Abstract
Description
[Technical field]
[0001] The present invention relates to new drug combinations that are particularly useful for the therapeutic management of moderate to severe pain. [Background technology]
[0002] Pain is a distressing sensation that significantly affects the patient's quality of life. Pain management is perhaps one of the most important challenges for medical professionals and one of the main subjects of current medical research.
[0003] Pain can be defined as a distressing experience associated with actual or potential tissue damage, with sensory, emotional, cognitive and social components (Williams et al., Pain 2016, 157, 2420-23).
[0004] The causes of pain can be diverse and may, for example, result from an injury or an underlying disease or condition, either malignant or non-malignant, or may be the result of a surgical intervention.
[0005] Other classifications of pain distinguish between nociceptive pain, which is a normal physical response to injury, and neuropathic pain, which results from disorders affecting the central or peripheral nervous system. Furthermore, the two main types of nociceptive pain are somatic pain and visceral pain. Somatic pain refers to injuries to the musculoskeletal system, such as skin, muscle, and bone, whereas visceral pain is associated with visceral tissues and may be sensed indirectly. Pain is also often associated with inflammatory responses.
[0006] From the viewpoint of its duration, pain can be classified into acute pain and chronic pain. Acute pain is generally short-term, usually up to several weeks, occurs in response to tissue trauma, and decreases with healing of the underlying injury or disease, thus serving as a warning function of the possibility or extent of injury. Conversely, chronic pain is pain that persists beyond the normal healing time, and thus lacks the acute warning function of physiological nociception.
[0007] One common cause of acute pain is surgical trauma, and acute post-operative pain is one of the causes of delayed recovery in patients who have undergone surgery.
[0008] Nonsteroidal anti-inflammatory drugs (NSAIDs) are the most commonly prescribed drugs for the treatment of both acute and chronic pain, among which ibuprofen is perhaps one of the most widely used analgesics.
[0009] Like most NSAIDs, ibuprofen is believed to exert its analgesic effect primarily by inhibiting the enzyme cyclooxygenase (COX), which is involved in the synthesis of prostaglandins. Although ibuprofen is safer than other NSAIDs, it can also cause some undesirable side effects, including stomach irritation and bleeding, among others.
[0010] Another drawback of using NSAIDs as analgesics is that they may not be sufficient to adequately relieve pain of a certain intensity.
[0011] According to the results of several controlled clinical trials, salt forms of ibuprofen, such as the salt with arginine (ibuprofen arginate), the salt with lysine (ibuprofen lysinate) or the sodium salt, are advantageous over standard ibuprofen (acid form) as they are reported to provide a more rapid onset of pain relief and superior analgesic efficacy (Moore et al. Faster, higher, stronger? Evidence for formulation and efficacy for ibuprofen in acute pain. Pain 2013, 155 (1), 14-21).
[0012] Furthermore, it has been reported that ibuprofen arginate causes fewer gastroscopic disorders and has a significantly lower incidence of clinical side effects than ibuprofen, which can be explained by the increased NO synthesis induced by arginine (Gallego-Sandin et al., Effect of ibuprofen on cyclooxygenase and nitric oxide synthase of gastric mucosa: correlation with endoscopic lesions and adverse reactions, Dig. Dis. Sci., 2004, 49 (9), 1538-44).
[0013] Another class of drugs frequently used in pain management are the opioids. The effectiveness of opioid analgesics in managing severe pain is widely recognized.
[0014] For example, tramadol is a centrally acting analgesic with opioid agonist properties and is frequently used to treat moderate to severe pain of chronic or acute origin. Its mechanism of action is dual in nature, since it acts on the μ-opioid receptor, to which it binds with low affinity, while inhibiting the reuptake of noradrenaline and serotonin (J Pharmacol. Exp. Ther., 1992, 260 (1), 275-85).
[0015] Despite their excellent analgesic effect, the administration of opioids to treat pain is associated with several side effects, such as gastrointestinal side effects (e.g., nausea or vomiting), or side effects affecting the central nervous system (e.g., respiratory depression, cognitive impairment, hallucinations or allodynia), or cardiovascular side effects (e.g., hypotension or bradycardia), or cutaneous side effects, as well as hormonal or immunological side effects (Arthur et al, Safe opioid use. Management of opioid-related adverse effects and aberrant behaviors, Hematol. Oncol. Clin. N. Am., 2018, 32, 387-403).
[0016] Therefore, healthcare professionals must carefully weigh the risks and benefits associated with opioids when prescribing them for pain relief. The potential risks associated with opioids are of particular concern in the treatment of pre- and post-operative pain, where effective and safe pain relief is critical.
[0017] One way to reduce the risk of opioid-related side effects is to reduce the recommended dosage whenever possible, although such dosage reduction must not come at the expense of the effectiveness of treatment.
[0018] One strategy to maximize the efficacy and reduce side effects of analgesics is to appropriately combine analgesics with different mechanisms of action. In particular, some combinations of NSAIDs and opioids can provide synergistic effects, maximizing efficacy and reducing side effects.
[0019] In particular, the combination of tramadol with different NSAIDs has been reported in the background art as an effective strategy for pain management.
[0020] The synergistic effect of such a combination allows the amount of tramadol to be reduced while maintaining good analgesia.
[0021] However, adjusting the dose of each drug in the combination to minimize side effects while retaining analgesic efficacy is not a trivial matter. Even if it is desirable to minimize the amount of the opioid, tramadol, for safety reasons, it is still important to retain maximum analgesic efficacy.
[0022] There are several medicines available on the market that contain a combination of tramadol and an NSAID, which exploit the synergistic effects of the combination. Thus, for example, Enanplus® and Skudexa® are film-coated tablets that contain a combination of 75 mg of tramadol hydrochloride and 25 mg of dexketoprofen. The recommended dose is one tablet, with a minimum dosing interval of 8 hours.
[0023] Further examples of combination medicines are Tramacet® and Zaldiar®, which are film-coated tablets containing a combination of tramadol hydrochloride and paracetamol. The recommended dosage is an initial dose of two tablets equivalent to 75 mg tramadol and 650 mg paracetamol, with an interval of at least 6 hours, and a maximum daily dose of 300 mg tramadol hydrochloride and 2600 mg paracetamol.
[0024] Pharmaceutical compositions containing a fixed dose combination of ibuprofen arginate and tramadol hydrochloride are described in European Patent Application EP2474309-A1 and are believed to be useful for the treatment of pain. In particular, film-coated tablets and oral granules containing 400 mg of ibuprofen, 75 mg of tramadol hydrochloride and 340 mg of arginine are described.
