Description

[Technical field]

[0001] The compounds and methods described herein relate generally to the field of treatment of thyroid-mediated disorders, and in particular to mechanisms that reduce the side effects resulting from administration of thyroid hormone receptor agonists. [Background technology]

[0002] Thyroid hormones (THs) play vital roles in growth, development, metabolism, and homeostasis. They are produced by the thyroid gland as thyroxine (T4) and 3,5,3'-triiodo-L-thyronine (T3). T4 is the major secreted form in humans and is enzymatically deiodinated by deiodinase to the more active form T3 in peripheral tissues. THs exert their actions by interacting with thyroid hormone receptors (TRs), which belong to the nuclear hormone receptor superfamily, and regulate the transcription of target genes. THs form part of the thyroid axis, also known as the hypothalamic-pituitary-thyroid, or HPT, axis, which involves complex endocrine and paracrine feedback loops that link brain tissue with the endocrine system to exert global control over a host of issues, including overall metabolic rate, lipid secretion, cardiac function, and muscle and bone growth (see, e.g., Robins and Cotran: Pathologic Basis of Disease, Kumar, V. et al., eds. (2005), p. 1165, the contents of which are incorporated herein by reference).

[0003] TR is expressed in many tissues and exists as two isoforms (TRα and TRβ). Tissue distribution studies, mouse knockout studies, and evaluation of patients with thyroid hormone resistance (RTH) syndrome have established that TRα is the predominant isoform in the heart and regulates most cardiac functions, while the TRβ isoform predominates in the liver and pituitary gland, regulating cholesterol metabolism and thyroid-stimulating hormone (TSH) production, respectively. Recognizing the possible benefits associated with TR modulation, many approaches have been pursued to identify TR agonists suitable for lowering plasma cholesterol levels. However, these benefits have been counterbalanced by adverse cardiovascular side effects such as tachycardia, arrhythmias, elevated blood pressure, and heart failure, as well as effects on the thyroid hormone axis, muscle metabolism, and bone loss.

[0004] TR-mediated pathways are involved in regulating serum lipid levels, including cholesterol, triglycerides, and related lipoproteins. See Pearce, EN, Curr. Cardiol. Rep. 6:451-6 (2004) and Duntas, LH, Thyroid 12:287-93 (2002), both of which are incorporated herein by reference in their entirety. Elevated serum lipid levels are involved in the development of atherosclerosis and the aggravation of coronary artery disease. See Robins and Cotran: Pathologic Basis of Disease, Kumar, V. et al., eds. (2005), 523, 572-77, the entire contents of which are incorporated herein by reference. Clinical trials have demonstrated that lowering low-density lipoprotein / serum cholesterol levels reduces morbidity and mortality associated with cardiovascular disease. See Grundy, SM et al., Circulation 110:227-39 (2004), the entire contents of which are incorporated herein by reference. Drugs such as statins and PCSK-9 inhibitors, along with dietary and lifestyle interventions, can help treat hyperlipidemia in some patients, but many patients are unable to significantly lower their serum cholesterol levels, and many cannot tolerate high doses of statins. See Pearson, T. et al., Arch Intern Med. 160:459-467 (2000), the entire contents of which are incorporated herein by reference. Thus, there is an unmet medical need for additional orally administered lipid-modulating therapies.

[0005] Similarly, nonalcoholic fatty liver disease (NAFLD), a condition associated with a group of metabolic abnormalities known as metabolic syndrome, is defined by excess fat accumulation in the form of triglycerides (steatosis) in the liver. The condition may further include hepatocellular injury and inflammation leading to nonalcoholic steatohepatitis (NASH). NASH is commonly found in patients with type 2 diabetes, hypercholesterolemia, hypertriglyceridemia, and obesity. Patients with NASH are at risk of developing cirrhosis, liver failure, and hepatocellular carcinoma. Treatment of NASH is currently limited to lifestyle interventions. However, the role of thyroid hormone in regulating LDL-C and triglyceride levels makes the TR-mediated pathway a promising target for the treatment of NASH and NAFLD. For example, in animals, thyroid hormone mimetics have been shown to dramatically reduce liver fat content.

