Abstract
Description
TECHNICAL FIELD
[0001] The present invention relates to an electronic sphygmomanometer having a cuff attached to a blood pressure measuring site and a blood pressure calculating unit for calculating a blood pressure value from a cuff pressure, and a blood pressure measuring method using the sphygmomanometer. BACKGROUND OF THE INVENTION
[0002] Blood pressure is a type of indicator for diagnosing cardiovascular disease. Conducting risk assessment based on blood pressure is effective in preventing cardiovascular diseases such as stroke, cardiac arrest, and heart attack. Typically, a diagnosis for risk assessment is made based on blood pressure (casual blood pressure) measured in medical facilities during hospital visits and checkups. However, recent research has shown that blood pressure measured at home is more effective in diagnosing cardiovascular disease than casual blood pressure. Accompanying this, home blood pressure monitors are widely used.
[0003] Most of the currently commonly used electronic blood pressure monitors use the blood pressure calculation algorithm of the oscillometric method or the microphone method.
[0004] In the oscillometric method, the change in arterial volume is detected as a pressure pulse wave amplitude, which occurs when the cuff is inflated to a predetermined pressure (e.g., systolic blood pressure + 30 mmHg) and then gradually released at a predetermined rate (decompression rate). A predetermined algorithm is applied to the change in pressure pulse wave amplitude to calculate the blood pressure value. The change in arterial volume that occurs when the cuff is inflated gradually at a predetermined rate (decompression rate) can be detected as a pressure pulse wave amplitude, and a predetermined algorithm can be applied to the change in pressure pulse wave amplitude to calculate the blood pressure value.
[0005] In the microphone method, the appearance, attenuation and disappearance of the Korotkoff sounds, which occur when the cuff pressure is applied to a predetermined pressure (e.g. systolic blood pressure + 30 mmHg) and then gradually released at a predetermined rate (pressure release rate), are detected with a microphone arranged in the cuff for determining the systolic blood pressure and the diastolic blood pressure.
[0006] For both blood pressure calculation methods, the cuff pressure must be applied to a cuff pressure (e.g., systolic blood pressure + 30 mmHg) that is sufficiently greater than the systolic blood pressure. The pressure release rate and the pressure application rate must be set such that the information about the pressure pulse wave amplitude or the Korotkoff sounds can be fully preserved.
[0007] The document DE 10 2006 057 148 A1 discloses a vital sign monitoring system that can be used for multiple patients and uses historical patient data information for the patient to optimize the recording process of current vital sign measurements,
[0008] The document JP 3 117 972 U discloses a blood pressure monitor that can optimize the pressure applied to the wristband even when the maximum blood pressure is different between the morning and evening measurements or when it is used by several monitors. (State of the art pressurization value)
[0009] In the blood pressure measuring devices, methods such as (1) setting a pressurization value with a changeover switch before measurement (see JP S62 - 66 835A), (2) estimating a systolic blood pressure during pressurization and setting a pressure obtained by adding a predetermined pressure (e.g., 30 mmHg) to the estimated systolic blood pressure as a pressurization value (see JP 2 842 696 B2 and JP 3 393 432 B2), and (3) setting a pressure obtained by adding a predetermined pressure (e.g., 30 mmHg) to an average of the systolic blood pressure values up to the previous measurement as a pressurization value (see JP 2005 - 185 681 A) have been used so far. DISCLOSURE OF THE INVENTION PROBLEMS SOLVED BY THE INVENTION
[0010] Blood pressure varies greatly due to factors such as the time of measurement, the day of the week, the season, the environment, stress, illness, and various lifestyle-related activities (meals, exercise, smoking, rest), etc. Thus, in the method of (1), the patient's systolic blood pressure must be known in advance, but it does not necessarily need to be correctly determined, even if the pressurization value is selected based on the previously measured blood pressure value, because blood pressure fluctuates greatly once the measurement is performed.
[0011] Similarly, in the method (3), it does not necessarily need to be set correctly even if an average value or the like of a plurality of measurement results is used.
[0012] Furthermore, when calculating blood pressure during cuff pressure release, the cuff pressure must be applied as quickly as possible for accurate blood pressure measurement.
