Abstract
Description
[Technical field]
[0001] The present invention relates to a catheter, a catheter assembly, and a tubular body that are placed in a blood vessel and used for infusion or the like. [Background technology]
[0002] Catheters are used for treatments such as infusion of fluids to patients. The catheter is placed in the patient's blood vessel. An infusion line, for example, is connected to a catheter hub at the proximal end of the catheter. The fluid in the infusion line is administered to the patient via the catheter.
[0003] For example, Patent Document 1 describes a catheter assembly. [Prior art documents] [Patent documents]
[0004] [Patent Document 1] JP 2009-233007 A Summary of the Invention [Problem to be solved by the invention]
[0005] After a treatment such as infusion is completed, the catheter is no longer needed and is removed from the blood vessel and discarded. However, when the catheter is removed from the blood vessel, the elasticity of the catheter may cause the tip of the catheter to be significantly displaced. The displacement of the catheter causes blood adhering to the outer and inner surfaces of the catheter to splash around. If the patient has an infectious disease, the splashing of blood will contaminate the surrounding area and increase the risk of infection for medical staff. Furthermore, the splashing of blood imposes a workload on medical staff for cleaning and disinfection.
[0006] An object of the present invention is to solve the above-mentioned problems. [Means for solving the problem]
[0007] One aspect of the following disclosure is a catheter comprising: a catheter body having an inner lumen; a catheter hub fixed to a base end of the catheter body; a tubular body having openings at a tip end and a base end, the portion between the tip end and the base end being changeable in shape from a contracted state to an extended state; a tubular body having an inner diameter portion through which the catheter body and the catheter hub are passed; and a tubular body holding portion for holding the tubular body in the contracted state, wherein in the extended state, the tip end of the tubular body protrudes beyond the tip of the catheter body to cover the tip of the catheter body.
[0008] Another aspect is a catheter assembly comprising the catheter of the above aspect, an inner needle inserted into the lumen of the catheter body, and an inner needle hub fixed to a base end of the inner needle.
[0009] Yet another aspect is a tubular body that is attached to a catheter having a catheter body with an internal lumen and a catheter hub fixed to the base end of the catheter body, and covers the tip of the catheter body when the catheter body is removed, the tubular body having openings at the tip and base ends, a shape changeable between the tip and base ends from a contracted state to an extended state, and having an inner diameter portion through which the catheter body and the catheter hub can be inserted, and in the extended state, the tip protrudes beyond the tip of the catheter body to cover the tip of the catheter body. Effect of the Invention
[0010] The catheter, catheter assembly, and tubular body according to the above aspects prevent blood adhering to the catheter body from scattering by covering the tip of the catheter body with the tubular body. As a result, the catheter and catheter assembly reduce the risk of infection for medical personnel and lighten the workload of medical personnel. [Brief description of the drawings]

[0012] (First embodiment) The catheter assembly 10 according to this embodiment shown in Fig. 1 includes a catheter 12 and an inner needle assembly 14. The inner needle assembly 14 is used to introduce the catheter 12 into a patient's blood vessel 94 (Fig. 4). The inner needle assembly 14 is removed from the catheter 12 after the catheter 12 has been introduced into the blood vessel 94. The catheter 12 is placed in the patient's blood vessel 94 and used for treatments such as administration of infusion fluids.
[0013] The catheter 12 has a catheter body 16, a catheter hub 18, and a tubular body 20. The catheter body 16 has a cylindrical shape that extends long in the axial direction. The catheter body 16 has an internal lumen 16a. The lumen 16a penetrates the catheter body 16 in the axial direction and opens at a tip end 16b and a base end 16c of the catheter body 16. The inner diameter of the lumen 16a is larger than the outer diameter of the inner needle 22 of the inner needle assembly 14. The lumen 16a accommodates the inner needle 22 so that it can be inserted therethrough. The catheter body 16 is formed of a resin material having appropriate flexibility.
