[0022] For the purpose of promoting understanding of the principles of the present disclosure, reference will now be made to the implementations shown in the accompanying drawings, and specific language will be used to describe them. However, it should be understood that it is not intended to limit the scope of the present disclosure. Those skilled in the art in the technical field involved in the present disclosure are generally fully able to envision any changes and further modifications to the described devices, instruments, methods, and any further applications of the principles of the present disclosure. Specifically, it is fully conceivable that the features, components and / or steps described with reference to one or more implementations can be combined with the features, components and / or steps described with reference to other implementations of the present disclosure. For simplicity, in some cases, the same reference numerals are used in all drawings to refer to the same or similar parts.
[0023] Embodiments of the present disclosure provide an intraocular lens box ("IOL box") that folds an IOL after removing a cover. The cover may initially cover the mouth of the IOL box to protect the mouth from any damage during storage and / or transportation. The cover is removed to expose the mouth and also actuate the folding mechanism of the IOL box to fold the IOL for delivery into the eye. The IOL box may be a pre-loaded box with pre-loaded IOLs. The IOL box may be part of a modular delivery system that includes a universal interface for removably attaching to various handpieces including different types of drive mechanisms.
[0024] Specific embodiments of the present disclosure allow for interchange between different handpieces (such as disposable and reusable handpieces) and IOL box designs. Disposable handpieces may include a non-electric manual drive mechanism (e.g., manually actuated by pushing or twisting, and / or manually actuated by a fluid or elastic member such as a spring). Reusable handpieces may include a manual drive mechanism as well as an electric drive mechanism (e.g., a stator winding). It should be noted that these types of handpieces are examples, and other types of handpieces or drive mechanisms may be utilized according to specific embodiments of the present disclosure.
[0025] Certain embodiments of the present disclosure allow for the assembly of a handpiece to an IOL box, the delivery of an IOL, and the disengagement of the handpiece from the used IOL box when necessary. The universal interface allows different types of handpieces to be easily paired and used with the IOL box for IOL implantation. The handpiece can be secured to the IOL box by sliding the end of the handpiece over the plunger barrel of the IOL box to form an insertion tool. Once the insertion tool is formed, the IOL can be delivered into the eye. After the IOL is implanted, the IOL box can be easily removed from the handpiece (e.g., a reusable handpiece) by pulling the handpiece from the IOL box.
[0026] Figure 1 A top elevation view of an insertion tool 10 including an IOL cartridge 12 is shown according to some embodiments of the present disclosure. A removable cover ("cover") 16 may cover the IOL cartridge 12. The cover 16 may be removed from the IOL cartridge 12 (e.g., by pulling or pushing in the direction indicated by directional arrow 20) to expose a mouth (not shown) of the IOL cartridge 12. Additionally, the cover 16 may be removed to rotate the internal components of the IOL cartridge 12 to manipulate the IOL for delivery into the eye, for example, by folding and / or compressing the IOL (not shown).
[0027] The insertion tool 10 may include a handpiece 26 that is removably attached to the IOL cartridge 12. It should be noted that various handpieces may be used with different types of IOL cartridges. The handpiece 26 may include an electric or non-electric drive mechanism that may include a push rod 34 extending along the length of the handpiece 26. The push rod 34 may be movably disposed within the handpiece 26 and may be manually actuated via a rotatable knob 35 that may cooperate with a fluid or elastic member (such as a spring 37) or another actuation mechanism. The push rod 34, when actuated, may advance a plunger (not shown) that may be movably disposed within a plunger barrel 28. In general, for purposes of discussing and describing various components and features of the handpiece 26 and the IOL cartridge 12, reference to a proximal or proximal direction may refer to a direction that is more toward the end of the handpiece 26 that includes the rotatable knob 35 according to the directional arrow 23. Similarly, reference to a distal or distal direction may refer to a direction that is more toward the end of the IOL cartridge 12 that includes the cover 16 according to the directional arrow 20.
