The invention includes creation of a patients, hospitals / company, doctors, donors, admin and transplant sections within the web application for respective uses and local / regional / central monitoring portals within the web application to provide booking, tracking, real time monitoring of patients medical update / journey within the hospital from consultation / admission till discharge and follow up and provide timely medical and financial assistance and also communications between patients, hospitals and offices etc. It further relates to a web portal mediated provision of time based variable descending / static / ascending concessional model / total waive off in out patient / Laboratory / radiology services investigations and in patient bills. The invention includes a method of communicating with other portals / pharma companies / surgical item implant manufacturers / other offices via a commonly installed web application at user, health care setting and such office settings by integrated sign / cloud messaging system within and outside the web application to provide real time tracking, monitoring and provide medical and financial assistance. It further relates to web portal mediated provision of international standard equipments or private ward facility to such health schemes where these are not allowed.
