Abstract
Description
Technical Field
[0001] The present invention relates to medical devices, and more particularly to a holder for medical devices. Background Art
[0002] Medical devices for insertion into a patient must be kept sterile and undamaged prior to insertion. These medical devices, such as guidewires and catheters, are typically kept on the patient or in a sterile area prior to use or during intermittent use. In addition, wet sterile towels are often used to hold these devices and prevent them from falling or slipping off the patient. Such covers prevent users (e.g., medical professionals) from seeing them and knowing which devices are kept in which location. In addition, during insertion and use, the user usually needs another person to hold the unused portion of the device to prevent it from falling to the floor.
[0003] When kept in the operating room with other sterile instruments, wires and instruments are sometimes placed in buckets to keep them in a circular structure and prevent them from falling on the floor. During surgery, multiple sizes and types of guidewires are sometimes used, and these guidewires can be kept together, so it can be difficult to distinguish which guidewire is which when preparing the instruments for surgery. In addition, wires are often coiled and then twisted inside the coil to prevent them from bouncing off and causing part or all of the wire to fall on the non-sterile floor, which requires replacing the wire and increases the cost of the surgery.
[0004] The present application addresses these and other challenges associated with retaining or preserving a medical device to be inserted into a patient. Summary of the invention

[0066] The present application describes a retainer for a medical cylinder. The present application also describes a method for operating a retainer for a medical cylinder and a kit including a retainer for a medical cylinder (such as a guidewire or a catheter). The retainer for a medical cylinder of the present application may include a first outer member and a second outer member, which may be made of, for example, plastic or metal, and each has a connecting member firmly attached to each other. Each of the outer members includes an inner surface and an outer surface. In one or more embodiments, the first outer member and the second outer member may be, for example, circular, square, elliptical or polygonal.
[0067] The first outer member and the second outer member can each include a perimeter, and at least a portion of the first outer member is located in a first plane, and at least a portion of the second outer member is located in a second plane. In one or more embodiments, the outer member is a structure having a surface area and a thickness, wherein the thickness is less than the surface area. In one or more embodiments, the outer member can vary in height, width, diameter, and / or thickness along the length of the outer member (thus being non-uniform). The two outer members can have matching sizes, or in some embodiments, can have varying sizes.
[0068] The core structure can extend between the first outer member and the second outer member at a position inside the periphery of the first outer member and the second outer member. In one or more embodiments, the core can be integrated into the first and / or second outer member. In at least one embodiment, the core can be coupled to the first outer member and the second outer member. In at least one embodiment, the core can include a first connecting member coupled to the first outer member and a second connecting member coupled to the second outer member, and the corresponding connecting members can be configured to be firmly coupled to each other. The first outer member and the second outer member can be oriented so that their respective inner surfaces are relative to each other around the core, wherein the first plane and the second plane are oriented substantially parallel and spaced apart.
[0069] In one or more embodiments, the first outer member may include a surface configured to be written to include information thereon, such as information related to the medical procedure and / or medical device involved, the cylinder held by the retainer, or the patient. In at least one alternative embodiment, the first outer member may include a label for marking information thereon. The second outer member may include an adhesive on its outer surface that is configured to be removably attached to the surgical drape of the patient. The retainer may also include at least one retaining member, which may be made of a softer material (e.g., a compressible material) than the outer member, such as a plastic foam, an open-cell or closed-cell plastic material, or other soft polymers, wherein at least one retaining member is placed between the inner surfaces of the first outer member and the second outer member. For example, the retaining member may be disposed on at least a portion of one of the inner surfaces of the first outer member and the second outer member, or on a portion of both of them, or on all inner surfaces.
[0070] In one or more embodiments, the attachment of the first outer member and the second outer member to each other defines a space for holding the elongated medical body between the two retaining members. In at least one embodiment, when the first outer member and the second outer member are attached, the two retaining members contact each other. In one or more embodiments, the outer members are all tapered from the edge of the outer surface to a point on the inner surface (e.g., the edge of the inner surface) to define an annular funnel, which allows the retainer to more easily receive and capture the elongated medical body therein. Then, at least a portion of the medical cylinder can be fed to the retaining member via the funnel, for example, continuously capturing a continuous ring of the medical cylinder retained therein, and then the medical cylinder is retained between the retaining members via friction (or retained between the inner surface of the retaining member and one of the outer members via friction). The retaining member is compressible so that when the medical cylinder is pushed into the instrument, the medical cylinder slides between the retaining members and is retained by the retaining member pressing against the more rigid outer member. During use of the cylinder during a surgical procedure, as a portion of the medical cylinder is pulled out of the holder, the pulling of the medical cylinder can cause a portion of the medical cylinder to be cleaned by both retaining members as it is released. In one or more embodiments, the retaining members can be wetted with a sterile or antiseptic fluid so that when the portion of the medical cylinder is pulled out of the holder, the medical cylinder is wetted with the fluid (e.g., for lubricity prior to insertion into a patient) or cleaned.
[0071] In at least one embodiment, the retainer can include an outer member without two retaining members, and in such embodiments, the taper of the outer member captures the medical cylinder such that the medical cylinder is retained between the outer members.
[0072] The retainer of the present application allows a medical professional (e.g., a surgeon) to perform a medical procedure requiring the insertion of a medical cylinder into a patient without the need for another assistant to hold the outer length of the medical cylinder as it is inserted, thereby preventing accidental dropping or contamination of the medical cylinder.
[0073] As further shown and described herein, the apparatus and methods of the present invention provide an effective solution for holding elongated medical bodies (e.g., medical cylinders) of different sizes and diameters (e.g., guidewires, catheters, and thin tools that can be inserted into a patient's body for minimally invasive medical surgery). In addition, in one or more embodiments, the holder of the present application can hold multiple medical cylinders at a time.