[0013] If the pressurization is performed at a low speed, blood will stagnate at the peripheral side (forearm in the case of upper arm measurement) of the measurement site before the artery is completely closed, making accurate measurement impossible. Therefore, the pressurization rate is set as high as possible. As a result, the pulse wave information during pressurization becomes less, and the error in the estimated systolic blood pressure becomes large in method (2), and thus the pressurization value may not be accurately determined.
[0014] In view of the problems described above, it is an object of the present invention to provide an electronic blood pressure monitor and a blood pressure measurement method capable of setting an optimal blood pressure measurement parameter for each user by correcting a blood pressure measurement parameter based on information recorded in a past measurement which is close to the conditions at the time of blood pressure measurement, and improving the user's level of satisfaction. [0031] An embodiment of the present invention will be described below with reference to the drawings. [Example]
[0032] As in Fig. 1, an electronic blood pressure monitor 2600 of a first embodiment includes a cuff 2101, an air tube 2102, a pressure sensor 2103, a pump 2104, a valve 2105, an oscillation circuit 2111, a pump control circuit 2112, a valve control circuit 2113, a timing control unit 2115, a power supply unit 2116, a CPU 2120, a display unit 2121, a memory (for processing) 2122, a memory (for recording) 2123, an operation unit 2630, an interface 2171, and a recording medium 2172. Fig. 1 is a block diagram showing a configuration of the electronic blood pressure monitor 2600 of the first embodiment.
[0033] The cuff 2101 is a band-shaped member connected to the air tube 2102 and attached to a user's blood pressure measuring site for applying air pressure.
[0034] The pressure sensor 2103 is an electrostatic capacitive pressure sensor in which a capacitance value changes according to the pressure in the cuff (cuff pressure).
[0035] Pump 2104 and valve 2105 apply pressure to the cuff and adjust (control) the pressure in the cuff. Oscillating circuit 2111 outputs a signal with a frequency corresponding to the capacitance value of pressure sensor 2103. Pump control circuit 2112 and valve control circuit 2113 drive pump 2104 and valve 2105, respectively.
[0036] The timing unit 2115 is a device for timing the current date and time and transmits the time-controlled date and time to the central unit 2120 as required. The power supply unit 2116 provides electrical power to each configuration unit.
[0037] The central unit 2120 carries out the control of the pump 2104, the valve 2105, the display unit 2121, the memories 2122, 2123, the operation unit 2630 and the interface 2171, the setting process for the blood pressure and the management of the recorded values.
[0038] The display unit 2121 is constituted by a display device such as a liquid crystal display, and displays the blood pressure value according to a signal transmitted from the central unit 2120.
[0039] The memory (for processing) 2122 stores the control program or the like of the blood pressure monitor.
[0040] The memory (for recording) 2123 stores the blood pressure value and also stores the date and time, the measured value and the measurement status by linking.
[0041] In addition to the power switch 2131, the measurement switch 2132, the stop switch 2133, and the data output switch 2141, the operation unit 2630 further includes a measurement state input switch 2641 for inputting measurement states in which operation input, such as turning the blood pressure monitor on / off and starting measurement, is permitted. The input signal is transmitted to the central processing unit 2120. The interface 2171 performs the recording / reading of the blood pressure value with respect to the recording medium 2172 under the control of the central processing unit 2120.
[0042] The measuring state input switch 2641 is designed to allow the input of the dosing status whether or not after dosing for the measuring state.
[0043] The blood pressure measuring operation using the blood pressure monitor 2600 configured as described above is carried out in accordance with the flowchart of Fig. 2 are described. Fig. 2 is a flowchart showing the blood pressure measurement operation in the present example.
[0044] When the power button of the blood pressure monitor is pressed (step S2601), the central unit 2120 first initializes the operation memory of the blood pressure monitor and performs a 0 mmHg setting of the pressure sensor 2103 (step S2602).
