Description

TECHNICAL FIELD OF THE INVENTION

[0001] The invention comprises an agent for the treatment of periodontitis. BACKGROUND OF THE INVENTION

[0002] In many cases, wound healing cannot yet be optimally supported and promoted therapeutically. Science continues to strive to find ways to prevent complications or infections, avoid delayed healing, and achieve the best possible cosmetic outcome. There are still patients who suffer from chronic or non-healing wounds.

[0003] The published patent application EP 2 777 708 A1 provides an agent for treating mouth ulcers, which contains a flowable, pasty adhesive composition which is suitable for adhering to the oral mucosa after application, and a pharmaceutical active composition comprising an organic or inorganic chlorine compound and at least one oxidizing agent.

[0004] US Patent No. 6,280,775 B1 discloses a mouthwash for treating and reducing dental diseases. This mouthwash is based on a mixture of a water-soluble metal chlorite solution and a solution containing sodium persulfate and hydrogen peroxide. The resulting composition contains chlorine dioxide.

[0005] Published patent application WO 99 / 34773 A1 describes an antimicrobial care product based on a peroxycarboxylic acid. The product is provided in particular in the form of shampoos, roller sticks, gels, rinses, and oral care compositions, such as mouthwashes, toothpastes, tooth powders, mouth sprays, chewing gums, lozenges, sachets, and bleaching kits, etc., for the treatment of topically treatable microbial infections, in particular diseases of the oral cavity, such as plaque, gingivitis, periodontitis, and bad breath, as well as anaerobic infections of the skin.

[0006] The published patent application WO 2004 / 000372 A2 describes a composition for disinfecting dentures, toothbrushes, razors and the like, comprising a salt-form chloride or bromide compound, at least one oxidizing agent with an oxidation potential which, in solution, is higher than the oxidation potential of Cl 1-< / Cl 0< or Br 1-

[0007] Publication WO 00 / 19981 A1 discloses antimicrobial pharmaceutical preparations (e.g., solutions, gels, ointments, creams, delayed-release preparations, etc.) containing chlorite (e.g., a metal salt of chlorite) in combination with a peroxide compound (e.g., hydrogen peroxide) for use in disinfecting objects or surfaces (e.g., contact lenses, drapes, etc.), antisepsis of the skin or other body parts, preventing scarring, and / or treating and prophylaxis of skin or mucous membrane diseases (e.g., wounds, burns, infections, frostbite, ulceration, psoriasis, acne, or other scarring lesions). Publication US 2004 / 037891 A1 also discloses an antimicrobial preparation containing chlorite in combination with a peroxide compound. However, this composition remains stable even during storage at room temperature and does not generate chlorine dioxide.

[0008] The article "Chlorhexidine - An antiseptic in periodontics" by Dutt et al. (IOSR Journal of Dental and Medical Sciences, Vol. 13, No. 9, September 1, 2014, pages 85-88) states that chlorhexidine is the most effective chemical anti-plaque agent available to date. Specifically, the use of chlorhexidine as an adjunct to mechanical therapies to control plaque formation is recommended. TASK

[0009] It is an object of the present invention to provide an agent for pain relief and wound healing. In particular, the aim is to provide an agent that can be easily applied, preferably by the patient themselves. In particular, the aim is to find an agent that, when applied topically, contributes to pain relief and wound healing.

[0010] Furthermore, it is an object of the present invention to provide an agent (mixture of substances) and a dosage form which enables a simple, localized and sustainable treatment.

[0011] For example, the objective is to provide a product for use on chapped skin or cracked skin, e.g., on the soles of the feet, heels, toes, or between toes; on the hands, fingers, or between fingers; on the lips, corners of the mouth, corners of the nose, in the flexures of joints, or in the perianal region. The particular goal is to provide a product that successfully soothes and heals chapped skin or cracked skin, especially cracks on calloused skin.

[0012] It is a further object of the present invention to provide an agent for use in the mouth or throat, particularly in cases of gum problems or periodontitis.

[0013] It is a further object of the present invention to provide an agent for topical application in the case of injuries to the mucous membrane in the mouth and throat, in particular the gums or gingiva. SUMMARY

[0014] The invention relates to an agent for use in the treatment of periodontitis, comprising at least one salt-form chloride compound selected from the group of alkali or alkaline earth metal salts and at least one oxidizing agent selected from the group of persulfate compounds.