[0074] In addition, although the elongated medical body held by the holder of the present application is generally described herein as being generally "cylindrical" in shape, the medical body may also be of other shapes, including elongated shapes with non-uniform cross-sections and / or sizes. As used herein, the terms "medical cylinder", "cylinder", and "elongated medical body" are used interchangeably, but it should be noted that "medical cylinder" or "cylinder" is a preferred embodiment of an elongated medical body.
[0075] The retainer of the present invention and the method for operating the retainer for medical cylinders are now described more fully with reference to the accompanying drawings, in which one or more illustrated embodiments and / or arrangements of the method are shown. The device and method are not limited in any way to the illustrated embodiments and / or arrangements, because the illustrated embodiments and / or arrangements described below are merely examples of the method, and as understood by those skilled in the art, the method can be implemented in various forms. Therefore, it should be understood that any structural and functional details disclosed herein should not be interpreted as limiting the device and method, but are provided as representative embodiments and / or arrangements to teach those skilled in the art one or more ways to implement the device and method. In addition, the terms and phrases used herein are not restrictive, but provide an understandable description of the device and method. In addition, as used in this application, when used in conjunction with a numerical value, the term "approximately" refers to any number within about 5%, 3% or 1% of the quoted numerical value, including the quoted numerical value. As used in this application, the terms "approximately" or "substantially" or "approximately" with respect to the orientation of two objects (e.g., approximately parallel to each other, approximately in parallel, or substantially parallel to each other) refer to any value within about 0-20 degrees of the specified term (e.g., within about 0-20 degrees of parallel).
[0076] Figures 1A-1H Various views of a holder 100 for a medical cylinder are shown according to one or more embodiments. Referring now to Figures 1A-1D, the retainer 100 includes a first outer member 105 and a second outer member 110. The first outer member 105 and the second outer member 110 may be made of a plastic material, a metal, or a similar durable material. The first outer member 105 includes an inner surface 115 having a first connecting member 120. Figures 1A-1D As shown, in one or more embodiments, the first connecting member 120 can be two pins or pin-like attachments. The second outer member 110 also includes an inner surface 125, and the inner surface 125 includes a second connecting member 130. In one or more embodiments, as shown in FIG. Figure 1A As shown, the second connecting member 130 may be two openings, and the size and shape of the two pins (first connecting member 120) are designed to fit securely into and attach to the corresponding openings. Figures 1A-1H In some embodiments, when the pin 120 is inserted through the corresponding opening 130, the protruding edge of the pin can latch onto the edge of the opening 130 to provide a secure attachment between the pin and the opening. In other embodiments, the pin 120 can be disposed on one outer member with the corresponding opening on another outer member. It should be understood that in at least one embodiment, the first connection member 120 and the second connection member 130 can be attached to each other via other mechanisms, such as screws, adhesives, or press-fit attachments.
[0077] Continue to refer Figures 1A-1H , the first outer member 105 also includes an outer surface 135, which optionally includes a recess 140. In one or more embodiments, the outer surface 135 or the recess 140 (if present) may be configured to receive an adhesive member 142. In one or more embodiments, the adhesive member 142 is a double-sided adhesive member, where one side is attached to the outer surface 135 or the recess 140 and the other side is attached to the surface of the object. The adhesive member 142 allows the retainer to be selectively removable and reattachable to the object. In one or more embodiments, the object is a medical drape, and the adhesive member 142 is specifically configured to be removable and reattachable to the medical drape.
[0078] In many medical procedures in which one or more medical cylinders (e.g., guidewires, catheters) are inserted into a patient's body, such as cardiac or any vascular catheterization procedures, a portion of the patient's body is covered with a medical (surgical) drape. Reusable surgical drapes are typically made of a woven material that can be cleaned and sterilized before reuse. Disposable (single-use) drapes are typically made of a non-woven material (e.g., a paper base with a liquid-proof film). Therefore, in one or more embodiments, the adhesive member 142 on the outer surface 135 or recess 140 is specifically designed to be selectively removable and reattachable to the material of the reusable surgical drape and / or disposable surgical drape. Although the adhesive member 142 can be selectively removed and reattached to the drape, the attachment strength of the adhesive member 142 has sufficient strength to remain attached to the drape and holder 100 during normal storage, pushing and pulling of the medical cylinder (e.g., guidewire) in the holder 100 during medical surgery. The adhesive member 142 allows the retainer to be attached to a surgical drape in any direction or orientation. The outer surface 135 or recess 140 is flat or substantially flat, so that attaching the outer surface 135 or recess 140 to the medical drape via the adhesive member 142 maintains the retainer 100 as tightly attached as possible and unobtrusive relative to the medical drape, so that the retainer 100 is less likely to accidentally shift or move during a medical procedure. Although as described herein, the retainer 100 is generally described as being adhered to a medical drape for a patient, it should be understood that the retainer can also be configured to attach to a medical drape or clothing of a medical professional or technician, or other objects, such as a table or surgical tray. In addition, in one or more embodiments, the retainer can be selectively and removably attached to one or more different surfaces (e.g., a drape, table, etc.) via the adhesive member during use of the retainer (e.g., during a medical procedure).
[0079] In one or more embodiments, the adhesive member 142 is any adhesive that can be sterilized and can adhere to the medical drape firmly enough so that it can be removed and reapplied to the drape multiple times. For example, the adhesive member 142 can be a sterilizable double-sided tackified acrylic adhesive on a clear polyethylene carrier. In at least one embodiment, the adhesive member 142 is a double-sided coated medical polyethylene film tape, such as 3M product number 1509. One side of the adhesive member 142 is firmly adhered to the outer surface 135 or recess 140 of the retainer 100, and the other side of the adhesive member 142 is removably adhered to the medical drape.
