Description

DOSAGE OF ANTIBODY DRUG CONJUGATE RINATABART SESUTECAN TECHNICAL FIELD

[0001] The present disclosure generally relates to dosage schedules for antibody-drug conjugates (ADC) for human subjects, and in particular, to the dosage schedule of ADC rinatabart sesutecan (Rina-S) for human subjects in the treatment of cancer, such as solid tumors. It also relates to such ADCs for use in such dosage regimens. It also relates to combination therapies, where the ADC is used in combination with a further anti-cancer drug. BACKGROUND

[0002] Folate Receptor 1 (FOLR1), also known as Folate Receptor-alpha (FRα), or Folate Binding Protein (FBP), is an N-glycosylated protein expressed on the plasma membrane of cells. FOLR1 is overexpressed in the vast majority of ovarian cancers, as well as in many uterine, endometrial, pancreatic, renal, lung, and breast cancers, while the expression of FOLR1 on normal tissues is restricted to the apical membrane of epithelial cells in the kidney proximal tubules, alveolar pneumocytes of the lung, bladder, testes, choroid plexus, and thyroid (Weitman S D, et al., Cancer Res 52: 3396-3401 (1992); Antony A C, Annu Rev Nutr 16: 501-521 (1996); Kalli K R, et al. Gynecol Oncol 108: 619-626 (2008)). The antibody-drug conjugate (ADC) rinatabart sesutecan (Rina-S; previously also referred to as PRO1184) is developed to target FOLR1 and includes a FOLR1 antibody and drug units, each of which comprises exatecan connected to the antibody through a hydrophilic linker. Rina-S has been shown to induce cytotoxicity in cancer cells and demonstrate anti-tumor activities, both in vitro and in vivo. However, there is still a need for effective methods of administering Rina-S to human subjects. Preferably, the methods comprise optimized dosage and administration schedules that maximize efficacy and minimize toxicity of Rina-S for therapeutic use in human subjects. There is also a need to provide combination therapies for treating cancers which are positive for FOLR1.SUMMARY

The term "biosimilar" (e.g., of an approved reference product / biological drug) as used herein refers to a biologic product that is similar to the reference product based on data from (a) analytical studies demonstrating that the biological product is highly similar to the reference product notwithstanding minor differences in clinically inactive components; (b) animal studies (including the assessment of toxicity); and / or (c) a clinical study or studies (including the assessment of immunogenicity and pharmacokinetics or pharmacodynamics) that are sufficient to demonstrate safety, purity, and potency in one or more appropriate conditions of use for which the reference product is approved and intended to be usedand for which approval is sought (e.g., that there are no clinically meaningful differences between the biological product and the reference product in terms of the safety, purity, and potency of the product). In some embodiments, the biosimilar biological product and reference product utilizes the same mechanism or mechanisms of action for the condition or conditions of use prescribed, recommended, or suggested in the proposed labeling, but only to the extent the mechanism or mechanisms of action are known for the reference product. In some embodiments, the condition or conditions of use prescribed, recommended, or suggested in the labeling proposed for the biological product have been previously approved for the reference product. In some embodiments, the route of administration, the dosage form, and / or the strength of the biological product are the same as those of the reference product. A biosimilar can be, e.g., a presently known antibody having the same primary amino acid sequence as a marketed antibody, but may be made in different cell types or by different production, purification, or formulation methods. A biosimilar ADC typically is structurally highly similar to the reference product, including the antibody part, the cytotoxic payload, and the linker, but may be produced via slightly different production processes.

In certain embodiments, the human subject is treated with Rina-S according to the invention at a time where there has not been a determination of the percentage of cancer cells which are FOLR1 positive in a sample of the solid tumor of the human subject; in certain embodiments thereof, the human subject has an ovarian cancer.

[0062] In some embodiments, the methods of the present invention can be used to treat cancer (e.g., solid tumor) that is localized. In some embodiments, the method of the present invention can be used to treat cancer (e.g., solid tumor) that is metastatic. In some embodiments the patient has an advanced solid tumor. In some embodiments, the patient has a locally advanced tumor. In some embodiments the tumor has spread so that the patient has secondary solid tumors in addition to the primary tumor. In some embodiments, the tumor cannot be removed by surgery, i.e. the tumor is unresectable. In some embodiments, the tumor has both spread (the cancer is metastatic) and at least one solid tumor is present which is unresectable.

