USE OF COMPOUND 1 IN TREATMENT OR PREVENTION OF DISEASE RELATED TO RET FUSION AND / OR RET MUTATIONTECHNICAL FIELD

[0001] The present invention relates to use of N- (3-chloro-5- (trifluoromethyl) phenyl) -3- ( (6- (4-hydroxypiperidin-1-yl) imidazo [1, 2-b] pyridazin-3-yl) ethynyl) -2-methylbenzamide or a pharmaceutically acceptable salt thereof in the treatment or prevention of a disease related to a RET fusion and / or a RET mutation.BACKGROUND

[0002] RET is a transmembrane tyrosine receptor kinase, which is capable of binding to receptors and activating downstream signaling pathways, and playing a role in cell differentiation, growth, migration, and survival. The occurrence of loss of function mutations in RET leads to Hirschsprung disease, while the occurrence of gain of function mutations is associated with a variety of human tumors.

[0003] As the RET gene is subjected to activating variations, which mainly include RET gene point mutations and RET gene fusions, overdrive function of the RET gene leads to backward signal transmission and activation of downstream PI3K and MAPK signaling pathways, resulting in cell proliferation and further causing tumor diseases. The RET point mutations occur mainly in multiple endocrine neoplasias (MEN2A and MEN2B) and medullary thyroid cancer (MTC) , while RET fusion proteins are mainly found in papillary thyroid cancer and non-small cell lung cancer. In papillary thyroid cancer, there are more than a dozen types of partners that form fusion proteins with RET, mainly including CCDC6-RET and NCOA4-RET. In non-small cell lung cancer, the primary fusion protein is RET-KIF5B. The RET mutations also occur in cancers such as pancreatic cancer, salivary gland cancer, colorectal cancer, ovarian cancer, chronic myelomonocytic leukemia, and breast cancer (Saha D, Ryan K, Lakkaniga N, et al. Targeting Rearranged during Transfection in Cancer: A Perspective on Small-Molecule Inhibitors and Their Clinical Development. [J] . Journal of medicinal chemistry, 2021, 64 (16) : 11747-11773. DOI: 10.1021 / acs. jmedchem. 0c02167. ) . There are many RET mutants, and in addition to the above-mentioned ones, the point mutations also include, but are not limited to, RET (G533C) , RET (C609F / G / R / S / Y / W) , RET (C611F / G / R / S / Y / W) , RET (D613Y) , RET (C618F / G / R / S / Y / W) , RET (C620F / G / R / S / Y / W) , RET (C630R / Y / F / S) , RET (C634W / F / G / R / S / Y / K) , RET (K666E) , RET (G691S) , RET (R749T) , RET (E762Q) , RET (E768D) , RET (V778I) , RET (L790F) , RET (Y791F) , RET (V804L / E / M) , RET (Y806H) , RET (R813Q) , RET (A883F) , RET (S891A) , RET (S904F / A) , RET (R912P) , RET (M918T) , and the like; the fusion proteins further include, but are not limited to, ACBD5, AFAP1L2, AKAP13, ANKRD26, DLG5, ERC1, FKBP15, GOLGA5, HOOK3, KIAA1468, NCOA4, PCM1, PDCD10, PPFIBP2, PRKAR1A, SPECC1L, SQSTM1, TBL1XR1, TRIM24, TRIM27, and UEVLD. Sometimes amino acid mutations also occur in the fusion proteins.

[0004] The RET mutations and / or fusion proteins play a driving role in various tumors, and it is clinically significant to develop inhibitors against RET mutations. Currently, several kinase inhibitors targeting RET have been approved by the FDA in the United States, such as Vandetanib, Cabozantinib, Sorafenib, Lenvatinib, Pralsetinib, and Selpercatinib, wherein the multi-target kinase inhibitors, such as Vandetanib, Cabozantinib, Sorafenib, and Lenvatinib, have poor selectivity and may cause great side effects to patients during use. Pralsetinib and Selpercatinib are highly selective RET inhibitors. However, in addition to inhibiting RET kinase, Selpercatinib also has strong inhibitory effects on kinases such as KDR, VEGFR2, PLK4, and SAK. Moreover, animal safety concerns are raised when Selpercatinib is used at doses higher than 30 mg / kg. In addition to inhibiting RET kinase, Pralsetinib also has strong inhibitory effects on kinases such as DDR1, JAK1, TRKC, FLT3, JAK2, and TRKA, and also poses safety concerns at high doses. Similar to most targeted drugs, the two drugs also develop resistance during use, such as RET G810R / C / N and V804E. Currently, no drugs targeting these resistance mechanisms have been approved to be on the market. Therefore, there is an urgent clinical need for a new generation of drugs that can overcome the existing drug resistance mechanisms.

