DETAILED DESCRIPTION OF THE PREFERRED EMBODIMENTS

[0015] Benzbromarone is an effective uricosuric agent for lowering serum uric acid (sUA) and treating gout. It has been found that treatment with benzbromarone can result in lowering sUA even after a single dose and can continue to do so after multiple doses, and that chronic treatment can result in a target level of sUA of <6 mg / dL. However, in certain patients, benzbromarone is associated with hepatotoxicity. A high percentage of these patients developed acute liver failure leading to death or emergency liver transplantation. As a result, benzbromarone was never approved for use in the United States. In addition, benzbromarone's hepatotoxicity led to its withdrawal in Europe in 2003. Benzbromarone is converted to reactive metabolites by CYP2C9. Benzbromarone is metabolized by CYP2C9 via 6-OH benzbromarone to 5,6-dihydroxybenzbromarone and subsequently to a reactive ortho-quinone intermediate by oxidation of 5,6-dihydroxybenzbromarone. The mechanism of benzbromarone hepatotoxicity appears to be the result of its hepatic metabolism by CYP2C9 and the possible effect of 6-OH benzbromarone and its further metabolites on mitochondrial function (Iwamura et al., Drug Metabolism and Disposition, 2011, 39, 838-846; Uchida et al., Drug Metab. Pharmacokinet., 2010, 25, 605-610).

The result can be a reduction and / or alleviation of the signs, symptoms or causes of the disease or any other desired alteration of a biological system. For example, an "effective amount" for therapeutic use is the amount of a composition including a compound as disclosed herein that is required to provide a clinically significant reduction in disease symptoms without undue adverse side effects. The appropriate "effective amount" in any individual case can be determined using techniques such as dose escalation studies. The term "therapeutically effective amount" includes, for example, a prophylactically effective amount. An "effective amount" of a compound disclosed herein is an amount that is effective to achieve a desired pharmacological effect or therapeutic improvement without undue adverse side effects. It is understood that an "effective amount" or "therapeutically effective amount" may vary from subject to subject, depending on the variability in the metabolism of Compound 1, the age, weight, general condition of the subject, the condition being treated, the severity of the condition being treated, and the judgment of the prescribing physician. By way of example only, a therapeutically effective amount can be determined by a dose escalation clinical trial.

[0053] The term "enhance" or "enhancing" means to increase or prolong a desired effect, either in potency or duration. By way of example, "enhancing" the effect of a therapeutic agent refers to the ability to increase or prolong, either in potency or duration, the effect of the therapeutic agent during the treatment of a disease, disorder or condition. An "enhancing-effective amount," as used herein, refers to an amount adequate to enhance the effect of a therapeutic agent in the treatment of a disease, disorder or condition. When used in a patient, the amount effective for this use will depend on the severity and course of the disease, disorder or condition, previous treatments, the patient's health status and response to the drug, and the judgment of the treating physician.

In some embodiments, the pharmaceutical formulation is administered in two or three or four capsules or tablets.

[0075] In some embodiments, solid dosage forms, such as tablets, effervescent tablets and capsules, are prepared by mixing particles of Compound 1, the compound of formula (I), or the compound of formula (II) with one or more pharmaceutical excipients to form a bulk blend composition. When these bulk blend compositions are referred to as homogeneous, it means that the particles of Compound 1, the compound of formula (I), or the compound of formula (II) are evenly distributed throughout the composition, so that the composition can be easily subdivided into equally effective unit dosage forms, such as tablets, pills and capsules. The individual unit dosages can further include a film coating that disintegrates upon oral ingestion or contact with diluent. These formulations can be manufactured by conventional pharmacological techniques.

[0076] Conventional pharmacological techniques include, for example, one or a combination of the following methods: (1) dry blending, (2) direct compression, (3) milling, (4) dry or non-aqueous granulation, (5) wet granulation, or (6) fusion. See, for example, Lachman et al., The Theory and Practice of Industrial Pharmacy (1986). Other methods include, for example, spray drying, pan coating, melt granulation, granulation, fluidized bed spray drying or coating (e.g., Wurster coating), tangential coating, top spray, tabletting, extrusion, and the like.

[0105] The term "delayed release" as used herein refers to delivery such that release can be achieved at some generally predictable location in the intestine distal to the location that would have been achieved had there been no delayed release change. In some embodiments, the method for delayed release is a coating. Any coating should be applied to a sufficient thickness such that the entire coating does not dissolve in gastrointestinal fluids at a pH below about 5, but dissolves at a pH of about 5 or greater. It is expected that any anionic polymer that exhibits a pH-dependent solubility profile can be used as an enteric coating in the methods and compositions described herein to achieve delivery to the lower gastrointestinal tract. In some embodiments, the polymers described herein are anionic carboxylic acid polymers. In other embodiments, the polymers and their compatible mixtures, as well as some of their properties, include, but are not limited to, the following:

[0106] Shellac, also called refined lac, is a refined product obtained from the resinous secretions of insects. This coating dissolves in media with a pH > 7;

Examples of suitable acrylic polymers include methacrylic acid copolymers and ammonium methacrylate copolymers. Eudragit series E, L, S, RL, RS, and NE (Rohm Pharma) are available when solubilized in organic solvents, aqueous dispersions, or dry powders. Eudragit series RL, NE, and RS are insoluble in the gastrointestinal tract but permeable and are primarily used for colon targeting. Eudragit series E dissolves in the stomach. Eudragit series L, L-30D, and S are insoluble in the stomach and dissolve in the intestine;

[0108] Cellulose derivatives. Examples of suitable cellulose derivatives are ethyl cellulose; and reaction mixtures of partial acetate esters of cellulose with phthalic anhydride. Performance may vary based on the degree and type of substitution. Cellulose acetate phthalate (CAP) dissolves at pH>6. Aquateric (FMC) is an aqueous-based system and is a spray-dried CAP pseudolatex with particles of <1 μm. Other components in Aquateric may include Pluronics, Tweens, and acetylated monoglycerides. Performance may vary based on the degree and type of substitution. For example, HPMCP such as HP-50, HP-55, HP-55S, or HP-55F grades are suitable. Performance may vary based on the degree and type of substitution. For example, suitable grades of hydroxypropyl methylcellulose acetate succinate include, but are not limited to, AS-LG (LF), which dissolves at pH 5, AS-MG (MF), which dissolves at pH 5.5, and AS-HG (HF), which dissolves at higher pH. These polymers are provided as granules or as aqueous dispersions; fine powders for polyvinyl acetate phthalate (PVAP). PVAP dissolves at pH>5, which is much less permeable to water vapor and gastric juices.

In particular, anionic carboxylic acrylic polymers usually contain 10-25% by weight of a plasticizer, especially dibutyl phthalate, polyethylene glycol, triethyl citrate, and triacetin. Conventional coating techniques, such as spray or pan coating, are used to apply the coating. The coating thickness must be sufficient to ensure that the oral dosage form remains intact until localized delivery at the desired site in the intestinal tract is reached.

[0110] Colorants, antiblocking agents, surfactants, defoamers, lubricants (e.g., carnauba wax or PEG) can be added to the coating in addition to plasticizers to solubilize or disperse the coating materials and to improve coating performance and the coating product.

[0111] In other embodiments, the formulations described herein that contain Compound 1, a compound of formula (I), or a compound of formula (II) are delivered using a pulsatile dosage form.