[0063] In some embodiments, the subject may have high-grade epithelial ovarian cancer. In some embodiments, the subject may have a high-grade serous ovarian cancer.

[0064] In certain of the embodiments set out herein, the subject may have a tumor that is a platinum-resistant tumor. In some embodiments, the subject may have high-grade serous or endometrioid EOC (epithelial ovarian cancer), primary peritoneal cancer, or fallopian tube cancer, resistant to platinum chemotherapy. In one preferred embodiment, the subject has platinum-resistant ovarian cancer. Alternatively, the subject may have a tumor that is platinum-sensitive. In some embodiments, the subject may have platinum-sensitive ovarian cancer.

[0065] In some embodiments, the subject to be treated has a histologically or cytologically confirmed metastatic or unresectable solid malignancy. In some embodiments, the subject has ovarian cancer and also epithelial ovarian cancer, primary peritoneal cancer, or fallopian tube cancer. In some embodiments, the subject has endometrial cancer. In some embodiments, the subject has non-small cell lung cancer (NSCLC). In some embodiments, the subject has EGFR-mutated NSCLC. In some embodiments, the subject has breast cancer. In some embodiments, the subject has hormone receptor-positive, HER2-negative or triple-negative breast cancer. In some embodiments the subject has mesothelioma. In one embodiment, the cancer is selected from histologically or cytologically confirmed metastatic or unresectable solid malignancy including ovarian cancer (must have epithelial ovarian cancer, primary peritoneal cancer, or fallopian tube cancer), endometrial cancer, non-small cell lung cancer, epidermal growth factor receptor (EGFR)-mutated NSCLC, breast cancer (hormone receptor-positive, HER2-negative and triple-negative), and mesothelioma.

[0066] In some embodiments, the subject to be treated is selected from one with high-grade serious ovarian cancer, primary peritoneal cancer or fallopian tube cancer (in particular excluding endometrioid, clear cell carcinoma, mucinous, low grade, and those with a sarcomatous or neuroendocrine element).

[0067] In one embodiment, the patient has already had their FRα expression status determined, for instance using the Ventana FOLR1-2.1 RxDx Assay. In one embodiment, the patient has high FRα expression (ie, FRα expression ≥75% PS2+), for instance as measured using the Ventana FOLR1- 2.1 RxDx Assay. In another embodiment, the patient does not have high FRα expression, and in particular has a FRα expression <75% PS2+, for instance as measured using the Ventana FOLR1-2.1 RxDx Assay. In further embodiments, the patient has a FRα expression <25% PS2+, for instance as measured using the Ventana FOLR1-2.1 RxDx Assay. In certain embodiments, the patient has not already had their FRα expression status determined. In such embodiments the patient may be treatedwith Rina-S according to the invention absent predetermination of FRα expression status, e.g. in patients having ovarian cancer.

[0068] In some embodiments, a method of the present invention may comprise a step of determining whether or not the tumor expresses FOLR1. In further embodiments, it may comprise the step of determining the level of expression of FOLR1. In other embodiments, it may not comprise either of those steps. In certain embodiments thereof, the human subject has ovarian cancer. In certain embodiments, the patients have been pretreated with other therapies against the tumor, e.g. with at least 1, 2, 3, 4, or more lines of therapy. In certain embodiments, the patients are treated with the ADC, in particular Rina-S, at a dose level of 100 mg / m2. In certain embodiments, the patients are treated with the ADC, in particular Rina-S, at a dose level of or 120 mg / m2. In certain embodiments, the dosage is administered to the patients about every three weeks (Q3W). In certain embodiments, the administration of the ADC, in particular Rina-S, is via intravenous administration.

[0069] In some embodiments, whether or not there is FOLR1 expression is not determined beyond determining the presence or absence of any expression of FOLR1. In some embodiments, the level of FOLR1 expression in the solid tumor is not determined at all prior to treatment. For instance, as a large majority of ovarian cancers display FOLR1 expression it may be that a subject with ovarian cancer is simply treated according to the invention without determining the presence, or absence, or level of FOLR1 expression, because of the high chance the tumor will express FOLR1 and also because the exact level of FOLR1 expression may be less important for Rina-S than for other cancer drugs.

