pattern smoothness (PQPS), S / R peak wave relation (S / R%), R-T wave peak timing patterns and variation (R-Tx), Ectopic heart beat index (ECTx), Atrial Fibrillation Index (AFibX), Breathing Index (BI), Respiratory Sinus Arrythmia (RSA), Respiratory Sinus Arrythmia Synchronization (RSAsync), Respiratory Sinus Smoothness (RSS), Respiratory rate adjusted according to heart rate variability (Rhrv), Cardiac Sinus Smoothness (CSS) and Parasy mphatetic cardiac influence index (PCIx).

The method of claim 3, wherein a plurality of indices, such as at least two of the biomarkers are calculated and combined into a matrix and there comparing with Artificial Intelligence the latest test results to the persons historical values and reference group values, and from these results establishing a trend which forms the basis for a personalized health care monitoring and diagnostics.
