USE OF COMPOUND 1 IN TREATMENT OR PREVENTION OF DISEASES SUCH AS PULMONARY FIBROSIS OR LUNG DISEASES WITH FIBROSISTECHNICAL FIELD

[0001] The present invention relates to use of N- (3-chloro-5- (trifluoromethyl) phenyl) -3- ( (6- (4-hydroxypiperidin-1-yl) imidazo [1, 2-b] pyridazin-3-yl) ethynyl) -2-methylbenzamide (also referred to herein as compound 1) or a pharmaceutically acceptable salt thereof in the treatment or prevention of diseases such as pulmonary fibrosis or lung diseases with fibrosis.BACKGROUND

[0002] Pulmonary fibrosis is the ultimate outcome of the development, evolution, and scarring of many lung diseases. It is a disease process characterized by alveolar epithelial cell injury, aggregation and proliferation of inflammatory cells, activation of fibroblasts and myofibroblasts, transformation of extracellular matrix (ECM) components, and activation of signaling pathways and gene expression. The ultimate result of this process is the loss of lung elasticity and the loss of alveolar surface area, resulting in impairment of gas exchange efficiency and lung function. Pulmonary fibrosis has various etiologies; for example, acute severe viral pneumonia, post-healed pulmonary tuberculosis, and pneumoconiosis late stage cause secondary pulmonary fibrosis. However, some have unknown etiologies, such as idiopathic pulmonary fibrosis (IPF) ; IPF develops in a chronic and progressive manner with a complicated pathogenesis and a pathological manifestation of usual interstitial pneumonia (UIP) , and the average survival time after diagnosis is about 3 years, with a 5-year survival rate of 30-50%, indicating an extremely poor prognosis. Currently, pirfenidone and nintedanib are clinically used for treatment to relieve disease progression. However, for progressive pulmonary fibrosis, pirfenidone treatment does not significantly improve mortality, DLCO (diffusing capacity of the lung for carbon monoxide) , and respiratory symptoms; in addition, the risk of gastrointestinal discomfort is increased by 1.8 times, the risk of photosensitivity is increased by 4.9 times, and the overall risk of adverse events is increased by 1.2 times. Nintedanib treatment can reduce the annual decline in FVC (forced vital capacity) in CTD-ILD (connective tissue disease-associated interstitial lung disease) , fibrotic NSIP (nonspecific interstitial pneumonia) , and fibrotic occupational lung disease. However, it has no significant effect on fibrotic HP (hypersensitivity pneumonitis) , sarcoidosis, and unclassified ILD (interstitial lung disease) , and does not significantly improve mortality; in addition, the overall risk of adverse events is increased by 1.1 times, the adverse events mainly including gastrointestinal tract issues, elevated liver transferases,  and the like. Therefore, there is still an urgent need to develop effective and safe pulmonary fibrosis drugs.

[0003] DDR1 / 2 inhibitor drugs have not yet entered the clinical trial stage for pulmonary fibrosis. Two marketed TRK inhibitor drugs (larotrectinib and entrectinib) are both targeted at tumor patients with NTRK gene mutations. There are no marketed drugs for pulmonary fibrosis indications, nor clinical trials and preclinical studies for pulmonary fibrosis indications. For the PDGFR target and CSF1R target, only one marketed multi-target inhibitor, nintedanib, is used for anti-pulmonary fibrosis treatment.

[0004] The bleomycin (BLM) -induced pulmonary fibrosis model is the most commonly used experimental pulmonary fibrosis model. BLM induces DNA strand breakage, generates free radicals, induces oxidative stress responses, causes cell apoptosis or necrosis, and induces inflammatory responses and fibrosis. This model is employed in the present invention to test the therapeutic effect of the compound on pulmonary fibrosis. It is known in the art that the bleomycin-induced pulmonary fibrosis model is the most commonly used experimental pulmonary fibrosis model, as the histopathological changes of bleomycin-induced pulmonary fibrosis are very similar to those of human pulmonary fibrosis.

[0006] The present invention relates to use of N- (3-chloro-5- (trifluoromethyl) phenyl) -3- ( (6- (4-hydroxypiperidin-1-yl) imidazo [1, 2-b] pyridazin-3-yl) ethynyl) -2-methylbenzamide (also referred to herein as compound 1) or a pharmaceutically acceptable salt thereof in the treatment of pulmonary fibrosis or lung diseases with fibrosis. In the present invention, the pulmonary fibrosis or lung diseases with fibrosis include idiopathic pulmonary fibrosis (IPF) , non-idiopathic pulmonary fibrosis, and progressive pulmonary fibrosis. For example, in the present invention, the pulmonary fibrosis or lung diseases with fibrosis include idiopathic pulmonary fibrosis, giant cell interstitial pneumonia, sarcoidosis, cystic fibrosis, respiratory distress syndrome, granulomatosis, silicosis, drug-induced pulmonary fibrosis (e.g., pulmonary fibrosis induced by drugs such as bleomycin, carmustine, cyclophosphamide, amiodarone, procainamide, penicillamine, and nitrofurantoin) , and asbestosis.