[0070] In one embodiment, a patient with one of the recited cancer types herein is treated without any determination of whether or not the solid tumor displays FOLR1 expression. In certain embodiments thereof, the human subject has ovarian cancer. In certain embodiments, the human subjects have been pretreated with other therapies against the tumor, e.g. with at least 1, 2, 3, 4, or more lines of therapy. In certain embodiments, the human subjects are treated with ADC, in particular Rina-S, at a dose level of 100 mg / m2. In certain embodiments, the human subjects aretreated with the ADC, in particular Rina-S, at a dose level of or 120 mg / m2. In certain embodiments, the dosage is administered to the human subjects about every three weeks (Q3W). In certain embodiments, the administration of the ADC, in particular Rina-S, is via intravenous administration. Dosages, dosage regimens, routes of administration, and patient assessment

The person of ordinary skill will realize that a variety of factors, such as age, general health, specific organ function or weight, as well as effects of prior therapy on specific organ systems may be considered in selecting an optimal dosage of ADC, and that the dosage and / or frequency of administration may be increased or decreased during the course of therapy.

[0075] In some embodiments, the treatments of the present invention result in an improved treatment outcome for the subject. Such is especially the case when the human subject has failed a previous treatment. In certain embodiments, the improved treatment outcome is an objective response selected from stable disease, a partial response, or a complete response. In certain embodiments, the improved treatment outcome is a stable disease, indicating the progression of the solid tumor has been halted. In certain embodiments, the improved treatment outcome is a partial response. In certain embodiments, the partial response indicates that the solid tumor size has been reduced by at least 20%, at least 25%, at least 30%, at least 40%, or at least 50%. In certainembodiments, the partial response indicates that the solid tumor size has been reduced by at least 30%. In one embodiment, it has been reduced by at least 30% according to those criteria. In certain embodiments, the improved treatment outcome is a complete response. In certain embodiments, the complete response indicates that there is no detectable cancer. In certain embodiments, the improved treatment outcome is reduced tumor burden. In certain embodiments, the reduce tumor burden indicates a reduction of the solid tumor size by at least 20%, at least 25%, at least 30%, at least 40%, or at least 50%. In certain embodiments, the improved treatment outcome is progression-free survival or disease-free survival. In certain embodiments, the progression-free survival or disease- free survival is assessed after 1, 2, 3, 4, 5, or 10 years.

[0076] Any appropriate approach may be used to assess tumors and the treatment. In some embodiments the treatment is assessed per RECIST v1.1. In embodiments, where the cancer is pleural mesothelioma treatment may be assessed using mRECIST v1.1. The RECIST ver 1.1 criteria were published by Eisenhauer et al in the European Journal of Cancer (2009) 228-247 and may be used to define the degree of response. Eisenhauer et al is incorporated in its entirety and specifically in relation to the RECIST ver 1.1 criteria. In some embodiments, Best Overall Response (BOR) may be assessed. In some embodiments, overall response rate (ORR) may be assessed. In some embodiments Disease Control Rate (DCR) may be assessed. [0227] The goal of this study 1 is to determine the safety, tolerability and maximum tolerated dose (MTD) of rinatabart sesutecan (Rina-S; PRO1184).

[0228] FIG.1 shows the study 1 design for the PRO1184-001 drug dosage schedule. Part A (dose escalation) included five tumor types. Part B (dose expansion) included four tumor specific cohorts and one basket cohort. Incremental doses of Rina-S were administered to 5 different cancer patients over 21 days, ranging from 60 mg / m2, to 180 mg / m2at the end of the maximum therapeutic dose. Four tumor-specific cohorts and one basket cohort (ovarian cancer) were included in the study 1.