[0028] The IOL cartridge 12 may include a plunger barrel 28. The plunger barrel 28 may be a rigid, hollow tubular member that may be inserted into the handpiece 26 and secured therein via a protrusion 30 extending from the plunger barrel 28. The protrusion 30 mates with the handpiece 26 via a slot 32 of the handpiece 26 (e.g., via rotation).
[0029] Figure 2 Some embodiments of the present disclosure are shown Figure 1 Insertion tool 10, wherein cover 16 is not shown to show components below cover 16, which are configured to be in an initial non-actuated position. IOL box 12 includes a mouth 36, which is located at the distal end 38 of the housing 40 of IOL box 12. Housing 40 may include a compartment 41, which may be in fluid communication with mouth 36. Compartment 41 may accommodate IOL 25. IOL 25 may be any suitable intraocular lens. IOL 25 may include a lens portion 27 and a haptic extension 31. Haptic extension 31 may be a side support (or other suitable extension) extending from lens portion 27, which side support may stabilize IOL 25 when it may be arranged in the patient's eye. It should be understood that IOL 25 is merely exemplary and the technology disclosed herein may be used with any suitable IOL. For example, a modular IOL (not shown) may also be used, which includes a lens portion that may be arranged in a base with a haptic extension.
[0030] The housing 40 may include a movable or slidable actuator, such as a slider 42, which is movably disposed within a track 44 of the housing 40. The track 44 may be integrated into a side portion 46 of the housing 40. The cover 16 (e.g., Figure 1The cover 16 (shown) can cover the housing 40 and the mouth 36. The inner portion (not shown) of the cover 16 can contact the slider 42 and can move the slider 42 in the direction indicated by the directional arrow 20 as the cover 16 is removed. For example, the slider 42 can be moved from an initial first position to a second position. In the illustrated embodiment, the slider 42 is in an initial unadvanced position or first position. In this initial position, the cover 16 has not been removed, and the IOL 25 is in an unfolded and uncompressed state.
[0031] Figure 3 A cutaway side view of a cover 16 according to some embodiments of the present disclosure is shown, the cover at least partially covering the IOL box 12, wherein the slider 42 is in an initial unadvanced position. In the illustrated embodiment, the inner portion 48 (e.g., the inner top portion) of the cover 16 may include a latch 52 that extends (e.g., upward) into a recess 54 on the bottom side of an extension 56 of the slider 42 to removably secure the cover 16 to the slider 42. As the cover 16 is pulled in the direction of the directional arrow 20, the cover 16 moves the slider 42 forward in the direction 24. Moving the cover 16 disengages the latch 52 of the cover 16 from the recess 54 of the slider 42 due to the latch 52 dropping in height along the tapered portion 58 of the mouth 36, thereby releasing the cover 16 from the IOL box 12. As previously described, the cover 16 covers the mouth 36 and the housing 40 (including the slider 42) of the IOL box 12. The plunger 29 may be movably disposed within the plunger barrel 28. The plunger 29 may extend lengthwise within the plunger barrel 28. It should be noted that only a portion of the plunger 29 is shown.
[0032] Figure 4 Some embodiments of the present disclosure are shown Figure 3 1 and 1 . A bottom view of the slider 42 of the IOL cartridge 12 of the embodiment of the present invention is shown in an initial, unadvanced position (i.e., the cover 16 has not been removed from the IOL cartridge 12 and the IOL 25 is in an unfolded state). In the illustrated embodiment, at least a portion of the slider 42 (e.g., a bottom portion 43 of the slider 42) can completely circumferentially surround the housing 40. The bottom portion 43 can be configured to contact a slider stop 45 of the housing 40. The slider stop 45 can extend outwardly from a bottom outer portion 47 of the housing 40. The slider stop 45 can prevent the slider 42 from sliding off or separating from the housing 40 of the IOL cartridge 12 as the slider 42 moves forward when the cover 16 is removed.