[0015] Further developments and / or advantageous embodiments of the invention are the subject of the dependent patent claims. The uses of the agent according to the invention described below, which do not relate to the treatment of periodontitis, are not part of the invention.

[0016] The agent according to the invention is also particularly suitable for use in the treatment of cracks in the skin, such as rhagades, particularly on feet and hands, in particular on the soles of the feet, the edge of the plantar surfaces, heels, toes, palms and / or fingers, in the treatment of poorly healing wounds, including e.g. wounds of diabetics, in particular wounds on the limbs, e.g. on legs, arms, feet, hands, toes and fingers, in the treatment of infected wounds, in particular bacterially infected wounds or fungal infected wounds, for disinfecting wounds, in the treatment of bleeding wounds, in particular e.g. bleeding gums and / or in the treatment of therapy-resistant (i.e. non-healing) open wounds on the skin or mucous membranes, these uses in themselves are not part of the claimed invention.

[0017] It can also be used to treat slow-healing, chronic, or non-healing wounds—that is, wounds that have not healed or shown no improvement for two days, one week, four weeks, or longer. Wound healing is generally accelerated. The agent according to the invention is therefore suitable for accelerating wound healing.

[0018] Furthermore, the agent according to the invention acts as an anti-inflammatory or disinfectant, particularly when applied to open skin areas or gums. Bleeding gums can be reduced or prevented, particularly after dental treatment and / or in cases of periodontitis. The agent according to the invention is particularly suitable for the treatment of periodontitis, especially marginal periodontitis, or as an adjunct treatment (after dental intervention) for apical periodontitis.

[0019] The agent according to the invention also has a pain-relieving or analgesic effect in the skin and mucous membrane defects or skin and mucous membrane wounds mentioned herein, in particular after dental treatment, such as a tooth cleaning treatment, or in the case of skin cracks, in particular rhagades, and can thus be used additionally for pain relief.

[0020] The agent according to the invention is particularly suitable for use in the treatment of periodontitis in combination with a tooth cleaning treatment, in particular a scaling and root planing (SRP) treatment. The agent according to the invention is expediently applied following a tooth cleaning treatment, in particular following a scaling and root planing (SRP) treatment. The agent according to the invention is preferably applied at least on the same day following a tooth cleaning treatment or scaling and root planing (SRP) treatment. The agent according to the invention is expediently applied to the gums, in particular to the gum line, preferably for 1 to 3 hours. In the days following the tooth cleaning treatment or scaling and root planing (SRP) treatment (preferably for at least three, more preferably at least 5 consecutive days), regular dental care (e.g.at least once a day) with an aqueous solution of the agent according to the invention.

[0021] Chapped skin or skin tears, particularly split-shaped tears in the skin, are often the result of overstretching with reduced elasticity of the skin, caused, for example, by dehydration, cold (climatic influences) or illness (e.g. skin diseases) on the lips, corners of the mouth, base of the nostrils, soles of the feet (particularly on the heels or on or between the toes), hands and / or fingers, joint folds or in the perianal region. In addition to environmental influences and illness, skin cracks can also have other causes, e.g. a bacterial infection of a skin injury. A tear (i.e. a tear) in the skin, particularly a split-shaped tear in the skin as a result of overstretching with reduced elasticity, is medically known as a rhagade and colloquially also as a fissure.

[0022] Tears, or fissures, can also develop. These tears are unsightly, can be painful, can pose hygiene problems, and are potential entry points for pathogens. If healing is delayed, such tears can have a restrictive effect. Measures to promote healing are therefore desirable.

[0023] Furthermore, it has been shown that the inventive agent also leads to improvement and ultimately healing in other types of wounds. The inventive agent can be used advantageously in wounds that are slow to heal (e.g., due to circulatory disorders, the use of certain medications, wound infections, or other reasons that can adversely affect the healing process). In particular, wounds of diabetics can be successfully treated with the inventive agent.

[0024] It has also been shown that gum injuries and gingivitis in particular can be quickly alleviated.

[0025] It has been found that the agent according to the invention can be successfully applied topically.

