Abstract

The present invention provides an anti-OX40L / IL-31Rot bispecific antibody and uses thereof. Specifically, the present invention provides for the first time a bispecific antibody that simultaneously targets OX40L and IL-31Rot for the treatment of skin diseases such as atopic 5 dermatitis (AD). The anti-OX40L / IL-31Rot bispecific antibody of the present invention exhibits high binding activity to both OX40L and IL-31Rot, synergistically regulates pathogenic T cells, effectively suppresses pruritus while providing long-lasting therapeutic efficacy, thereby offering patients a better treatment experience. The bispecific antibody of the present invention has promising applications in the treatment of skin diseases.

Anti-OX40L / IL-31Rα Bispecific Antibody and Uses ThereofTECHNICAL FIELD

[0001] The present invention relates to the field of biomedicine, and specifically to an anti-OX40L / IL-31Rα bispecific antibody and uses thereof.BACKGROUND ART

[0002] According to the Global Burden of Disease, the global prevalence of atopic dermatitis (AD) has increased annually over the past 30 years (1987-2017) , with up to one-fifth of the population in developed countries affected by AD. AD is a disease with complex etiology and high heterogeneity. It manifests as a wide range of diverse symptoms, severely impacting patients' quality of life. Statistics indicate that it affects approximately 15%-20%of children and 10%of adults in the United States. Additionally, two studies from the United States reported that the total direct and indirect costs for children and adults reached as high as $3, 300 per person per year in 2013. In recent years, the number of AD patients in China has shown an increasing trend annually, and AD has developed into a common refractory skin disease in dermatology.

[0003] Studies have shown that T-cell imbalance is the fundamental cause of AD. OX40 / OX40L, as one of the major cytokines regulating T cells, is one of the therapeutic targets for AD. Several anti-OX40 / OX40L monoclonal antibodies, represented by Sanofi's Amlitelimab, have demonstrated positive clinical effects. Amlitelimab, which targets OX40L, not only shows efficacy not inferior to another approved AD-specific drug (the IL-4R-targeting monoclonal antibody dupilumab) , but also exhibits excellent long-lasting activity in its Phase IIb 52-week data. This may be closely related to its broader regulatory activity on T cells compared to IL-4R, validating the effectiveness of targeting OX40 / OX40L in the treatment of AD.

[0004] The cytokine IL-31 plays a key role in the pathogenesis of both atopic dermatitis and prurigo nodularis (PN) . Prurigo nodularis (PN) is a refractory chronic pruritic disease and the most common subtype of chronic prurigo. It is estimated that more than 180,000 people in the United States suffer from PN. Among various chronic pruritic diseases, PN has the highest intensity and frequency of itching. Long-term recurrent severe itching severely affects patients' quality of life. Patients with PN suffer from a range of debilitating symptoms over the long term, including, in addition to itching, skin nodules covering large areas of the body and poor sleep quality.

[0005] IL-31 initiates itching by binding to the heterodimeric receptor IL-31Rα on itch-sensing neurons and is a cytokine directly involved in pruritus. Monoclonal antibodies targeting IL-31Rα can specifically inhibit IL-31 cytokine signaling, thereby suppressing the pruritus induced by such signaling, as well as pathological conditions including pro-inflammatory responses and epidermal fibrosis mediated by this signaling pathway. Based on its positive performance in the Phase III OLYMPIA clinical trial, Nemluvio (Nemolizumab) from Galderma received FDA approval for marketing in August 2024, and also met all clinical endpoints in two Phase III ARCADIA clinical trials for the treatment of AD.

[0006] Bispecific antibodies represent a novel class of therapeutic antibodies with potential therapeutic advantages in clinical applications. However, there are currently no bispecific antibodies in the art that simultaneously target OX40L and IL-31Rα for the treatment of pruritic skin diseases.

[0007] Therefore, there is an urgent need in the art to develop bispecific antibodies that simultaneously target OX40L and IL-31Rα.SUMMARY OF THE INVENTION

[0008] An object of the present invention is to provide a bispecific antibody that simultaneously targets OX40L and IL-31Rα.

[0009] In a first aspect of the present invention, provided is a bispecific antibody, wherein the bispecific antibody comprises:

[0010] a first antigen-binding domain D1, and

[0012] wherein the D1 specifically binds to the target molecule OX40L protein, and the D2 specifically binds to the target molecule IL-31Rα protein.

