[0003] Field

[0003] The present disclosure relates to the field of autoimmune diseases, and more particularly, to compositions and methods for diagnosing primary biliary cirrhosis. [Background technology]

[0004] background

[0004] Primary biliary cirrhosis (PBC), now known as primary biliary cholangitis, is a chronic inflammatory autoimmune disease of the liver. PBC is characterized by progressive damage to bile ducts in the liver. Recent studies have identified the presence of circulating autoantibodies that can be used as biomarkers for PBC. See Norman et al., Liver Int., 35 (2): 642-651 (2015);Hirschfield et al., Annu Rev Pathol., 8: 303-330 (2013);Bogdanos et al., Dig Dis., 30 Suppl 1: 20-31 (2012). However, a subpopulation of PBC patients remains serologically negative for known autoantibodies, and the diagnosis is not confirmed until more advanced disease has developed. Thus, there is a need for new and / or more specific biomarkers that detect additional autoantibody species or improve diagnostic accuracy or sensitivity. The present disclosure addresses this need and provides related advantages as well. Summary of the Invention [Means for solving the problem]

[0046] Detailed Description

[0051] The present disclosure is based in part on the discovery of Kelch-like 12 (KLHL12) epitopes that are specific to autoantibodies against KLHL12 protein and can be used to detect anti-KLHL12 antibodies as an indicator of primary biliary cirrhosis (PBC), now known as primary biliary cholangitis. Thus, the present disclosure provides benefits to PBC subjects, for example, by specifically detecting anti-KLHL12 antibodies that indicate the presence of PBC, such as in subjects who are seronegative for known biomarkers of PBC. Furthermore, such benefits allow subjects at risk of PBC or subjects with early PBC to prevent or reduce disease progression and associated symptoms.

[0051]

[0056] As used herein, the term "Kelch-like 12" or "KLHL12" (also known as Kelch-like family member 12, DKIR, HDKIR, CUL3-interacting protein 1, C3IP1, and Kelch-like protein C3IP1) refers to a polypeptide that acts as a substrate adaptor for the Cullin 3 ubiquitin ligase complex to facilitate ubiquitination. See Rondou et al., J Biol Chem., 283: 11083-11096, (2008). KLHL12 is expressed in a variety of tissues, including liver, pancreas, gallbladder, colon, adrenal tissue, and small intestine, to name a few. [0073]

[0077] In some embodiments, the solid support may have an isolated KLHL12 fragment attached to its surface. Any isolated KLHL12 fragment, exemplified by comprising an amino acid sequence selected from the group consisting of SEQ ID NOs: 1-25, may be attached to the solid support. In some embodiments, any isolated KLHL12 fragment may be immobilized to the solid support via a linker molecule. In some embodiments, all that is required is that the molecule, such as any isolated KLHL12 fragment, remain immobilized or attached to the support under conditions for which the support is intended to be used, e.g., applications requiring antibody binding or detection. Please note that the term "immobilized" is used interchangeably with "attached" unless otherwise indicated, either explicitly or by context, and both terms are intended to include covalent and non-covalent attachment.

[0074]

[0078] As used herein, the term "antibody" is used interchangeably with immunoglobulin (Ig) and refers to a polypeptide product of B cells, or recombinant equivalents thereof, that is capable of binding to a specific molecular antigen and consists of two heavy chains and two light chains. Each amino-terminal portion of each chain contains a variable region that confers binding specificity. See Borrebaeck (ed.), Antibody Engineering, Second Edition, Oxford University Press. (1995); Kuby, Immunology, Third Edition, WH Freeman and Company, New York (1997). The term encompasses autoantibodies and antibodies used as detection probes in the methods and kits disclosed herein. Antibodies may exhibit specific binding affinity that binds to a single molecular species or panspecific binding affinity that selectively binds to more than one related molecular species. In the context of this disclosure, specific molecular antigens that may be bound by the antibodies of this disclosure include anti-KLHL12 antibodies, including, for example, any of the KLHL12 fragments having SEQ ID NOs: 1-25 or anti-KLHL12 antibodies specific for any one or more of the KLHL12 fragments having SEQ ID NOs: 1-25. The antibodies of the present disclosure may be derived from any mammalian organism, including mouse, rabbit, goat, chicken, donkey, etc. Furthermore, the primary or secondary antibodies may be monoclonal, polyclonal, chimeric, or humanized. The antibodies provided herein may also be used in the methods and kits of the present disclosure.