[0025] It is noteworthy that in all medicinal products containing a combination of tramadol and a second analgesic approved so far in European countries, the minimum dose of tramadol hydrochloride is 75 mg. Moreover, in the field of tramadol, the recommended dose of tramadol hydrochloride is 100 mg and never less than 50 mg.
[0026] Despite currently available alternatives for pain management, there remains a need for additional analgesics with good therapeutic efficacy and fewer side effects. Summary of the Invention
[0027] Subject of the Invention The subject of the present invention is a combination of ibuprofen in the form of a pharma- ceutically acceptable salt, and tramadol or a pharma- ceutically acceptable salt thereof, in specific dosage amounts, for use in the treatment of pain.
[0028] Another object of the present invention is a pharmaceutical composition comprising ibuprofen in the form of a pharma- ceutically acceptable salt in a particular dosage amount, tramadol or a pharmaceutical salt thereof in a particular dosage amount, and at least one pharma- ceutically acceptable excipient. [Brief description of the drawings]
[0029] [Figure 1] FIG. 1 is a diagram representing the change in pain intensity measured on a visual analog scale (VAS) of 0-100 mm (or 0-10 cm) according to the results of a comparative clinical trial described in Example 1. Patients suffering from moderate to severe pain after dental surgery were treated intravenously with a combination of 37.5 mg tramadol HCl and 400 mg ibuprofen (as ibuprofen alginate) (group 1), or a combination of 75 mg tramadol HCl and 400 mg ibuprofen (as ibuprofen alginate) (group 2), or 100 mg tramadol HCl (group 3), or placebo (group 4). The VAS values displayed on the graph are the average values for each group. Time point 0 represents the starting point at which the treatment was administered to the patients. DETAILED DESCRIPTION OF THE PREFERRED EMBODIMENTS
[0030] A subject of the present invention is a combination of ibuprofen in the form of a pharma- ceutically acceptable salt with tramadol or a pharma- ceutically acceptable salt thereof, for use in the treatment of pain in humans, characterized in that the dose of ibuprofen in the combination is between 350 mg and 450 mg and the dose of tramadol or a pharma- ceutically acceptable salt thereof, expressed as an equivalent amount of tramadol hydrochloride, is between 35 mg and 40 mg.
[0031] The inventors have developed a specific combination of ibuprofen and tramadol, which maximizes the synergistic analgesic effect of both drugs.Surprisingly, the analgesic effect of such a combination, using a significantly low dose of tramadol, which has been considered subtherapeutic until now, is more effective than that achieved by the combination of ibuprofen and a high dose of tramadol. This finding is particularly important and unexpected, allowing for the simultaneous enhancement of analgesic effect and the reduction of potential side effects associated with the use of tramadol at doses commonly used in clinical practice.
[0032] A subject of the present invention may also be defined as a method of treating pain in a human subject in need thereof, the method comprising administering to the subject a combination of ibuprofen in the form of a pharma- ceutically acceptable salt and tramadol or a pharma- ceutically acceptable salt thereof, wherein the dosage of ibuprofen in the combination is between 350 mg and 450 mg, and the dosage of tramadol or a pharma- ceutically acceptable salt thereof in the combination is between 35 mg and 40 mg, expressed as an equivalent amount of tramadol hydrochloride.
[0033] In the present specification and claims, the singular forms preceded by the articles "a," "an," or "the" are generally intended to include the plural forms, unless the context clearly indicates otherwise. Furthermore, numerical values preceded by the term "about" are intended to include a certain variation around such value, i.e., a variation or ±2% of the stated amount. Numerical ranges defined by lower endpoints and upper endpoints are meant to include the aforementioned endpoints.
[0034] Tramadol Tramadol is the International Nonproprietary Name (INN) of the substance (±)-cis-2-[(dimethylamino)methyl]-1-(3-methoxyphenyl)cyclohexanol.
[0035] Tramadol is a centrally acting analgesic compound with a pleiotropic mechanism of action, as it acts as an agonist at μ-opioid receptors and as a serotonin (5-HT) and noradrenaline (NE) reuptake inhibitor. In addition, its metabolite, O-desmethyltramadol, also has activity at μ-opioid receptors.
[0036] The tramadol molecule has two chiral centers and can therefore exist in four different stereoisomers corresponding to two pairs of enantiomers, namely (1R,2R) and (1S,2S) (cis forms) and (1R,2S) and (1S,2R) (trans forms).
[0037] Within the scope of the present invention, the term tramadol includes any of the cis forms (including any stereoisomers, mixtures of stereoisomers in any ratio, in particular the racemate, i.e. the racemic (1:1) mixture of the (1R,2R) and (1S,2S) stereoisomers).
[0038] Tramadol is a basic compound that contains an amine group. Pharmaceutically acceptable salts of tramadol include its acid addition salts, including any solvates and crystalline forms thereof.
[0039] The pharmaceutically acceptable addition salts of tramadol can be prepared by conventional methods well known to those skilled in the art using pharmaceutically acceptable substantially non-toxic organic or inorganic acids.Suitable acids include, inter alia, hydrochloric acid, nitric acid, sulfuric acid, phosphoric acid, acetic acid, propionic acid, maleic acid, malonic acid, succinic acid, citric acid, tartaric acid, malic acid, salicylic acid and phthalic acid.Preferably, hydrochloric acid is used.
[0040] In a preferred embodiment, the tramadol is in the form of the hydrochloride salt.
[0041] In a particularly preferred embodiment, the tramadol is in the form of the hydrochloride salt of racemic tramadol.
[0042] Tramadol is commercially available from several sources and can also be prepared according to the methods described in US Pat. No. 3,652,589.
[0043] In this specification and claims, the amounts of tramadol or its pharma- ceutically acceptable salts are always expressed as tramadol hydrochloride equivalents, whether tramadol is used as the free base or in other salt form. For example, 32.9 mg of tramadol free base is 37.5 mg when expressed as tramadol hydrochloride equivalents.
[0044] Ibuprofen Ibuprofen is the International Nonproprietary Name (INN) of the compound (RS)-2-(4-(2-methylpropyl)phenyl)propanoic acid.
[0045] Ibuprofen is a drug that belongs to the group of nonsteroidal anti-inflammatory drugs (NSAIDs) that have analgesic, antipyretic and anti-inflammatory properties.