[0045] After the initialization process is completed, the measurement conditions, such as after dosing, are input (step S2603) and the measurement switch is pressed (step S2604), so that the central unit 2120 acquires the current date and time from the time control unit 2115 (step S2605) and sets one or more of the blood pressure measurement parameters, such as the pressurization value, the pressurization rate, and the depressurization rate (step S2606). The method of setting the blood pressure measurement parameters in step 2606 will be described later.
[0046] The cuff pressure is then pressurized by the pump 2104 to a predetermined pressurization value (steps S2607 to S2608). The pressurization rate and pressurization value used here are the pressurization rate and pressurization value set in step S2606. After pressurizing to the predetermined pressurization value, the central unit 2120 gradually releases the cuff pressure with the valve 2105 (step S2609). The central unit 2120 extracts the pressure change component involved in the volume change in the artery, which is superimposed on the cuff pressure obtained during the depressurization, and calculates the blood pressure using a predetermined calculation (step S2610). After calculating the blood pressure (step S2611: YES), the central unit 2120 opens the valve 2105 and releases the air in the cuff. The central unit 2120 displays the calculated blood pressure value on the display unit 2121 (step S2612) and records the same in the memory (for recording) 2123 in association with the date and time, the measured value, and the measurement state (dosage status) (step S2613).
[0047] In step S2606, the central unit 2120 sets the measurement parameter for blood pressure in the following manner. (If the measurement parameter for blood pressure is the pressurization value)
[0048] The pressurization value must generally be set to a cuff pressure sufficiently greater than the systolic blood pressure. In particular, the pressurization value must be approximately equal to the systolic blood pressure + 30 mmHg. The pressurization value is set based on the recorded measurement data. In particular, the central unit 2120 outputs a table as shown in Fig. 3, which is recorded in the memory (for recording) 2123. The table recorded in the memory (for recording) 2123 used herein records the measurement date and time ("YY (Year) / MM (Month) / DD (Day)" ... "HH (Hour) : mm (Minute) ... in the figure), the dosing status (measurement state "after dosing" in the figure), and each measurement value, such as the blood pressure value and the pulse rate ("SYS 1 (Systolic Blood Pressure)", "DIA 1 (Diastolic Blood Pressure)", "PLS 1 (Pulse Rate)") in the figure) in association with each other, as shown in Fig. 3 shown.
[0049] The central unit 2120 then extracts the data closest to the current measurement date and time and / or the data in which the measurement condition indicating the dosing status or the like, such as whether or not after dosing, is the same from the measurement data recorded in the table.
[0050] The central unit 2120 sets the pressure obtained by adding 30 mmHg to the systolic blood pressure of the latest data of the measurement data that agrees with the above condition as the pressurization value.
[0051] When reference is made here to closest to the measurement date and time, it means that at least one or more of the conditions, including the same month of measurement, measurement in the same week, measurement on the same day of the week, and measurement at the same time, coincide with each other.
[0052] Not limited to using the systolic blood pressure of the most recent data of the measurement data consistent with the above conditions as described above, an average value of the systolic blood pressures from a plurality of measurement data consistent with the conditions may be used, and the pressure at which 30 mmHg is added to this average value may be set as the pressurization value.
[0053] Alternatively, a representative systolic blood pressure value from a plurality of measurement data consistent with the conditions may be used, and the pressure at which 30 mmHg is added to this representative value may be set as the pressurization value. The representative value may be a maximum value, a median value, or the like from the plurality of data.
[0054] If measurement data consistent with the measurement condition does not exist, such as when the blood pressure monitor is first used, the preset pressure value (e.g., 180 mmHg) can be set. The systolic blood pressure can be estimated during pressure application, and the pressure at which 30 mmHg is added to the thus estimated value can be set as the pressure value, as described in JP 2 842 696 B2 and JP 3 393 432 B2. (If the measurement parameter for blood pressure is the pressure relief value)
[0055] The pressure release rate must be set at a rate at which pulse wave amplitude information can be acquired in a reasonable proportion to the number required for blood pressure measurement. The required number of pulse wave amplitude pieces is approximately five or more, as described in JP 3 149 873 B2. As described in the same document, the pressure release rate that satisfies this requirement can be calculated using the systolic blood pressure (SYS), the diastolic blood pressure (DIA), and the pulse rate (PLS).