[0080] Similar to the first outer member 105, in one or more embodiments, the second outer member 110 includes an outer surface 145, which may optionally include a recess 150 (see Figure 1E). In one or more embodiments, the outer surface 145 or recess 150 can be configured to write thereon. Specifically, the outer surface 145 or recess 150 can include a material that allows writing via, for example, a marker, pen, or pencil. A physician or medical professional can write on the recess to indicate, for example, various aspects of the medical cylinder held by the retainer 100 or the patient undergoing surgery, such as the size of the medical cylinder (e.g., a 0.035 inch guidewire), the patient's name, or the surgery to be performed on the patient, or which doctor is responsible for the item held in the retainer. Therefore, the outer surface 145 or recess 150 can be a flat or substantially flat surface to allow clear writing.
[0081] In at least one alternative embodiment, instead of outer surface 145 or recess 150 being configured to be written on, outer surface 145 or recess 150 may be configured to receive a label of a material that permits writing by, for example, a marker, pen, or pencil.
[0082] Although, as described above, the adhesive member is described as being configured for attachment to outer surface 135, and outer surface 145 is described as being configured for writing thereon or receiving a label for writing thereon (i.e., a "writable surface"), it should be understood that in some embodiments, the adhesive may be configured for attachment to outer surface 145, and the writable surface may be outer surface 135.
[0083] Thus, as described throughout the specification, in certain embodiments, the adhesive member is configured to attach to an outer surface of the first outer member or an outer surface of the second outer member, and the writable surface is an outer surface of the first outer member or the second outer member (or a corresponding label).
[0084] refer to Figure 1GIn one or more embodiments, the retainer 100 further includes two retaining members 155 placed between the inner surface 115 of the first outer member 105 and the inner surface 125 of the second outer member 130. Each retaining member 155 includes a through hole for accommodating the first connecting member 120 and / or the second connecting member 130 when the first outer member and the second outer member are attached. In one or more embodiments, the retaining member 155 can be adhered to the corresponding inner surface of the outer member (e.g., via an adhesive) and provide friction to the medical cylinder. In at least one embodiment, when the connecting members 120 and 130 are attached, the retaining member 155 can rotate freely within the retainer. When the first outer member and the second outer member are attached via their respective connecting members 120 and 130, a space for retaining a medical cylinder can be defined between the two retaining members 155. In at least one embodiment, when the medical cylinder is not in the holder 100, there is no space between the retaining members 155, but because the retaining members are compressible, once the medical cylinder is fed into the holder 100, the retaining members 155 form a space for the medical cylinder between them. The retaining members 155 are configured to hold the medical cylinder between them via tension when the first outer member and the second outer member are attached. The retaining members 155 can be made of foam (e.g., plastic foam, open or closed cell plastic material or other soft polymer), silicone, rubber or similar deformable or flexible material, which can firmly hold the medical cylinder without damaging the medical cylinder.
[0085] In one or more embodiments, the retaining member 155 can be moistened with an antiseptic or antimicrobial liquid so that when a medical cylinder (e.g., a guide wire) is pulled through the retainer 100, the medical cylinder slides along the surface of the retaining member 155, thereby sterilizing or further cleaning the medical cylinder before insertion into the body. In one or more embodiments, the retaining member 155 can also be moistened by the user with sterile saline or water to clean or moisten the medical cylinder.
[0086] The size and shape of the first and second outer members 105, 110 and the retaining member 155 can be designed to fit a specific type of medical cylinder, particularly medical cylinders of different sizes. For example, in the case of a guidewire, the size and shape of the first and second outer members 105, 110 and the retaining member 155 can be designed to securely hold, for example, a 035 guidewire (a guidewire having a diameter of 0.035 inches) or, alternatively, a 014 guidewire (a guidewire having a diameter of 0.014 inches).
[0087] like Figure 1FAs shown, the first outer member 105 and the second outer member 110 are both tapered from the edge of the respective outer surface to the edge of the respective inner surface, thereby defining an annular funnel 160 configured to capture a medical cylinder. More specifically, each of the first outer member 105 and the second outer member 110 has a respective outer periphery 152 and 153, and a respective center 154 and 156 (see Figure 1A and 1B ) and corresponding surfaces 157 and 158, which taper from the outer periphery 152, 153 to a point between the outer periphery 152, 153 and the center 154, 156, thereby defining an annular funnel 160 configured to capture a medical cylinder.
[0088] The funnel shape of the outer member also aids in the ability of the holder 100 to clean medical cylinders. Specifically, the retaining member of the holder 100 holds the cylinder 165 (e.g., a guide wire) in a static state, but when an operator applies force, such as by pulling the wire, the wire can pass through (see Figure 1H ). The funnel shape of the retainer 100 will naturally filter out large debris as the wire passes through, and small debris will be removed as the wire passes through the retaining member. The fluid can be absorbed into the retaining member, thereby completely cleaning the medical cylinder (e.g., a guide wire).