[0014] The catheter hub 18 is joined to the vicinity of the base end 16c of the catheter body 16 to support the catheter body 16. The catheter hub 18 has an opening 18b having a substantially cylindrical shape on the base end side. The catheter hub 18 has a small diameter section 181 located on the tip side and having a relatively small outer diameter, and a large diameter section 182 located on the base end side and having a relatively large outer diameter. The small diameter section 181 is joined to the catheter body 16 to support the catheter body 16. The small diameter section 181 has a tapered shape in which the outer diameter gradually increases toward the base end side.
[0015] A step 183 is formed at the base end of the small diameter portion 181. The small diameter portion 181 is connected to the large diameter portion 182 via the step 183. The large diameter portion 182 has an outer diameter dimension that allows a user to easily grip and operate it. The large diameter portion 182 has a tapered shape in which the outer diameter gradually increases toward the base end. The large diameter portion 182 has a flange 184 at its base end. The flange 184 protrudes slightly radially outward from the large diameter portion 182. The flange 184 is used to connect to the connector of the infusion line 90.
[0016] The catheter hub 18 has a hollow portion 18a therein. The hollow portion 18a extends in the axial direction of the catheter hub 18. The hollow portion 18a communicates with the lumen 16a of the catheter body 16 at the tip side. The hollow portion 18a also extends to the base end side of the catheter hub 18 and forms an opening 18b at the base end of the catheter hub 18. The opening 18b and its vicinity have an inner diameter capable of accommodating the inner needle hub 32. The inner needle assembly 14 is inserted into the catheter 12 through the hollow portion 18a.
[0017] As shown in FIG. 1 and FIG. 2, the cylindrical body 20 is attached to the small diameter portion 181 of the catheter hub 18. The cylindrical body 20 is a tube-shaped member formed of a flexible resin film. The cylindrical body 20 has openings at the tip end 20b and the base end 20d. Between the tip end 20b and the base end 20d of the cylindrical body 20, a hollow cylinder having an inner diameter portion 20a on the inside is formed. The catheter hub 18 is inserted into the inner diameter portion 20a of the cylindrical body 20. The cylindrical body 20 has a ring portion 20c joined to the tip end 20b. The ring portion 20c is a ring-shaped member formed of a material having higher rigidity than other portions of the cylindrical body 20. The ring portion 20c is formed of a hard material such as resin or metal. The ring portion 20c is configured to be difficult to spread and not to easily go over a rib 24 described later. The ring portion 20c may be made of a rubber material that is elastically deformable against an external force. If the ring portion 20c made of a rubber material is shaped so as to contract in the circumferential direction to close the tip portion 20b, it is preferable that the opening of the tip portion 20b can be closed after the tubular body 20 is extended. The ring portion 20c moves integrally with the tip portion 20b of the tubular body 20. The base end portion 20d of the tubular body 20 is joined to the step portion 183 of the catheter hub 18.
[0018] As shown in FIG. 2, the tubular body 20 is initially maintained in a contracted state in which it is folded in an accordion-like manner in the axial direction. The tubular body 20 may be maintained in a contracted state by being wound around the outer periphery of the ring portion 20c. In order to prevent the tubular body 20 from elongating during use of the catheter 12, the catheter hub 18 has a rib 24. The rib 24 is a ring-shaped protrusion protruding from the outer periphery of the small diameter portion 181. The rib 24 means a protrusion formed by a part of the outer periphery of the small diameter portion 181 protruding, and its shape is not limited to a ring shape. The rib 24 has an outer diameter slightly larger than the inner diameter of the ring portion 20c of the tubular body 20. The rib 24 abuts against the ring portion 20c to prevent the ring portion 20c from displacing toward the tip, thereby maintaining the tubular body 20 in a contracted state. That is, the ring portion 20c and the rib 24 constitute a tubular body holding portion 26 that holds the tubular body 20 to the catheter hub 18.
