A METHOD FOR DIAGNOSING OR PREDICTING PREECLAMPSIATechnical Field

[0001] This invention generally relates to a method for diagnosing or predicting diseases, and particularly to a method for diagnosing or predicting preeclampsia.Background

[0002] Preeclampsia is a disorder of pregnancy associated with new-onset hypertension, which occurs most often after 20 weeks of gestation. Worldwide, 2-5%of pregnant women suffer from preeclampsia, and the rate is even higher in developing countries. With clinical manifestation of hypertension and proteinuria or critical organ function damages like visual disturbances, headaches, epigastria pain, and rapid development of oedema, preeclampsia is a major cause of maternal and perinatal morbidity and mortality. Serious outcomes of preeclampsia may include cerebral haemorrhage, disseminated intravascular coagulation, kidney failure of the mother, and stillbirth or preterm delivery of the infant, which all endanger the health of the mother and infant. Termination of pregnancy is often required to avoid severe comorbidities. According to the guidelines of American College of Obstetricians and Gynecologists (ACOG) and Society of Obstetricians and Gynaecologists of Canada (SOGC) , or “Williams Obstetrics: 24rd Edition” , preeclampsia diagnosis is made subjectively based on hypertension and proteinuria or hypertension and other symptoms. However, proteinuria diagnosed by a single dipstick reading is not reliable., and those non-specific clinical signs and symptoms are tedious; there is no accurate, objective test widely available to confirm an exact diagnosis. Therefore, there is an unmet need to develop a simple, objective test for preeclampsia diagnosis and screening.Summary of the Invention

[0003] In one aspect, the present disclosure provides a method for diagnosing or predicting preeclampsia in a pregnant woman, comprising: a) obtaining at least one maternal or fetal parameter or factor, b) determining if the urine of said pregnant woman contains misfolded proteins or misfolded protein aggregates, and c) using the combination of the at least one maternal or fetal parameter or factor obtained in step a) and the presence or absence of misfolded proteins or misfolded protein aggregates in the urine of said pregnant woman to diagnose or predict preeclampsia.

[0009] The inventors surprisingly discovered that combining a test for urine misfolded proteins and at least one maternal or fetal parameter or factor produces synergies and results in a significantly improved diagnostic or predictive accuracy for preeclampsia.

[0010] The term "misfolded protein" as used herein is relative to a correctly folded protein. The most frequent destiny of misfolded proteins is self-aggregation due to the exposure of hydrophobic protein surfaces that are normally buried inside the proteins, resulting in a high degree of stickiness. The β-sheet structural motif offers the most favorable organization for these intermolecular aggregates, and as a result misfolded proteins often exist as aggregates (oligomers and fibrous aggregates) . In the present invention, the meaning of "misfolded protein" includes such aggregates formed by misfolded proteins.

[0011] For example, a study found that both urine and placental tissues contain a large number of misfolded proteins and their aggregates in preeclampsia patients, which can specifically bind to Congo red, and when observed under electronic microscope, these congophilic misfolded protein aggregates have a fibrous structure very similar to amyloid-like structures, whereas in normal pregnant women there is no such aggregate (Buhimschi et al., SCIENCE Translational Medicine, 2014, 6 (245) : 245ra92) .

[0012] Once the conformation occurs and the misfolding cannot be repaired or eliminated, different proteins even with no homology in structure and sequence can form a β-sheet structure. The β-sheet structures of different proteins can interact to form aggregates containing different misfolded proteins, which can bind specifically to Congo Red.