[0056] Similar to the pressurization value, the data closest to the current measurement date and time and / or the data where the measurement state is the same are extracted from the recorded measurement data, and the pressure release rate can be calculated based on the systolic blood pressure, diastolic blood pressure and pulse rate recorded in the relevant data.
[0057] Similar to the pressurization value, the depressurization rate (e.g., 5.5 mmHg / sec) can be set in advance when measurement data consistent with the measurement condition does not exist, such as when the blood pressure monitor is first used. Alternatively, the systolic blood pressure, diastolic blood pressure, and pulse rate can be estimated during pressurization, as described in JP 3 149 873 B2, and the depressurization rate calculated from such an estimated value can be set.
[0058] The sphygmomanometer of the present example uses a method of calculating blood pressure during depressurization, but it can be applied to a sphygmomanometer with a method of calculating blood pressure during pressurization. In this case, the pressurization rate can be set instead of the depressurization rate. The setting method is similar to that of the depressurization rate, and thus its description will be omitted.
[0059] Blood pressure changes depending on the surrounding environment, such as the temperature at the time of measurement. With the electronic blood pressure monitor, blood pressure is measured with the dosage status as the measurement condition, but the measurement condition is not limited to the dosage status and can be the measurement date and time or the temperature at the time of measurement. As shown in Fig. 4, in this case, a table in which the measurement date and time, the temperature (temperature at the time of measurement is "25°C" in the figure) at the time of measurement, and each measured value, such as blood pressure and pulse rate, are linked, is recorded in the memory 2123. The temperature changes during the date (season) and time, and thus only the measurement date and time and each measured value can be stored in association with each other in the memory 2123.
[0060] In this case, the user may input the information of the temperature using the measurement state input switch 2641, or a temperature measuring means may be included in the electronic sphygmomanometer 2600 for detecting the information of the temperature based on the detection result of the relevant means.
[0061] The measurement state is not limited to the dosage state, and may include dosage-related information, such as the type of medication taken, or at least one of the information related to exercise content, such as dosage information, measurement location, disease information, or the extent to which exercise is performed. Lifestyle information related to meals, exercise, smoking, rest (break), or the like may be used as the measurement state. In this case, the measurement state input switch 2641 is configured to allow various types of information to be input.
[0062] For example, blood pressure changes depending on different types of activities that are related to lifestyle habits and cause a difference in blood pressure between before and after a meal, before physical exercise and during rest, and before and after smoking. As in Fig. 5, a table in which the measurement date and time ("Y1 (year) / M1 (month) / D1 (day)" ..., "h1 (hour):m1 (minute)" ... in the figure), the information on the living habits (measurement state "after meal", "after exercise", "after smoking" and "during rest" ... in the figure), and each measurement value such as the blood pressure value and the pulse rate are linked together can be used for the table stored in the memory 2123.
[0063] There is a difference in blood pressure due to the psychological stress the user is exposed to at the time of measurement. Therefore, it is assumed that this difference in blood pressure occurs due to the measurement location, which is a measurement condition. For example, the user is in a relaxed state with little stress at home, and often in a state of high tension at the office, so there may be a difference in the measurement result when blood pressure is measured at home and when it is measured in the office.
[0064] A table in which the measurement date and time, the location of the measurement (location of the measurement “home”, “office”, “hospital”, etc. in the figure) and each measured value, such as the blood pressure value and the pulse rate, are linked to each other, as in Fig. 6 can be used for the table stored in the memory 21243.
