Abstract
Description
[0001] USE OF MELANOCORTIN-4 RECEPTOR AGONISTS FOR THE TREATMENT OF PCSK1 DEFICIENCY INDICATIONS CLAIM OF PRIORITY This application claims priority to KR Patent Application No. 10-2023-0130273, filed on September 27, 2023. The entire contents of the application as filed is incorporated herein by reference. TECHNICAL FIELD The present invention relates to the use of a compound of Formula 1 or a pharmaceutically acceptable salt thereof for the purpose of preventing or treating a rare genetic obesity disease associated with deficiency of Proprotein Convertase Subtilisin / Kexin Type 1 (PCSK1), which is related to the melanocortin-4 receptor (MC4R) pathway and the mechanism of action thereof: [Formula 1] In Formula 1 above, R1 is C2-C5alkyl. BACKGROUND Proprotein convertase subtilisin / kexin type 1(PCSK1) is a member of the subtilisin-like proprotein convertase family, which includes proteases that process peptide precursors and is involved in the processing of hormones and other protein precursors. Substrates include pro- opiomelanocortin (POMC), renin, enkephalin, dynorphin, somatostatin, insulin, and AGRP. PCSK1 activity is essential for the cleavage activation of many peptide hormone precursors such as proopiomelanocortin, proinsulin, proglucagon, and proghrelin, which are involved in the regulation of food intake, glucose homeostasis, and energy homeostasis. Proopiomelanocortin is derived from the POMC gene and is mainly expressed in the hypothalamus pituitary gland, brain, and stem, and the like of the brain, and is post-translationally and enzymatically modified to become neuropeptides such as α-melanocyte stimulating hormone (MSH), β-MSH, γ-MSH, adrenocorticotrophic hormone, or the like, which exert physiological effects. Melanocortin peptides regulate various physiological phenomena depending on their affinity for each MCR, and these physiological phenomena are known to include skin pigmentation, body temperature, inflammation, appetite, behavior control functions, and the like (Reference: Mountjoy KG, et al., Mol Cell Endocrinol 1997, 128, 171; Haskell-Luevano C. et al., Drug News Perspect 1999, 12, 197; Wikberg, JES, et al., Pharmacol Res 2000, 42, 393). Melanocortin receptor (MCR) is a type of G-protein coupled receptor (GPCR), whose main role is to activate secondary messengers to transmit signals and regulate cellular responses to many physiological stimuli. A total of five types of melanocortin receptors have been identified as melanocortin receptors to date. MC1R is expressed in melanocytes and macrophages, and determines skin and hair color by regulating melanin pigment in melanocytes. MC2R is expressed in the adrenal gland and adipose tissue, and the function thereof in mediating the regulation of adrenal hormone secretion by adrenocorticotropic hormone in the adrenal gland is well known. MC3R, MC4R, and MC5R are expressed not only in nerve terminals but also in the brain and are thought to mediate central nervous system actions by melanocortin peptides that result in effects on behavior, learning, memory, appetite, and neurogenesis and regeneration. To date, MC3R is known to be involved in erectile dysfunction and inflammatory responses, and MC4R is known to be involved in obesity and diabetes, and research on the specificity of action of each receptor is actively being conducted (MacNeil DJ et al., Eur J Pharmacol 2002, 450, 93). As a result, genetic studies in humans with active obesity have shown that MC4R is deeply involved, and knockout mice with MC4R deletion have been shown to develop obesity due to overeating, proving that this receptor plays an important role in appetite control (Lu D, Willard D et al., Nature 1994, 371(6500), 799; Huszar D et al., Cell 1997,88(1), 131; Hinney A et al., J Clin Endocrinol Metab 1990, 84(4), 1483). The main obesity treatment drugs developed to date are appetite suppressants that act on the central nervous system, and most of them are drugs that regulate the action of neurotransmitters. Examples include noradrenalin agents (phentermine and mazindol) and fluoxetine and sibutramine, which are serotonergic agents. However, the above neurotransmitter regulators have a wide range of effects on various physiological functions in addition to appetite suppression