Description

[0001] The invention relates to a method for controlling a laser of a laser device. Furthermore, the invention relates to a laser device, a computer program, and a computer-readable medium.

[0002] Clouding and scarring within the cornea, also known as the cornea, which can result from inflammation, injury, or congenital diseases, impair vision. Clear vision is significantly impaired, particularly if these pathological and / or abnormally altered areas of the cornea are located in the visual axis of the eye. Furthermore, other visual disorders, such as reduced visual acuity or corneal curvature, can also impair vision. Various state-of-the-art laser procedures using appropriate laser devices are available for this purpose. These can detach a solid body from the cornea and thus improve vision for a patient. For example, photodisruptive and ablative procedures are known for this purpose, which create corresponding interfaces using laser pulses and can thus, for example, remove a solid body from the cornea, whereby the injured or diseased area can be modified in such a way that vision is improved again.

[0003] Furthermore, methods are already known in the art in which a focused femtosecond laser with low energy and high repetition rate is used to non-surgically change the refractive index of transparent materials and tissues, such as corneas, lenses, contact lenses, and artificial lenses, thereby altering their light refraction properties. This method is also referred to as LIRIC (Laser-Induced Refractive Index Change). In living tissue, the method does not trigger wound healing or scarring.

[0004] According to the known LIRIC methods, cylindrical or spiral paths are used to deliver the phase-interleaved optical corrections. The treatment outcome depends on the speed of the laser pulses, whereby a constant speed is specified in the state of the art. However, this is particularly difficult to achieve with concentric paths. Depending on the position of the laser pulses, the laser speed can no longer be maintained, for example, in the center of the circular path, or corresponding corrections at the edge of the treatment area are no longer feasible due to limitations of the laser device with regard to energy delivery.

[0005] The described refractive correctors according to WO 2017 / 106321 A1 include, among others, intraocular lenses (IOLs), contact lenses, corneal inlays, and other optical components or devices having a continuous central phase zone and peripheral phase discontinuities. Further embodiments relate to a method of using a laser to alter the refractive properties of refractive correctors to form such a continuous central phase zone and peripheral phase discontinuities, as well as other applications. The refractive corrector and methods adapt a Fresnel lens structure to include continuous phase-retarding regions with a wavefront height greater than a design wavelength in a central zone of a refractive corrector to enhance human vision applications while maintaining the benefits of phase wrapping in the peripheral region.

[0006] US 2018 / 0 243 082 A1 describes methods and systems in which laser-induced refractive index changes are carried out by focused femtosecond laser pulses in optical polymer materials or optical tissues in order to enable various types of vision correction.

[0007] It is therefore an object of the present invention to provide a method, a laser device, a computer program and a computer-readable medium by means of which the disadvantages of the prior art are overcome and in particular an improved treatment of a polymer material, in particular a cornea, can be realized.

[0008] This object is achieved by a method, a laser device, a computer program, and a computer-readable medium according to the independent patent claims. Advantageous embodiments with expedient refinements of the invention are specified in the respective subclaims, wherein advantageous embodiments of the method are to be regarded as advantageous embodiments of the laser device, the computer program, and the computer-readable medium, and vice versa.

[0009] One aspect of the invention relates to a method for controlling a laser of a laser device. A plurality of first laser pulses are generated with a first energy density within a predetermined energy range and below a photodisruption regime of a polymer material of a region of an optical element. A core region of the region is irradiated with the first laser pulses, wherein a refractive index of the polymer material changes as a function thereof at each irradiation point irradiated with the first laser pulses. A plurality of first irradiation lines are generated within the core region by means of a plurality of irradiation points, thereby producing a first optical correction in the core region.A plurality of second laser pulses are generated with a second energy density within the energy range and below a photodisruption regime of the polymer material of the region of the optical element, wherein the second energy density is different from the first energy density. The edge region of the region, which at least partially surrounds the core region, is irradiated with the second laser pulses, wherein the refractive index of the polymer material changes as a function thereof at each of the irradiation points irradiated with the second laser pulses. A plurality of second irradiation lines are generated within the edge region by means of the plurality of irradiation points, and thereby a second optical correction is generated in the edge region that is different from the first optical correction.