[0033] Figure 5 is a cut-away top elevation view of an IOL cartridge 12 according to a particular embodiment of the present disclosure, wherein the cover 16 (eg, Figure 1 shown) and a slider 42 (eg, Figure 24 (shown) to show the components in an initial non-actuated configuration below the slider 42. As shown, the IOL box 12 is coupled to the handpiece 26. The inner portion 60 of the housing 40 of the IOL box 12 may include a compartment 41. The compartment 41 may include one or more mechanisms (which may include one or more sets of IOL manipulators) for engaging with the IOL 25 to fold, unfold, straighten, or otherwise manipulate the IOL 25. For example, the folding mechanism may include an edge roller 62 pivotally attached to the inner portion 60. Pins (not shown) may extend laterally from the edge roller 62 into the inner portion 60 of the housing 40, thereby rotating the edge roller 62 (e.g., in a vertical direction). The inner portion 60 may also include compression arms 64, which are also pivotally arranged within the inner portion 60 via pins 66, which vertically pass through the compression arms 64 into the inner portion 60 to allow the compression arms 64 to rotate (e.g., in a lateral direction). The compression arms 64 may include protrusions 68 that may extend upward to contact an inner ramp (not shown) of the slider 42. The IOL 25 may be disposed at the center of the inner portion 60, within the compartment 41, and between the compression arms 64 and the edge rollers 62. The IOL 25 may be preloaded within the grooves 65 of the edge rollers 62 and held in place. The edge rollers 62 rotate inwardly and downwardly (e.g., vertically) to fold the IOL 25 when actuated (i.e., the cover 16 is removed). At the same time, when actuated, the compression arms 64 rotate laterally inwardly to compress the haptic extensions 31. Once compressed and folded, the IOL 25 is ready to be delivered through the mouth 36.
[0034] Figure 6 According to a specific embodiment of the present disclosure Figure 5 1. A top view of an IOL box 12. A pin 63 extends from the edge roller 62 into the inner portion 60, thereby securing the cover 16 (e.g., Figure 1 16 is removed to enable the edge rollers 62 to rotate. Removing the cover 16 allows the edge rollers 62 and the compression arms 64 to rotate, thereby folding and compressing the IOL 25 for delivery through the mouth 36.
[0035] Figure 7 A cutaway top view of an IOL box 12 is shown with a cover 16 disposed on the IOL box 12 according to a particular embodiment of the present disclosure. The cover 16 provides a protective covering to the IOL box 12 and secures the IOL 25 (e.g., Figure 5The slide 42 may include inner ramps 70 that align with the projections 68 of the compression arms 64. The inner ramps 70 may extend inwardly from the side portions 72 of the slide 42. The inner ramps 70 may be configured to guide the projections 68 along the inner ramps 70 as the cover 16 is removed. The compression arms 64 may rotate inwardly (e.g., rotate sideways) as the cover 16 is removed and the projections 68 move along the inner ramps 70, thereby compressing the loop extensions 31 (e.g., Figure 5 as shown).
[0036] Figure 8 A cutaway side elevation view of an IOL cartridge 12 is shown with the slider 42 in an initial, unadvanced position according to a particular embodiment of the present disclosure. The cover 16 is not shown to illustrate the initial, unadvanced position of the slider 42 prior to actuation of the edge rollers 62 and compression arms 64, as shown in FIG. Figure 5 and Figure 6 Although not shown, the opposite side of the IOL box 12 is configured similarly to the side depicted. The slider 42 may include an inner ramp 74 that aligns with a lower portion 76 of the edge roller 62. The inner ramp 74 may extend upwardly and may be configured to guide and receive the lower portion 76 of the edge roller 62 when the cover 16 is removed. As the lower portion 76 is contacted by the inner ramp 74, the edge roller 62 may be rotated vertically to fold the IOL 25 (e.g., Figure 5 As the cover 16 is removed, the inner ramp 74 of the slider 42 can be advanced to contact the lower portion 76 of the edge roller 62, which can cause the lower portion 76 of the edge roller 62 to be lifted or raised. Lifting or raising the lower portion 76 can cause the edge roller 62 to rotate, thereby causing other portions of the edge roller 62 to contact the IOL 25, such as the groove 65, to push the IOL 25 downward, thereby folding the IOL 25.