[0046] The ibuprofen molecule has one chiral center and therefore can exist in two enantiomeric forms. Within the scope of the present invention, the term ibuprofen includes ibuprofen in its racemic form ((R,S)-ibuprofen), the (S) enantiomer of ibuprofen ((S)-ibuprofen) and mixtures of the (R) and (S) enantiomers of ibuprofen in any ratio, preferably mixtures enriched in form (S). Preferably, the ibuprofen used in the present invention is selected from (R,S)-ibuprofen and (S)-ibuprofen. More preferably, it is racemic (R,S)-ibuprofen.
[0047] Ibuprofen is commercially available and can also be prepared, for example, according to the procedures described in British Patent Application GB-A-971700. The resolution of the enantiomers of ibuprofen is described, for example, in Brushan et al. (Resolution of enantiomers of ibuprofen by liquid chromatography: a review, Biomed. Chromatogr., 1998, 12 (6), 309).
[0048] Ibuprofen is an acidic molecule that contains a carboxylic acid moiety. In the present invention, ibuprofen is used in the form of its pharma- ceutically acceptable salts.
[0049] In an embodiment of the invention, the pharma- ceutically acceptable salt of ibuprofen is selected from the salt with arginine (ibuprofen arginate), the salt with lysine (ibuprofen lysinate) and sodium ibuprofen.
[0050] In another embodiment of the invention, the pharma- ceutically acceptable salt of ibuprofen is selected from ibuprofen alginate and ibuprofen lysinate.
[0051] In a preferred embodiment of the invention, ibuprofen is in the form of a salt with arginine (ibuprofen arginate).
[0052] When ibuprofen is in the form of a salt with arginine, the pharmaceutical composition according to the invention can be prepared by using a preformed salt of ibuprofen arginate in a conventional manner, or by using ibuprofen and arginine separately as separate components of the composition, the latter option avoiding a fixed 1:1 stoichiometric molar ratio of ibuprofen and arginine, as occurs when a preformed salt is used.
[0053] Therefore, in the context of the present invention, the term ibuprofen in the form of ibuprofen arginate or a salt with arginine is used to indicate that ibuprofen is administered together with arginine in any molar ratio. Generally, the molar ratio of ibuprofen:arginine is 1.2:1 to 1:1.2, preferably 1:1 to 1:1.2, and even more preferably 1:1.05 to 1:1.1.
[0054] If ibuprofen alginate is used in the form of a preformed salt (so the molar ratio is 1:1), this salt can be prepared, for example, as described in Spanish patent application ES435416.
[0055] On the other hand, arginine is an α-amino acid and is found in nature in its enantiomeric form L. In the context of the present invention, the term arginine includes any of its enantiomeric forms: L-arginine, D-arginine and mixtures thereof. Preferably, arginine is in the form of L-arginine.
[0056] Arginine is commercially available from a variety of sources.
[0057] In another embodiment of the invention, ibuprofen is in the form of a salt with lysine (ibuprofen lysinate).
[0058] When ibuprofen is in the form of a salt with lysine, the pharmaceutical composition according to the invention can be prepared by using a preformed salt of ibuprofen lysinate in a conventional manner, or by using ibuprofen and lysine separately as separate components of the composition, the latter option avoiding a fixed 1:1 stoichiometric molar ratio of ibuprofen and lysine, as occurs when a preformed salt is used.
[0059] Thus, in the context of the present invention, the term ibuprofen in the form of ibuprofen lysinate or a salt with lysine is used to indicate that ibuprofen is administered together with lysine in any molar ratio. Generally, the molar ratio of ibuprofen:lysine is between 1.2:1 and 1:1.2, preferably between 1.1:1 and 1:1.1, and even more preferably about 1:1.
[0060] If ibuprofen lysinate is used in the form of a preformed salt (so the molar ratio is 1:1), this salt can be prepared, for example, as described in Spanish patent application ES435416.
[0061] Lysine is an α-amino acid that is found in nature in its enantiomeric form L. In the context of the present invention, the term lysine includes any of its enantiomeric forms: L-lysine, D-lysine and mixtures thereof. Preferably, lysine is in the form of L-lysine.
[0062] Lysine is also commercially available from a variety of sources.
[0063] In this specification and claims, the amounts of ibuprofen salts described are always expressed as the weight of ibuprofen free acid. For example, 737.8 mg of ibuprofen alginate (1:1 molar ratio) is equivalent to 400.0 mg of ibuprofen free acid.
[0064] Combination treatment The therapeutic use according to the invention comprises a combination of ibuprofen in the form of a pharma- ceutically acceptable salt and tramadol or a pharma- ceutically acceptable salt thereof, The combination is, the dosage of ibuprofen in the combination is between 350 mg and 450 mg, preferably between 360 mg and 440 mg, more preferably between 370 mg and 430 mg, even more preferably between 380 mg and 420 mg, even more preferably between 390 mg and 410 mg, even more preferably about 400 mg, even more preferably 400 mg; the dosage of tramadol or a pharma- ceutically acceptable salt thereof in the combination, expressed as the equivalent amount of tramadol hydrochloride, is between 35 mg and 40 mg, preferably between 36 mg and 39 mg, more preferably about 37.5 mg, even more preferably 37.5 mg; Based on the superior analgesic effect of the combination, the ibuprofen salt is preferably selected from ibuprofen alginate, ibuprofen lysinate and ibuprofen sodium, more preferably selected from ibuprofen alginate and ibuprofen lysinate, even more preferably ibuprofen alginate, even more preferably ibuprofen alginate, wherein the amount of arginine is an amount resulting in an ibuprofen:arginine molar ratio of 1.2:1 to 1:1.2, more preferably 1:1 to 1:1.2, even more preferably 1:1.05 to 1:1.1.
[0065] The term "dose" or "dosage" as used in this specification and claims means the therapeutically effective amounts (by weight) of the two drugs in a drug combination administered to a patient in a single dose.
[0066] Such dose or dosage may be repeated once or twice or more depending on the need for treatment and / or according to the prescription of each particular case. When administered repeatedly, the administration interval is generally about 4 hours to about 8 hours, for example, the administration interval may be about 4 hours, or about 6 hours, or about 8 hours. Other less frequent administration schedules, i.e., administration schedules with longer administration intervals, are also included within the scope of the present invention, so that the dosage can be adjusted to meet specific treatment requirements according to the intensity and duration of pain.