[0010] In particular, it is proposed that the area to be treated be divided into a core area and a peripheral area. The core area is irradiated with the first energy density, and the peripheral area with the second energy density. Energy density is understood to mean, in particular, the energy dose, i.e., the energy per unit area. This can be the energy of one laser pulse per area, but also the energy of several pulses within a “scan section” per area of the scan path, i.e., the irradiation line. In particular, energy density can also be viewed as power density, i.e., energy per unit time and per unit area. Furthermore, the so-called “fluence,” i.e., energy per unit volume, can also be viewed as energy density. The energy density can be described by the density of the irradiation lines, the so-called interline distance, or by the density of the irradiation points within the lines, i.e., the density of the irradiation points, which is also referred to as the interspot distance. Furthermore, the energy of the emitted laser pulses for different lines can also be understood as energy density.

[0011] The energy range describes a range in which the energy can be adjusted by a user of the laser device and the method according to the invention can be carried out. In particular, the energy range is limited upwards by the energy for the photodisruption regime, i.e., at which energy level photodisruption bubbles, which can also be referred to as cavitation bubbles, form, and downwards by the energy at which the refractive index in the polymer material still changes. The energy range is therefore particularly dependent on the polymer material itself.

[0012] The polymer material is, in particular, a human or animal cornea and / or a lens of an eye. It should also be mentioned at this point that the method can also be applied to artificial contact lenses or artificial intraocular lenses. In particular, the polymer material is, for example, a biopolymer material that is synthesized in a cell of a living being, for example in the form of polysaccharides, proteins, nucleic acids, or the like. Furthermore, however, artificial polymer materials, such as collagen, can also be modified accordingly using the method according to the invention. For example, the cornea or the lens of the eye can then be considered biopolymer material, while collagen, for example, can be considered contact lenses or intraocular lenses.

[0013] The optical element can then be, for example, an eye or a vitreous body. The region, in turn, describes the area within the polymer material that encompasses, for example, the diseased area and / or the area to be treated.

[0014] In particular, the laser pulses with the predefined energy or energy density are generated in such a way that they are generated below the photodisruption regime, which means that no photodisruption bubbles are generated within the cornea, which only changes the refractive index within the irradiation points, which in turn causes light rays in the area to be refracted differently after the treatment than before, which leads to a correction of the optical element.

[0015] Furthermore, it is thus preferably provided that so-called phase wrapping is carried out within the region. In particular, during so-called phase wrapping, a technical limit is reached, particularly in the edge region, which can also be referred to as the periphery, at which the resolution of the laser device can no longer allow "finer" irradiation lines. According to the invention, it is now provided that a progressive reduction of the irradiation line profile can be carried out, particularly in the edge region, wherein this can be carried out in particular in a so-called transition zone. The method can also be used for presbyopic correction, i.e., a correction of age-related farsightedness.

[0016] A second optical correction is produced in the peripheral area which is smaller than the first optical correction. In particular, it is therefore provided that different corrections are carried out in the peripheral area and in the core area. For example, due to the limitations of the laser device, it is no longer possible to carry out the corresponding correction in the peripheral area as in the core area. In particular, however, it is now possible to carry out a smaller correction in the peripheral area, which can lead to an improvement in vision, for example. The values of the corrections therefore preferably differ in their magnitude but not in their sign. For example, the first correction can be five diopters and the second correction less than five diopters, for example four diopters or two diopters. This is purely an example and in no way to be regarded as exhaustive.

[0017] Furthermore, it has proven advantageous if the first irradiation lines and / or the second irradiation lines are generated in a substantially annular manner in the region. In particular, a type of Fresnel lens can thus be provided, with both the first irradiation lines in the core region and the second irradiation lines in the edge region thus being annular.

[0018] It is further advantageous if the first irradiation lines and / or the second irradiation lines are generated concentrically with one another. In particular, the first irradiation lines and / or the second irradiation lines thus form concentric circles with one another. In particular, the concentric circles in the core region have a smaller radius than the concentric circles in the edge region. This allows for reliable correction of the polymer material.

[0019] In a further advantageous embodiment, a transition from the core region to the edge region is specified depending on at least one parameter limiting the laser device. In particular, when the laser device has reached a corresponding limit with regard to the potentially achievable energy density, in particular with regard to the laser pulses to be emitted or the irradiation point spacing, the transition to the edge region occurs in order to perform further correction there with a lower energy density. Thus, a corresponding correction can also be performed in the edge region, which can lead to an improvement in the visibility restriction.