[0075]

[0079] As used herein, the term "anti-KLHL12 antibody," when used in reference to an autoantibody, is intended to mean an autoantibody specific or pan-specific to one or more KLHL12 fragments having SEQ ID NOs: 1-25. Anti-KLHL12 antibodies may include any antibody class (IgG, IgM, IgD, IgA, and IgE) and subclass (IgG1, IgG2, IgG3, IgG4, IgA1, and IgA2) or functional fragments thereof. In some embodiments, anti-KLHL12 antibodies include human autoantibodies against human KLHL12.

[0076]

[0080] The term "autoantibody" should be understood to refer to immunoglobulins against components of the tissue of the subject that produces the autoantibody. The term is intended to include antibodies produced by the subject's immune system against one or more of the subject's own polypeptides or antigens. Thus, autoantibodies can be produced by the subject's immune system when the immune system fails in whole or in part to distinguish between self and non-self tissue components. As provided herein, exemplary autoantibodies include anti-KLHL12 antibodies described herein.

[0077]

[0081] As used herein, the term "signal-to-noise ratio" refers to the ratio between the specific binding affinity of a protein to its target and the non-specific binding affinity of the protein to its non-target.Those skilled in the art will understand that methods for measuring the binding affinity between proteins are well known in the art.In some embodiments, the binding between isolated KLHL12 fragments and anti-KLHL12 antibodies, including anti-KLHL12 autoantibodies, shows a signal-to-noise ratio standard score (ZS) of 25 or less.

[0078]

[0082] The present disclosure provides a kit for diagnosing PBC. The kit may include one or more isolated KLHL12 fragments of the present disclosure. Exemplary isolated KLHL12 fragments include SEQ ID NOs: 1-25 as disclosed herein and found in Table 2 below. In other embodiments, the isolated KLHL12 fragment begins and ends at the listed amino acid residues. The isolated KLHL12 fragment may be chemically synthesized, recombinantly synthesized, or isolated from a natural source as disclosed herein. The isolated KLHL12 fragment may specifically bind to anti-KLHL12 antibodies, including anti-KLHL12 autoantibodies, and the binding exhibits a signal-to-noise standard score (ZS) of 25 or less.

[0079]

[0083] The kit can include a detection probe specific to anti-KLHL12 antibody. As used herein, the term "detection probe" refers to a binding agent that can specifically bind to a target. Such binding agents include, for example, antibodies and antibody-specific binding polypeptides. The detection probes provided herein can also be used in the methods of the present disclosure.

[0080]

[0084] Antibodies include full-length antibodies and functional fragments, such as those exemplified below. As used herein, the term "functional fragment" when used in reference to an antibody is intended to refer to a portion of an antibody comprising a heavy or light chain polypeptide that retains some or all of the antibody binding activity of the antibody from which the fragment is derived. Such functional fragments can include, for example, Fd, Fv, Fab, F(ab'), F(ab)2, F(ab')2, single chain Fv (scFv), diabodies, triabodies, tetrabodies and minibodies, which can be found, for example, in Harlow and Lane, Antibodies: A Laboratory Manual, Cold Spring Harbor Laboratory, New York (1989); Myers (ed.), Molec. Biology and Biotechnology: A Comprehensive Desk Reference, New York: VCH Publisher, Inc.; Huston et al., Cell Biophysics, 22: 189-224 (1993); Pluckthun and Skerra, Meth. Enzymol., 178: 497-515 (1989) and Day, ED, Advanced Immunochemistry, Second Ed., Wiley-Liss, Inc., New York, NY (1990). Antibody functional fragments provided herein can be used in the methods and compositions of the disclosure.