[0006] Selective TRβ agonists have been developed as a means to suppress the cardiac side effects of non-specific TR agonists while retaining the possible beneficial effects of TRβ activation, such as reduced cholesterol and serum lipid levels, and reduced obesity due to increased cellular metabolism. See Fujitaki, JM et al., Drug Metab. Disp. 36(11)2393-403 (2008), the entire contents of which are incorporated herein by reference. However, it has been shown that even targeted TRβ agonists can result in suppression of the thyroid hormone axis (see Erion, MD, PNAS USA 104(39):15490-5 (2007), the entire contents of which are incorporated herein by reference), which can result in side effects ranging from depression and fatigue to muscle wasting and bone loss. Thus, there is a need for compositions and methods for producing activation of TRβ while reducing the suppression of the HPT axis and its associated side effects. [0482] In previous clinical trials, daily administration of these compounds to dogs resulted in a dose-dependent reduction in serum lipid levels and suppression of the HPT axis with unknown dose-dependence. Surprisingly, given the dose-dependence of the initial lipid-lowering effect, changing the administration schedule to every other day did not result in a decrease in efficacy in terms of lowering plasma cholesterol, but did reduce the suppression of the HPT axis. Thus, some embodiments include methods of administering Compound 1, Compound 2, and related compounds in such a way as to preserve their primary effect as agonists of TRβ and the reduction of clinical symptoms achieved thereby, while improving or eliminating the suppression of the HPT axis and the side effects associated with such suppression.

[0483] According to the methods disclosed herein, reduction of HPT suppression side effects associated with administration of the above-mentioned compounds may be achieved by adjusting the administration schedule so that the subject experiences periodic partial or complete reductions in administration followed by resumption of administration over a period of time. In some embodiments, a dose is administered daily for 1-30 days, followed by a rest period of 1-30 days. In some embodiments, no doses are administered during the rest period. In some further embodiments, the compound and its metabolites are allowed to be completely cleared from the subject's body before the next dose is administered. In some other embodiments, less than the normal daily dose is administered during the rest period. [0506] In any of the foregoing embodiments, the daily dose can be administered in a single dose or doses administered once per day, or in two or more divided doses administered multiple times per day. For example, the compounds described herein can be administered once per day, twice per day, three times per day, or four times per day.

[0507] In some embodiments, the subject's T3, T4, or TSH levels are monitored so that administration of the daily dose can be eliminated or reduced on any day that the T3, T4, or TSH levels fall below a predetermined threshold. If the T3, T4, or TSH levels rise above a predetermined threshold during a drug holiday, normal daily administration can be continued. EXAMPLES

[0508] Example 1 Alternative dosing study of Compound 2 in dogs The purpose of this study was to determine the effect of oral administration of Compound 2, once daily for 14 days followed by 14 days of every other day administration, on plasma cholesterol levels and indices of thyroid function in beagle dogs. Compound 2 was formulated with Lutrol F68 NF (Poloxomer 188) and carboxymethylcellulose (CMC; sodium salt / high viscosity) and administered as a suspension in 0.5% CMC / 1% Lutrol in deionized water. Twelve beagle dogs (9-15 kg) were randomized into six treatment groups (one male and one female per group) and administered Compound 2, at doses of 0.1, 0.3, 1, 3, or 10 mg / day, in a 0.5% CMC / 1% Lutrol F68 suspension, or vehicle, by gavage once daily for 14 days. At the end of the treatment cycle (cycle 1), the dogs were given a 4-week washout period and then entered a second 14-day treatment cycle. Cycle 2 used the same dosing paradigm as cycle 1, but animals were randomized into cycle 2 such that each combination dosing group from cycle 2 consisted of 4 different animals (2 males, 2 females). At the end of cycle 2, dosing was continued every other day for an additional 14 days (cycle 2 extension). Blood samples were then taken at baseline and appropriate time intervals and analyzed for total plasma cholesterol levels, total T4 (tT4), free T4 (fT4), total T3 (tT3), free T3 (fT3), and thyroid stimulating hormone (TSH).