through numerous subtype receptors. Therefore, the above regulators have a major disadvantage of lacking selectivity for each subtype and cause various side effects when administered for a long period of time. On the other hand, melanocortin agonists are neuropeptides rather than neurotransmitters, and considering the fact that all other functions other than energy metabolism are normal in MC4R gene KO mice, melanocortin agonists may have an advantage as a point of action in being capable of only inducing weight loss through appetite suppression without affecting other physiological functions. In particular, the receptor is a G-protein coupled receptor (GPCR), which belongs to the most successful category of new drug action points developed to date, and is significantly different from existing action points in that it is relatively easy to secure selectivity for subtype receptors. As examples of utilizing these melanocortin receptors as the point of action, International Publication Nos. WO 2008 / 007930 and WO 2010 / 056022 disclose compounds as agonists of the melanocortin receptors. However, a person skilled in the art can prepare the compound of Formula 1 using various methods based on the structure of Formula 1, and all of these methods should be interpreted as falling within the scope of the present invention. In other words, the compound of Formula 1 can be prepared by arbitrarily combining various synthetic methods described herein or disclosed in the prior art, and this will be understood as falling within the scope of the present invention, and the method for preparing the compound of Formula 1 is not limited to that described below. In one embodiment, the hydrochloride salt of the compound of Formula 1 of the present invention can be prepared according to Reaction Formula 1 below. [Reaction Formula 1] In Reaction Formula 1 above, R2is C1-C5alkyl; R3 is C3-C8 cycloalkyl unsubstituted or substituted with 1 or 2 C1-C5 alkyl; and R4 and R5 are each independently hydrogen or halogen. In defining the compound represented by Formula 1 throughout this specification, the concepts defined as below are used. The following definitions also apply to terms used individually or as part of larger groups throughout this specification, unless specifically indicated otherwise. Terms and abbreviations used herein have their original meanings unless otherwise defined. As used herein, the term “substitution” may mean that a hydrogen atom bonded to each substituent listed in the compound or the above substituent group is replaced and bonded with any substituent At this time the position to be substituted is not limited as long as it is a position in which a hydrogen atom bonded to each substituent listed in the compound or the above substituent group exists, that is, a position in which the hydrogen atom can be substituted with a substituent. When two or more are substituted, the two or more substituents may be the same or different from each other. Conversely, “unsubstituted” can mean that no hydrogen atom bonded to a carbon atom has been replaced by any substituent. Any substituent applicable to the above “substitution” may be appropriately selected from the substituents listed in the substituent group as described herein, but the scope of substituents available for substitution is not limited to this substituent group. When a substituent is not separately described or mentioned in the chemical formula described herein, it may be considered that hydrogen is bonded, and in some cases, the substituent may be absent depending on the ring member forming the heterocycle. As used herein, the term “alkyl” by itself or as part of another substituent, unless otherwise stated, means a straight- chain or branched-chain monovalent hydrocarbon having the number of carbon atoms indicated (that is, C1-C5 means 1 to 5 carbons). Examples of the above alkyl include methyl, ethyl, n-propyl, isopropyl, n-butyl, isobutyl, tert-butyl, sec-butyl, 1-methyl-butyl, 1-ethyl-butyl, n-pentyl, isopentyl, neopentyl, tert-pentyl, and the like, but these are non-limiting examples, and the scope of the present invention is not limited thereto. As used herein, the term “cycloalkyl” refers to a monovalent aliphatic carbocycle in which one hydrogen atom is missing from any position