[0065] As described above, an electronic sphygmomanometer 2600 is provided, which comprises biological information acquisition means for measuring a blood pressure value, recording means (memory 2123) for recording the blood pressure value, means (memory 2122) for storing a control program of the sphygmomanometer or the like, operation means (operation unit 2630) for performing operations such as blood pressure management, correction means (central unit 2120) for correcting the biological information acquired by the biological information acquisition means based on separately acquired correction information, and output means (display unit 2121) for outputting the corrected information (blood pressure value) after the correction, and further comprising a cuff 2101 attached to a blood pressure measurement site, pressurization and depressurization means 2104,2105 for adjusting the pressure applied to the cuff 2101, a pressure detection means (pressure sensor 2103) for detecting the pressure in the cuff, and blood pressure calculation means (central unit 2120) for calculating a blood pressure value from the cuff pressure, wherein the electronic sphygmomanometer further comprises state information acquisition means (measurement state input switch 2641) for acquiring information related to a state at the time of the current blood pressure measurement, wherein the recording means is adapted to store the state at the time of blood pressure measurement acquired by the state information acquisition means as measurement log information together with the blood pressure value, and a measurement log information acquisition means (central unit 2120,which executes step S2606) for acquiring past measurement log information close to the state at the time of blood pressure measurement of the recording means, wherein the correction means (central unit 2120 which executes step S2606) is designed to correct a measurement parameter for blood pressure based on the measurement log information when the past measurement log information close to the state at the time of blood pressure measurement is acquired by the measurement log information acquiring means.
[0066] According to the configuration described above, an effect is achieved that the optimal measurement parameter for blood pressure can be set for each user.
[0067] Furthermore, an appropriate measurement parameter for blood pressure is set according to the change in blood pressure caused by the difference in the surrounding environment (herein, the temperature) at the time of measurement by correcting the measurement parameter for blood pressure with the condition at the time of blood pressure measurement as the measurement date and time or as the temperature at the time of measurement.
[0068] The measurement state input switch 2641, which serves as input means for inputting the state at the time of blood pressure measurement, is arranged on the operation unit 2630 as the state information acquisition means. The state input to the measurement state input switch 2641 is recorded in the memory 2123 with the blood pressure value, so that various types of information relevant to the factors that change blood pressure can be easily acquired. The appropriate blood pressure measurement parameter that takes each factor into account can thus be set.
[0069] In the present example, the blood pressure measurement parameter is corrected by focusing on one of the conditions at the time of blood pressure measurement, including, but not necessarily limited to, the dosage status, temperature, lifestyle information, measurement location, and the like. Some of these different conditions may be combined. In this case, a more appropriate blood pressure measurement parameter can be set by using measurement log information where different measurement conditions are consistent with each other.
[0070] For this reason, when a configuration of using the measurement log information in which a plurality of measurement states agree with each other is adopted, the measurement log information should preferably be extracted from even better matching measurement states.
[0071] However, there may be a difference in the blood pressure value in a case where the time of measurement (in the morning, at night) or the timing of measurement (after meal, before meal, etc.) is different, even if the dosage status and a few other measurement conditions are consistent with each other.
[0072] The measurement log information in which a certain specific state and a few other states coincide with each other may be outside the target of extraction in a case where another certain specific state is different. In this case, a table in which the coincident measurement state and the measurement state determined to be outside the target of extraction when the states are different are linked is stored in the memory 2123 or the like, and the central unit 2121 can determine the measurement log information as outside the target of extraction when the measurement state determined to be outside the target of extraction is different.
[0073] As described above, the electronic blood pressure monitor 2600 of the present example has input means (measurement condition input switch 2641) for inputting measurement conditions to the operation means (operation unit 2630), and records the measurement condition such as the measurement location, disease information, dosage status, and exercise together with the blood pressure value in the recording medium (memory 2122, 2123).
[0074] The present invention is not limited only to the configuration of the embodiments described above, and a large number of embodiments can be realized.
[0075] For example, the electronic blood pressure monitor 2600 may be configured to download a suitable parameter, threshold, algorithm, or the like from a dedicated server to expand its functionality. In this case, the software version can be upgraded with the existing hardware, or optimization can be easily implemented by the user.
[0076] The functional expansion of the 2600 electronic blood pressure monitor can be performed from a user-owned operating terminal, such as a personal computer, without using the server. In this case, the parameters, threshold values, algorithms, and the like can be downloaded from a recording medium, such as a CD-ROM.
[0077] The 2600 electronic blood pressure monitor can be connected directly and wirelessly or via a wired connection to another biological information sensing device, such as a body composition monitor, a pedometer, or an electronic thermometer. In this case, data can also be transmitted and received reciprocally to improve individual accuracy. INDUSTRIAL APPLICABILITY