[0081]

[0085] Antibody specific binding polypeptides are provided herein and include any polypeptide that can specifically recognize antibodies and functional fragments thereof. Exemplary IgG binding proteins include A protein and G protein. Antibody specific binding polypeptides of the present disclosure can be obtained or synthesized by methods described herein or known in the art, including, for example, chemically synthesized, purified from natural sources, or recombinantly produced. Thus, the antibody specific binding polypeptides described herein can be mammalian, such as mouse, rabbit, goat, chicken, donkey, etc. All such antibody specific binding polypeptides provided herein can be used in the methods and compositions of the present disclosure.

[0082]

[0086] When referring to specific binding to a target, the detection probe of the present disclosure may directly bind to the target, or it may be made specific to the target by indirect means. For example, a detection probe that directly binds to an anti-KLHL12 antibody may include KLHL12 or any of the isolated KLHL12 fragments disclosed herein. Direct binders may also include, for example, antibodies or other antibody-specific binding polypeptides that specifically recognize the KLHL12:anti-KLHL12 antibody complex and antibodies or other antibody-specific binding polypeptides that specifically bind to the anti-KLHL12 antibody. A detection probe of the present disclosure that may be made specific to a target by indirect means may include, for example, anti-Ig or other antibody-specific binding polypeptides that bind Ig. Such antibodies and antibody-specific binding polypeptides may be made specific to an anti-KLHL12 antibody, for example, by capturing the anti-KLHL12 antibody with the isolated KLHL12 fragments described herein and washing away non-anti-KLHL12 Ig before adding anti-Ig or other antibody-specific binding polypeptides that bind Ig. Many other configurations for isolating or separating such binding complexes to achieve specific binding to a target are known in the art, all of which can be used as indirect means to generate target-specific detection probes. Thus, "detection probes specific for anti-KLHL12 antibodies" include, for example, KLHL12, KLHL12:anti-KLHL12 antibody complex binders, anti-KLHL12 antibody binders, and Ig binders.

[0083]

[0087] Thus, in some embodiments, the antibody specific for the anti-KLHL12 antibody is goat anti-human IgG or a functional fragment thereof. In some embodiments, the binding polypeptide specific for the anti-KLHL12 antibody is protein A or protein G or a functional fragment thereof.

[0084]

[0088] The detection probe may include a reporter tag. As used herein, the term "reporter tag" refers to a molecule capable of generating an assay signal indicative of the detection of a biomarker. Radioactive labels include, for example, carbon-14 or tritium. Small molecule labels include biotin, resins such as agarose beads and fluorescently labeled magnetic beads or nanoparticles such as colloidal gold. Polypeptide or functional fragment labels include avidin, streptavidin or neutravidin, which have affinity for biotin. Polypeptide or functional fragment labels also include hemagglutinin (HA), glutathione-S-transferase (GST) or c-myc. Reporter tags and labels provided herein can also be used in the methods of the disclosure.

[0086]

[0090] The label of the present disclosure may be conjugated to any of the detection probes identified herein. Conjugation may include non-covalent or covalent crosslinking as described above. In some configurations, the label conjugated to the detection probe requires an additional substrate or binder as described above. As an example, the HRP label conjugated to the detection probe requires a substrate as disclosed above to detect the detection probe. Many other configurations for labels are known in the art. The present disclosure includes all label configurations exemplified herein and / or known in the art. In some embodiments, the label configuration may include PE conjugated to an isolated KLHL12 fragment having an amino acid sequence selected from the group consisting of SEQ ID NOs: 1-25, a KLHL12:anti-KLHL12 antibody complex binder, an anti-KLHL12 antibody binder, or an Ig binder.

[0109] The present disclosure provides a method for diagnosing PBC, such as in a subject who is seronegative for known PBC antibodies. Such a method includes: (a) contacting a biological sample from a subject suspected of having PBC, including a subject who is seronegative for known PBC antibodies, with an isolated KLHL12 fragment as described herein; and (b) detecting the presence of anti-KLHL12 antibody in the biological sample, wherein the presence of bound anti-KLHL12 antibody indicates PBC.