[0005] CN113831344B discloses a series of TRK inhibitors, including N- (3-chloro-5- (trifluoromethyl) phenyl) -3- ( (6- (4-hydroxypiperidin-1-yl) imidazo [1, 2-B] pyridazin-3-yl) ethynyl) -2-methylbenzamide (hereinafter referred to as compound 1) .SUMMARY OF THE INVENTION

[0006] The present invention relates to use of N- (3-chloro-5- (trifluoromethyl) phenyl) -3- ( (6- (4-hydroxypiperidin-1-yl) imidazo [1, 2-b] pyridazin-3-yl) ethynyl) -2-methylbenzamide (also referred to herein as compound 1) or a pharmaceutically acceptable salt thereof in the treatment or prevention of a disease related to a RET fusion and / or a RET mutation.

[0007] In a first aspect, the present invention provides use of compound 1 or the pharmaceutically acceptable salt thereof in preparation of a medicament for treating or preventing a disease related to a RET fusion and / or a RET mutation.

[0008] In a second aspect, the present invention provides a method for treating or preventing a disease related to a RET fusion and / or a RET mutation, comprising administering to a subject in need thereof a therapeutically effective amount of compound 1 or the pharmaceutically acceptable salt thereof.

[0009] In a third aspect, the present invention provides a pharmaceutical composition for treating or preventing a disease related to a RET fusion and / or a RET mutation, comprising compound 1 or the pharmaceutically acceptable salt thereof, and a pharmaceutically acceptable excipient.

[0010] In a fourth aspect, the present invention provides compound 1 or a pharmaceutically acceptable salt thereof for use in the treatment or prevention of a disease related to a RET fusion and / or a RET mutation.

[0019] Example 1: Inhibition Effect of Compound 1 on RET Mutations

[0020] The test compound 1 was dissolved using DMSO and well mixed by vortex shaking. A 4-fold gradient dilution with DMSO was performed with a first concentration of 10 μM. The final assay concentrations of the compounds were 10000 nM, 2500 nM, 625 nM, 156.25 nM, 39.06 nM, 9.77 nM, 2.44 nM, 0.61 nM, 0.15 nM, and 0.038 nM. The blank control was 1%DMSO.

[0021] ADP-Glo assay was used to test the inhibition effect of compound 1 on RET R749T and RET A883F: 2× ATP / substrate solution and 2× kinase solution with kinase reaction buffer (Formulation of substrate mixture in table 1) were prepared. 40 nL of the compound dilution was transferred into the 384 assay plate using Echo 655; the 384 assay plate was centrifuged and then 2 μL of 2× kinase solution was added to the 384 assay plate, which was further centrifuged at 1000 rpm for 1 minute and incubated at 25℃ for 10 minutes. 2 μL of 2× substrate and ATP solution were added to the 384 assay plate, which was centrifuged at 1000 rpm for 1 minuted, and incubated at 25℃ for 60 / 120 minutes. 4 μL of ADP-Glo was transferred into the 384 assay plate, which was centrifuged at 1000 rpm for 1 minute, and incubated at 25℃ for 40 minutes. 8 μL of the assay solution was transferred into a 384 plate, which was centrifuged at 1000 rpm for 1 minute, and incubated at 25℃ for 40 minutes. The luminescence signal was read by using a microplate reader.

[0022] HTRF assay was used to test the inhibition effect of compound 1 on RET E762Q, RET G691S, RET G810C, RET G810S, RET L730I, RET L730M, RET M918T, RET M918T G810S, RET R813Q, RET R912P, RET S891A, RET S904A, RET V778I, RET V804L, and RET Y806H: 2×ATP / substrate solution and 2× Kinase solution with kinase reaction buffer (Formulation of substrate mixture in table 1) were prepared. 50 nL of the compound dilution was transferred into the 384 assay plate using Echo 655; the 384 assay plate was centrifuged and then 2.5 μL of 2×Kinase solution was added to the 384 assay plate, which was centrifuged at 1000 rpm for 1  minute and incubated at 25℃ for 10 minutes. 2.5 μL of 2× substrate and ATP solution were added to the 384 assay plate, which was centrifuged at 1000 rpm for 1 minute, and incubated at 25℃ for 30 minutes. 2× XL665 and antibody detection reagents were formulated with assay buffer. 5 μL of kinase detection reagent was added to the assay plate, which was incubated at 25℃. The assay plate was centrifuged at 1000 rpm for 1 minute and further incubated at 25℃ for 1 hour. The fluorescence signal at 615 nm (Cryptate) and 665 nm (XL665) was read by using a microplate reader.