[0067] The duration of treatment using the combination according to the present invention may vary widely depending on the type and duration of the pain to be treated, and can be easily adjusted by the physician for each specific situation, as is common practice in the field. In general, the duration of treatment can vary from a single administration to repeated administrations, for example, over several days (e.g., 1, 2, 3, 4, 5, 6 or 7 days), or up to several weeks, or up to several months.
[0068] Each dose of the combination according to the invention may be administered in a preferred unique pharmaceutical composition comprising a fixed dose combination of both drugs, i.e. ibuprofen in the form of its acceptable salts and tramadol or a pharmaceutically acceptable salt thereof, or both drugs may be administered simultaneously using two separate pharmaceutical dosage forms, i.e. one for ibuprofen in the form of its pharmaceutically acceptable salts and one for tramadol or a pharmaceutically acceptable salt thereof. By simultaneous administration, it is meant either administration substantially at the same time or successive administration with a short time interval, e.g. usually less than 5 minutes.
[0069] The use of fixed dose combinations in a single administration is generally preferred because they are easier to administer, thus facilitating drug compliance and increasing drug safety.
[0070] The combination according to the invention may be administered by any suitable route, ie by oral route or by intravenous injection.
[0071] In a preferred embodiment of the invention, the combination is administered by the oral route.
[0072] In another preferred embodiment of the invention, the combination is administered intravenously.
[0073] In another embodiment of the invention, ibuprofen in the form of a pharma- ceutically acceptable salt is administered orally and tramadol or a pharma- ceutically acceptable salt thereof is administered intravenously.
[0074] In another embodiment of the invention, ibuprofen in the form of a pharma- ceutically acceptable salt is administered intravenously and tramadol or a pharma- ceutically acceptable salt thereof is administered orally.
[0075] The combinations according to the invention are intended for the treatment of pain. In this context, the expression treatment of pain has its usual meaning in the medical field, i.e. it includes eliminating the sensation of pain or reducing the intensity of pain or preventing the occurrence of foreseeable pain.
[0076] Treatment of pain according to the present invention refers to the treatment of a subject suffering from pain or prone to suffering from pain, the term "subject" relating to a human.
[0077] Pain is a subjective experience, so it is impossible to measure pain intensity in a completely objective way. Therefore, patient self-reporting is commonly used to measure pain intensity. Available rating scales for measuring pain intensity include, for example, the visual analog scale (VAS), numeric rating scale (NRS) and word rating scale (VRS) described in the Guideline on the clinical development of medicinal products intended for the treatment of pain (EMA / CHMP / 970057 / 2011).
[0078] In particular, visual analog scales (VAS) are sometimes used to measure pain intensity, which varies continuously on a 10 cm (or 100 mm) line representing "no pain" (0 mm) at one end point to "worst pain imaginable" (100 mm) at the other end point.
[0079] Example 1 shows the results of a controlled clinical trial in patients suffering from moderate to severe pain after dental surgery, i.e. after surgical extraction of at least two third molars, at least one of which is a mandibular third molar and at least one of which is an impacted third molar, with bone removal, which is a well-known model for moderate to severe acute pain, as described in the guideline EMA / CHMP / 970057 / 2011.
[0080] Although the VAS cut-off value for including patients in the study was ≥ 55 mm, it is noteworthy that the baseline VAS values at time 0 before the start of treatment were approximately 70 mm in all groups, i.e., patients undergoing the assay were suffering from particularly severe pain.
[0081] As described in Example 1, it was surprisingly found that the pain relief in patients (group 1) treated with the combination of ibuprofen alginate / tramadol HCl 400 / 37.5mg was better than that in patients (group 2) treated with a similar combination containing twice the amount of tramadol, i.e., the combination of ibuprofen alginate / tramadol HCl 400 / 75mg (see FIG. 1).In fact, it was completely surprising and unexpected that the combination of ibuprofen alginate / tramadol HCl 400 / 37.5mg provided better pain relief than the combination of ibuprofen alginate / tramadol HCl 400 / 75mg.
[0082] It is also particularly noteworthy that the pain relief achieved by the combination of the present invention (400 / 37.5 mg ibuprofen / tramadol, Group 1) is far superior to that obtained using a much higher dose of tramadol alone (100 mg tramadol HCl, Group 3), which is one of the current standard strategies for the management of acute severe pain.
[0083] Although there is no clear boundary between acute and chronic pain, acute pain is thought to be adaptive in that it has a warning function, is short in duration (usually up to a few weeks), and diminishes as the underlying injury or disease (e.g., post-operative pain) heals.
[0084] Chronic pain, on the other hand, can be defined as pain that persists beyond the normal healing time and therefore lacks an acute warning function. Pain is usually considered chronic if it lasts for more than 3-6 months or is recurrent.
[0085] The combinations according to the invention are suitable for the treatment of virtually any type of pain, including chronic and acute pain, preferably pain of moderate to severe intensity.
[0086] In one embodiment the combination according to the invention is for use in the treatment of chronic pain.
[0087] In another preferred embodiment, the combination according to the invention is a combination for use in the treatment of acute pain.
[0088] As noted above when discussing the results of Example 1, the combination according to the present invention is particularly effective in alleviating severe pain (generally defined as pain with a VAS value of about 7 or greater). Thus, in one embodiment, the combination according to the present invention is a combination for use in treating severe pain.
[0089] The pain treated with the combinations according to the invention may be located in any part or area of the body and may have any aetiology, i.e., may result from any underlying disease or condition or injury, or may result from a medical procedure, including surgical intervention. Pain may be either nociceptive (somatic or visceral) or neuropathic.
[0090] In one embodiment the combination of the invention is a combination for the treatment of nociceptive pain.
[0091] In particular, effective post-operative pain management is essential in any surgical procedure, and inadequate pain management can result in increased mortality, delayed recovery, and increased hospital costs.
[0092] Thus, in one embodiment the combination according to the invention is for use in the treatment of acute post-operative pain.
[0093] As shown in the accompanying examples, the analgesic combination treatment of the present invention is generally intended for adults. Therefore, in one embodiment, the combination according to the present invention is a combination for use in adults, including young adults, i.e., patients aged 12 years or older. In another particular embodiment, the combination according to the present invention is a combination for use in adult patients aged 18 years or older.
[0094] Pharmaceutical dosage form According to one embodiment of the present invention, separate dosage forms are prepared for each active ingredient, i.e., for ibuprofen in the form of a pharma- ceutically acceptable salt and for tramadol or a pharma- ceutically acceptable salt thereof, which are used in combination by administering together or sequentially in appropriate doses according to the present invention.
