[0004] Glutaminyl-peptide cyclotransferase (QPCT) , also known as glutaminyl cyclase (GC) , was first isolated by Messer from the latex of the tropical plant Carica papaya in 1963 (Messer, M. Nature (1963) 4874: 1299) . Twenty four years later, a corresponding enzymatic activity was discovered in animal pituitary (Busby, W.H.J. et al. J Biol Chem (1987) 262 (18) : 8532-8536; Fischer, W.H. and Spiess, J. Proc Natl Acad Sci U S A. (1987) 84 (11) : 3628–3632) . QPCT catalyzes the conversion of N-terminal L-glutaminyl residues of peptides to pyroglutamyl groups, and is present in the pituitary and adrenal glands, where it is important for the generation of the N-terminal pyroglutamyl groups of peptide hormones such as neurotensin and thyrotropin-releasing hormone. QPCT also catalyzes the conversion of N-terminal L-glutamyl residues to pyroglutamyl residues. This activity may contribute to the formation of several amyloid-related plaque forming peptides, contributing to Alzheimer's disease pathology. QPCT is also considered to be a diagnostic marker of thyroid tumors.

[0005] Glutaminyl-peptide cyclotransferase-like protein (QPCTL) , an isoenzyme of QPCT, catalyzes the cyclization of N-terminal glutamine and glutamic acid residues on target proteins such as CCL2, CCL7 and CX3CL1, forming pyroglutamate residues at the NH2-terminus of the proteins, and thereby influencing their biological properties. Recent studies reported that the formation of pyroglutamate on CD47 mediated by QPCTL is essential for its binding to SIRPαand function as “don’ t eat me” signal. QPCTL deficiency significantly enhances the macrophage-mediated phagocytosis of tumor cells. Moreover, inhibition of QPCTL can enhance the efficacy of PD-1 blockade via reshaping the infiltration of myeloid cells. These studies indicate that QPCTL is an attractive target for the treatment of cancers. (Lei Yu et. al. Signal Transduction and Targeted Therapy (2023) 8: 454)

[0006] QPCT and QPCTL are considered attractive therapeutic targets in many human disorders, such as neurodegenerative diseases, and a range of inflammatory conditions, as well as for cancer immunotherapy. In recent decades, small molecule inhibitors targeting QPCT and QPCTL have been developed, and some of QPCT inhibitors including PBD150, PQ912 and SEN177 also exhibited inhibitory activity against QPCTL.

[0007] In spite of numerous treatment options for cancer patients, for example, alkylating agents, antimetabolites, hormones and immunotherapy, there remains a need for effective and safe therapeutic agents and a need for their preferential use in combination therapy.Summary of the Invention

[0008] In the first aspect, the present disclosure provides a pharmaceutical combination for use in treatment of cancer in a subject, comprising a first anti-cancer agent which is an inhibitor of glutaminyl-peptide cyclotransferase-like protein (QPCTL)  / glutaminyl-peptide cyclotransferase protein (QPCT) , a second anti-cancer agent, and optionally a third anti-cancer agent. The first anti-cancer agent, the second anti-cancer agent and the optional third anti-cancer agent may be administered to the subject simultaneously, separately or sequentially.

[0439] On Day 28 of the experiment, compared with the Vehicle group, the Compound A 10 mg / kg monotherapy group, the Cisplatin monotherapy group, the Compound A 10 mg / kg + anti-PD-1 10 mg / kg dual therapy group, the Compound A 10 mg / kg + anti-PD-L1 10 mg / kg dual therapy group, the Compound A 10 mg / kg + Cisplatin 3 mg / kg dual therapy group, the Compound A 10 mg / kg + anti-PD-1 10 mg / kg + Cisplatin 3 mg / kg triple therapy group and the Compound A 10 mg / kg + anti-PD-L1 10 mg / kg + Cisplatin 3 mg / kg have the relative tumor growth rates (T / C) of 69.70%, 60.29%, 75.92%, 47.28%, 59.63%, 65.83%, and 42.12%, respectively, and the tumor growth inhibition rates (TGI) of 35.23%, 46.00%, 27.49%, 60.74%, 46.37%, 39.44%, and 66.84%, respectively, demonstrating statistically significant tumor growth inhibition (p < 0.05) , while no significant changes were observed in the other groups (p > 0.05) . Compared with Compound A alone, both the dual combination (Compound A + anti-PD-L1) and triple combination (Compound A + anti-PD-L1 + cisplatin) achieved statistically significant differences in endpoints (p < 0.05) . The q values were calculated as described in Example 2 for evaluating combination therapy. The q value for Compound A combined with anti-PD-L1 antibody was 1.35, indicating a significant synergistic effect between the two drugs. However, there was no significant additive or synergistic effect between Compound A and an anti-PD-1 antibody or cisplatin. Trend analysis suggests that the tumor inhibition effect of the triple combination (Compound A + anti-PD-1 + cisplatin) was slightly enhanced compared to the dual combination (Compound A + anti-PD-1) , but not significantly.