[0037] Fig. 9 A cutaway top view of an IOL cartridge 12 is shown with the cover 16 removed and the slider 42 in the advanced position according to a particular embodiment of the present disclosure. Figure 1 ) is removed, the inner bevel 70 of the slider 42 has been advanced, and the protrusion 68 of the compression arm 64 has moved along the inner bevel 70 of the slider 42 to the flat portion 78. The flat portion 78 is adjacent to the apex 80 of the inner bevel 70. In the illustrated configuration, the compression arm 64 is rotated laterally inward, and the loop extension 31 ( Figure 5 and Figure 6 shown) are compressed.
[0038] Fig.10A cut-away side elevation view of an IOL cartridge 12 according to a particular embodiment of the present disclosure is shown with the cover 16 removed and the slider 42 in an advanced position. As previously described, the opposite side of the slider 42 can be configured similarly to the side shown. In the illustrated embodiment, the slider 42 is in the advanced position and the IOL 25 is in a folded state (not shown). The inner ramp 74 has been advanced and the lower portion 76 of the edge roller 62 has been vertically rotated and positioned on the flat portion 82, which is adjacent to the apex 84 of the inner ramp 74. As the inner ramp 74 of the slider 42 is advanced, the lower portion 76 of the edge roller 62 is vertically rotated and positioned on the flat portion 82, and the IOL 25 is in the folded state.
[0039] Fig.11 A cutaway top view of an IOL cartridge 12 is shown with edge rollers 62 and compression arms 64 in their rotated positions according to certain embodiments of the present disclosure. The cover 16 has been removed to advance the slide 42 (e.g., Fig.10 ), thereby actuating edge roller 62 and compression arm 64. In the illustrated embodiment, slider 42 is not shown to allow viewing of the position of edge roller 62 and compression arm 64 after their rotation (actuation). Edge roller 62 rotates vertically to fold IOL 25, and compression arm 64 rotates laterally to compress haptic extension 31 of IOL 25 ( Figure 5 and Figure 6 as shown).
[0040] Fig.12 A side cross-sectional view of an insertion tool 10 according to a specific embodiment of the present disclosure is shown. In the illustrated embodiment, the plunger barrel 28 can be disposed in the housing 92 of the handpiece 26 and fixed therein via the protrusion 30. The plunger barrel 28 is in fluid communication with the compartment 41. The inner portion 93 of the slider 42 is exposed to the compartment 41. As previously described, the plunger 29 can be movably disposed in the plunger barrel 28. After the cover 16 is removed to advance the slider 42 forward, the push rod 34 of the handpiece 26 can be actuated to advance the plunger 29 forward, thereby delivering the IOL 25 in a folded and compressed state from the compartment 41 through the passage 94 and the mouth 36 to the patient's eye. The passage 94 can extend from the compartment 41 through the mouth 36. After delivering the IOL 25, the mouth 36 can be removed from the patient's eye. Then, the handpiece 26 can be rotated to release the protrusion 30 of the IOL box 12 from the slot 32 of the handpiece 26. Next, the hand piece 26 may be pulled from the IOL cartridge 12 to disengage the IOL cartridge 12 from the hand piece 26. The spent IOL cartridge 12 may then be disposed of.
[0041] refer to Figures 1 to 12, an exemplary technique for assembling the IOL cartridge 12 to the handpiece 26 to form the insertion tool 10 according to certain embodiments of the present disclosure is described as follows.