[0095] According to another embodiment of the present invention, both drugs are administered in the form of fixed dose combination.Fixed dose combination typically refers to a single pharmaceutical composition that contains both active ingredients combined with at least one pharmaceutically acceptable excipient. The term "fixed dose combination" means that both ingredients are inseparably formulated together in the composition, so that the ratio of both ingredients cannot be changed.
[0096] Thus, another aspect of the present invention is a pharmaceutical composition comprising ibuprofen in the form of a pharma- ceutically acceptable salt, tramadol or a pharma- ceutically acceptable salt thereof, and at least one pharma- ceutically acceptable excipient, the amount of ibuprofen is between 350 mg and 450 mg, preferably between 360 mg and 440 mg, more preferably between 370 mg and 430 mg, even more preferably between 380 mg and 420 mg, even more preferably between 390 mg and 410 mg, even more preferably about 400 mg, even more preferably 400 mg, the amount of tramadol or a pharma- ceutically acceptable salt thereof, expressed as the equivalent amount of tramadol hydrochloride, is between 35 mg and 40 mg, preferably between 36 mg and 39 mg, more preferably about 37.5 mg, even more preferably 37.5 mg; The pharmaceutical composition is preferably one in which the ibuprofen salt is selected from ibuprofen alginate, ibuprofen lysinate and ibuprofen sodium, more preferably selected from ibuprofen alginate and ibuprofen lysinate, even more preferably ibuprofen alginate, and even more preferably ibuprofen alginate, wherein the arginine is in an amount corresponding to a molar ratio of ibuprofen:arginine of 1.2:1 to 1:1.2, more preferably 1:1 to 1:1.2, even more preferably 1:1.05 to 1:1.1.
[0097] Preferably, the pharmaceutical composition comprises ibuprofen in the form of a pharma- ceutically acceptable salt in the amounts and preferred forms described above, tramadol or a pharma- ceutically acceptable salt thereof in the amounts and preferred forms described above, and at least one pharma- ceutically acceptable excipient, i.e., the composition does not contain any additional active ingredients.
[0098] Another aspect of the invention is a composition of the above fixed dose combination for use in the treatment of pain.
[0099] According to one embodiment, the fixed dose combination according to the invention is in an oral pharmaceutical dosage form.
[0100] Suitable pharmaceutical forms for oral administration are solid pharmaceutical forms, such as tablets, capsules, granules, powders and pills, as well as liquid pharmaceutical forms, such as solutions, syrups and suspensions.
[0101] Preferably, the oral composition is selected from the group consisting of powders or granules for oral solution, oral solutions, capsules and tablets.
[0102] According to another embodiment, the fixed dose combination according to the invention is in an injectable form suitable for intravenous administration.
[0103] These pharmaceutical compositions can be prepared by conventional procedures well known to those skilled in the pharmaceutical art, as described in reference books in this field, such as Aulton's Pharmaceutics. The design and manufacture of medicines Aulton's Pharmaceutics (ME Aulton and KMG Taylor, editors, Churchill Livingstone Elsevier, Fourth Edition, 2013) or Remington Essentials of Pharmaceutics (L. Felton, editor, Pharmaceutical Press, 2013) or Pharmaceutics. Basic principles and application to pharmacy practice (AK Dash, S. Singh, and J. Tolman, editors, Academic Press, Elsevier, 2014).
[0104] The dosage form comprises an active ingredient and at least one pharma- ceutically active excipient.
[0105] Suitable excipients for use in preparing pharmaceutical dosage forms are described, for example, in the book Handbook of pharmaceutical excipients (RC Rowe, PJ Sheskey and ME Quinn, editors, Pharmaceutical Press, sixth edition, 2009).
[0106] In one embodiment, the fixed dose combination according to the present invention is an oral pharmaceutical dosage form selected from a tablet, a capsule, and a granule or powder.
[0107] As is well known in the art, common excipients used in formulating solid dosage forms include, for example, diluents (also known as fillers or bulking agents), binders, lubricants, glidants, anti-caking agents, alkalizing agents, disintegrants and wetting agents. Other optional excipients include sweeteners, flavoring agents and coloring agents.
[0108] The preparation of solid formulations generally involves a granulation step, either by a wet method involving liquid in the process, or by a dry method not involving liquid. Powders and granules can be pharmaceutical dosage forms themselves, which can be placed in a sachet and ingested, typically dissolved or dispersed in a liquid beforehand. Alternatively, powders and granules can be compressed into tablets or placed in capsules.
[0109] The preparation of an oral formulation containing a fixed dose combination of ibuprofen alginate and tramadol or a pharmaceutical salt thereof is described in European patent application EP2474309-A1.
[0110] A particularly preferred dosage form is a fixed-dose combination for oral administration selected from granules, powders, tablets and capsules, comprising 350 mg to 450 mg, preferably 360 mg to 440 mg, more preferably 370 mg to 430 mg, even more preferably 380 mg to 420 mg, even more preferably 390 mg to 410 mg, even more preferably about 400 mg, and even more preferably 400 mg of ibuprofen, 35 mg to 40 mg, preferably 36 mg to 39 mg, more preferably about 37.5 mg, and even more preferably 37.5 mg of tramadol or a pharma- ceutically acceptable salt thereof, preferably tramadol hydrochloride, when expressed as an equivalent amount of tramadol hydrochloride, and arginine in an amount such that the molar ratio of ibuprofen:arginine is 1.2:1 to 1:1.2, more preferably 1:1 to 1:1.2, and even more preferably 1:1.05 to 1:1.1, and at least one pharma- ceutically acceptable excipient.
[0111] In a preferred embodiment, the composition for oral administration comprises about 37.5 mg of tramadol hydrochloride, about 400 mg of ibuprofen, arginine in an amount such that the molar ratio of ibuprofen:arginine is 1.2:1 to 1:1.2, and at least one pharma- ceutically acceptable excipient, wherein the pharmaceutical composition is selected from granules, powders, tablets, and capsules.
[0112] Preferably, the oral composition consists of the above-mentioned fixed dose combination of ibuprofen, arginine and tramadol or a pharma- ceutically acceptable salt, and at least one pharma- ceutically acceptable excipient, i.e., the composition does not contain any additional active ingredients.
[0113] In a preferred embodiment, the formulation is a powder or granule.
[0114] In a preferred embodiment, the formulation is a tablet.
[0115] In a preferred embodiment, the formulation is a capsule.