[0008] In a first aspect, the present invention provides use of compound 1 or a pharmaceutically acceptable salt thereof in the preparation of a medicament for treating or preventing pulmonary fibrosis or lung diseases with fibrosis.

[0009] In a second aspect, the present invention provides a method for treating or preventing pulmonary fibrosis or lung diseases with fibrosis, comprising administering to a subject in need thereof a therapeutically effective amount of compound 1 or the pharmaceutically acceptable salt thereof.

[0010] In a third aspect, the present invention provides a pharmaceutical composition for treating or preventing pulmonary fibrosis or lung diseases with fibrosis, comprising compound 1 or the pharmaceutically acceptable salt thereof, and a pharmaceutically acceptable excipient.

[0011] In a fourth aspect, the present invention provides compound 1 or a pharmaceutically acceptable salt thereof for use in the treatment or prevention of pulmonary fibrosis or lung diseases with fibrosis.

[0037] Example 1: Inhibitory Effect of Compound 1 on Enzyme Activities of TRK, DDR, and CSF1R

[0038] Assay principle of HTRF method:

[0039] A kinase phosphorylates a substrate, and an antibody labeled with Eu-CryPtate binds to a phosphorylation site in the substrate; streptavidin-XL665 binds to the substrate biotin; when Eu and XL665 are in close proximity, Eu as a donor, after being excited by a light source (320 nm) , emits emission light (620 nm) and transfers energy resonance to a nearby XL665 receptor, and the receptor emits emission light (665 nm) after being excited; the specific signal is proportional to the phosphorylated substrate; when an inhibitor is added, the phosphorylation level is inhibited, the emission light at 665 nm is undetectable, and only the emission light at 620 nm is detectable, such that the level of inhibition of kinase activity by the compound can be evaluated.

[0041] Compound 1 was serially diluted in DMSO from a starting concentration of 1 μM to give a total of 10 concentrations after four-fold dilution. Replicate wells were set for the assay. 25 nL of the compound was transferred to a 384 reaction plate (Catalog No. 784075, Greiner) using Echo 665.2× kinase solution was prepared with 1× kinase reaction buffer (5× Buffer, 5 mM MgCl2, 1 mM DTT, H2O, and 1 mM MnCl2) . 2.5 μL of TRKA (1.5 nM, 08-186, Carna) solution was transferred to the 384 reaction plate. The plate was centrifuged at 1000 rpm for 60 s using a centrifuge and incubated at 25 ℃ for 10 min. A mixed solution of 2× biotin-labeled tyrosine kinase substrate (1 μM) (Catalog No. 61TK0BLE, Cisbio –PerkinElmer) and ATP (25 μM) was prepared using the kinase reaction buffer. 2.5 μL of the mixed solution of the substrate and ATP was added to the reaction plate to start the reaction, and the plate was centrifuged at 1000 rpm for 60 s using the centrifuge. The plate was sealed with a plate-sealing film and incubated at 25 ℃ for 40 min. 5 μL of a mixed solution of Eu-CryPtate-labeled anti-tyrosine kinase antibody and streptavidin-XL665 was added to each well of the reaction plate. The plate was centrifuged at 1000 rpm for 1 min and incubated at 25 ℃ for 60 min. The fluorescence signals at 620 nm (Cryptate) and 665 nm (XL665) were read using a BMG microplate reader. The ratio of the fluorescence signals, i.e., (665 nm / 620 nm) ×10000, in each well was calculated, and the ratio represented the degree of the kinase activity. TRKB enzyme activity assay:

[0048] Compound 1 was serially diluted in DMSO from a starting concentration of 1 μM to give a total of 10 concentrations after four-fold dilution. Replicate wells were set for the assay. 2×kinase solution was prepared with 1× kinase reaction buffer (5× Buffer, 5 mM MgCl2, 1 mM DTT, H2O, and 1 mM MnCl2) . 2.5 μL of DDR2 (1.3 nM, 08-114, Carna) solution was transferred to a 384 reaction plate. The plate was centrifuged at 1000 rpm for 60 s using a centrifuge and incubated at 25 ℃ for 10 min. A mixed solution of 2× biotin-labeled tyrosine kinase substrate (1 μM) and ATP (25 μM) was prepared using the kinase reaction buffer. 2.5 μL of the mixed solution of the substrate and ATP was added to the reaction plate to start the reaction, and the plate was centrifuged at 1000 rpm for 60 s using the centrifuge. The plate was sealed with a plate-sealing film and incubated at 25 ℃ for 40 min. 5 μL of a mixed solution of Eu-CryPtate-labeled anti-tyrosine kinase antibody and streptavidin-XL665 was added to each well of the reaction plate. The plate was centrifuged at 1000 rpm for 1 min and incubated at 25 ℃ for 60 min. The fluorescence signals at 620 nm (Cryptate) and 665 nm (XL665) were read using a BMG microplate reader. The ratio of the fluorescence signals, i.e., (665 nm / 620 nm) × 10000, in each well was calculated, and the ratio represented the degree of the kinase activity.