[0019] The cylindrical body holding portion 26 prevents the cylindrical body 20 from extending toward the distal end during use of the catheter 12, facilitating use of the catheter 12. The cylindrical body holding portion 26 may have only the ring portion 20c without the rib 24. In this case, the ring portion 20c fits into the small diameter portion 181 to hold the cylindrical body 20 to the catheter hub 18 by friction. In order to increase the friction, an adhesive portion (adhesive layer) may be formed on at least one of the ring portion 20c and the small diameter portion 181. The cylindrical body holding portion 26 may also have the ring portion 20c and an adhesive that releasably fixes the ring portion 20c to the small diameter portion 181. Furthermore, instead of the rib 24, the ring portion 20c and the small diameter portion 181 may be joined by an easily breakable welded portion. An adhesive or welded portion that is easily breakable is one aspect of a breakable fitting portion, and various other aspects may be taken.
[0020] 3, the overall length of the tubular body 20 in an extended state is longer than the overall length of the catheter body 16. When the tubular body 20 is extended, the tip portion 20b protrudes further toward the tip side than the tip 16b of the catheter body 16. The tubular body 20 in the protruding state covers the entire catheter body 16.
[0021] As shown in Fig. 1, the inner needle assembly 14 is inserted inside the catheter 12 in the initial state. The inner needle assembly 14 has an inner needle 22 having a sharp needle tip 28, and an inner needle hub 32 that supports the base end of the inner needle 22. In the initial state, the inner needle 22 is disposed inside the lumen 16a of the catheter body 16. The needle tip 28 of the inner needle 22 protrudes from the tip 16b of the catheter body 16. The inner needle hub 32 fits into the hollow portion 18a of the catheter hub 18 in the initial state.
[0022] The operation of the catheter assembly 10 and the catheter 12 of this embodiment will be described below together with the method of use.
[0023] The user grasps the catheter assembly 10, punctures the inner needle 22 through the patient's skin 92, and inserts the inner needle 22 into a blood vessel 94. The user then grasps the inner needle hub 32 of the inner needle assembly 14, and inserts the catheter body 16 into the blood vessel 94. Once the catheter body 16 has secured the blood vessel 94, the user pulls the inner needle assembly 14 out of the catheter 12 and removes it. The catheter 12 is placed in the patient's blood vessel 94. The user then connects an infusion line 90 to the catheter hub 18 of the catheter 12, and performs treatment such as infusion.
[0024] As shown in FIG. 4, after the treatment such as infusion is completed, the user performs the step of removing the catheter 12. The step of removing the catheter 12 includes the steps of extending the tubular body 20 to cover the catheter main body 16 with the tubular body 20, and pulling the catheter main body 16 out of the skin 92. The operation of extending the tubular body 20 is started by the user pressing the ring portion 20c toward the distal end. This operation causes the ring portion 20c to overcome the rib 24, making the tubular body 20 extendable. Next, the user grasps the ring portion 20c and stretches the tubular body 20 toward the distal end of the catheter main body 16. When the tubular body 20 extends to the vicinity of the patient's skin 92, the user covers the patient's skin 92 and the puncture portion of the catheter main body 16 with the ring portion 20c. After that, the user pulls the catheter hub 18 toward the proximal end to pull the catheter main body 16 out of the skin 92.
[0025] In the above-mentioned removal step, the catheter body 16 is pulled out while being covered with the tubular body 20. Even if the tip 16b of the catheter body 16 is displaced in a bouncing manner due to the elasticity of the catheter body 16 when the catheter body 16 is pulled out from the skin 92, blood adhering to the lumen 16a and the outer peripheral surface of the catheter body 16 is prevented from scattering. Therefore, the catheter assembly 10 and the catheter 12 of this embodiment can prevent blood from scattering, and reduce the risk of infection and the workload of medical personnel.
[0026] Second embodiment A catheter 12A of this embodiment shown in Figure 5 differs from the catheter 12 described with reference to Figures 1 to 4 in a catheter hub 18A, a cylindrical body 20A, and a cylindrical body holding portion 26A. Note that in the catheter 12A, the same components as those in the catheter 12 are denoted by the same reference numerals, and detailed description thereof will be omitted.