[0106]

[0110] The presence of increased anti-KLHL12 antibodies in a subject compared to healthy control individuals may indicate the presence of PBC or the risk of developing PBC. Thus, a measurable increase in autoantibodies against KLHL12 is used to diagnose PBC. [0108]

[0112] In some embodiments, detection of increased levels of anti-KLHL12 antibodies compared to healthy control individuals indicates a subject with PBC. In some embodiments, after diagnosing PBC using the compositions and methods provided herein, the presence of PBC can be further confirmed based on various symptoms associated with the onset or presence of PBC. Clinical symptoms associated with PBC include, for example, fatigue, pruritus, sicca syndrome, and upper abdominal discomfort. See Norman et al., Liver Int., 35 (2): 642-651 (2015); Onofrio et al.,. Gastroenterol Hepatol (NY), 15 (3): 145-154 (2019).

[0109]

[0113] In some embodiments, detection of an increased level of anti-KLHL12 antibody compared to a healthy control individual indicates that the subject is at risk of developing clinical symptoms of PBC. In some embodiments, the subject may be at risk of developing clinical symptoms of PBC within less than 3 months, less than 6 months, less than 9 months, less than 12 months, less than 18 months, less than 2 years, less than 3 years, less than 4 years, less than 5 years, less than 6 years, less than 7 years, less than 8 years, less than 9 years, less than 10 years, less than 12 years, less than 14 years, or less than 16 years from the determination of an increased level of anti-KLHL12 antibody.

[0110]

[0114] In some embodiments, the presence of an increased level of anti-KLHL12 antibodies compared to a healthy control individual indicates that the subject is more than 5%, more than 10%, more than 15%, more than 20%, more than 25%, more than 30%, more than 35%, more than 40%, more than 45%, more than 50%, more than 60%, more than 70%, or more than 80%, or more than 90% likely to develop clinical symptoms of PBC within 5 years after determining the increased level of anti-KLHL12 antibodies. In some embodiments, the presence of an increased level of anti-KLHL12 antibodies may indicate that the subject is more than 2-fold, more than 3-fold, more than 4-fold, more than 5-fold, more than 6-fold, more than 7-fold, more than 8-fold, more than 9-fold, or more than 10-fold more likely to develop clinical symptoms of PBC within 5 years after determining the increased level of anti-KLHL12 antibodies compared to a healthy control individual.

[0122]

[0126] The baseline or standard that determines or defines a subject as a non-PBC subject is the reference range. In diagnostic or health-related fields, the reference range is the range of values observed in reference subjects, which may be healthy control individuals, designated by a particular percentile. The reference range may be any range of values determined by a person skilled in the art. See CLSI, "How to define and determine reference intervals in the clinical laboratory: approved guideline" C28: A2 (2000). In some cases, depending on the particular analyte being measured or the disease being studied, the reference range may be strict or less strict. A person skilled in the art will understand the appropriate stringency to use when determining the reference range. Thus, in some embodiments, the reference range may be set at the 95th percentile. To increase specificity and decrease sensitivity, e.g., to increase stringency, a higher cutoff such as the 96th percentile or 97, or 98, or 99, may be used.

[0123]

[0127] In the present disclosure, anti-KLHL12 antibodies may be considered to be increased in a subject if the anti-KLHL12 antibody level is at least above the 95th percentile compared to the anti-KLHL12 antibody level in a healthy control subject. In some embodiments, anti-KLHL12 antibodies may be considered to be increased in a sample if the signal in the anti-KLHL12 antibody detection assay is at least two standard deviations above the noise, such as the mean or median signal of control samples. In some embodiments, anti-KLHL12 antibodies may be considered to be present in a sample if the level of anti-KLHL12 antibodies exceeds a predetermined threshold level. The threshold level of anti-KLHL12 antibodies can be determined by one of skill in the art, such as a clinician, based on various factors, such as the purpose of a particular clinical trial or the diagnostic and prognostic significance of a particular anti-KLHL12 antibody level, or the results of another diagnostic test for PBC that does not involve detection of anti-KLHL12 antibody levels.

For example, the isolated KLHL12 fragment can be immobilized on the wall of a microtiter plate well or cuvette. In some embodiments, contacting the sample from the subject with the isolated KLHL12 fragment can include exposing the sample to the isolated KLHL12 fragment.