[0023] The negative control read value was set as 0%inhibition rate, the positive control read value was set as 100%inhibition rate, and the inhibition rate of each test solution was calculated.

[0028] Example 2: In Vitro Inhibition of Activity of RET C634W Mutant by Compound 1

[0029] TT cells were cultured at 37 ℃ with 5%CO2. After the logarithmic growth phase, the cell suspension was centrifuged at 1000 rpm for 5 minutes. The cell pellet was collected, and the supernatant was discarded. The cell pellet was then resuspended in fresh culture medium, and the cells were adjusted to a proper cell density, ensuring a cell viability of over 90%. The cells were then seeded in a 96-well white cell culture plate (Biosharp BS-MP-96W) at a volume of 95 μL per well (2000 cells / well) , and cell-free culture medium (containing 0.1%DMSO) was added to Min control wells.

[0030] After 24 hours of incubation overnight, 50 μL of upper culture medium was removed, and 45 μL of fresh culture medium was further added. 5 μL of 20× compound 1 was then added to the culture plate containing 95 μL of cell suspension. The resulting mixture was mixed well, and 5 μL of a mixture of DMSO-cell culture medium was added to the Max control. The final concentration of DMSO was 0.1%. The culture plate was incubated in an incubator for 72 hours.

[0031] CellTiter-Glo buffer was added to a flask of CellTiter-Glo substrate to dissolve the substrate, thereby preparing a CellTiter-Glo working solution. 50 μL (equal to half the volume of cell culture medium in each well) of the CellTiter-Glo working solution was added to each well. The culture plate was shaken on an orbital shaker for 2 minutes to induce cell lysis. The culture plate was left to stand at room temperature for 10 minutes to stabilize the luminescence signal. The luminescence signal was detected on a SpectraMax Paradigm plate reader. The cell proliferation inhibition rate data were processed using the following formula: inhibition rate (Inh%) = 100 -(RLUDrug -RLUMin)  /  (RLUMax -RLUMin) × 100%. The inhibition rates corresponding to different concentrations of compound 1 were calculated in EXCEL, and then GraphPad Prism software was used to plot inhibition rate curves and calculate relevant parameters including the maximum and minimum cell inhibition rates, and IC50 values.

[0032] The experimental results showed that compound 1 had an IC50 value of 1.036 μM for TT cells.

[0033] Example 3: Evaluation of Activity of Compound 1 on Ba / F3 Cell Lines with RET Fusion Mutations

[0034] Each Ba / F3 cell line carrying a fusion mutation gene was cultured at 37 ℃ with 5%CO2. Cells in the logarithmic growth phase were taken for plating. Compound 1 was prepared into a 20 mM solution in DMSO, which was then diluted in a 4-fold gradient from the highest concentration to the lowest concentration with DMSO to give a total of 9 concentrations.

[0035] Cell culture medium was added to a 96-well plate, and the solution of compound 1 was pipetted from the 1000× compound 1 stock plate and added to the cell culture medium in the 96-well plate. The mixture was mixed well by pipetting to prepare a 20× compound 1 working solution. DMSO was added to the control.

[0040] From the above experimental results, it can be seen that compound 1 of the present invention can effectively inhibit Ba / F3-KIF5B-RET, Ba / F3-KIF5B-RET-G810N, Ba / F3-KIF5B-RET-G810C, and Ba / F3-KIF5B-RET-G810S cell lines, and can be used for treating various cancers  caused by the above RET gene fusion mutations, such as papillary thyroid cancer, non-small cell lung cancer, pancreatic cancer, salivary gland cancer, colorectal cancer, ovarian cancer, chronic myelomonocytic leukemia, and breast cancer (Saha D, Ryan K, Lakkaniga N, et al. Targeting Rearranged during Transfection in Cancer: A Perspective on Small-Molecule Inhibitors and Their Clinical Development. [J] . Journal of medicinal chemistry, 2021, 64 (16) : 11747-11773. DOI: 10.1021 / acs. jmedchem. 0c02167. ) .