[0027] The cylindrical body 20A of this embodiment has a basic structure similar to that of the cylindrical body 20 of FIG. 2. However, the cylindrical body 20A is different from the cylindrical body 20 of FIG. 2 in that an elastic member 36 is arranged in the inner diameter portion 20a or the outer diameter portion. The elastic member 36 is a coil spring that can expand and contract in the axial direction. In the natural state, the elastic member 36 has a length approximately equal to the total length of the cylindrical body 20A in the expanded state. In the cylindrical body 20A in the contracted state, the elastic member 36 is maintained in a state compressed in the axial direction. The tip of the elastic member 36 is joined to the ring portion 20c, and the base end of the elastic member 36 is abutted against the step portion 183. Therefore, in the contracted state, the elastic member 36 urges the ring portion 20c toward the tip direction by the elastic restoring force.
[0028] The catheter hub 18A is similar to the catheter hub 18 (FIG. 2), but differs from the catheter hub 18 in that it further includes a hub-side fixing part 38. The catheter hub 18A has two hub-side fixing parts 38. The hub-side fixing parts 38 are arranged at positions spaced 180° apart from each other in the circumferential direction of the catheter hub 18A. The hub-side fixing part 38 includes a support part 40 and a lever 42. The support part 40 protrudes radially outward from the large diameter part 182 of the catheter hub 18A. The support part 40 includes a shaft part 44 that rotatably supports the lever 42. The shaft part 44 includes a biasing member (e.g., a torsion spring) not shown. The biasing member biases the claw part 46 (described later) of the lever 42 in a direction approaching the center of the catheter hub 18A.
[0029] The lever 42 is a rod-shaped member that is elongated in the axial direction. The lever 42 is attached to the support part 40 at a portion closer to the base end. The tip 42a of the lever 42 extends to near the center of the small diameter part 181 in the axial direction. The tip 42a of the lever 42 has a claw part 46 that is bent toward the center of the catheter hub 18A. The claw part 46 protrudes slightly toward the inside in the radial direction of the catheter hub 18A. The claw part 46 abuts against the ring part 20c of the tubular body 20A to prevent the ring part 20c from displacing toward the tip. That is, the tubular body holding part 26A of this embodiment is composed of the hub side fixing part 38 and the ring part 20c.
[0030] The levers 42 have operating parts 48 extending from the support part 40 in the proximal direction. The operating parts 48 are subjected to a pressing force by the user's fingers. When the user grasps the operating parts 48 of the two levers 42 and presses them toward the center of the catheter hub 18A, the levers 42 rotate relative to the support part 40. When the levers 42 rotate, the claw parts 46 are displaced outward from the tubular body 20A, and the engagement between the claw parts 46 and the ring part 20c is released. As a result, the holding of the tubular body 20A by the tubular body holding part 26A is released.
[0031] The catheter 12A of this embodiment has the above-mentioned configuration. Next, the operation of the catheter 12A will be described together with the method of use.
[0032] As shown in Fig. 6A, the catheter 12A of this embodiment extends the tubular body 20A in the removal process. The extension operation of the tubular body 20A of this embodiment is performed by pressing the operating portion 48 of the lever 42 as shown by the arrow in the figure. When the operating portion 48 is pressed toward the center of the catheter hub 18A, the lever 42 rotates around the shaft portion 44. The rotation of the lever 42 displaces the tip of the lever 42 radially outward. As a result, the claw portion 46 of the lever 42 disengages from the ring portion 20c of the tubular body 20A.
[0033] As shown in Fig. 6B, when the claw portion 46 is released from the ring portion 20c, the ring portion 20c becomes displaceable toward the axial tip. The ring portion 20c is displaced toward the axial tip by the elastic force of the elastic member 36. The tip of the tubular body 20A displaces toward the tip following the ring portion 20c. As a result, the tubular body 20A unfolds toward the axial tip and covers the entire catheter body 16.
[0034] As a result, the catheter 12A of this embodiment can prevent blood adhering to the outer periphery and inner cavity 16a of the catheter main body 16 from scattering by the tubular body 20A.