[0089] Figures 2A-2H Another embodiment of the holder for medical cylinder of the present application is disclosed, which has Figures 1A-1H Similar components of the embodiments illustrated in the drawings have similar properties unless otherwise specified. Figures 2A-2H , the retainer 200 includes a first outer member 205 and a second outer member 210, which can be made of plastic material, metal or similar durable materials. The first outer member 205 includes an inner surface 215 having a first connecting member 220. Figure 2B-2D As shown, in one or more embodiments, the first connecting member 220 can be two pins or pin-like attachments. The second outer member 210 also includes an inner surface 225, and the inner surface 225 includes a second connecting member 230. In one or more embodiments, as shown in FIG. Figure 2A As shown, the second connecting member 230 may be two openings, and the size and shape of the two pins (first connecting member 220) are designed to fit securely into and attach to the corresponding openings. Figures 2A-2HIn an embodiment, when the pin 220 is inserted through the corresponding opening 230, the protruding edge of the pin can latch onto the edge of the opening 230 to provide a secure attachment between the pin and the opening. In another embodiment, one outer member can have a pin and an opening, and the other member has a corresponding opening and a pin for securing the two outer members together. It should be understood that in at least one embodiment, the first connecting member 220 and the second connecting member 230 can be attached to each other via other mechanisms, such as screws, adhesives, or press-fit attachments.
[0090] Continue to refer Figures 2A-2H , the first outer member 205 also includes an outer surface 235, which optionally includes a recess 240. The outer surface 235 or the recess 240 can be configured to receive an adhesive member 242. In one or more embodiments, the adhesive member 242 is a double-sided adhesive member, where one side is attached to the outer surface 235 or the recess 240 and the other side is attached to the surface of an object (e.g., a medical drape). In one or more embodiments, the adhesive member 242 is selectively removable and reattachable to the medical drape. The outer surface 235 or the recess 240 is flat or substantially flat, so that attaching the outer surface 235 or the recess 240 to the medical drape via the adhesive member 242 maintains the retainer 200 as tightly attached as possible and unobtrusive relative to the medical drape.
[0091] The second outer member 210 includes an outer surface 245, which optionally includes a recess 250, and the outer surface 245 or recess 250 can be configured to write on it. Specifically, the outer surface 245 or recess 250 can include a material that allows writing via, for example, a marker, a pen, or a pencil, so that a medical professional can write on the outer surface or recess to indicate various aspects related to the medical cylinder, surgery, or patient. The outer surface 245 or recess 250 can be a flat or substantially flat surface to allow easy-to-read writing. In at least one alternative embodiment, the outer surface 245 or recess 250 can be configured to receive a label with a material that allows writing, rather than the outer surface 245 or recess 250 being configured to write on it.
[0092] refer to Figures 2E to 2G In one or more embodiments, the retainer 200 further includes two retaining members 255 placed between the inner surface 215 of the first outer member 205 and the inner surface 225 of the second outer member 230. Each of the retaining members 255 includes a through hole for accommodating the first connecting member 220 and / or the second connecting member 230 when the first and second outer members are attached.
[0093] When the first outer member and the second outer member are attached via their respective connecting members 220 and 230, a space for holding the medical cylinder can be defined between the two holding members 255, or can be defined between the holding members 255 when the medical cylinder is inserted. The holding member 255 is configured to hold the medical cylinder between them via tension when the first outer member and the second outer member are attached. The holding member 255 can be made of foam (e.g., plastic foam or open-cell or closed-cell plastic material), silicone, rubber, or similar deformable or flexible material that can firmly hold the medical cylinder without damaging the medical cylinder.
[0094] In one or more preferred embodiments, the retaining members 255 can be made of silicone, and in one or more embodiments, the retaining members 255 can include a plurality of protrusions 260 on the inner wall of each retaining member. The protrusions 260 can have varying sizes on the inner wall of each retaining member, and the protrusions are configured to capture portions of the medical cylinder (see Figure 2H ). For example, in the case where the medical cylinder 265 is a guide wire, the wire can be captured between the two retaining members 255 by the protrusions 260. In one or more embodiments, the size and density of the protrusions 260 increase as one moves from the outer edge of the inner wall of each retaining member 225 to the center of each retaining member 255. The variable protrusion size allows the cylinder (e.g., a guide wire) to be more or less firmly held depending on the diameter of the cylinder and the depth to which the operator presses the cylinder into the retainer 200. Although in Figure 2F-2H In the exemplary embodiment of FIG. 2 , protrusion 260 is shown as a circle, but in one or more embodiments, protrusion 260 may have any geometric shape, such as square, triangle, oval, or any mixture thereof.
[0095] In one or more embodiments, retaining member 255 can be adhered to the corresponding inner surface of the outer member (eg, via an adhesive). In at least one embodiment, retaining member 255 can rotate freely within the holder when connecting members 220 and 230 are attached.
[0096] In one or more embodiments, the retaining member 255 can be moistened with an antiseptic or antibacterial liquid so that when a medical cylinder (e.g., a guide wire) is pulled through the retainer 200, the medical cylinder slides along the surface of the retaining member 255, thereby sterilizing or further cleaning the medical cylinder before inserting it into the body. In one or more embodiments, the retaining member 255 can also be moistened by the user with sterile saline or water to clean or moisten the medical cylinder.
[0097] The sizes and shapes of the first and second outer members 205, 210, retaining member 255, and protrusion 260 can be designed to fit a specific type of medical cylinder, particularly medical cylinders of different sizes. For example, in the case of a guidewire, the sizes and shapes of the first and second outer members 205, 210, retaining member 255, and protrusion 260 can be respectively set to securely hold a unique, different, or varying range of diameters of guidewires.