[0229] Safety assessments include physical examination, vital signs and body measurements, standard clinical laboratory assessments such as hematology, blood biochemistry, and monitoring of adverse events and serious adverse events. Table 4 shows the summary of safety data. Table 4: Summary of Safety Data Rina-S dose level ) ) ) )(7.1) (25.0) )

[0230] Rina-S was well tolerated at least up to 120 mg / m2(140 mg / m2is under evaluation), with common adverse events including hematologic cytopenias, gastrointestinal adverse events and fatigue. Most TRAEs were mild or moderate. Grade≥3 treatment related adverse events occurred in 8 (34.8%) patients. Treatment-related Serious Adverse Events (SAEs) occurred in 3 patients and were all cytopenias. Dose-limiting toxicities were observed at the 140 mg / m2and 180 mg / m2dose levels and were all hematologic cytopenias. No interstitial lung disease / pneumonitis, infusion- related reactions, keratopathy was observed.

[0231] The most common treatment-related adverse events (AEs) were nausea (n=5), decreased white blood cell counts (n=3), and fatigue, decreased lymphocyte counts, and decreased neutrophil counts (n=2 each); most events were Grade 1 or 2. Treatment-related ≥ Grade 3 hematologic AEs were reported for 2 patients treated at 120 mg / m2. No ocular toxicity or interstitial lung disease was observed. No DLTs were observed. Example 3: Determination of anti-tumor activity of Rina-S in the study 1

[0232] Fifteen patients with at least one post-baseline scan tumor assessment. Ten patients were considered efficacy evaluable (had a baseline scan and at least 1 post-baseline tumor assessment). Treatment duration ranged from 0.6 weeks to 31.0 weeks. Seventeen of 23 patients remain on treatment. FIG.2 shows the best change from baseline in target lesion tumor burden: ovarian and endometrial cancer. FIG.3 shows the percentage change in tumor size over time: ovarian and endometrial cancer. FIG.4 shows the reduction in CA-125 levels.

[0233] The overall response rate was 20.0% (95% CI: 2.5, 55.6), with 2 patients with partial response (including 1 confirmed response) and 4 patients with stable disease. Anti-tumor responses appeared to deepen over time.

[0234] Rina- S showed encouraging antitumor activity across FOLR1 expression levels in this heavily pretreated patient population and most patients had a reduction in target lesion.

[0235] Antitumor activity was observed at both dose levels and in patients with high, medium, and low FOLR1 expression, including an ongoing confirmed partial response in a patient with endometrial cancer and decreased tumor measurements in additional patients. Patients who present equivocal findings of progression may be allowed to continue the study drug with approval of the medical monitor and if, in the opinion of the investigator, the patient is clinically stable. If progression is confirmed unequivocally on the next response assessment, study drug will be discontinued. Example 6: Determination of anti-tumor activity of Rina-S in the study 2 Efficacy Analysis Primary Endpoint: PFS per Investigator Assessment

[0243] This study is designed to evaluate PFS in patients with PROC receiving either Rina-S or IC, with the hypothesis that Rina-S is superior to IC in PFS. The study is considered to have met this objective if Rina-S is superior to IC in PFS at the final analysis of PFS, using the PAS.

[0244] PFS is defined as the time from randomization to the first documentation of disease progression (PD) per RECIST v1.1 according to investigator assessment, or to death due to any cause, whichever comes first.

[0245] The non-parametric Kaplan-Meier method will be used to estimate the PFS curve in each treatment arm. The median PFS and its two-sided 95% confidence interval (CI) using the complementary log-log transformation method (Collett, London: Chapman & Hall, pp.237-251 (1994)) will be calculated. The treatment difference in PFS will be assessed by the stratified log- rank test. A stratified Cox proportional hazard model with Efron's method of tie handling will be used to assess the magnitude of the treatment difference (i.e., hazard ratio) between the treatment arms. The hazard ratio and its 95% CI from the stratified Cox model with Efron's method of tie handling and with a single treatment covariate will be reported. The same stratification factors usedfor randomization will be applied to both the stratified log-rank test and the stratified Cox model.

[0246] Since progression is assessed periodically, PD can occur any time in the time interval between the last assessment where PD was not documented and the assessment when PD is documented. For the primary analysis, for patients who have PD, the true date of disease progression will be approximated by the date of the first assessment at which PD is objectively documented using RECIST v1.1, regardless of discontinuation of study drug.