[0116] In another embodiment, the fixed dose combination according to the present invention is an intravenously injectable composition.Typically, the intravenously injectable composition is an aqueous solution containing both active ingredients.Other common ingredients of an intravenously injectable composition are, for example, pH adjusting agents, tonicity agents and preservatives, as are well known in the art.
[0117] The intravenously injectable compositions are typically prepared by dissolving the active ingredient and any additional excipients in water for injection. The solution is usually sterilized, for example by filtration and / or autoclaving, and then placed into a suitable container.
[0118] The preparation of an intravenously injectable composition comprising ibuprofen alginate and tramadol or a pharma- ceutically acceptable salt thereof is described in International Patent Application WO2013 / 124498-A1.
[0119] A particularly preferred dosage form is a fixed-dose combination in the form of an aqueous solution for intravenous injection, comprising 350 mg to 450 mg, preferably 360 mg to 440 mg, more preferably 370 mg to 430 mg, even more preferably 380 mg to 420 mg, even more preferably 390 mg to 410 mg, even more preferably about 400 mg, and even more preferably 400 mg of ibuprofen, 35 mg to 40 mg, preferably 36 mg to 39 mg, more preferably about 37.5 mg, even more preferably 37.5 mg of tramadol or a pharma- ceutically acceptable salt thereof, preferably tramadol hydrochloride, when expressed as an equivalent amount of tramadol hydrochloride, and arginine in an amount such that the molar ratio of ibuprofen:arginine is 1.2:1 to 1:1.2, more preferably 1:1 to 1:1.2, even more preferably 1:1.05 to 1:1.1, and at least one pharma- ceutically acceptable excipient.
[0120] In a preferred embodiment, the composition is an aqueous solution for intravenous injection comprising about 37.5 mg of tramadol hydrochloride, about 400 mg of ibuprofen, arginine in an amount such that the molar ratio of ibuprofen:arginine is 1.2:1 to 1:1.2, and at least one pharma- ceutically acceptable excipient.
[0121] Preferably, the intravenously injectable composition consists of the above-mentioned fixed dose combination of ibuprofen, arginine and tramadol or a pharma- ceutically acceptable salt, and at least one pharma- ceutically acceptable excipient, i.e., the composition does not contain any additional active ingredients. EXAMPLES
[0122] Example 1: A controlled clinical trial to evaluate the efficacy of intravenous ibuprofen alginate in combination with different doses of tramadol in patients with moderate to severe pain after dental surgery A phase III, multicenter, randomized, double-blind, placebo-controlled pilot study was conducted to evaluate the comparative efficacy and safety of intravenous ibuprofen alginate (400 mg) in combination with two different doses of tramadol hydrochloride (37.5 and 75 mg) and intravenous tramadol 100 mg in patients suffering from moderate to severe pain after dental surgery.
[0123] Pain intensity was assessed using a visual analogue scale (VAS).
[0124] Patients were divided into four groups and received the following medications: Group 1: Ibuprofen alginate / Tramadol HCl 400mg / 37.5mg (16 patients) Group 2: Ibuprofen alginate / Tramadol HCl 400mg / 75mg (18 patients) Group 3: Tramadol HCl 100 mg (18 patients) - Group 4: Placebo (saline) (17 patients)
[0125] The amount of ibuprofen in acid form represents the amount of ibuprofen arginate (400 mg). The amount of L-arginine in each composition of Groups 1 and 2 was 356 mg, which corresponds to an ibuprofen:arginine molar ratio of 1:1.054.
[0126] The compositions of groups 1 to 3 were aqueous solutions of the drug prepared in water for injection. Other excipients included in the compositions were sodium chloride (used as an isotonicity agent to reach an osmolality of 310-360 mOsm / kg), as well as hydrochloric acid and sodium hydroxide (to adjust the pH of the solution to a value of 6.8-7.8). The volume of the compositions in all groups was 100 mL and was infused intravenously for 30 minutes in a single dose. The preparation of the composition used in group 1 was similar to that described in Example 5. The preparation of the composition used in group 2 was the same, except that 75 mg of tramadol HCl was used instead of 37.5 mg of tramadol HCl.
[0127] Patients selected for the study were subjects aged 18 years or older, weighing 50–110 kg, and suffering from moderate to severe pain (VAS cut-off value 0–100 ≥ 55 mm) after surgical extraction of at least two third molars (at least one of which was a mandibular third molar and at least one of which was an impacted third molar) with bone removal.
[0128] Therefore, after dental surgery, each patient's pain intensity was evaluated according to the VAS test until the patient's pain intensity reached a value of 55 mm or more. At that point, the patient was randomized into one of four groups and administered the corresponding medication within 15 minutes after randomization and within 25 minutes after the VAS test. A new VAS test was performed at the beginning of the injection, which was taken as the baseline value. If the patient did not reach the required pain intensity 4 hours after surgery, it was considered a recruitment failure.
[0129] It is noteworthy that although the cutoff VAS value was 55 mm, in fact all treated patients, except for the placebo group, had much higher basal VAS values, from approximately 70 mm.
[0130] After administration of each medication, the patient's progress was followed for 4 hours by assessing the change in pain intensity.
[0131] Until 4 hours after administration of study medication, patients were not to take any other analgesics, except for those defined as rescue medications per protocol.
[0132] Patients were allowed to request rescue medication during the period starting 30 minutes after administration of each treatment. Rescue medication was 1 g of paracetamol administered by the oral route. If a second rescue medication was required, a 575 mg capsule of metamizole was administered. If rescue medication was administered before 4 hours had elapsed after administration of study medication, the patient was terminated from the study (but continued to be included to evaluate safety parameters and monitored for an additional 4 hours).
[0133] Pain intensity was assessed for each patient at the following time points: 0 (basal), 15, 30, 45, 60, 75, 90, 105, 120, 150, 180, 210 and 240 minutes after drug administration. Mean VAS values were calculated for each of the four groups.
[0134] The results are shown in Table 1 below and graphically in FIG.
[0135] [Table 1]
[0136] Surprisingly, although the dose of tramadol administered in group 1 was significantly less than that administered in group 2, i.e., half the dose of tramadol administered in group 2 (37.5 mg (group 1) vs. 75 mg (group 2)), pain relief was found to be better in group 1 patients than in group 2 patients.
[0137] These results were surprising because it was completely unexpected that the ibuprofen alginate / tramadol HCl 400 / 37.5 mg combination would provide better pain relief than the ibuprofen alginate / tramadol HCl 400 / 75 mg combination, which contains twice the dose of tramadol.