[0020] In another advantageous embodiment, the edge area is used as a transition zone between the area to be treated and the area not to be treated. This transition zone can also be referred to as a transition zone (TZ). This creates an improved transition between the area to be treated and the area not to be treated, thereby improving the visual impairment.

[0021] It is also advantageous if a predefined correction of the optical element is performed in the core area. In particular, the predefined correction can be specified, for example, based on pachymetric data of the eye. This predefined correction enables, in particular, improved vision. A type of transition area can then be created in the peripheral area, which does not have the predefined correction but can still improve visual acuity even in the peripheral area, thus providing a better transition to the area not to be treated.

[0022] According to a further advantageous embodiment, at least the first energy density is adjusted as a function of the respective distance between the irradiation points and / or as a function of the respective distances between the first irradiation lines and / or as a function of a laser pulse energy of the respective laser pulses. In particular, the energy density is thus not only dependent on a single pulse, but can be viewed as a function of the energy dose, i.e. the energy per unit area. Thus, the energy of one laser pulse per area, but also the energy of multiple pulses within the area can influence the energy density. Furthermore, the energy with which the laser pulses are generated can also influence the energy density accordingly. Furthermore, the energy density can also be understood as a type of power density, i.e. the energy per unit time and per unit area.Furthermore, energy density can also be understood as fluence, i.e., the energy per unit volume. Thus, energy density can be influenced in various ways.

[0023] Furthermore, it has proven advantageous if the method is used to generate an axicon shape of the optical region. This can, for example, create a special, conical region. Axicons are conical lenses that produce a ring-shaped beam profile. An axicon images a point source onto a line along the optical axis or transforms a laser beam into a ring, allowing optical corrections to be made within the region. The axicon shape can, for example, be generated based on different heights of the irradiation lines. For example, the height of the irradiation lines can decrease from the center of the core region towards the edge region.

[0024] It is also advantageous if, for generating the second optical correction, at least a height of the second irradiation line is generated differently than the height of the first irradiation line and / or a distance between the second irradiation lines is generated differently than a distance between the first irradiation lines and / or the laser pulses are generated with a lower predefined second energy for the second irradiation lines than the laser pulses with the predefined first energy for the first irradiation lines. In particular, an optical correction can thus be carried out with different interspot distances, interline distances and different energy densities of the laser pulses themselves. Furthermore, a corresponding height of the irradiation lines can also be different. Thus, different optical corrections can be carried out in the core region and in the edge region.

[0025] In a further advantageous embodiment, different second corrections are generated in the edge region from an inner edge facing the core region to an outer edge facing away from the core region. In particular, the different second corrections depend on the corresponding limitations of the laser device. In particular, a transition zone can thus be created that has different second corrections, so that a smooth transition from the core region to the outermost edge of the edge region can be created, thereby realizing an improved optical correction.

[0026] Another advantage is that the outer edge of the peripheral area is less corrected than the inner edge. This allows for a smooth transition from the core area through the peripheral area to the non-treatment area.

[0027] It has also proven advantageous if the laser pulses are emitted in a wavelength range between 200 nm and 2 µm, in particular between 400 nm and 1450 nm, with a respective pulse duration between 1 fs and 1 ps, in particular between 10 fs and 100 fs, and a repetition frequency greater than 10 kHz, in particular between 1 MHz and 100 MHz. In particular, this allows the laser pulses to be generated below the photodisruption regime, resulting only in a change in the refractive index. This allows the procedure, and in particular the change in the refractive index, to be reliably performed without performing an invasive procedure in the cornea.

[0028] In a further advantageous embodiment, topographical and / or pachymetric and / or morphological data of the optical element, in particular of the eye, in particular of the cornea and / or the lens, are taken into account when controlling the laser. In particular, this data can be determined, for example, before a treatment. The treatment can then be reliably carried out based on this data.

[0029] A second aspect of the invention relates to a laser device having at least one ophthalmic surgical laser and having at least one control device for the laser(s), which is designed to carry out the steps of the method according to the first aspect.

[0030] The laser device is preferably designed as a rotary scanner and, for this purpose, has, for example, a beam deflection device. The laser device is also preferably designed as an ophthalmic surgical treatment device.