[0042] First, the plunger barrel 28 of the IOL cartridge 12 (with the cap 16 disposed thereon) may be inserted into the handpiece 26 to secure the IOL cartridge 12 to the handpiece 26, thereby forming the insertion tool 10, as shown in FIG. Figure 1 As shown. Then, the cover 16 can be removed to expose the mouth 36. With the cover 16 removed, the IOL 25 is manipulated to be in a folded and compressed configuration, as shown. Fig.11 As previously described, in additional or alternative embodiments, in addition to or in lieu of folding the IOL 25, the cover 16, slide 42, and / or housing 40 of the IOL cartridge 12 may be configured such that removal of the cover 16 causes one or more portions of the IOL 25, such as the haptic extension 31, to be straightened or unfolded. Upon actuation of the push rod 34, the plunger 29 may be advanced forward to deliver the IOL 25 from the compartment 41 through the mouth 36. To disassemble the insertion tool 10 after delivery of the IOL 25, the IOL cartridge 12 may be rotated and pulled out of the hand piece 26, thereby causing the plunger barrel 28 to slide out of the hand piece 26 to form a separate IOL cartridge 12 and a separate hand piece 26.
[0043] Fig.13 and Fig.14 An exemplary technique for implanting an IOL 25 into a patient's eye 96 is presented in accordance with certain embodiments of the present disclosure. Fig.13 An incision 100 is shown that a surgeon may create in an eye 96. For example, the incision 100 may be created through the sclera of the eye 96. The incision 100 may be of a suitable width or length. Without limitation, a suitable width and / or length may be less than 3 mm, and in some cases may be less than 2 mm. After creating the incision 100, the mouth 36 of the insertion tool 10 may be inserted through the incision 100 into the interior of the eye 96. The insertion tool 10 may be actuated to dispense the IOL 25 into the capsular bag 108 of the eye 96, as shown in FIG. Fig.14 shown.
[0044] The IOL 25 can be delivered in a folded (or rolled) configuration and allowed to unfold after being ejected from the insertion tool 10. After dispensing, the IOL 25 should unfold and seat within the capsular bag 108 of the eye 96, as shown in FIG. Fig.14 For example, the haptic extension 31 can be manipulated to engage the equatorial portion of the capsular bag 108. The haptic extension 31 can engage the capsular bag 108 to secure the IOL 25 in the capsular bag 108.
[0045] Compared with other IOL delivery systems, using the method and system described herein can provide many benefits and advantages. For example, the folding or other manipulations of IOL can be simplified. The integrated function of folding or other manipulations of IOL 25 by removing the cover 16 of IOL box 12 can ensure that IOL insertion tool 10 is correctly used and can prevent potential user errors. For example, in order to expose the mouth 36 of IOL box, first the cover 16 must be removed, so that IOL 25 is folded and placed in the appropriate configuration so as to deliver. Therefore, this integrated function of removable cover 16 can ensure that the appropriate sequence of the step for preparing, configuring and delivering IOL is followed and therefore can prevent IOL 25 from being delivered or ejected prematurely before the appropriate folding or other configurations.
[0046] In addition, the interchangeable use between different drive mechanisms and preloaded IOL boxes provides a simplified and unified process for pairing drive mechanisms with preloaded IOL boxes. Therefore, various handpieces using different types of drive mechanisms can be used with each of a plurality of different types of IOL boxes, and therefore can be used with different types of IOLs. For example, depending on the type of IOL and / or the type of IOL box, a user can easily choose between the multiple types of drive mechanisms he or she wishes to use. In addition, while one user may prefer to use a first type of drive mechanism handpiece for a given IOL box, a different user may choose to use a second type of drive mechanism handpiece for the same given type of IOL box. Importantly, by allowing the handpiece to be fixed to the IOL box with a universal or standard interface, the user experience of fixing the handpiece to the IOL box can be substantially the same, regardless of the type of drive mechanism used or the IOL box and therefore the type of IOL, which can also improve the operator's ease of use and simplify the IOL delivery procedure.
[0047] It is believed that the operation and construction of the present disclosure will be apparent from the foregoing description. While the apparatus and methods shown or described above have been characterized as being preferred, various changes and modifications may be made therein without departing from the spirit and scope of the present disclosure as defined in the appended claims.