[0035] (Modification of the second embodiment) The catheter 12B of this modified example shown in Figure 7 has a catheter hub 18B. The catheter hub 18B differs from the catheter hub 18A of Figure 5 in the hub side fixing part 38B. The configuration of the catheter 12B other than the hub side fixing part 38B is similar to that of the catheter 12A of Figure 5. In the configuration of the catheter 12B, the same components as those of the catheter 12A of Figure 5 are denoted by the same reference numerals and detailed description thereof will be omitted.
[0036] The catheter hub 18B has two hub side fixing parts 38B as the cylindrical body holding part 26B. One hub side fixing part 38B and the other hub side fixing part 38B are arranged circumferentially at an angle of 180°. Each hub side fixing part 38B has a support part 40B, a lever 42, and a joint part 44B. The support part 40B and the lever 42 are integrally joined at the joint part 44B. In the initial state, the joint part 44B joins the lever 42 to the support part 40B with the tip of the lever 42 tilted inward. Therefore, in the initial state, the claw part 46 of the lever 42 holds the ring part 20c of the cylindrical body 20A.
[0037] The joint 44B in this embodiment is elastically deformable. When the operating portion 48 of the lever 42 is pressed inward, the joint 44B is elastically deformed, causing the lever 42 to rotate relative to the support portion 40B. The joint 44B functions in the same manner as the shaft portion 44. The rotation of the lever 42 causes the claw portion 46 to be displaced outward. As a result, the lever 42 releases the holding of the tubular body 20A, allowing the tubular body 20A to be extended. In this modification, the large diameter portion 182, the support portion 40B, the lever 42, and the claw portion 46 can be formed as an integral member. The catheter 12B of this modification has a simpler structure than the catheter 12A having the shaft portion 44, and the number of parts can be reduced.
[0038] (Other embodiments) In the above-described catheter 12, the ring portion 20c of the tubular body 20 may be configured to fit into the small diameter portion 181 of the catheter hub 18, and the tip portion 20b of the tubular body 20 may be fixed to the catheter hub 18 by friction with the outer circumferential surface of the catheter hub 18. In this embodiment, the tubular body holding portion 26 can be configured only by the ring portion 20c, thereby simplifying the structure.
[0039] In the above-mentioned catheter 12A, the tubular body holding portion 26A is provided on the catheter hub 18A and has a hub side fixing portion 38 that fixes the tip portion 20b of the tubular body 20A, and the hub side fixing portion 38 may hold the tip portion 20b of the tubular body 20A by an engagement portion, a fitting portion, or a breakable joint (including a welded portion).
[0040] In the above-described catheter 12, the tubular body 20 may be made of a rigid member that changes irreversibly only from a contracted state to an expanded state.
[0041] The above embodiments and their effects are summarized below.
[0042] One embodiment includes a catheter body 16 having an inner lumen 16a, a catheter hub 18, 18A fixed to a base end 16c of the catheter body, a tubular body 20, 20A having openings at a tip end 20b and a base end 20d, respectively, the portion between the tip end and the base end being capable of changing shape from a contracted state to an extended state, through an inner diameter portion 20a through which the catheter body and the catheter hub are passed, and a tubular body holding portion 26, 26A that holds the tubular body in the contracted state, and the tubular body is in a catheter 12, 12A whose tip end protrudes beyond the tip of the catheter body and covers the tip of the catheter body in the extended state.
[0043] The catheter described above can prevent blood adhering to the catheter body from scattering when the catheter body is removed by covering the catheter body with the cylindrical body.
[0044] In the above catheter, the base end of the tubular body may be fixed to the outer periphery of the catheter hub, the tubular body may be folded and contracted in the contracted state, and the tubular body holding part may fix the tip end of the tubular body in the contracted state at a position closer to the base end than the tip of the catheter hub. This catheter can hold the tubular body at a position closer to the base end than the tip of the catheter hub when in use. Therefore, the tubular body does not interfere with the use of the catheter.
[0045] In the above catheter, the tubular body may include a ring portion 20c attached to the tip portion, and the ring portion may fix the tip portion of the tubular body to the catheter hub by friction with the outer circumferential surface of the catheter hub in the contracted state. With this configuration, the tubular body holding portion can be composed of only the ring portion, thereby simplifying the structure.