[0138] It was also surprisingly found that pain relief in patients in Group 1 was also significantly better than pain relief in patients in Group 3, despite the significantly lower dose of the opioid tramadol (37.5 mg vs. 100 mg).
[0139] The number of patients in each group who required emergency medication and the number of patients who reported adverse events (AEs) are shown in Table 2.
[0140] [Table 2]
[0141] It is noteworthy that the proportion of patients undergoing emergency treatment in group 1 was lower than the proportion of patients undergoing emergency treatment in groups 2, 3 and 4. Thus, it was further confirmed that the combination of the present invention provides particularly effective pain relief.
[0142] Furthermore, the proportion of patients reporting side effects was also lower in group 1, confirming that this was the safest therapy.
[0143] In summary, these results show that the ibuprofen arginine / tramadol HCl 400 / 37.5 mg combination according to the invention administered intravenously is effective in treating moderate to severe acute post-operative pain, and that such combination is surprisingly more effective than the ibuprofen arginine / tramadol HCl 400 / 75 mg combination despite containing half the amount of tramadol, and furthermore is significantly more effective than the use of a much higher dose (100 mg) of tramadol HCl alone.
[0144] Furthermore, the combination of the present invention offers the best option not only in terms of efficacy but also in terms of safety.
[0145] Example 2: Controlled Clinical Trial to Evaluate the Efficacy of Oral Ibuprofen Alginate / Tramadol (400 / 37.5) in Patients with Moderate to Severe Pain Following Dental Surgery A phase III, multicenter, randomized, double-blind, placebo-controlled study was conducted to evaluate the comparative efficacy and safety of the combination of ibuprofen arginate / tramadol HCl (400 mg ibuprofen / 37.5 mg tramadol HCl) administered by the oral route compared with oral tramadol (50 mg), oral ibuprofen arginate (400 mg ibuprofen), and placebo for treating adult patients (age 18 and older) suffering from moderate to severe somatic pain after dental surgery.
[0146] Pain intensity was assessed using a visual analogue scale (VAS).
[0147] Patients selected for the study were adults suffering from moderate to severe pain (VAS0-100 ≥ 45 mm) after surgical extraction of at least two third molars with bone removal (if only two third molars were extracted, they had to be ipsilateral, at least one of which was an impacted third molar).
[0148] Patients were divided into four groups and received the following medications: Group 1: Ibuprofen alginate / Tramadol HCl 400mg / 37.5mg (95 patients) Group 2: Ibuprofen alginate 400mg (97 patients) Group 3: Tramadol HCl 50 mg (98 patients) - Group 4: Placebo (93 patients)
[0149] The amount of ibuprofen alginate (400 mg) is expressed as the amount of ibuprofen in its acid form.
[0150] The ibuprofen arginate / tramadol HCl combination administered to patients in Group 1 was in the form of granules for oral administration (described in Example 4). Each dose contained 340 mg of L-arginine.
[0151] The 400 mg ibuprofen alginate administered to patients in Group 2 corresponded to 400 mg of the commercially available drug Espidifen®, with each dose containing 400 mg of ibuprofen given as 770 mg of ibuprofen alginate.
[0152] The 50 mg dose of tramadol HCl 50 mg in Group 3 was administered using the commercially available Adolonta® 100 mg / mL oral solution.
[0153] After dental surgery, each patient's pain intensity was evaluated according to the VAS test until the pain intensity of each patient reached a value of 45 mm or more. At that time, the patient was randomly assigned to one of four groups and administered the corresponding medication. After the first medication administration, a new VAS test was performed, which was taken as the baseline value. Each medication was administered four times with an administration interval of 6 hours.
[0154] After administration of each medication, the patient's progress was followed by assessing the change in pain intensity up to 24 hours after the first administration.
[0155] Patients were allowed to request emergency medication during this period, starting 30 minutes after the first dose. Emergency medication consisted of 1 g of paracetamol administered by the oral route. If a second emergency medication was required, a 575 mg capsule of metamizole was administered.
[0156] Pain intensity was assessed for each patient at the following time points: 0 (baseline), 1, 2, 3, 4, 5, 6, 8, 12 and 24 hours after drug administration. Mean VAS values were calculated for each of the four groups.
[0157] For example, at 6 hours, patients in Group 1 treated with the combination according to the invention achieved maximum pain relief, with mean pain intensity differences (Pain Intensity Difference, PID) observed to be 9 mm, 13 mm and 18 mm for Group 2 (ibuprofen alginate), Group 3 (tramadol HCl) and Group 4 (placebo), respectively, as shown in Table 3 below (row 1, total population, baseline VAS > 45 mm).
[0158] Moreover, it was surprisingly found that such differences in VAS values were generally increased in the subpopulations of patients with higher baseline VAS values (see Table 3, rows corresponding to baseline VAS ≥ 50, 55 and 60 mm). In other words, the superiority of the treatment according to the invention (group 1) over the comparison controls (groups 2, 3 and 4) was even higher in patients suffering from more severe pain.
[0159] [Table 3]
[0160] As shown in Table 4 below, the number of patients who required emergency medication in group 1 who received the medicament according to the present invention was only 4.2% of the patients, whereas the number of patients who required emergency medication in groups 2, 4 and 5 was 6.2%, 38.8% and 55.9% of the patients.
[0161] [Table 4]
[0162] These results show that the combination of ibuprofen arginine / tramadol HCl 400 / 37.5mg according to the present invention, administered orally, is effective in treating moderate to severe post-dental pain, and is particularly effective in patients suffering from more severe pain. These results also show that such a combination is more effective than ibuprofen arginine 400mg, tramadol HCl 50mg and placebo, and is safer than tramadol 50mg.
[0163] Example 3: Controlled Clinical Trial to Evaluate the Efficacy of Oral Ibuprofen Alkynate / Tramadol HCl (400 / 37.5) in Patients with Moderate to Severe Pain After Hysterectomy A phase III, multicenter, randomized, double-blind, placebo-controlled study was conducted to evaluate the comparative efficacy and safety of the combination of ibuprofen arginate / tramadol HCl (400 mg ibuprofen / 37.5 mg tramadol HCl) administered by the oral route compared with oral tramadol (50 mg), oral ibuprofen arginate (400 mg ibuprofen), and placebo for treating adult patients (age 18 years or older) suffering from moderate to severe pain of visceral origin following non-neoplastic abdominal hysterectomy.