[0138]

[0142] In some embodiments, the ELISA can be a direct ELISA. In some embodiments, the direct ELISA can include the first step of immobilizing the isolated KLHL12 fragment on any of the solid supports disclosed herein. For example, the isolated KLHL12 fragment can be immobilized on the wall of a microtiter plate well or cuvette. In some embodiments, contacting the sample from the subject with the isolated KLHL12 fragment can include exposing the anti-KLHL12 antibody contained in the patient sample to the immobilized isolated KLHL12 fragment. Any of the immunoassays disclosed herein (see above) and in the art can be used or modified for use in any of the methods disclosed herein.

[0139]

[0143] As used herein, the term "particle-based multi-analyte test (PMAT)" is intended to mean an immunoassay that allows for the simultaneous measurement of two or more analytes in a single assay. For example, in PMAT, different types of particles are used simultaneously, each type immobilizing a specific binding partner for a specific molecular species on its particle surface. In solution, the analyte molecules to be detected are bound to their binding partners on the corresponding particle type. The binding is then visually detected by adding a secondary marker that marks the analyte molecules bound to all particles of the multiplex assay. PMAT can be performed using various formats known in the art, such as flow cytometry, capture sandwich immunoassays, or competitive immunoassays. For example, a dual-laser flow-based detection instrument can be used to detect the binding of analyte fractions, such as autoantibodies, via the fluorescence of the secondary marker. In some embodiments, the PMAT particles can be beads. Using PMAT, the presence of one or more autoantibodies specifically associated with an autoimmune disease can be identified, and patients can be diagnosed with an autoimmune disease specifically associated with the autoantibodies identified by PMAT.

[0140]

[0144] In some embodiments, anti-KLHL12 antibodies can be detected in biological samples using dot blot or line immunoassay (LIA). Methods and protocols for dot blot, including estimation of polypeptide concentration, are well known in the art. See Joint ProteomicS Laboratory (JPSL) of the Ludwig Institute for Cancer Research, Estimating protein concentration by dot blotting of multiple samples, Cold Spring Harbor Protocols, New York (2006).

[0141]

[0145] The immunoassay may further include a blocking step, a washing step, and additionally or alternatively an elution step. The blocking step may include contacting the solid support of the immunoassay in a blocking buffer for a time and temperature sufficient to allow blocking. Exemplary blocking buffers are identified above. The washing step includes contacting the solid support of the immunoassay with a washing buffer to eliminate non-specific binding of the polypeptide to the solid support. Exemplary washing buffers are described above. The elution buffer may include any of a variety of elution buffers known in the art or disclosed herein. The elution buffer may include, for example, a 0.1 M glycine:HCl solution at pH 2.5-3. The polypeptide complex may be eluted from the solid support of the immunoassay to aid in the detection and measurement of, for example, KLHL12:anti-KLHL12 antibody complexes.

[0142]

[0146] The detection is carried out on a solid support. Exemplary solid supports include beads, spheres, particles, membranes, chips, slides, plates, wells, and test tubes disclosed herein. In some embodiments, the beads, spheres, or particles comprise micrometer or nanometer dimensions. In some embodiments, the membrane is selected from the group consisting of nitrocellulose, nylon, polyvinylidene fluoride (PVDF), and polyvinylidene difluoride.

[0147] The sequences in Table 2 are exemplary amino acid sequences of the isolated KLHL12 fragments described herein.

[0144] [Table 2] EXAMPLES

[0145] Example I Selection of KLHL12 fragment candidates through epitope mapping of the KLHL12 protein

[0148] This example illustrates the selection process of KLHL12 fragment candidates via epitope mapping of the KLHL12 protein.

[0146]

[0149] For epitope mapping, two serum pools were used. The first pool consisted of sera containing anti-KLHL12 antibodies, and the second pool consisted of sera from systemic lupus erythematosus (SLE) patients who were negative for anti-KLHL12 antibodies. The two pools were used to perform epitope mapping on custom sequences overlapping with KLHL12 fragments synthesized on a solid-phase matrix (PEPperPRINT GmbH, Heidelberg, Germany).