[0247] PFS will be calculated as follows: PFS (days) = (date of first documented PD or death – date of randomization) + 1

[0248] Patients without documented PD or death will be censored at the last disease assessment date. PFS will also be censored at the date of the last disease assessment documenting absence of PD or death in patients who are given anti-tumor treatment other than the study drug or are removed from the study prior to the documentation of tumor progression. If a patient does not have any on- study tumor assessments, PFS will be censored on Day 1.

[0249] PFS per BICR will be analyzed using the same methodology as a sensitivity analysis. In addition, the concordance in the PD assessment between investigator assessment and BICR assessment will be evaluated. The discordance between the investigator and BICR assessments in censoring status, time to PD, and time to censoring will be tabulated, and the data for the discordance will be listed. Key Secondary Endpoints: Overall Survival

[0250] OS is defined as the time from randomization to death due to any cause. OS will be calculated as follows: OS (days) = (date of death – date of randomization + 1).

[0251] Patients who are alive (with no OS event) will be censored at the last known date alive.

[0252] The non-parametric Kaplan-Meier method will be used to estimate the OS curve in each treatment arm. The median OS and its two-sided 95% CI using the complementary log-log transformation method (Collett, London: Chapman & Hall, pp.237-251 (1994)) will be calculated. The treatment difference in OS will be assessed by the stratified log-rank test. The same stratification factors used for randomization will be applied to both the stratified log-rank test and the stratified Cox model. The Kaplan-Meier estimates of OS rate at 12 months will be compared between the Rina-S arm and IC arm and proportionality of hazard ratio over time will be explored. Objective Response Rate

[0253] ORR is defined as the percentage of the patients who have a best response of confirmed CR or PR using RECIST v1.1. Responses per investigator will be used as the primary analysis of ORR and responses based on BICR, incorporating status of CA-125 normalization for CR, will be used as a sensitivity analysis.

[0254] If PFS per investigator is declared positive at the primary analysis and OS is declared positive, a final analysis of ORR per investigator assessment will be performed. The following hypothesis will be tested at a one-sided α level of 0.025: H0: ORRRina-S= ORRICvs Ha: ORRRina-S> ORRICwhere ORRRina-S is the ORR in the Rina-S arm and ORRIC is the ORR in the IC arm.

[0255] ORR will be analyzed using a Cochran-Mantel-Haenszel (CMH) test (Cochran, Biometrics 10, 417-451 (1954); Mantel and Haenszel, J Natl Cancer Inst 22, 719-748 (1959)) stratified by the randomization stratification factors to test for differences between the Rina-S arm and the IC arm. P- value, odds ratio and its 95% CI will be reported. The 95% CIs for ORR by treatment arm based on Clopper-Pearson method (Clopper and Pearson, Biometrika 26, 404-413 (1934)) will also be provided. The PAS population will serve as the primary population for ORR and a supportive analysis of ORR will be carried out using the patients in the ITT and EE analysis set.

[0256] ORR per BICR will be analyzed using the same methodology based on PAS as a sensitivity analysis. In addition, the concordance in the best overall response (BOR) between investigator assessment and BICR assessment will be evaluated. The discordance between the investigator and BICR assessments in BOR and time to BOR will be tabulated, and the data for the discordance will be listed. Determination of Sample Size

[0257] The primary endpoint of this study is PFS as assessed by the investigator, with OS and ORR as assessed by the investigator as key secondary endpoints.

[0258] The primary analysis will be conducted on the Primary Analysis Set (PAS). This set excludes patients with high FRα expression who would otherwise be candidates for mirvetuximab soravtansine, but who have not received this prior therapy due to being enrolled in regions where it is not available. It is expected that approximately 80% of the study population will be included in the PAS.