[0164] Pain intensity was assessed using a visual analogue scale (VAS).
[0165] Patients selected for the study were adults suffering from moderate to severe pain (VAS 0-100 ≥ 40 mm) after total or partial abdominal hysterectomy for benign non-neoplastic disease, requiring hospitalization for at least 48 hours.
[0166] Patients were divided into four groups and received the following medications: Group 1: Ibuprofen alginate / Tramadol HCl 400mg / 37.5mg (91 patients) Group 2: Ibuprofen alginate 400 mg (94 patients) Group 3: Tramadol HCl 50 mg (98 patients) - Group 4: Placebo (93 patients)
[0167] The amount of ibuprofen alginate (400 mg) is expressed as the amount of ibuprofen in its acid form.
[0168] The ibuprofen arginate / tramadol HCl combination administered to patients in Group 1 was in the form of granules for oral administration (described in Example 4 below). Each dose contained 340 mg of L-arginine.
[0169] Patients in Group 2 were administered the over-the-counter drug Espidifen® 400 mg, with each dose containing 400 mg of ibuprofen as 770 mg of ibuprofen alginate.
[0170] Patients in Group 3 were administered the over-the-counter Adolonta® 100 mg / mL oral solution and a dose of 50 mg of tramadol HCl.
[0171] After surgery, each patient's pain intensity was evaluated according to the VAS test until the pain intensity of each patient reached a value of 40 mm or more. At that time, each patient was recruited, randomly assigned to one of four groups, and administered the corresponding medication. After the first medication administration, a new VAS test was performed, which was taken as the baseline value. Then, the assigned medication was administered seven more times with an interval of 6 hours, for a total of eight administrations for each patient.
[0172] After the first dose of each medication, the patient's progress was followed by assessing the change in pain intensity up to 48 hours after the first dose.
[0173] Thirty minutes after the first dose, patients could request emergency medication if necessary. Emergency medication consisted of 1 g of paracetamol administered by the oral route. If a second emergency medication was required, a 575 mg capsule of metamizol was administered.
[0174] Pain intensity was assessed for each patient at the following time points: 0 (baseline), 1, 2, 4, 6, 8, 10, 12, 18, 24, 30, 36, 42 and 48 hours after the first drug administration. The mean VAS values at each time point were calculated for each of the four groups.
[0175] Group 1 was found to have superior pain relief compared to all other groups during all periods evaluated.
[0176] As shown in Table 5 below, at 24 hours, the difference in the mean VAS values of group 1 compared to group 2 (ibuprofen), group 3 (tramadol) and group 4 (placebo) was 7 mm, 12 mm and 22 mm, respectively (row 1, total population, baseline VAS≧40 mm). Moreover, it was surprisingly found that such differences in VAS values (pain intensity difference, PID) generally increased in the subpopulation of patients with higher baseline VAS values (see rows corresponding to table 5, baseline VAS≧50 and 60 mm). In other words, the superiority of the treatment according to the invention (group 1) over the comparison controls (groups 2, 3 and 4) was even higher in patients suffering from more severe pain.
[0177] [Table 5]
[0178] Compared to tramadol (Group 3) and placebo (Group 4), the composition according to the invention (Group 1) showed greater pain relief with a difference of more than 1 cm in the VAS test from 6 hours to the end of the study.
[0179] Group 1 was the only group to show greater than 10 mm of pain reduction (pain intensity difference, PID) over the 48 hour period of treatment, with the greatest difference at 12 hours.
[0180] Table 6 below shows the patients in each group who required rescue medication in the first 24 hours of the study (patients treated in rescue in 24 hours) and all patients who took rescue medication. The last row shows the average time until rescue medication was required.
[0181] [Table 6]
[0182] There was no difference in the number of side effects reported in Group 1 (ibuprofen / tramadol) and Group 4 (placebo group), supporting the safety of the claimed combination.
[0183] The number of side effects reported in group 3 (tramadol) was twice as high as the number reported in group 1 (ibuprofen / tramadol).
[0184] These results show that the combination of ibuprofen arginine / tramadol HCl 400 / 37.5mg according to the present invention, administered orally, is effective in treating moderate to severe visceral postoperative pain, and is particularly effective in patients suffering from more severe pain.Such a combination is also shown to be more effective than ibuprofen arginine 400mg, tramadol HCl 50mg and placebo, and is safer than tramadol 50mg.
[0185] Example 4: Preparation of an oral fixed dose composition containing 400 mg of ibuprofen (as ibuprofen alkynyl acetate) and 37.5 mg of tramadol hydrochloride The oral compositions prepared were in the form of granules for oral solution, ie, intended to be dissolved in water before ingestion.
[0186] Each dose contained 1400 mg of granules containing 400 mg of ibuprofen, 340 mg of L-arginine, and 37.5 mg of tramadol hydrochloride. The composition was packaged in a single-dose sachet.
[0187] The list of ingredients is given in Table 5.
[0188] [Table 7]
[0189] The granules were prepared according to standard techniques well known in the art. Thus, the granulation solution was prepared by dissolving tramadol HCl and a portion of sucrose in purified water. The ibuprofen, another portion of sucrose, disintegrant and humectant were first premixed and then granulated in a mixer by adding the granulation solution. The granules were dried in a fluid bed and mixed with the extragranular ingredients (L-arginine, the remainder of the sucrose, alkalizing agent, sweetener, flavor and glidant) and thoroughly mixed in the mixer. The resulting granules were loaded into sachets, each containing 1400 mg. The molar ratio of ibuprofen:arginine in the formulation was 1:1.0066.
[0190] Example 5: Preparation of an intravenously injectable fixed dose composition containing 400 mg of ibuprofen (as ibuprofen alginate) and 37.5 mg of tramadol hydrochloride An aqueous injection formulation for intravenous infusion was prepared. The unit dose was bottled, with each bottle containing 100 mL of the formulation. The ingredients used to prepare the intravenous injection formulation are shown in Table 7.
[0191] [Table 8]
[0192] The manufacturing process is as follows: A portion of the water for injection (about 70%) was introduced into a reactor, and tramadol HCl, sodium chloride, arginine and ibuprofen were added with continuous stirring until a clear solution was obtained. The pH was adjusted to a value of 6.8±0.2 with HCl / NaOH. Finally, the remaining water was added to reach the final volume. The resulting solution was filtered, filled into bottles and sterilized by autoclaving.
[0193] The molar ratio of ibuprofen:arginine in the formulation was 1:1.054.