[0026] The at least one chloride compound in saline form and the at least one oxidizing agent define an active composition, which may optionally contain other substances. When the active composition is mixed with an adhesive composition, which is particularly available as a paste, cream, ointment, or gel, the treatment is particularly long-lasting and intensive due to the special adhesive properties of the agent. [0033] Additional features, advantages, and objects of the present invention will become apparent from the following description. The advantageous embodiments listed below, alone or in combination with one another, lead to further improvements of the inventive device. DESCRIPTION

[0034] The active composition is characterized by containing at least one chloride compound in saline form, selected from the group of alkali or alkaline earth metal salts, and at least one oxidizing agent selected from the group of persulfate compounds. This active composition has the advantage that, following application of the agent to an injured and / or inflamed area, contact with moisture (i.e., water) produces chlorine oxide or hypochlorite, which appears to be responsible for the healing effect of the agent. The adhesive composition—where present—forms a barrier layer that simultaneously contains the pharmaceutically active substances. However, these only develop their effect when moisture reaches them.

[0035] In the context of this description, the term "active composition" refers to components of the inventive agent that exhibit cosmetic and pharmaceutical efficacy, either alone or in combination. The term "adhesive composition," on the other hand, refers to components that can serve, alone or in combination, as carriers and application media for the active composition. The adhesive composition also serves the purpose of forming a barrier layer over the area to be treated (e.g., a tear or injury and / or possibly inflammation) and gradually releasing the active ingredients. Localized application and adhesion of the inventive agent due to the adhesive composition enables local and thus targeted application of the active ingredients.

[0036] The adhesive composition advantageously has an adhesive strength of more than 5 N [Newton], more preferably more than 7 N (in particular when measured according to ISO standard 10873:2010, in particular in a 1-minute test and / or a 10-minute test at 25°C).

[0037] The adhesive composition advantageously has a viscosity in the range of 20 to 50 mPa s [millipascal second], more preferably 25 to 45 mPa s (in particular measurement with the Brookfield viscometer, advantageously at 100 RPM with spindle (cylinder) No. 4 and at 25°C, in particular a mixture of 5 g of propylene glycol and 1 g of the paste according to the invention).

[0038] The adhesive composition advantageously has a consistency value in the range of 10 to 50 mm [millimeters], more preferably 20 to 40 mm (in particular when measured according to BE EN 30193-1:1994, in particular for a weight of 1 kg at 25°C).

[0039] The chlorine atom can be present in the starting compound either in the form of an alkali or alkaline earth metal salt (a compound that releases chloride ions in aqueous solution). A mixture of the aforementioned chlorine-containing substances can also be used.

[0040] The agent according to the invention is preferably characterized in that it contains no fluoride salts, preferably no fluoride compounds at all.

[0041] An oxidizing agent is used which is capable of splitting off chlorine or chlorine oxide from the chlorine compound in aqueous solution. Suitable oxidizing agents are, for example, persulfate compounds, in particular hydrogen peroxosulfate compounds. In combination with at least one suitable oxidizing agent, chlorine (Cl 2 ) or dichlorine oxide (Cl 2 O -< ) can be formed by reaction with dissolved chloride ions (Cl-). The advantage of such an active composition is that the chlorine is released gradually. The oxidation potential of the oxidizing agent in solution is advantageously higher than the oxidation potential of Cl -< / Cl o< , at least in the preferred pH range < 6 or < 5, respectively, so that the desired reaction can occur.

[0042] Although in principle different oxidizing agents can be used, the active composition contains a persulfate compound, in particular a hydrogen peroxosulfate compound, as the oxidizing agent. Persulfate compounds include peroxomonosulfate compounds (i.e. compounds containing the anion [SO 5 ] 2-< ) and peroxodisulfate compounds (i.e. compounds containing the anion [S 2 O 8 ] 2-< ). Examples of these compounds are peroxomonosulfates such as Na 2 SO 5 or KHSO 5 and peroxodisulfates such as Na 2 S 2 O 8 . Hydrogen peroxosulfate compounds are particularly preferred as oxidizing agents. An advantageous agent is obtained using potassium hydrogen monopersulfate (KHSO 5 ) as the oxidizing agent and a salt-form chloride compound, preferably from the group of alkali salts, e.g. table salt (NaCl). This has proven to be particularly effective.

[0043] The agent according to the invention, in particular the active composition, expediently contains a carboxylic acid, preferably a mono- or dicarboxylic acid such as citric acid. These organic carboxylic acids ensure an acidic pH, which can further enhance the effectiveness of the agent.