[0259] The sample size calculation for the final PFS analysis (PFS primary analysis) focuses on demonstrating superior PFS in the Rina-S arm compared to the IC arm and is based on the following assumptions: • PFS follows an exponential distribution with a median of 4 months in the IC arm • The hazard ratio between the Rina-S arm and the IC arm is 0.67 (corresponding to a median PFS of 6 months in the Rina-S arm)• An enrollment period of 21 months for a total of approximately 530 patients in order to reach approximately 420 patients in the primary population

[0260] A yearly censoring rate of 10% for PFS.A PFS hazard ratio of 0.67 is proposed for hypothesis testing in this study, equivalent to an approximate 2-month improvement in the median PFS assuming exponential distribution, which may be considered clinically meaningful. In patients with PROC, improvement in PFS has been associated with decreased tumor-related symptoms, improvement in patient-reported outcomes, and improvement in overall survival.

[0261] With 262 PFS events, the study has 90% power to demonstrate superiority of the Rina-S arm over the IC arm at a one-sided α = 0.025 when the true hazard ratio is 0.67. To ensure sufficient follow-up for the treated patients, the PFS primary analysis will be conducted when at least 262 PFS events have occurred and patients in the PAS have had the opportunity to be followed-up for a minimum of 3 months after randomization (or have documented PD, died, or been withdrawn from the study within 3 months after randomization).

[0262] If the primary endpoint of PFS is statistically significant at a one-sided α level of 0.025, the hierarchical testing procedure will be used to test the two key secondary endpoints in the following order at a one-sided α level of 0.025 to control the study-wide type I error rate. • OS • ORR per RECIST v1.1 as assessed by the investigator.

[0263] The key secondary endpoint of OS will be tested only after PFS is declared positive. At that timepoint, an interim analysis of OS will be performed (with approximately 131 OS events [53%] projected), and a final analysis of OS will be performed when at least 248 OS events have occurred, or at the time of study closure, when all treated patients have had the opportunity to be followed for at least 3 years after randomization, or have died or been withdrawn from the study, whichever comes first. The O’Brien-Fleming spending function will be applied to these two OS analysis timepoints at the one-sided α level of 0.025. The exact α allocation at the time of OS interim analysis depends on the number of OS events accrued by that time. With 248 OS events, the study has 80% power to demonstrate superiority of the Rina-S arm over the IC arm at a one-sided α = 0.025 when the true hazard ratio is 0.70. In addition, the sample size calculation of OS analyses assumes a median OS in the IC arm of 13 months, a censoring rate of 2% per year, and the same enrollment schedule as specified in the sample size calculation for PFS analysis.

[0264] ORR per investigator will be tested after the efficacy bound for OS has been crossed. Since all patients will be enrolled at that time, the following hypothesis will be tested at 1-sided 2.5% level with no group sequential spending function applied: H0: ORRRina-S= ORRICvs Ha: ORRRina-S> ORRICwhere ORRRina-S is the ORR in the Rina-S arm and ORRIC is the ORR in the IC arm.

[0265] Assuming the true ORRRina-S is 30% and the true ORRIC is 16%, a sample size of approximately 420 patients (210 patients / arm) achieves approximately 93% power to detect a difference in ORR of 14% (30% vs 16%) using the method of Farrington and Manning (Farrington and Manning, Stat Med 9, 1447-1454 (1990)) for a study to test the difference between two binomial event rates.

[0266] The sample size calculation for PFS, OS, and ORR are carried out using R package gsDesign. Example 7: Dose escalation and dose expansion of the Study 1

[0267] As continuation of the Study 1, 53 patients regardless of FRα expression were treated in Part A (dose escalation), including 32 patients with previously treated ovarian cancer (OC), 11patients with previously treated endometrial cancer (EC), and 10 patients with other tumor types (NSCLC, breast cancer, and mesothelioma). The median numbers of prior therapies of the patients with ovarian cancer and endometrial cancer were 5 (1-14) and 3 (1-14), respectively.75% of the patients with ovarian cancer had received bevacizumab, and 91% were platinum resistant. All the patients with endometrial cancer received PD-1 inhibitor. Rina-S doses from 60-180 mg / m2were evaluated in Part A. The MTD was 140 mg / m2. Doses of 100 and 120 mg / m2were selected for evaluation in Part B (dose expansion).

[0268] In Part B, 35 patients with ovarian cancer and 13 patients with endometrial cancer were treated. The median numbers of prior therapies of the patients with ovarian cancer and endometrial cancer were 3 (1-4) and 3 (1-7), respectively.