[0147]

[0150] To avoid truncated KLHL12 fragments, the sequence of the KLHL12 protein (NCBI gene ID: 59349) was extended at the C- and N-termini with the natural GSGSGSGSG (SEQ ID NO: 26) linker. The extended KLHL12 protein sequence was translated into a 15 amino acid KLHL12 fragment containing an interpeptide overlap of 14 amino acids. The resulting KLHL12 fragment was printed in duplicate and framed by an additional control peptide (YPYDVPDYAG (SEQ ID NO: 27)). One copy of the microarray containing both the resulting KLHL12 fragment and the control peptide was subsequently incubated with a pool of sera containing anti-KLHL12 antibodies. The other copy of the microarray was incubated with a pool of sera negative for anti-KLHL12 antibodies.

[0148]

[0151] The signal intensity of each spot in the two microarrays was quantified by an imaging instrument. Spots with high signal intensity or high signal-to-noise ratio, as shown in Figure 1, suggested KLHL12 fragment candidates for further analysis. These KLHL12 fragment candidates were synthesized and evaluated via several solid-phase assays, including ELISA and bead-based technology.

[0149]

[0152] This example demonstrates that KLHL12 fragments disclosed herein that have the ability to specifically bind to anti-KLHL12 antibodies can be systematically screened and evaluated.

[0150] Example II Performance of isolated KLHL12 fragments in detecting anti-KLHL12 antibodies in serum specimens from PBC patients

[0153] This example illustrates the ability of isolated KLHL12 fragments to detect anti-KLHL12 antibodies in serum specimens from PBC patients.

[0151]

[0154] The KLHL12 fragment consisting of the amino acid sequence of SEQ ID NO:1 was labeled with biotin at either the C-terminus (for P2623-1) or N-terminus (for P2738-1). The labeled KLHL12 fragment was further coated at 5 μg / ml onto either Costar high binding microwell plates (for P2623-1) or Inova streptavidin coated plates (for P2738-1) using standard Inova proprietary protocols and materials.

[0152]

[0155] Detection of anti-KLHL12 antibodies in serum specimens was performed by (1) making a sample diluent of 1:101 dilution of serum specimen in horseradish peroxidase (HR), (2) applying 100 μl of diluted specimen to each well, and (3) incubating the diluted specimen with the wells containing immobilized KLHL12 fragments at room temperature for 30 minutes. Upon completion of incubation, the wells were washed three times with 200-300 μl of HRP wash solution to wash away unbound anti-KLHL12 antibodies. Each well was then incubated for 30 minutes at room temperature with 100 μl of HRP-conjugated goat anti-human IgG. Upon completion of incubation, the wells were washed three times with 200-300 μl of HRP wash solution to wash away unbound HRP-conjugated goat anti-human IgG. Each well was then incubated with 100 μl of TMB (3,3',5,5;-tetramethylbenzidine) chromagen for 30 min. The reaction was stopped by adding 100 μl of 0.1 N H2SO4 stop solution. Reactivity was assessed by measuring the intensity of the developed color spectrophotometrically at 450 / 620 nm.

[0153]

[0156] To demonstrate the selective reactivity of serum specimens from PBC patients to KLHL12 fragments, serum specimens were also tested for reactivity to cyclic citrullinated peptides (CCPs), which are targets of antibodies present in many patients with rheumatoid arthritis (RA). The CCP-coated plates used herein were obtained from QUANTA Lite® CCP3 IgG ELISA kit (Inova Diagnostics, San Diego, CA). Receiver operating characteristic (ROC) analysis of the results clearly demonstrated the selective binding of serum specimens from PBC patients to KLHL12 fragments. In contrast, serum specimens from PBC patients showed only negligible binding to CCP, an RA-associated antigen. See Figure 2.

[0154]

[0157] To demonstrate the effectiveness of KLHL12 fragments in detecting anti-KLHL12 antibodies in serum specimens from PBC patients, serum specimens were also tested for reactivity with full-length recombinant KLHL12 protein with a GST tag at the N-terminus. KLHL12 protein was commercially obtained from Abnova Corporation (Taipei, Taiwan). Receiver operating characteristic (ROC) analysis of the results clearly showed that KLHL12 fragments were as effective as full-length KLHL12 protein in binding to anti-KLHL12 antibodies in serum specimens from PBC patients. See Figure 3.