[0013] Urine misfolded proteins can be detected by a capillary-based method for point-of-care testing of misfold proteins or misfold protein aggregates in pregnant women using urine samples. In the process of detecting misfolded proteins based on the capillary-based method (aso-called “CapCord” method as used in Example 3) , the diffusion area of a dye in a filter paper is used to determine the presence or absence of misfolded proteins. If a biological sample does not contain misfolded proteins or misfolded protein aggregates, the detection result obtained by the method is negative. That is, when the mixture of a negative sample and the dye is released into the filter paper by a capillary tube, no significant diffusion blot is formed on the filter paper, or in some embodiments a smaller diffusion blot is formed and the radius of the diffusion blot is less than a reference value, which may be determined by the maximum value of the radiuses of the diffusion blots formed by a certain number (e.g., 50-100) patient samples that clinically determined to be negative. If a biological sample contains misfolded proteins or misfolded protein aggregates, the detection result obtained by the method is positive. That is, when the mixture of a positive sample and the dye is released into the filter paper by a capillary tube, a larger diffusion blot is formed on the filter paper. In some embodiments of the invention, the positive sample produces a larger diffusion blot having a radius greater than or equal to a specific reference value, which may be determined by the minimum value of the radiuses of the diffusion blots formed by a certain number (e.g., 50-100) patient samples that clinically determined to be positive. It is worth noting that in some cases, a pseudopodium-or pseudopodia-like diffusion blot is produced with a radius larger than the radius of the diffusion blot formed by a negative sample, or greater than the above reference value. Even so, this type of diffusion result is still classified to be negative.

[0014] Urine misfolded proteins can also be detected a chromatographic method. When a mixture of Congo Red and urine from a pregnant woman is loaded onto a chromatographic column mounted in a detection cuvette, the presence (positive) or absence (negative) of misfolded proteins can be determined based on the color of the eluent in the cuvette. The eluent was also be applied for quantitative measurement using a Spectrophotometer such as HACH DR1900 instrument by detecting the absorbance at a wavelength of 520-600 nanometers.

[0218] Among these models as evaluated, the integration of the misfolded protein with MAP, PE history, antihypertensive medication, ΔBMI / GA, and UTP / cre yielded an AUC of 0.95 (Model 1) , indicating a robust predictive capacity. Models that combined the misfolded protein, MAP, PE history, and antihypertensive medication with either ΔBMI / GA (Model 6) or ΔBMI (Model 9) maintained high AUC values of 0.937, suggesting that the exclusion of UTP or UTP / cre still provided significant predictive capability. Additionally, Model 17, which excluded any form of UTP and BMI, achieved an AUC value of 0.932, albeit slightly lower than those incorporating more indices, but provided a simpler model with valuable predictive strength. Furthermore, variations in index combinations resulted in distinct AUC values, as illustrated in Figure 5, which shows the area differences under the ROC curve for 47 models and the single misfolded protein indicator.