[0098] Figures 3A-3J Another embodiment of the holder for medical cylinder of the present application is disclosed, which has Figures 1A-1H and Figures 2A-2H Reference is now made to the embodiment illustrated in FIG. Figures 3A-3J , the holder 300 includes a first outer member 305 and a second outer member 310, which can be made of a plastic material or metal or a similar durable material. In one or more embodiments, each of the first outer member 305 and the second outer member 310 can include a plurality of extensions 312 on their respective edges. The extensions 312 allow the holder 300 to balance or stand vertically on a flat surface, such as a surgical table or a surface including a medical drape (see Figure 3J , right). These extensions 312 also serve to allow for easier rotation or locking of the instrument, as discussed in further detail below. In one or more embodiments, each of the outer members 305 and 310 may include 2-4 extensions 312. In an exemplary embodiment, as Figures 3A-3J As shown, each of the outer members 305 and 310 can include four extensions 312. In at least one embodiment, the edges of the extensions 312 and the adjacent edges of the outer members 305 and 310 can include an adhesive for attachment to a medical drape so that the retainer 300 can be stabilized in an upright position on the medical drape.
[0099] The first outer member 305 includes an inner surface 315 having a first connecting member 320. Figure 3B As shown, in one or more embodiments, the first connecting member 320 is a protruding member ("key").
[0100] Similarly, the second outer member 310 also includes an inner surface 325, and the inner surface 325 includes a second connecting member 330. In one or more embodiments, as shown in FIG. Figure 3B As shown, the second connecting member 330 includes a notch ("lock") configured to securely receive a protruding member or key. When the protruding member 320 is inserted into the notch 330 and then the first or second outer member is rotated in a first direction (e.g., clockwise), the first outer member 305 and the second outer member 310 can be in a locked position (closed position - Figure 3F) are securely attached to each other. Similarly, after locking, in one or more embodiments, the first outer member 305 and the second outer member 310 can be unlocked by rotating the first or second outer member in a second direction (e.g., counterclockwise) (unlocked position - Figure 3G ). In one or more embodiments, locking (or unlocking) of the attachment between the first outer member 305 and the second outer member 310 is achieved by rotating the first outer member or the second outer member by 1 / 8 of a turn in a specified direction.
[0101] In one or more embodiments, the protruding member or key may include a compressible member 332 that is configured to assist in releasing the first and second outer members from the locked position when the first or second outer member is rotated in the second direction. The compressible member 332 is located at the center of the protruding member (first connecting member 320) to allow for auxiliary release from the locked state to the unlocked state when the retainer is rotated in the second direction (e.g., counterclockwise). In one or more embodiments, the compressible member is a spring, such as Figure 3G and Fig. 3I shown.
[0102] Continue to refer Figures 3A-3I , the first outer member 305 also includes an outer surface 335, which optionally includes a recess 340. The outer surface 335 or the recess 340 can be configured to receive an adhesive member 342. In one or more embodiments, the adhesive member 342 is a double-sided adhesive member, where one side is attached to the outer surface 335 or the recess 340 and the other side is attached to the surface of an object (e.g., a medical drape). In one or more embodiments, the adhesive member 342 is selectively removable and reattachable to a medical drape (e.g., a reusable medical drape or a disposable drape). The outer surface 335 or the recess 340 is flat or substantially flat, so that attaching the outer surface 335 or the recess 340 to the medical drape via the adhesive member 342 maintains the retainer 300 as tightly attached as possible and unobtrusive relative to the medical drape.
[0103] Similarly, the second outer member 310 includes an outer surface 345, which optionally includes a recess 350, wherein the outer surface 345 or recess 350 is configured to write thereon. Specifically, the outer surface 345 or recess 350 may include a material that allows writing via, for example, a marker, a pen, or a pencil. The outer surface 345 or recess 350 may be a flat or substantially flat surface to allow legible writing. In at least one alternative embodiment, instead of the outer surface 345 or recess 350 being configured to write thereon, the outer surface 345 or recess 350 may be configured to receive a label made of a material that allows writing.
[0104] Specific reference Fig. 3IIn one or more embodiments, the retainer 300 further includes two retaining members 355 placed between the inner surface 315 of the first outer member 305 and the inner surface 325 of the second outer member 330. Each retaining member 355 includes a through hole for accommodating the first connecting member 320 and / or the second connecting member 330 when the first outer member and the second outer member are attached.
[0105] When the first and second outer members are attached via their respective connecting members 320 and 330, the space for holding the medical cylinder can be defined between the two retaining members 355, or can be defined between the retaining members 255 when the medical cylinder is inserted. The retaining member 355 is configured to retain the medical cylinder between them via tension when the first and second outer members are attached. The retaining member 355 can be made of foam (e.g., plastic foam or open-cell or closed-cell plastic material), silicone, rubber or similar deformable or flexible material that can firmly hold the medical cylinder without damaging the medical cylinder. In one or more preferred embodiments, the retaining member 355 can be made of silicone.
[0106] In one or more embodiments, the retaining member 355 can be moistened with an antiseptic or antimicrobial liquid so that when a medical cylinder (e.g., a guide wire) is pulled through the retainer 300, the medical cylinder slides along the surface of the retaining member 355, thereby sterilizing or further cleaning the medical cylinder before inserting it into the body. In one or more embodiments, the retaining member 355 can also be moistened by the user with sterile saline or water to clean or moisten the medical cylinder.
[0107] In one or more embodiments, the size and shape of the first and second outer members 305, 310 and the retaining member 355 can be designed to fit a specific type of medical cylinder, particularly medical cylinders of different sizes. For example, in the case of a guidewire, the size and shape of the first and second outer members 305, 310 and the retaining member 355 can be designed to securely hold a unique, different or varying range of diameters of the guidewire.