[0509] Treatment with Compound 2 for 14 days resulted in a progressive dose-dependent reduction in total plasma cholesterol levels, with a mean reduction on day 15 of about 28 mg / dL or about 22% from baseline at a dose of 0.3 mg / kg / day and about 71 mg / dL or about 47% from baseline at the highest dose evaluated (10 mg / kg / day) (see Figure 1). The lowest dose of Compound 2 evaluated, 0.1 mg / kg / day, had the least effect on total plasma cholesterol levels (Figure 1). During the alternate day dosing period of Cycle 2 (Cycle 2 Extended), total plasma cholesterol levels in Compound 2-treated groups maintained a reduction similar to or greater than that observed after once-daily dosing compared to vehicle-treated animals (see Figure 2). Once-daily treatment with Compound 2 resulted in a dose-dependent decrease in serum tT4 (about 20-54%, see Figure 3), a dose-dependent decrease in fT4 (about 8-39%, see Figure 4), and a non-dose-dependent decrease in fT3 (about 15-32%, see Figure 6) from baseline levels. Compared to baseline levels, there was no meaningful change in tT3 levels in any of the Compound 2-treated groups beyond those observed in the vehicle-treated group (see Figure 5). The effect of once-daily Compound 2 treatment on serum TSH levels was variable, being completely suppressed in some animals and showing up to a four-fold increase in others on day 8. On day 15, TSH levels were non-dose-dependently decreased from baseline by about 6-27% in the Compound 2-treated groups (see Figure 7). During the cycle 2 extension period, the levels of tT4 and fT4 suppressed by once-daily treatment gradually recovered to levels approaching those of vehicle-treated animals (see Figures 8, 9). A more variable recovery of fT3 and TSH levels was observed, which was absent across all Compound 2 dose groups.

[0510] In conclusion, once-daily oral treatment of beagle dogs with compound 2 (0.1-10 mg / kg) for 14 days resulted in a dose-dependent decrease in mean total plasma cholesterol levels (up to 47% from baseline), accompanied by a dose-dependent decrease in serum tT4 levels, a dose-dependent decrease in fT4 levels, and a significant variation in serum TSH levels. Levels of tT3 were not affected by treatment, whereas levels of fT3 were decreased non-dose-dependently. Switching from once-daily to every-other-day administration of compound 2 in cycle 2 extension did not impair the cholesterol-lowering effect, but gradually restored levels of tT4 and fT4, and in some dose groups, fT3 and TSH, which were suppressed by once-daily oral compound 2 treatment. Thus, every-other-day administration is an effective alternative to once-daily administration with reduced effects on the thyroid hormone axis.

[0511] Example 2 First Alternative Dosing Study of Compound 1 in Dogs The purpose of this study is to investigate the effect of alternative dosing schedules on various clinical parameters in beagle dogs. Five beagle dogs per group of a single sex were randomized into five groups. One group received daily administration of vehicle only. Test groups received either: 1) daily administration of test sample; 2) one day of administration followed by a one-day washout period; 3) one day of administration followed by a two-day washout period; 4) three consecutive days of administration followed by a four-day washout period; or 5) five consecutive days of administration followed by a two-day washout period. Dogs are from a non-naive colony. Administration is by once-daily administration. Test sample is administered at a dose of 10 mg / kg of compound 1. Treatment is by oral gavage once daily on each treatment day and is 3 weeks (21 days) in duration. No recovery period is used. The vehicle for administration is 0.5% CMC / 1% Kolliphor P188 in deionized water, which is prepared once weekly and refrigerated. Food consumption is not monitored and veterinary tests are not performed unless necessary. Blood is drawn from each subject 7 days prior to the start of dosing, 4 days prior to the start of dosing, and 2, 4, 6, 8, 10, 12, 14, 16, 18, 20, and 22 days after the start of dosing. Subjects are evaluated for their plasma cholesterol levels, aspartate transaminase (AST), and alanine transaminase (ALT) levels, and thyroid function. Total T4, total T3, free T4, free T3, and thyroid stimulating hormone are evaluated. After 22 days, data are compiled and subjected to appropriate statistical analysis.

[0512] Example 3 Second Alternative Dosing Study of Compound 1 in Dogs The purpose of this study is to investigate the effect of alternative dosing regimens on various clinical parameters in beagle dogs. Four, five, or six beagle dogs per group of a single sex were randomized into five groups. One group received daily administration of vehicle only. Test groups received either: 1) daily administration of test sample; 2) one day of dosing followed by a two-day washout period; 3) three consecutive days of dosing followed by a four-day washout period; or 4) four consecutive days of dosing followed by a three-day washout period. Dogs are from a non-naive colony. Treatment is by oral gavage once daily on each treatment day and is three weeks in duration. No recovery period is used. The vehicle for dosing is 0.5% CMC / 1% Lutrol F68 in deionized water, which is prepared once weekly and refrigerated. Food consumption is not monitored and veterinary examinations are not performed unless necessary. Blood is drawn from each subject every two days or every three to four days. Subjects are assessed for their plasma cholesterol levels, aspartate transaminase (AST) and alanine transaminase (ALT) levels, and thyroid function. Total T4, total T3, free T4, free T3, and thyroid stimulating hormone are assessed. After 14 days and again after 22 days, data are pooled and subjected to appropriate statistical analysis.

