Abstract

A system for guiding members along personalized health pathways includes a database storing health-related data and completed actions for multiple members. A recommendation module accesses this data to determine the next best health-related action by calculating a propensity score for each possible action, reflecting the member's likelihood to complete it, and a health score, reflecting the estimated health impact. These scores are combined to identify the optimal next action. A communication module then relays this action to the member via a network and receives completion data confirming the action's completion.