1.A method for diagnosing or predicting preeclampsia in a pregnant woman or determining if a pregnant woman has a risk of preeclampsia, comprising:a) obtaining at least one (such as two or more) maternal or fetal parameter or factor of the pregnant woman, selected from a group comprising: (i) one or more of mean artery pressure (MAP) , BMIsampling (namely, the BMI of said pregnant woman at the sampling of the urine) , ΔBMI (namely, BMIsampling -BMIini (the BMI of said pregnant woman before pregnancy, or about the beginning of pregnancy, or within the first month of first pregnancy) ) , Gestational age (GA, optionally calculated in week) and ΔBMI / GA; (ii) Maternal age (optionally calculated in year) ; (iii) one or more of Urinary protein level and the ratio of Urinary protein level (such as Urine Total Protein (UTP) )  / Urinary Creatinine level (Cre) (UTP / Cre) ; (iv) one or more of preeclampsia in previous pregnancy (PEhis) and having received antihypertensive medication (Antihyper) ; and (v) any combination of (i) - (iv) ,b) determining if the urine of said pregnant woman contains misfolded proteins or misfolded protein aggregates, andc) combining the at least one maternal or fetal parameter or factor obtained in step a) and the content level of misfolded proteins or misfolded protein aggregates in the urine of said pregnant woman to diagnose or predict preeclampsia or determine the risk of preeclampsia.2.The method of Claim 1, wherein the content level of misfolded proteins or misfolded protein aggregates is determined as a qualitative measure (presence or absence) , a quantitative measure or a semi-quantitative measure.3.The method of Claim 1 or 2, wherein the content level of misfolded proteins or misfolded protein aggregates is determined as a qualitative measure (namely, presence or absence) , a quantitative measure or a semi-quantitative measure.4.The method of any of the preceding claims, wherein the combining step c) is performed by an algorithm, such as a regression algorithm (e.g., a linear regression algorithm, e.g., a two-class regression algorithm or a multiple linear regression algorithm) , to obtain a score p, and optionally, the score p being higher or lower than a predetermined threshold indicates that the pregnant woman is suffering from preeclampsia or is predicted to develop preeclampsia or is in an increased or decreased risk of having preeclampsia.5.The method of any of the preceding Claims, wherein the at least one maternal or fetal parameter or factor comprises (i) MAP and BMIsampling, (ii) MAP and maternal age; or (iii) MAP, BMIsampling, and maternal age.6.The method of any of the preceding claims, wherein said predicting comprises long-term predicting, such as predicting preeclampsia occurrence more than 4 weeks (e.g., after 6, 8, 10, 12, 14, 16, or 20 weeks) after said determining step, and optionally the determining step is at no earlier than gestational age of 20 weeks, such as no earlier than gestational age of 17 weeks, for example at the gestational age of 10-16 weeks, e.g. 11, 12, 13, 14, 15, or 16 weeks.7.The method of any of claims 1-4, wherein the at least one maternal or fetal parameter or factor comprises: at least one (such as two, three, four, five, six, seven or more) maternal or fetal parameter or factor of the pregnant woman, selected from a group comprising MAP, PEhis, Antihyper, ΔBMI, ΔBMI GA.8.The method of Claim 7, wherein said predicting comprises short-term predicting, such as predicting pre-eclampsia occurrence within 4 weeks (e.g., within 1, 2, 3, or 4 weeks) after said determining step, and optionally the urine is determined later than gestational age of 20 weeks, such as at the gestational age of 20-37 weeks, e.g., 22, 24, 26, 28, 30, 32, 34 or 36 weeks.9.The method of any of the preceding Claims, wherein the pregnant woman is a woman with suspected pre-eclampsia or in a high risk of having pre-eclampsia.10.A method for treating or preventing or managing pre-eclampsia in a pregnant woman, comprising:a) obtaining the at least one (such as two or more) maternal or fetal parameter or factor of the pregnant woman as defined in any of the preceding Claims,b) determining if the urine of said pregnant woman contains misfolded proteins or misfolded protein aggregates,c) combining the at least one maternal or fetal parameter or factor obtained in step a) and the content level of misfolded proteins or misfolded protein aggregates in the urine of said pregnant woman, by an algorithm, such as a regression algorithm (e.g., a linear regression algorithm, e.g., a two-class regression algorithm or a multiple linear regression algorithm) , to obtain a score p,wherein the score p being higher or lower than a predetermined threshold indicates that the pregnant woman is suffering from pre-eclampsia or is predicted to develop pre-eclampsia or is in an increased or decreased risk of having preeclampsia; andd) if the pregnant woman is diagnosed with preeclampsia or is predicted to develop preeclampsia or is in an increased risk of having preeclampsia, administering medication to the pregnant woman and / or managing the condition of the pregnant woman to prolong pregnancy.11.The method of Claim 10, wherein administering medication comprises administering aspirin, and / or managing the condition of the pregnant woman comprises losing weight, controlling blood pressure and / or blood sugar, and / or maintaining a regular exercise routine.12.A computer-implemented method for diagnosing or predicting pre-eclampsia in a pregnant woman or determining if a pregnant woman has a risk of pre-eclampsia, comprising:a) obtaining the at least one (such as two or more) maternal or fetal parameter or factor of the pregnant woman as defined in any of Claims 1-10,b) obtaining the content level of misfolded proteins or misfolded protein aggregates in the urine of said pregnant woman, andc) combining the at least one maternal or fetal parameter or factor obtained in step a) and the content level of misfolded proteins or misfolded protein aggregates in the urine of said pregnant woman to diagnose or predict preeclampsia or determine the risk of preeclampsia.13.A data processing apparatus, device or system, comprising means for carrying out the steps of the method of any of Claims 1-12 or comprising a processor adapted to or configured to perform the steps of the method of any of Claims 1-12.14.A computer program product or a computer-readable medium, comprising instructions which, when executed by a computer, cause the computer to carry out the steps of the method of any of Claims 1-12.