A bispecific antibody, characterized in that the bispecific antibody comprises:a first antigen-binding domain D1, anda second antigen-binding domain D2;wherein said D1 specifically binds to the target molecule OX40L protein, and said D2 specifically binds to the target molecule IL-31Rα protein.The bispecific antibody according to claim 1, characterized in that said D1 is an anti-OX40L antibody or an antigen-binding fragment thereof; and said D2 is an anti-IL-31Rα antibody or an antigen-binding fragment thereof.The bispecific antibody according to claim 1, characterized in that said bispecific antibody comprises an anti-IL-31Rα IgG antibody and an anti-OX40L VHH, wherein said anti-OX40L VHH is tandemly linked to the anti-IL-31Rα IgG antibody via a linker.The bispecific antibody according to claim 1, characterized in that said bispecific antibody comprises a monomer, and said monomer has the structure of Formula I from the N-terminus to the C-terminus:wherein,VHH represents an anti-OX40L VHH;VH represents the heavy chain variable region of the anti-IL-31Rα antibody;VL represents the light chain variable region of the anti-IL-31Rα antibody;CH represents the heavy chain constant region;CL represents the light chain constant region;L represents a linker or bond;“~” represents a disulfide bond;“-” represents a peptide bond.The bispecific antibody according to claim 3, characterized in that said anti-OX40L VHH comprises the following three complementarity-determining regions HCDR:HCDR1 as set forth in SEQ ID NO: 1 or 2; HCDR2 as set forth in SEQ ID NO: 3; and HCDR3 as set forth in SEQ ID NO: 4 or 5.The bispecific antibody according to claim 3, characterized in that the amino acid sequence of said anti-OX40L VHH is as set forth in any one of SEQ ID NO: 6, 8, or 10.The bispecific antibody according to claim 2, characterized in that the anti-IL-31Rα antibody comprises a heavy chain complementarity-determining region HCDR selected from the group consisting of:(1) HCDR1 as set forth in SEQ ID NO: 12; HCDR2 as set forth in SEQ ID NO: 13; and HCDR3 as set forth in SEQ ID NO: 14; or(2) HCDR1 as set forth in SEQ ID NO: 15; HCDR2 as set forth in SEQ ID NO: 16; and HCDR3 as set forth in SEQ ID NO: 14;and said anti-IL-31Rα antibody comprises the following three light chain complementarity-determining regions LCDR:LCDR1 as set forth in SEQ ID NO: 17; LCDR2 as set forth in SEQ ID NO: 18; and LCDR3 as set forth in SEQ ID NO: 19.The bispecific antibody according to claim 2, characterized in that the VH sequence of said anti-IL-31Rα antibody is as set forth in SEQ ID NO: 20 or 22; and the VL sequence of the anti-IL-31Rα antibody is as set forth in SEQ ID NO: 24.The bispecific antibody according to claim 1, characterized in that the sequence of said bispecific antibody is as follows:(1) said bispecific antibody has a heavy chain amino acid sequence as set forth in any one of SEQ ID NO: 27, 29, 31, 33, 35, or 37, and said bispecific antibody has a light chain amino acid sequence as set forth in SEQ ID NO: 39;(2) a polypeptide derived from (1) , wherein said derivation refers to a sequence formed by substitution, deletion, or addition of one or more amino acid residues, and said polypeptide has a polypeptide sequence with both anti-OX40L activity and anti-IL-31Rα activity.A polynucleotide molecule, characterized in that said polynucleotide molecule encodes the bispecific antibody according to claim 1.An expression vector, characterized in that said expression vector contains the polynucleotide molecule according to claim 10.A host cell. characterized in that said host cell contains the expression vector according to claim 11.A method for preparing the bispecific antibody according to claim 1, characterized in that said preparation method comprises the following steps:a) culturing the host cell according to claim 12 under expression conditions, thereby expressing the bispecific antibody;b) isolating and purifying the bispecific antibody from step a) .A pharmaceutical composition, characterized in that said pharmaceutical composition comprises an effective amount of the bispecific antibody according to claim 1 and one or more pharmaceutically acceptable carriers, diluents, or excipients.Use of the bispecific antibody according to claim 1, or the pharmaceutical composition according to claim 14, in the manufacture of a medicament for treating a skin disease, pruritic disease, or inflammatory disease.An immunoconjugate, characterized in that said immunoconjugate comprises:(a) the bispecific antibody according to claim 1; and(b) a conjugate moiety selected from the group consisting of: a detectable label, a drug, a toxin, a cytokine, a radionuclide, or an enzyme.A method for treating a skin disease, pruritic disease, or inflammatory disease, characterized in that said method comprises administering to a subject in need thereof the bispecific antibody according to claim 1, the pharmaceutical composition according to claim 14, or the immuno
