Abstract
Description
[Technical field]
[0001] The present disclosure relates to medical devices and, more particularly, to medical visualization and intubation systems. [Background technology]
[0002] Laryngoscopy may be performed for many reasons, e.g., to view the internal structures of the throat, such as the larynx, to facilitate endotracheal intubation using an endotracheal tube (ETT), to perform biopsy procedures, etc. In general, direct laryngoscopy refers to the use of a handheld laryngoscope to view the internal structures of the throat along a direct line of sight, whereas indirect laryngoscopy refers to the use of a laryngoscope in combination with an optical device, e.g., a mirror or prism, a fiberoptic stylet, or a video laryngoscope, to visualize the larynx or trachea along an indirect line of sight. Summary of the Invention [Problem to be solved by the invention]
[0003] However, for some medical procedures, direct laryngoscopy often presents challenges due to unplanned and unplanned scenarios such as anterior vocal cords, obesity, large tongue, or other irregular airway anatomical features that make a direct line of sight difficult. [0005] Embodiments of the present disclosure are now described with reference to the drawings, in which like reference numerals refer to like parts throughout.
[0006] Advantageously, embodiments of the present disclosure provide a system that extends the use of a standard laryngoscope beyond direct laryngoscopy to video-based indirect laryngoscopy for a variety of medical procedures, such as intubation, visualization, etc.
[0007] In one embodiment, the laryngoscope enhancement system includes a holder and a video stylet. The holder includes an adjustable mount and an end cap. The adjustable mount is configured to removably position an electronic device having a display. The end cap is fixedly coupled to the adjustable mount and defines a central cavity having a lower opening. The end cap is configured to removably attach the holder to an upper end of a laryngoscope handle, the lower opening having a diameter that matches the diameter of the upper end of the laryngoscope handle. The video stylet includes a number of wires, a connector coupled to the wires, a flexible portion, a malleable portion, and a video camera coupled to one or more of the wires. The connector is removably attachable to the electronic device. The flexible portion includes a flexible sheath that surrounds the wires. The malleable portion includes a malleable sheath that surrounds the malleable wire and the wires. The malleable sheath has a smaller diameter than the flexible sheath.
[0008] FIG. 1 illustrates an IS 100 according to an embodiment of the present disclosure.
[0009] In various embodiments, the IS 100 includes a laryngoscope 110 , an electronic device 120 , an ETT 130 , and a LES 200 .
[0010] The laryngoscope 110 includes a handle 112, a blade 114, and a connector 116 that attaches the blade 114 to the lower end of the handle 112. The connector 116 may be fixed or removable to support various sizes and types of blades 114. For example, a portion of the connector 116 may be fixed or integrated into the lower end of the handle 112, and a mating portion of the connector 116 may be fixed or integrated into the proximal end of the blade 114. The blade 114 may be a Macintosh (curved) blade, a Miller (straight) blade, or the like, and generally includes a spatula for passing over the lingual surface of the tongue, a flange for displacing the tongue, and a tip for elevating the epiglottis. The handle 112 is cylindrical in shape with a generally uniform diameter and has an upper end and a lower end.
[0011] Electronic device 120 includes a display 122 for displaying images from video stylet 210. More specifically, electronic device 120 is a mobile, portable, computer-based electronic device having a touch screen interface and a data / charging port (e.g., Lighting port, USB-C port, etc.), such as a smart phone (e.g., Apple iPhone®, Samsung Galaxy S / A / Z, etc.), a tablet (e.g., Apple iPad® mini, Samsung Galaxy Tab, etc.), etc. In some embodiments, electronic device 120 may include a network interface for connecting to a cellular network, a WiFi network, etc.
[0012] The ETT 130 includes, among other things, a tube 132, a proximal end cap 133 that defines a proximal opening, and a beveled distal end 134 that defines a distal opening.

The video stylet 210 includes, among other things, a connector 216 that is removably attachable to a data / charging port of the electronic device 120, a flexible portion (shown in solid lines) disposed generally outside the tube 132, a malleable portion (shown in dotted lines) disposed generally within the tube 132, and a video camera 214. The malleable portion of the video stylet 210 is deformable into a desired shape or curvature and may extend beyond the proximal end of the ETT 130, i.e., outside the tube 132. The combination of flexible and malleable portions of the video stylet 210 is advantageously lightweight and easily manipulated when advancing the ETT 130 into a patient's airway with one hand.
[0014] FIG. 2 illustrates in situ operation of IS100 according to an embodiment of the present disclosure.
[0015] During an intubation procedure on a patient 104, the healthcare professional 102 holds the handle 112 of the laryngoscope 110 in one hand and the tube 132 of the ETT 130 in the other hand. More specifically, the healthcare professional 102 first manipulates the video stylet 210 into a preferred curvature and then inserts the video stylet 210 into the opening defined by the proximal end cap 133 of the ETT 130 such that the video camera 214 is positioned near the opening defined by the beveled distal end 134 of the ETT 130. Once inserted, the ETT 130 generally conforms to the curvature of the malleable portion of the video stylet 210. With the aid of the laryngoscope 110, the healthcare professional 102 introduces the ETT 130 into the mouth of the patient 104. After initial placement of the ETT 130, the healthcare professional 102 uses indirect laryngoscopy to advance the ETT 130 toward the vocal cords of the patient 104 while an image of the upper airway captured by the video camera 214 is presented on the display 122 for viewing by the healthcare professional 102.
[0016] During a visualization procedure on the patient 104, the ETT 130 is not used, and the healthcare professional 102 holds the handle 112 of the laryngoscope 110 in one hand and the video stylet 210 in the other. More specifically, with the aid of the laryngoscope 110, the healthcare professional 102 introduces the video stylet 210 into the mouth of the patient 104. After initial placement of the video stylet 210, the healthcare professional 102 advances the video stylet 210 toward the vocal cords of the patient 104 using indirect laryngoscopy, while an image of the upper airway captured by the video camera 214 is presented on the display 122 for viewing by the healthcare professional 102.
[0017] The IS 100 is advantageous in incorporating the same handheld laryngoscope into these procedures, allowing the healthcare professional 102 to perform the same physical actions as before while integrating indirect line of sight capabilities.