[0155]

[0158] Serum specimens from 254 PBC patients from the Liver Unit of the Hospital Clinic of Barcelona, Spain and 40 PBC patients from Saint-Antoine Hospital of Paris, France were collected. The collected specimens were tested for anti-KLHL12 antibody prevalence using the QUANTA Lite® ELISA kit (Inova Diagnostics, San Diego, CA). The amino acid sequence of the KLHL12 fragment used was IECYDPIIDSWEVVTSMG (SEQ ID NO: 25), consisting of the amino acid sequence of SEQ ID NO: 2, with three additional amino acids at the C-terminus. The KLHL12 fragment was labeled with biotin at the C-terminus.

[0156]

[0159] Anti-KLHL12 antibodies were positive in 22.8% of a Spanish cohort of PBC patients and in 20% of a French cohort of PBC patients, demonstrating that KLHL12 fragments detect anti-KLHL12 antibodies in serum specimens from PBC patients originating from different geographic locations.

[0157]

[0160] This example demonstrates that the KLHL12 fragments disclosed herein are capable of diagnosing PBC through detecting anti-KLHL12 antibodies in serum specimens from PBC patients.

[0158] Example III Improving the sensitivity of PBC diagnosis using isolated KLHL12 fragments

[0161] This example illustrates that the addition of anti-KLHL12 antibody test using the KLHL12 fragment disclosed herein can improve the sensitivity of PBC diagnosis. The amino acid sequence of the KLHL12 fragment used in this example was IECYDPIIDSWEVVTSMG (SEQ ID NO: 25), which consists of the amino acid sequence of SEQ ID NO: 2, with three additional amino acids at the C-terminus.The KLHL12 fragment was labeled with biotin at the C-terminus.

[0159]

[0162] Serum specimens from a total of 487 patients with clinically documented PBC or PBC / AIH overlap diagnosed according to the European Association for the Study of the Liver (EASL) guidelines were collected at five specialized clinical sites (Barcelona, Spain; Salamanca, Spain; Calgary, Canada; Edmonton, Canada; Warsaw, Poland). Collected specimens were tested for the presence of antibodies against hexokinase 1 (HK-1) and KLHL12 using QUANTA Lite® ELISA kits (Inova Diagnostics, San Diego, CA). All serum specimens were also tested for anti-MIT3 autoantibodies.

[0160]

[0163] By adding anti-HK-1 and anti-KLHL12 antibody testing to anti-MIT3 antibody testing, the sensitivity for detecting PBC increased from 85.0% for anti-MIT3 alone to 90.8% for the combination of anti-MIT3, anti-HK-1, and anti-KLHL12 for the cohort of samples. See Figure 4.

[0161]

[0164] This example demonstrates that the KLHL12 fragments disclosed herein can be used to improve the sensitivity of PBC diagnosis.

[0162] Example IV Improving clinical reliability of PBC diagnosis using isolated KLHL12 fragments

[0165] This example illustrates that the addition of anti-KLHL12 antibody test using the KLHL12 fragment disclosed herein can improve the clinical reliability of PBC diagnosis.

[0163]

[0166] Serum specimens tested here were from 40 PBC patients from the Saint-Antoine Hospital of Paris, France, as described above in Example II. Specimens were tested for the presence of antibodies to MIT3, LKM-1, SLA, LC-1, sp100, gp210, HK-1, and KLHL12 using Aptiva® Liver Reagents (for research use only, Inova Diagnostics, San Diego, CA).

[0164]

[0167] The frequency and overlap of autoimmune liver disease-related biomarkers detected in 40 PBC patients are shown in Figure 5. Anti-KLHL12 antibodies were shown to be present together with other PBC-specific biomarkers. Furthermore, among MIT3-negative PBC patients, 25% (2 / 8) were positive for anti-KLHL12 antibodies as well as anti-gp210 and anti-HK-1 antibodies. See Figure 6.

[0165]

[0168] This example demonstrates that the KLHL12 fragments disclosed herein can be included as components of multi-analyte assays, thereby improving the clinical reliability of PBC diagnosis.