[0269] Planned tumor-specific dose expansion includes OC, EC, and EGFR-mutant NSCLC regardless of FRα expression (FRα levels retrospectively assessed). The patients with ovarian cancer in Part B (cohort B1, ovarian cancer dose expansion) was randomized 1:1 to Rina-S 100 mg / m2or Rina-S 120 mg / m2. The inclusion criteria for cohort B1 are as shown below. 1) Histologically or cytologically confirmed OC (must have epithelial ovarian cancer, primary peritoneal cancer, or fallopian tube cancer) 2) Prior treatment (1-3 lines for PROC or 4 lines regardless of platinum-sensitivity status) 3) ECOG PS 0-1 4) Measurable disease per RECIST v1.1 5) Adequate hematologic, hepatic, renal, and cardiac function

[0273] As continuation of the Study 1, the antitumor activity of Rina-S was evaluated in dose escalation and dose expansion.

[0274] In Part A (dose escalation), Rina-S showed encouraging antitumor activity in heavily pretreated patients with OC and EC. Table 7 shows the results of antitumor activity for Part A. Fig.6 shows the best change in target lesion for OC and EC dose escalation. Response assessment per RECIST v1.1.bBased on investigator assessment.

[0275] In Part B, Rina-S showed encouraging antitumor activity at 120 mg / m2, including a complete response, in heavily pretreated patients with OC. Table 8 shows the results of antitumor activity for cohort B1 of Part B. The Treatment duration range was 3.0-42.0+ weeks. The Median on-study follow-up was 24 weeks. Fig.7 shows the best change in target lesion for OC dose expansion. Table 8: Cohort B1: OC Dose ExpansionaRina-S Rina-SaBased on investibResponse-evaluable population.cOne patient in the 120 mg / m2cohort with prior mirvetuximab soravtansine was not response-evaluable.

[0276] As shown in Fig.8, most responses with Rina-S 120 mg / m2were observed early (at week6) and all confirmed responses with 120 mg / m2were ongoing at the time of data cutoff in patients with heavily pretreated OC. As shown in Fig.9, responses in patients with OC were observed across all FRα expression levels.Example 9: Activity of combinations between Rina-S and standard-of-care therapy drugs invitro

[0277] Cells were seeded in a 96-well plate (the wells being flat bottomed) at an appropriate density depending on the cell line (typically 1,000-3,000 cells per well in 100 µL of appropriate culture media). After overnight incubation, cells were treated with increasing concentrations of Rina-S, carboplatin or olaparib alone or Rina-S in combination with carboplatin (12.5 μM) or olaparib (30 μM) (concentrations were chosen based on IC50 values). Cell viability was evaluated after 4 days. Cell-Titer Glo (Promega, CAT# G7572) was added into the wells with luciferase readings collected after 5 minutes and analyzed by Microplate readers. All readings were normalized as percentage of viable cells in the untreated control wells and the IC50values were calculated by GraphPad Prism® software.

[0278] As shown in Figs.10A and 10B, the combination of Rina-S with carboplatin did not enhance the cytotoxicity of Rina-S on OVCAR-8, while the combination of Rina-S with olaparib enhanced the cytotoxicity effect of Rina-S on HEC-1-A. Example 10: Induction of hallmarks of immunogenic cell death (ICD) by exatecan and Rina-S

[0279] Hall markers of ICD induced by Rina-S and exatecan were evaluated in FOLRα-positive cells. KB and 3LL-FRα were seeded at 5*103cells per well in a 96-well plate (for ATP and HMGB1 detection) or 4*104cells per well in a 24-well plate (for cell surface calreticulin detection) and incubated at 37 ℃ overnight. The wells of the plates used were flat bottomed. For extracellular ATP (Adenosine triphosphate) and HMGB1 (High box group 1 protein) detection, cell supernatants were removed and 200 μL serially diluted Rina-S (10-fold dilutions; 1000 to 0.1 nM) or exatecan (10-fold dilutions; 800 to 0.8 nM) were added to the wells, and incubated for 24-72 h. ATP and HMGB1released in the supernatant were detected by CTG kit (Promega; Cat No: G7572) and Lumit HMGB1 Immunoassay kit (Promega; Cat No: W6112) separately. For cell surface calreticulin detection, cell supernatants were removed and 500 μL serially diluted Rina-S (10-fold dilutions; 100 to 1 nM) or exatecan (10-fold dilutions; 80 to 0.8 nM) were added to the wells and incubated for 48 h. Cell surface calreticulin was detected by flow cytometry using anti-calreticulin monoclonal antibody (Enzolifesciences; Cat No: ADI-SPA-601PE-D).