among the carbon atoms constituting the ring. Examples of cycloalkyls include, but are not limited to, cyclopropyl, cyclobutyl, cyclopentyl, 3-methylcyclopentyl, 2,3-dimethylcyclopentyl, cyclohexyl, cyclohexylene 3- methylcyclohexyl, 4-methylcyclohexyl, 2,3-dimethylcyclohexyl, 3,4,5-trimethylcyclohexyl, 4-tert- butylcyclohexyl, cycloheptyl, cyclooctyl, and the like. As used herein, the term “halogen” includes, but is not limited to, for example, fluoro, chloro, bromo, or iodo. According to the present invention, the compound of the above Formula 1 can efficiently prevent or treat a rare genetic obesity disease associated with Proprotein Convertase Subtilisin / Kexin Type 1 (PCSK1) deficiency, but is not limited thereto. Proprotein Convertase Subtilisin / Kexin Type 1 (PCSK1) is a member of the subtilisin-like precursor protein convertase family, which includes proteases that process peptide precursors, and has proopiomelanocortin (POMC) as a substrate. POMC is a precursor polypeptide having 241 amino acid residues, which is synthesized in the anterior and intermediate pituitary gland and serve as a precursor of several peptide hormones such as melanocyte stimulating hormone (MSH), endorphin, and corticotrophin. Alpha-MSH, which is a ligand of the melanocortin-4 receptor (MC4R), acts in the upstream of MC4R and when the PCSK1 gene is deficient and not expressed normally, POMC is not converted to alpha-MSH, which can cause severe early-onset obesity. As used herein, the term "therapeutically effective amount" means an amount sufficient to achieve the above pharmacological effect, that is, a therapeutic effect, and the amount of the compound will vary depending on the condition of the subject and the severity thereof, the mode of administration, and the age of the subject to be treated, can be determined by those skilled in the art based on the own knowledge thereof. In one embodiment, a therapeutically effective dose of the compound of Formula 1 above, for example, a dose required to treat adults, typically ranges from approximately 0.1 to 500 mg per day depending on the frequency and intensity of administration. When administered intramuscularly or intravenously to adults, a total dose of approximately 0.1 to 300 mg per day, divided into single doses, will usually be sufficient, but higher daily doses may be desirable for some patients. In one embodiment, the compound of Formula 1 may be administered at a dose of 10 mg / kg or 30 mg / kg. As used herein, the term “carrier” refers to a compound that facilitates introduction of a compound into cells or tissues. For example, dimethyl sulfoxide (DMSO) is a common carrier that facilitates the introduction of many organic compounds into the cells or tissues of an organism. In one embodiment, the pharmaceutically acceptable carrier can include lactose, dextrose, sucrose, sorbitol, mannitol, xylitol, erythritol, maltitol, starch, acacia gum, alginate, gelatin, calcium phosphate, calcium silicate, cellulose, methylcellulose, microcrystalline cellulose, polyvinyl pyrrolidone, water, methyl hydroxybenzoate, propyl hydroxybenzoate, talc, magnesium stearate, mineral oil, and the like. As used herein, the term “diluent” refers to a compound that is diluted in water to not only stabilize the biologically active form of the subject compound, but also to dissolve the compound. Salts dissolved in buffer solutions are used as diluents in the field of art. A commonly used buffer solution is phosphate buffered saline, which mimics the salt form of human body fluids. Buffer salts can control the pH of a solution at low concentrations, and thus it is rare for buffer diluents to modify the biological activity of a compound. As used herein, the term “pharmaceutically acceptable” refers to a property that does not impair the biological activity and physical properties of a compound. In one embodiment, the pharmaceutically acceptable salts include, but are not limited to acid addition salts formed by inorganic acids such as hydrochloric acid, sulfuric acid, nitric acid, phosphoric acid, hydrobromic acid, and hydroiodic acid, organic carboxylic acids such as tartaric acid, formic acid, citric acid, acetic acid, trichloroacetic acid, trifluoroacetic acid, gluconic acid, benzoic acid, lactic acid, fumaric