[0108] like Figure 3F-3G As shown, the first outer member 305 and the second outer member 310 are both tapered from the edge of the respective outer surface to the edge of the respective inner surface, thereby defining an annular funnel 360 configured to capture a medical cylinder. More specifically, as Figure 3G As shown, each of the first outer member 305 and the second outer member 310 has a respective outer periphery 352 and 353, and a respective center 354 and 356 (see Figure 3B) and corresponding surfaces 357 and 358, which gradually taper from the outer periphery 352, 353 to a point between the outer periphery 352, 353 and the center 354, 356, thereby defining an annular funnel 360 configured to capture a medical cylinder.
[0109] When the first outer member 305 and the second outer member 310 are in the unlocked position (open position- Figure 3G ) are attached to each other, the medical cylinder can be fed into the space between the retaining members 355 via the annular funnel 360. When the first outer member 305 and the second outer member 310 are in the locked position (closed position - Figure 3F ) are attached to each other, retaining member 355 provides a more secure grip on the medical cylinder.
[0110] The funnel shape of the outer member also aids in the ability of the holder 300 to clean the medical cylinder. Figure 3F-3G As illustrated, the retaining member of the retainer 300 holds the cylinder 365 (e.g., a guide wire) in a static state, but when the operator applies force, such as by pulling the wire, the wire can pass through. As the wire passes through, the funnel shape of the retainer 300 will naturally filter out large debris, and as the wire passes through the retaining member, small debris will be removed.
[0111] Thus, the retainer of the present application can secure and release a medical cylinder (e.g., a guidewire, a catheter) without the user having to interact directly with the retainer. In other words, when the retainer is attached to a surgical drape, the user can push the cylinder (e.g., a wire) into the retainer (e.g., via a funnel), and the retainer will hold the wire without the user having to actually open the outer member or even touch the retainer. In this way, the retainer of the present application can hold and passively feed the cylinder during insertion into the patient by a doctor.
[0112] In at least one alternative embodiment, the retainer can be integrated into the surgical drape, thereby eliminating the need for adhesive. In one or more embodiments, the retainer can include an additional flat surface attached to the edge of the first outer member. The flat surface can include an adhesive on its bottom surface for attachment to the surgical drape so that the retainer stands upright when the flat surface is attached to the drape, or for attachment to a table or area where the medical cylinder is kept away from the patient when not in use.
[0113] exist Figures 4A-4C An exemplary embodiment of the attachment of a retainer to a medical drape is shown in FIG. Figure 4A A typical attachment of an adhesive member to an outer surface or recess of a retainer 100 / 200 / 300 as described herein is shown, which allows the retainer to adhere and lie flat on a medical drape. Figure 4B-4CAs shown, the retainer 400 may include an additional flat surface 405 attached to the edge of the first outer member by a hinge or other hinged connection. It should be understood that the retainer 400 may include substantially the same features as the retainers 100, 200 or 300 described herein. The flat surface 405 of the retainer 400 may include an adhesive 410 so that when the flat surface 405 is pulled down to a horizontal position (e.g., extending perpendicular to the outer surface of the retainer), the adhesive 410 adheres to the cover cloth 415. Alternatively, the flat surface 405 can be attached to a non-cover cloth support structure, such as a table, a patient gurney, or a surgical tray, for use near the patient during surgery. In one embodiment, the flat surface 405 can be hinged so that it maintains a selected angle between 0 degrees and 270 degrees relative to the outer surface of the retainer so as to maintain the retainer in an appropriate orientation relative to the surface to which the flat surface 405 is adhered.
[0114] In one or more embodiments, adhesive 410 may include a removable liner 420 for covering adhesive 410. It should be understood that adhesive 410 may include a sterilizable double-sided tackified acrylic adhesive, as discussed above with respect to other embodiments. It should also be understood that in one or more embodiments of the retainers described herein, the adhesive or adhesive member may include a removable liner to prevent the adhesive from being inadvertently attached to a surface prior to use of the retainer.
[0115] In one or more embodiments, the adhesive can be replaced with one or more magnets (e.g., a pair of magnets). For example, one or more magnets can be embedded in the outer member of the retainer or adhered to the outer surface of the outer member. In such an embodiment, a magnetic base can be placed on or within the medical drape, and the one or more magnets of the retainer can be selectively attached to the magnetic base of the drape. Thus, in this embodiment, the attachment of the retainer to the drape is still selectively removable, but no adhesive is used.
[0116] A kit comprising one or more retainers of the present application is also disclosed herein. In addition to (one or more) retainers, in one or more embodiments, the kit may include one or more labels for attaching to the outer surface of one of the outer members, a writing utensil (e.g., a marker) for writing on the label, and a removable cover for an adhesive on (one or more) outer surfaces of one of the outer members. In one or more embodiments, the kit may also include an instrument pad for wiping the cylinder. In at least one embodiment, the instrument pad may be an antibacterial instrument pad. In one or more embodiments, the kit may also include a card that allows one or more retainers to be fixed in a package before use, or to be attached to the adhesive of the retainer to store the retainer after the retainer is removed from being attached to the sterile cover. The kit may also include one or more medical cylinders, such as guide wires or catheters. For example, the kit may also include cylinders of different diameters or lengths. The kit is preferably provided in a sufficiently sterile package so that it can be taken and opened, and then its contents are used in a sterile patient operating environment without compromising the sterility of the operating room.
[0117] Also disclosed herein is a method for operating a retainer for a medical cylinder. According to one or more embodiments of the method, a retainer of the present application is provided. Then, a portion of the medical cylinder is fed between two retaining members. Then, the medical cylinder is retained between the two retaining members of the retainer via friction. In one or more embodiments of the method, the medical cylinder is a guide wire or a catheter or a surgical tool or an electrode for stimulating tissue. In at least one embodiment in which the cylinder is a guide wire, the method may also include a step of pulling the guide wire so that the portion of the guide wire retained between the two retaining members is released, wherein pulling the guide wire causes the portion of the guide wire to be cleaned by the two retaining members when it is released. In at least one embodiment, the method may also include a step of applying a cleaning solution to the retaining member.