[0280] Rina-S and exatecan upregulated ICD biomarkers in FOLRα-positive cells in a concentration-dependent manner, as indicated in Figs.11A-11F. Example 11: Anti-tumor activity of combinations of Rina-S with SoC agents in mouse models in vivo

[0281] Rina-S shows strong potentiation of anti-tumor activity in combination with carboplatin, bevacizumab, olaparib (PARPi), or immunotherapy. Combinations of Rina-S with standard-of-care (SOC) agents were evaluated in preclinical mouse models of ovarian and endometrial cancer in vivo. Around 15 and 17 days after tumor inoculation, mice with an average tumor size of 100.16~180.86 mm3were randomized into 4 groups (n = 5–6 per group). On either Day 0 or Day 1, following randomization (defined as Day 0), the mice received the test article or PBS at the indicated concentrations. In detail: female NDG (ultra-immunodeficient phenotype) mice bearing established OVCAR-8 ovarian cancer xenografts (n = 6 mice / group) were treated with Rina-S (3 mg / kg, single dose) or bevacizumab (5 mg / kg, single dose) (Fig.12A); Rina-S (3 mg / kg single dose) or carboplatin (60 mg / kg, QW * 4) (Fig.12B), IV, or IP, alone or in combination. Female BALB / c nude mice bearing established HEC-1-A endometrial cancer xenografts (n = 5 mice / group) were treated with Rina-S (5 mg / kg, single dose) or olaparib (50 mg / kg, daily for 5 days X 4 weeks) IV, or by oral gavage, alone or in combination (Fig.12C).

[0282] Combinations of Rina-S with immuno-oncology therapy agents were evaluated in preclinical models of lung cancer and colorectal cancer. Around 6 days after tumor inoculation, mice with average tumor size of 65~142 mm3were randomized into 4 or 6 groups (n = 6 per group). Oneither Day 0 or Day 1, following randomization (defined as Day 0), the mice received the test article or PBS at indicated concentrations. In detail: female C57BL / 6 bearing established lung cancer (3LL- FRα) xenografts (n = 6 mice / group) were treated with Rina-S (2.5 mg / kg, single dose) or CS1003 (nofazinlimab; anti-PD-1) (10 mg / kg, Q3D*4) IV, alone or in combination (Fig.12D). Female C57BL / 6 mice bearing established MC38-FRα colorectal cancer xenografts (n = 6 mice / group) were treated with Rina-S (1.5 or 2.5 mg / kg, single dose) or CS1003 (0.5 mg / kg, Q3D*4) IV, alone or in combination (Fig.12E).

[0283] Tumor size was measured twice a week in two dimensions using a caliper, and tumor volume was calculated in mm3using the formula: V = 0.5a x b2where a and b are the long and short diameters of the tumor, respectively. Tumor volume exceeding 2000 mm3was defined as reaching the end of the experiment. Body weight was measured twice a week. Animal body weight was monitored as an indirect measure of tolerability.

[0284] Combination of Rina-S with carboplatin or bevacizumab or olaparib resulted in potentiated antiproliferative effects in the OVCAR-8 and HEC-1-A model in vivo, suggesting a benefit of these combinations in relevant tumor types (such as ovarian cancer and endometrial) in the clinic. No significant weight loss was observed in any of the treatment groups, and there were no reports of morbidity or mortality during the treatment period.

[0285] Combinations of Rina-S with CS1003 (anti-PD-1) resulted in potentiated anti-tumor effects in the 3LL-FRα and MC38-FRα model in vivo, suggesting a benefit for combined treatment of Rina-S and immune checkpoint inhibitors (ICI) in the clinic.