[0031] In an advantageous embodiment of the laser device, the laser device has a storage device for at least temporarily storing at least one control data set, wherein the control data set(s) comprise control data for individual laser pulses on or in the optical element and comprise at least one beam deflection device for beam guidance and / or beam shaping and / or beam deflection and / or beam focusing of a laser beam of the laser. Said control data sets are typically generated based on a measured topography and / or pachymetry and / or morphology of the optical element to be treated, in particular the cornea or lens to be treated in the pathologically and / or abnormally altered region within the optical element.

[0032] It can be provided that the laser device has a single memory device and a single control device. Alternatively, it can be provided that different memory devices and control devices are formed within the laser device in order to carry out a corresponding control of the laser.

[0033] Further features and their advantages can be found in the descriptions of the first aspect of the invention, wherein advantageous embodiments of the first aspect of the invention are to be regarded as advantageous embodiments of the respective other aspect of the invention.

[0034] A third aspect relates to a computer program comprising instructions that cause the laser device according to the second aspect of the invention to execute the method steps according to the first aspect of the invention. A fourth aspect of the invention relates to a computer-readable medium on which the computer program according to the third aspect of the invention is stored.

[0035] Further features and their advantages can be found in the descriptions of the first and second aspects of the invention, wherein advantageous embodiments of each aspect of the invention are to be regarded as advantageous embodiments of the other aspect of the invention.

[0036] Further features emerge from the claims, the figures, and the description of the figures. The features and combinations of features mentioned above in the description, as well as the features and combinations of features mentioned below in the description of the figures and / or shown alone in the figures, can be used not only in the respective combination specified, but also in other combinations without departing from the scope of the invention. Thus, embodiments are also to be considered encompassed and disclosed by the invention that are not explicitly shown and explained in the figures, but which emerge and can be produced through separate combinations of features from the explained embodiments.

[0037] Showing: Fig. 1 is a schematic view of an embodiment of a laser device; Fig. 2 a schematic plan view of an optical element; and Fig. 3 a schematic sectional view of an optical element.

[0038] In the figures, identical or functionally identical elements are provided with the same reference numerals.

[0039] Fig. 1 shows a schematic representation of a laser device 10 with a laser 12 for, for example, the treatment of a patient, in particular for the treatment of an eye 14 of a patient, wherein the eye 14 is also referred to below as an optical element. The laser device 10 is thus designed as an ophthalmic surgical treatment device. It can be seen that, in addition to the laser 12, a control device 18 for the laser 12 is designed. This embodiment with a control device 18 is to be regarded purely as an example. It can be provided that the laser device 10 also has a plurality, in particular more than two, control devices 18. The control device 18 can, for example, generate pulsed laser pulses 40 (see Fig. 2) in a predefined pattern into the eye 14, for example, into an area 16, wherein the position of the area 16 in this embodiment is selected such that a pathological and / or abnormally altered area is enclosed within a stroma of the eye 14. The area 16 is thus an area 16 to be treated.

[0040] Furthermore, it can be seen that the laser beam 22 generated by the laser 12 is deflected in the direction of the eye 14 by means of a beam deflection device 24, such as a scanner, in particular a so-called rotary scanner. The beam deflection device 24 is also controlled by the control device 18, for example, to create irradiation lines 34, 38 (see Fig. 2). The beam deflection device 24 can, for example, have one or two mirrors designed to deflect the incident laser beam 22.

[0041] In the present exemplary embodiment, the laser 12 shown is a laser 12 that emits the laser pulses 40 in a wavelength range between 200 nm and 2 µm, in particular between 400 nm and 1450 nm, with a respective pulse duration between 1 fs and 1 ps, in particular between 10 fs and 100 fs, and a repetition frequency greater than 10 kHz, in particular between 1 MHz and 100 MHz. In particular, the laser pulses 40 can be generated below the photodisruption regime, which merely leads to a change in the refractive index. As a result, the method, and in particular the change in the refractive index, can be reliably performed without performing an invasive procedure, for example, in a cornea.

[0042] The control device 18 also has a storage device 28 for at least temporarily storing at least one control data set, wherein the control data set(s) comprise control data for positioning and / or focusing individual laser pulses 34 in or on the eye 14. The position data and / or focusing data of the individual laser pulses 40 are generated based on a previously measured topography and / or pachymetry and / or the morphology of the eye 14 and, for example, the pathological and / or abnormally altered region within the stroma of the eye 14.