[0050] Compound 1 was serially diluted in DMSO from a starting concentration of 1 μM to give a total of 10 concentrations after four-fold dilution. Replicate wells were set for the assay. 2×kinase solution was prepared with 1× kinase reaction buffer (5× Buffer, 5 mM MgCl2, 1 mM DTT, H2O, and 1 mM MnCl2) . 2.5 μL of CSF1R (1.3 nM, 08-114, Carna) solution was transferred to a 384 reaction plate. The plate was centrifuged at 1000 rpm for 60 s using a centrifuge and incubated at 25 ℃ for 10 min. A mixed solution of 2× biotin-labeled tyrosine kinase substrate (1 μM) and ATP (25 μM) was prepared using the kinase reaction buffer. 2.5 μL of the mixed solution of the substrate and ATP was added to the reaction plate to start the reaction, and the plate was centrifuged at 1000 rpm for 60 s using the centrifuge. The plate was sealed with a plate-sealing film and incubated at 25 ℃ for 40 min. 5 μL of a mixed solution of Eu-CryPtate-labeled anti-tyrosine kinase antibody and streptavidin-XL665 was added to each well of the reaction plate. The plate was centrifuged at 1000 rpm for 1 min and incubated at 25 ℃ for 60 min. The fluorescence signals at 620 nm (Cryptate) and 665 nm (XL665) were read using a BMG microplate reader. The ratio of the fluorescence signals,  i.e., (665 nm / 620 nm) × 10000, in each well was calculated, and the ratio represented the degree of the kinase activity.

[0052] Example 2: Evaluation of Therapeutic Effect of Compound 1 on Induced Pulmonary Fibrosis in Mice

[0053] Method

[0054] The pharmacodynamic experiment was designed with 5 groups, which were groups receiving the following treatments:

[0055] Group 1: sham group (sham surgery group) ;

[0056] Group 2: model group;

[0057] Group 3: compound 1, 1 mg / kg QD (hereinafter, mg / kg was abbreviated as mpk) ;

[0058] Group 4: compound 1, 5 mg / kg QD;

[0059] Group 5: compound 1, 25 mg / kg QD.

[0060] C57BL / 6 mice (male, aged 8 weeks, weighing 20-25 g) were from Jiangsu GemPharmatech Co., Ltd. On day 1 of the experiment, after anesthesia of mice, the mice in the model group and the administration group were intratracheally injected (i. t. ) with bleomycin (BLM) at a dose of 0.66 mg / kg and with a volume of 50 μL on day 1. The sham surgery group was intratracheally injected with normal saline with a volume of 50 μL. On days 5 to 21 of the experiment, intragastric administration was performed once a day; the sham surgery group and the model group were given a vehicle by intragastric administration with an administration volume of 10 mL / kg body weight once a day. The vehicle used was 5%DMSO + 5%Tween80 + 20%PEG400 + 70%H2O, and all compounds were freshly prepared before use.

[0062] On the last day of the experiment, two hours after administration, bronchoalveolar lavage fluid was collected; cytomorphological test was performed on the BALF using smear microscopy, and the total cells, monocytes, neutrophils, and lymphocytes were counted.

[0063] The left and right lungs were taken and placed in 10%NBF for fixation. After 24 h of fixation, conventional sectioning and Masson trichrome staining were performed. Pathological section results were scored using the modified Ashcroft Score, with the scoring criteria shown in Table 3, and Masson-positive staining areas were quantified. Significance analysis was performed using t-test, one-way ANOVA or two-way ANOVA, and post-hoc Dunnett’s test. The Mann-Whitney or Kruskal-Wallis nonparametric test was used when the sample size in the groups was small or did not conform to the normal distribution. Variance p < 0.05 indicates a significant difference.

[0069] In the sham surgery group (group 1) , animal lung tissues showed a normal lung structure with a small amount of collagen around the bronchioles. 21 days after tracheal injection of bleomycin, the model group (group 2) showed significant alveolar wall thickening, continuous fibrotic bands, and fibrotic masses, with a fibrotic area of up to 59%. The normal structure of lung tissues was extensively damaged (FIG. 6) , and the modified Ashcroft score of lung tissues was significantly increased (FIG. 7) , with a significant increase in fibrotic area (FIG. 8) . Compared with the model group (group 2) given the vehicle, test compound 1, administered intragastrically once a day at doses of 1 mpk, 5 mpk, and 25 mpk for 17 consecutive days, could significantly reduce the modified Ashcroft score (FIG. 7) and the fibrotic area stained with Masson trichrome stain (FIG. 8) in lung tissues of model mice.

[0071] In conclusion, compound 1, at doses of 1 mpk, 5 mpk, and 25 mpk, showed significant anti-inflammatory and anti-fibrotic effects in the bleomycin-induced mouse pulmonary fibrosis model, and could significantly counteract the body weight loss due to modeling.