[0513] Example 4 Alternative dosing studies of Compound 1 in humans A Phase 2, randomized, double-blind, placebo-controlled, multicenter study will be conducted to evaluate the efficacy, safety, and tolerability of Compound 1 administered for 12 weeks, followed by a 4-week washout phase, in patients with primary hypercholesterolemia and nonalcoholic fatty liver disease. Subjects are randomly assigned to one of seven individual treatment arms: daily placebo orally (PO); a daily dose of 5 mg Compound 1 PO after a loading dose of 250 mg Compound 1; a daily dose of 10 mg Compound 1 PO after a loading dose of 250 mg Compound 1; a daily dose of 10 mg Compound 1 PO after a loading dose of 250 mg Compound 1; a daily dose of 5 mg Compound 1 PO after a loading dose of 100 mg Compound 1; a daily dose of 10 mg Compound 1 PO after a loading dose of 100 mg Compound 1; and a dose of 10 mg Compound 1 PO after a loading dose of 100 mg Compound 1. Compound 1 is provided in the form of a tablet containing either 5 mg or 10 mg Compound 1.

[0516] Subjects will be assessed for their total thyroid function at various times during the course of treatment, at the end of treatment, and 4 weeks after treatment. When assessing total thyroid function, total T4, total T3, free T4, free T3, and thyroid stimulating hormone will be evaluated. Subjects will also be monitored periodically for indicators of cardiac side effects, such as by monitoring blood pressure, C-reactive protein, cardiac troponin (cTnI), creatine kinase (CK), creatine kinase MB isoenzyme, by regular electrocardiograms and / or Holter monitors, or by other means as appropriate. Evaluations of thyroid function, cardiac health, and clinical endpoints will be performed periodically throughout the treatment period. Final measurements will be taken upon discontinuation of treatment at week 12 and after a 4-week washout period at week 16. After 16 weeks, data will be pooled and subjected to appropriate statistical analysis.

[0517] Example 6 Alternative dosing studies of Compound 1 in humans A Phase 1b, randomized, double-blind, placebo-controlled study will be conducted to evaluate the efficacy, safety, and tolerability of Compound 1 administered for 12 weeks, followed by a 4-week washout phase, in patients with primary hypercholesterolemia and nonalcoholic fatty liver disease.

[0518] In this current clinical trial, the maximum cumulative weekly dose will not exceed 40 mg. Doses of Compound 1 of 5 mg daily and up to 40 mg once weekly over a 4-week treatment period in this Phase 1b study result in dose- and / or schedule-dependent reductions in LDL-C and triglycerides. The predicted systemic exposure (AUC0-168h [i.e., 1-week exposure]) of the active metabolite at 40 mg once weekly is 5570 ng·hr / mL. Pharmacokinetic modeling indicates an AUC0-168h of 18300 ng·hr / mL. Thus, the estimated exposure in humans receiving 40 mg once weekly is estimated to be approximately 3-fold lower than the previously determined no observable adverse effect level (NOAEL) exposure of 5 mg / kg / day.

[0519] Approximately 32 subjects will be enrolled and randomly assigned in a 6:2 ratio (Compound 1:placebo) to one of the following treatment groups: (1) 5 mg Compound 1 or placebo daily; (2) 20 mg Compound 1 or placebo once a week; (3) 40 mg Compound 1 or placebo once a week; (4) 10 mg Compound 1 or placebo every other day. The primary endpoint is the percent change in LDL-C from the baseline visit to day 29. Either a 20% reduction in LDL-C or a reduction in LDL-C into the normal range will be considered clinically meaningful. Subjects will also be monitored for adverse effects and / or clinically relevant changes in health status or results of laboratory analyses. Each subject will be screened for changes in health status or laboratory results 28 days after the last administration of test sample.

[0520] Clinical parameters observed during the study may include one or more of the following: physical examination; monitoring of concomitant medications; assessment / monitoring of hyperthyroidism / thyrotoxicosis; vitals (seated for ≥5 min); weight; assessment of alcohol intake; assessment / monitoring of lifestyle habits; 12-lead ECG (supine for ≥10 min); assessment / monitoring of drug compliance; AE / SAE assessment; and / or 24-hour Holter monitoring.