[0043] Fig. Figure 2 shows a schematic plan view of an eye 14. In particular, the Fig. 2 the area 16 which is to be treated and an area 20 not to be treated. The area 16 to be treated is divided into a core area 30 which is delimited by a first irradiation line 34 and an edge area 36 which is delimited at an outer edge by a second irradiation line 38 and at the inner edge by the first irradiation line 34.

[0044] The method according to the invention provides for a plurality of first laser pulses 40 having a first energy density 42 to be generated within a predetermined energy range and below a photodisruption regime of a polymer material 26 of the region 16 of the optical element. The core region 30 is irradiated with the first laser pulses 40, wherein, as a function thereof, a refractive index of refraction of the polymer material 26 changes at each irradiation point irradiated with the first laser pulses 40. A plurality of first irradiation lines 34 are generated within the core region 30 by means of a plurality of irradiation points, whereby a first optical correction 44 (see Fig. 3) is generated in the core region 30. A plurality of second laser pulses 40 are generated with a second energy density 46 within the energy range and below a photodisruption regime of the polymer material 26 of the region 16 of the optical element, wherein the second energy density 46 is different from the first energy density 42. The edge region 36 is irradiated with the second laser pulses 40, wherein the edge region 36 surrounds the core region 30 at least in part, and wherein, as a function thereof, the refractive index of the polymer material 26 changes at each irradiation point irradiated with the second laser pulses 40. A plurality of second irradiation lines 38 are generated within the edge region 36, whereby a second correction 48 that is different from the first optical correction 44 is generated in the edge region 36.

[0045] In particular, it is provided that the second optical correction 48 in the edge region 36 is generated to a lesser extent than the first optical correction 44 in the core region 30. Furthermore, it is shown in particular that the first irradiation lines 34 and / or the second irradiation lines 38 are generated substantially annularly in the region 16, wherein it is provided in particular that the first irradiation lines 34 and / or the second irradiation lines 38 are generated concentrically to one another.

[0046] Furthermore, it is particularly provided that a predefined correction of the optical element, in particular of the eye 14, is carried out at least in the core region 30. Furthermore, the Fig. 2, that different second corrections 48 are generated in the edge region 36 from an inner edge, which in this case is delimited by the first irradiation line 34 and which faces the core region 30, to an outer edge, which in this case is delimited by the second irradiation line 38 and faces away from the core region 30. In particular, a smaller correction can be generated at the outer edge of the edge region 36 than at the inner edge of the edge region 36.

[0047] Fig. 3 shows a schematic side view or sectional view of the area 16. In the present case, in particular, a type of sectional view of the Fig. 2 shown area 16.

[0048] The Fig. 3 shows that a transition from the core region 30 to the edge region 36 is predetermined depending on at least one parameter limiting the laser device 10. In particular, when the laser device 10 has reached a corresponding limit with respect to the energy density 42, 46, a transition occurs to the edge region 36 in order to perform a further correction, in particular the second optical correction 48, there with a lower energy density 42, 46. Thus, a corresponding correction can also be performed in the edge region 36, which can lead to an improvement in the visual restriction.

[0049] It can be provided that, for example, the edge area 36 is used as a transition zone from the area 16 to be treated to the area 20 not to be treated.

[0050] Furthermore, the Fig. 3, that at least the first energy density 42 is adjusted depending on the respective distance between the irradiation points and / or depending on the respective distances between the first irradiation lines 34 and / or depending on the laser pulse energy of the respective laser pulses 40. Furthermore, for generating the second optical correction 48, at least a height of the second irradiation line 38 can be generated differently than the height of the first irradiation line 34 and / or a distance between the second irradiation lines 38 can be generated differently than a distance of the first irradiation line 34 and / or the laser pulses 40 can be generated with a lower predefined second energy for the second irradiation lines 38 than the laser pulses 40 with the predefined first energy for the first irradiation lines 34.

[0051] The Fig.Figure 3 shows, in particular, that the heights of the first irradiation line 34 and the second irradiation line 38 are substantially equal. Alternatively, it may also be provided that an axicon shape of the optical region 16 is generated by means of the method.