[0041] Example 4: Evaluation of In Vivo Efficacy of Compound 1 on RET Fusion Mutations

[0042] Compound 1 was dissolved and uniformly mixed in 5%DMSO, 10%Solutol, and 85%Saline, and the resulting solution was administered at a dose of 60 mg / kg once a day; LOXO-292 (Selpercatinib) was dissolved in the same vehicle to serve as a positive control, and the resulting solution was administered at a dose of 10 mg / kg twice a day; the negative control was the pure solvent.

[0043] Each Ba / F3 cell line was cultured in RPMI1640 culture medium supplemented with 10%fetal bovine serum and 1%penicillin and streptomycin mixture at 37 ℃ with 5%CO2. The cells were passaged 2-3 times per week. When the cell saturation reached 80-90%and the required cell number was achieved, the cells were collected and counted.

[0044] 0.2 mL of cells (1 × 106 cells) (along with matrigel in a volume ratio of 1: 1) was subcutaneously inoculated on the right back of each Balb / c nude mouse (female, aged 6-8 weeks, weighing 18-22 g) . When the average tumor volume reached about 100-120 mm3 (as close as possible to 100 mm3) , the mice were grouped and subjected to intragastric administration.

[0045] After the inoculation was completed, the subcutaneous tumor-bearing status, activity ability, diet, body weight, eyes, hairs, and other abnormal behaviors, appearance signs, or other abnormal conditions were observed daily. Before grouping, the animals were weighed, and the tumor volumes were measured. The animals were then randomly grouped according to the data. The tumor volume and body weight were measured three times a week (Monday, Wednesday, and Friday) .

[0046] The tumor volume was measured by a vernier caliper, with the formula TV = 0.5 a × b2, where a is the long diameter of the tumor and b is the short diameter of the tumor. The tumor inhibition efficacy of compound 1 was evaluated by TGI (%) or relative tumor proliferation rate T / C (%) . Calculation of tumor growth inhibition rate TGI (%) : TGI (%) = [ (1 - (average tumor volume at the end of administration of a treatment group -average tumor volume at the start of administration of the treatment group) )  /  (average tumor volume at the end of treatment of a solvent control group -average tumor volume at the start of treatment of the solvent control group) ] × 100%. The calculation formula for the relative tumor proliferation rate T / C (%) was as follows: T / C %= TRTV  / CRTV × 100% (TRTV: RTV of a treatment group; CRTV: RTV of a negative control group) . Relative tumor volume (RTV) was calculated based on the tumor measurement results, and the calculation formula was RTV = Vt  / V0, where V0 is the average tumor volume measured at the time of grouping for administration (i.e., D0) , and Vt is the  average tumor volume at a certain measurement. TRTV and CRTV were derived from data measured on the same day.

[0047] At the end of the experiment, tumor weights were measured, and the Tweight / Cweight percentage was calculated, where Tweight and Cweight represent tumor weights of the administration group and the vehicle control group, respectively. If the animal’s health continued to deteriorate, or if the tumor volume exceeded 2,000 mm3, or if there was a severe disease or pain, euthanasia had to be imposed.

[0048] The one-way analysis of variance (one-way ANOVA) test method was used to determine whether there was a significant difference in tumor volume between the treatment group and the control group. P < 0.05 indicates a significant difference.

[0050] The body weight of each group of mice fluctuated within a normal range.

[0051] For the Ba / F3 KIF5B-RET-G810C mouse xenograft tumor model (FIG. 1) , on days 1-18 after administration, the tumor volume of the compound 1 administration group was significantly lower than that of the control group; on day 18, the mice were sacrificed; compound 1 showed a TGI value of 47%and a T / C value of 57%; it can be seen that compound 1 has a certain inhibitory effect on the Ba / F3 KIF5B-RET-G810C mouse xenograft tumor, and the effect is superior to that of the positive compound.

[0052] For the Ba / F3 KIF5B-RET-G810S mouse xenograft tumor model (FIG. 2) , on days 1-16 after administration, the tumor volume of the compound 1 administration group was significantly lower than that of the control group; on day 16, the mice were sacrificed; compound 1 showed a TGI value of 70%and a T / C value of 41%; it can be seen that compound 1 has a significant inhibitory effect on the Ba / F3 KIF5B-RET-G810S mouse xenograft tumor, and the effect is comparable to that of the positive compound.