[0118] In one or more embodiments, the holder of the present application may further include a fixing mechanism for fixing the cylinder in the holder. The fixing mechanism operates independently or in combination with a holding member, and as mentioned above, the holding member holds the cylinder by friction. Figures 5A-5R An exemplary fixing mechanism according to one or more embodiments is shown. Figures 5A-5R As shown, in one or more embodiments, the safety mechanism described further below can be used with any of the retainers of the present application (eg, retainers 100 / 200 / 300).
[0119] Figures 5A-5CA top view (left) and a side view (right) of an embodiment of a retainer are shown, wherein a second outer member of the retainer (i.e., the outer member not attached to the adhesive) includes a toggle button on its outer surface, wherein the toggle button (501, 502, 503) is used to selectively lock and unlock the clamping feature that secures the wire. Specifically, pressing one side of the toggle button configures a portion of the first outer member (top outer member) to move toward a cylinder 565 (see FIG. 5 ) secured within the retainer. Figure 5C ) applies additional pressure, thereby providing further fixation (clamping) of the cylinder 565 between the two outer members. Similarly, pressing the other side of the toggle button is configured to release the pressure applied by the second outer member. Figures 5A-5C As shown, the toggle button can be provided in a variety of different shapes, with "lock" and "unlock" portions on opposite sides, such as a crescent shape ( Figure 5A , 501), ellipse ( Figure 5B , 502) or the entire toggle button ( Figure 5C , 503), wherein the "locking" portion of the button constitutes all or substantially all of one side of the outer surface of the first outer member, and the "unlocking" portion of the button constitutes all or substantially all of the other side of the outer surface of the first outer member. Pressing the locking portion of button 503 results in holding half of the instrument at a time, such as Figure 5C exemplified.
[0120] Other exemplary securing mechanisms are Figure 5D-5Q shown in . Specifically, Figure 5D-5E A securing mechanism with an "eject" button 504 integrated into the first outer member (top outer member) is shown in accordance with one or more embodiments. Figure 5E As shown, when button 504 is pressed by the user, button 504 remains in the downward position (locked position) and applies pressure to cylinder 565. Once in the downward position, if button 504 is pressed again, button 504 is released back to the upward position and the pressure applied to the cylinder is released.
[0121] Figure 5F-5G A fixing mechanism with a squeeze locking mechanism according to one or more embodiments is shown. Fig. 5F In the embodiment illustrated in FIG. 1 , the securing mechanism includes two squeeze buttons 505 on opposite ends of the first outer member of the holder (i.e., the top outer member that does not include the adhesive). In this embodiment, the holder remains in the locked position (i.e., pressure is applied to the cylinder held in the holder) until the user squeezes the two buttons 505 together toward the middle of the first outer member (i.e., using two fingers). Squeezing the buttons 505 together releases the pressure applied by the first outer member on the cylinder 565, as shown in FIG. Figure 5G shown.
[0122] Figures 5H-5I A securing mechanism is shown that includes a latch 506 (e.g., an articulated latch) for latching one end of the first outer member to one end of the second outer member, thereby pressing the two outer members tightly together to apply pressure to the cylinder 565. The latch 506 can be attached to the end of the second outer member (the bottom outer member) and can have an opening through which a portion of the end of the first outer member fits when the latch 506 is engaged.
[0123] Figures 5J-5K A securing mechanism is shown that includes a plurality of locking tabs 507 on the edge of the second outer member (bottom outer member). Instead of locking by pressing the first and second outer members together onto the cylinder as done in the previously discussed securing mechanisms, the locking tabs 507 prevent the cylinder from sliding out of the holder by folding upward (see FIG. Figures 5J-5K , right). In one or more embodiments, there are multiple tabs 507 to allow for different types of placement of the cylinder. Figures 5J-5K In the exemplary embodiment of the present invention, there are four tabs 507; however, in other embodiments, there may be a different number of tabs, such as 2 or 3 or 5 or 6. In one or more embodiments, it is not necessary to use all of the tabs 507 at once, only the tabs that close the cylinder in the holder are needed. For example, when the cylinder is held on one side of the holder, it may only be necessary to fold up 2 or 3 tabs to hold the cylinder in place and prevent it from sliding out of the holder.
[0124] Figure 5L-5M A securing mechanism is shown in which a first outer member (top outer member) can be rotated to lock (apply additional pressure to the cylinder) or unlock (release the additional pressure on the cylinder) the retainer. In this embodiment, the first outer member (top outer member) and the second outer member (bottom outer member) of the retainer are not uniformly shaped, in that one end of each outer member tapers toward the outer edge, while the other end of each outer member does not taper and remains substantially flat. The first outer member is configured to selectively rotate approximately 180 degrees about the central axis 508 between a locked position and an unlocked position, in which in the locked position the cylinder is retained between matching portions of the non-tapered outer members so that additional pressure is applied to the cylinder 565 (see Figure 5M When the first outer member is rotated to the unlocked position, the cylinder 565 is then retained between the tapered portion of the first outer member and the flat portion of the second outer member (see Figure 5L ).