[0046] The tubular body may include a ring portion attached to the tip portion, the ring portion being made of an elastically deformable member shaped to contract in diameter to close the opening of the tip portion of the tubular body, and closing the tip portion after the tubular body is extended. This tubular body covers and closes the tip of the catheter main body after removal, thereby effectively preventing blood from splashing from the catheter main body.
[0047] In the above catheter, the tubular body holding part is provided on the catheter hub and has a hub side fixing part 38 that fixes the tip end of the tubular body, and the hub side fixing part may hold the tip end of the tubular body by engaging, fitting, or a breakable joint. This catheter can prevent the tubular body from elongating before being removed.
[0048] In the above catheter, the hub side fixing part may have a rib 24 that protrudes from the outer periphery of the catheter hub and extends annularly in the circumferential direction of the catheter hub, and the rib may hold the cylindrical body in the contracted state by engaging with the tip end of the cylindrical body. This catheter can simplify the structure of the cylindrical body holding part and reduce manufacturing costs.
[0049] In the catheter described above, the tubular body holding part may further have a ring part attached to the tip part of the tubular body, and the tubular body holding part may have an engagement part where the rib and the ring part engage with each other. This catheter can simplify the structure of the tubular body holding part and reduce manufacturing costs.
[0050] In the above catheter, the tubular body holding portion is provided on the catheter hub and has a claw portion 46 that hooks and holds the tip end of the tubular body from the outside of the tubular body, and the claw portion engages with the tip end of the tubular body to hold it in the contracted state. This catheter can hold the tubular body with the claw portion. Also, since the tubular body holding portion itself is provided on the catheter hub, the operating portion of the claw portion can be located on the catheter hub side. Therefore, this catheter makes it possible to release the contracted state of the tubular body with the hand that holds the catheter hub, improving operability.
[0051] In the catheter described above, the tubular body holding portion may further have a ring portion attached to the tip portion of the tubular body, and the claw portion may hold the tubular body in the contracted state by hooking onto the ring portion. This catheter can hold the ring portion with the claw portion. Therefore, a flexible film-like material can be used for the tubular body, and the tubular body can be reliably held through the ring portion.
[0052] In the above catheter, the cylindrical body holding part may be configured to include a lever 42 that supports the claw part and a support part 40 that supports the lever rotatably relative to the catheter hub, and when an operating part 48 at the base end of the lever is pressed, the claw part is displaced outward from the cylindrical body, releasing the engagement between the cylindrical body and the claw part. This catheter has improved operability because the engagement of the cylindrical body holding part can be released simply by pressing the lever.
[0053] In the above catheter, the tubular body may have an elastic member 36, and the elastic member may promote the extension of the tubular body by biasing the tip end of the tubular body in a direction away from the base end by an elastic restoring force. Since the tubular body of this catheter extends spontaneously, it is not necessary for the user to perform an operation to stretch the tubular body, and therefore the catheter has excellent operability.
[0054] Another aspect of the embodiment is a catheter assembly 10 including the above-mentioned catheter, an inner needle 22 inserted into the lumen of the catheter body, and an inner needle hub 32 fixed to the base end of the inner needle. This catheter assembly 10 can prevent blood from splashing when the catheter is removed.
[0055] Another aspect of the embodiment is a tubular body that is attached to a catheter having a catheter body with an internal lumen and a catheter hub fixed to a base end of the catheter body, covers the tip of the catheter body when the catheter body is removed, has openings at the tip and base ends, is shape-changeable between the tip and base ends from a contracted state to an extended state, has an inner diameter portion through which the catheter body and the catheter hub can be inserted, and in the extended state, the tip protrudes beyond the tip of the catheter body to cover the tip of the catheter body. This tubular body can prevent blood from splashing when the catheter is removed.
[0056] The present invention is not limited to the above-described embodiment, and various configurations can be adopted without departing from the gist of the present invention. [Explanation of symbols]