acid, and maleic acid, sulfonic acids such as methanesulfonic acid, benzenesulfonic acid, p-toluenesulfonic acid, and naphthalenesulfonic acid, or the like. Therefore, mice having a mutation (N222D) in the PCSK1 gene are an effective animal model reflecting the human genetic obesity disease, PC1 / PCSK1 deficiency obesity, and are mice that exhibit abnormal metabolic disease-related phenotypes such as hyperphagia and severe obesity. The pharmacological effects of N-((3S,5S)-1-((3S,4R)-1-(tert-butyl)-4-(4- chlorophenyl)pyrrolidine-3-carbonyl)-5-(morpholine-4-carbonyl)pyrrolidin-3-yl)- N-((1s,4R)-4- methylcyclohexyl)isobutyramide hydrochloride (hereinafter referred to as "Compound 100" or “Cmpd. 100”) obtained in Example 1 in a rare genetic obesity disease associated with PCSK1 deficiency in a Pcsk1-deficient mouse model were evaluated in the obesity models as below. Experimental Example 1-1: Heterozygous (PCSK1+ / -) mouse obesity model The anti-obesity efficacy was confirmed by repeated oral administration of the Compound 100 of the present invention at a dose of 10 mg / kg or 30 mg / kg for about 28 days to PCSK1+ / - mice heterozygous for a mutation (N222D) in the PCSK1 gene while feeding a 60 kcal% high-fat diet (diet with 60 kcal% fat) and the results are shown in FIGS.1 and 2. As can be seen from FIG. 1, it was confirmed that the Compound 100 of the present invention exhibited a significant weight loss effect. As of day 28, Compound 100 in PCSK1+ / - mice when administered at doses of 10 mg / kg and 30 mg / kg caused weight gain inhibition of -6.23% and -13.4% compared to the solvent-administered control group (Vehicle), respectively, exhibiting a statistically significant weight loss effect. Additionally, as can be seen from FIG.2, it was confirmed that the Compound 100 of the present invention exhibited a significant appetite suppressing effect. Based on the cumulative feed intake for one week, when Compound 100 was administered at doses of 10 mg / kg and 30 mg / kg in PCSK1+ / - mice, food intake (g) decreased compared to the solvent-administered control group (Vehicle), exhibiting a statistically significant appetite suppressing effect. Experimental Example 1-2: Homozygous (PCSK1- / -) mouse obesity model The anti-obesity efficacy was confirmed by repeated oral administration of the Compound 100 of the present invention at a dose of 10 mg / kg or 30 mg / kg for approximately 28 days to PCSK1- / - mice homozygous for a mutation (N222D) in the PCSK1 gene while feeding a 60 kcal% high-fat diet (diet with 60 kcal% fat) and the results are shown in FIGS.3 and 4. As can be seen from FIG. 3, it was confirmed that the Compound 100 of the present invention exhibited a significant weight loss effect. As of day 28, when Compound 100 was administered to PCSK1- / - mice at doses of 10 mg / kg and 30 mg / kg, it caused weight gain inhibition of -5.2% and - 10.1%, respectively, compared to the solvent-administered control group (Vehicle), exhibiting a statistically significant weight loss effect. Additionally, as can be seen from FIG.4, it was confirmed that the Compound 100 of the present invention exhibited a significant appetite suppressing effect. Based on the cumulative feed intake for one week, when Compound 100 was administered at doses of 10 mg / kg and 30 mg / kg in PCSK1- / - mice, food intake (g) decreased compared to the solvent-administered control group (Vehicle), exhibiting a statistically significant appetite suppressing effect. The results of Experimental Examples 1-1 and 1-2 are due to the excellent melanocortin-4 receptor agonistic activity of the Compound 100 of Example 1, for which it is expected that significant differences will be visible even at low administration doses, and have exhibited equal or better efficacy compared to other obesity treatment agents having the same MC4R agonist as the point of action, and thus it is expected that it will be able to exhibit excellent constitutional improvement effects and safety when actually applied clinically. In addition, the Compound 100 of the present invention is a small molecule compound rather than a peptide, and thus there is the advantage that the Compound 100 can be used as an oral preparation in addition to an injectable preparation compared to peptidyl MC4R agonists.