[0125] Figure 5N-5PAnother rotational fixing mechanism is shown. In this embodiment, each of the first outer member and the second outer member includes gear-like protrusions 509 at regular (or irregular) intervals along their respective inner surfaces. When the first outer member (the top outer member) is rotated about the central axis 508 by a specified amount (e.g., 30 degrees), the protrusions 509 of the first outer member and the second outer member are aligned with each other (i.e., on top of each other) or not aligned with each other. When the first outer member is rotated so that the cylinder 565 remains in a position where the protrusions 509 of both the first outer member and the second outer member are aligned (see Figure 5P ), additional pressure is applied to the cylinder 565 to secure the cylinder in the holder (locked position). Conversely, when the first outer member is rotated so that the cylinder 565 remains in a position where none of the protrusions 509 of the second outer member (bottom outer member) are aligned with the protrusions 509 of the first outer member (see Fig.5O ), less pressure is applied to the cylinder 565. Compared with the embodiments of 5L-5M, Figure 5N-5P The illustrated embodiments allow for a shorter turning radius or rotation (eg, 30 degrees) for moving the retainer to the locked position, with the 5L-5M embodiments utilizing approximately 180 degrees of rotation for locking.
[0126] Figure 5Q-5R Shows Figure 5L-5M or Figure 5N-5P Specifically, by Figure 5Q and 5R The illustrated embodiments may be implemented with Figure 5L-5M (with tapered outer edges) or Figure 5N-5P (having aligned protrusions on the inner surface of the outer member) operates in the same manner, except that the first and second outer members 510 are generally rectangular, or more preferably square rather than circular. In this embodiment, the first outer member 510 (the top outer member) is rotated a specified amount (e.g., 90 degrees) about the central axis to apply additional pressure (lock) to secure the cylinder in the holder or release additional pressure (unlock). For example, as Figure 5Q-5R As shown, when unlocked, the first outer member (top outer member) is at an angle relative to the second outer member (bottom outer member) (see Figure 5R ), but when in the locked position, the square edges of the outer member are visually aligned (e.g., parallel) (see Figure 5Q ). Thus, this embodiment provides a more visible indication of when the instrument is locked or unlocked.
[0127] In one or more embodiments, the retainer of the present application may also include an auxiliary attachment mechanism, by which the retainer of the present application can be fixed to the surgical cover cloth or the user's clothing. The auxiliary attachment mechanism can be attached to (e.g., integrated into) the first outer member or the second outer member, wherein the auxiliary attachment mechanism is configured to attach to the cover cloth or clothing. The auxiliary attachment mechanism can operate independently or in combination with an additional method (such as an adhesive on the second outer member (bottom outer member)). Figures 6A-6J An exemplary auxiliary attachment mechanism according to one or more embodiments is shown. Figures 6A-6J As shown, in one or more embodiments, the auxiliary attachment mechanism described further below can be used with any of the retainers of the present application (eg, retainers 100 / 200 / 300).
[0128] Advantageously, it has been found that the use of an auxiliary attachment mechanism allows the medical worker to use the holder as a third hand during use of the medical cylinder. For example, by attaching the holder to the medical worker's surgical gown (i.e., clothing), when the worker is working with the medical cylinder, at least a portion of the medical cylinder can be secured by the holder, and the worker can then remove the holder from the clothing and reattach it elsewhere, for example, to a surgical drape, table, or tray to secure the cylindrical medical body as needed, thereby freeing the medical worker to move away from the patient and preserve the cylindrical medical body.
[0129] Figure 6A-6B An embodiment is shown in which the holder of the present application is attached to a pocket card 670 having an auxiliary attachment mechanism 671. The pocket card 670 is used to stabilize the holder in its sterilizable packaging. The pocket card can include a pair of slits 672 so that the holder can be removably attached to the pocket card 670 by tucking the opposite edges of one of the outer members into the slits 672. The pocket card 670 may include an auxiliary attachment mechanism 671 for attachment to a drape or clothing, such as an alligator clip for attachment (see Figure 6B ) or teeth / protrusions for example for clamping in the manner of bread clips (see Fig. 6A ). In one or more embodiments, the pouch card 670 and the auxiliary attachment mechanism 671 may be additional parts of a kit including a retainer.
[0130] Figure 6C-6D One embodiment is shown in which the retainer of the present application includes an auxiliary attachment mechanism in the form of an integrated ratchet clip 673. Figure 6C and 6D As illustrated in FIG. 1 , in one or more embodiments, the ratchet clip 673 may be integrally molded with one of the outer members of the retainer. Fig.6DAs shown, when pressed together, the opposite ends 674 of the ratchet clip 673 can ratchet together to provide different degrees of tightness to push the protrusions 679 together sufficiently tightly around the fold of the cover cloth or clothing to secure the retainer thereto. In at least one alternative embodiment, the end 674 of the ratchet mechanism has only one attachment point and therefore has one tightness.
[0131] Figure 6E-6F An embodiment is shown in which the retainer of the present application includes one or more integrated teeth as an auxiliary attachment mechanism. The integrated teeth can be used to grip the folds of the cover cloth or clothing and can be molded into one of the outer members of the retainer of the present application. For example, Fig. 6E As shown in the embodiment of FIG. 6 , a portion of the cover cloth or garment can be pushed through four integrated teeth 675. Similarly, in Fig. 6F (and Fig. 6A ) embodiment, a portion of the cover cloth or garment can be pushed into and retained within the opening and two integrated teeth 676 in the manner of a bread binder.
[0132] Similarly, Figure 6G-6H One embodiment is shown in which a retainer of the present application may include a hole 677 molded into one of the outer members of the retainer, through which an auxiliary attachment mechanism 671 may be attached, such as a clip (e.g., an alligator clip) for attachment to a drape or garment.
[0133] Figures 6I-6J Another embodiment is shown in which the holder of the present application includes a spring-action clip 678 attached to one of the outer members of the holder.