[0044] An advantageous active composition, which can be produced cost-effectively, contains potassium hydrogen monopersulfate (KHSO ), table salt (NaCl), and an acid, preferably in the form of a monocarboxylic acid or a dicarboxylic acid such as oxalic acid, tartaric acid, succinic acid, or citric acid. The active composition preferably contains such an amount of acid that, after dissolving the active composition in an aqueous medium (5 g of active composition in 50 ml of water), a pH value of < 5, preferably < 4.5, is achieved. Citric acid is physiologically completely harmless, is rapidly degraded in nature, and is therefore preferably used in the active composition.

[0045] The active ingredient may additionally contain germicidal compounds such as potassium monopersulfate, potassium carbonates, sodium peroxycarbonate, sodium benzoate, subtilisin, potassium benzoate, chlorhexidine, a combination of chlorhexidine and thymol, cetylpyridinium chloride, halogen-releasing compounds such as PVP-iodine and cyanuric acid chloride, and / or formaldehyde-releasing compounds such as paraformaldehyde and / or methylol compounds, etc. Analgesic agents such as lidocaine, polidocanol, or benzydamine are also advantageously used.

[0046] The active composition expediently contains a binder, and optionally flavorings, colors, and excipients such as water softeners, fillers, and the like. Examples of binders that can be used include modified corn starch, microcrystalline cellulose, sorbitol, hydrogenated soy triglycerides, polyethylene glycol such as PEG 180, PEG 150, PEG 75, polyvinylpyrrolidone, a copolymer of ethylene oxide and propylene oxide, polyvinylpyrrolidone, or a copolymer of polyvinylpyrrolidone and vinyl acetate, etc. The weight proportion of the binder is expediently a maximum of approximately 30% by weight and is preferably between 5 and 30% by weight, more preferably between 5 and 25% by weight.-% of the total weight of the active composition. If an adhesion composition is used in conjunction with the active composition, a further binder as described above may be dispensed with, since the adhesion composition described herein as a whole absorbs the active composition and / or at least one of its components assumes the binding function for the active composition.

[0047] The adhesion composition preferably contains at least one water-soluble polymer, preferably selected from the group of cellulose derivatives, and at least one alkyl vinyl ether / maleic anhydride copolymer, and optionally refined paraffins (petroleum jelly), fats, and / or oils. The adhesion composition may further optionally contain zinc-containing substances, in particular zinc oxide, silicon dioxide, and polyethylene glycol. Possible such compositions and similar compositions that can be used as an adhesion composition are mentioned in the following patents and patent applications: US Patent Application No. 13 / 574,230, US Patent Application No. 13 / 574,224, US 4,758,630 and US 5,561,177, EP Patents EP 2 525 768 and EP 2 525 769.

[0048] Advantageously, the active ingredient is added to the adhesive composition in the form of a powdered solid mixture. This form of admixture can have a delayed-release effect on the active ingredient, for example, by only dissolving the powdered active ingredient upon gradual contact with saliva.

[0049] The active composition can optionally contain a surfactant. In principle, various surfactants known to those skilled in the art can be used. If carboxylic acids are used, the surfactants should be stable at a pH value below 7. In this context, "stable" means that a maximum of 10% of the surfactant used decomposes in solution at room temperature within 30 minutes. Preferred surfactants are fatty alcohol polyglycol ethers, alkylbenzenesulfonates, and alkylsulfonates. Anionic surfactants, preferably from the group of alkyl ether sulfates, such as fatty alcohol ether sulfate alkali salts, e.g., sodium n-alkyl C 12-14 diglycol ether sulfate or sodium n-alkyl C 12-14 diglycol ether sulfate, have proven particularly effective. Other suitable surfactants include sodium lauryl sulfate, sodium lauryl sulfoacetate, and trisodium phosphate.

[0050] An example active composition contains the following components: Material in wt. percent (%) preferred ranges in wt.% (%) citric acid 30 15 to 50 Lauryl sulfate (especially sodium lauryl sulfate) 15 2 to 30 Sodium bicarbonate and / or sodium carbonate 20 10 to 40 Sodium chloride 10 4 to 20 Potassium hydrogen monopersulfate 5 1 to 10 binder 20 0 to 30

[0051] To produce the cosmetically and / or pharmaceutically active agent (mixture of substances), the active composition is advantageously dispersed in an adhesive composition (i.e., an adhesive cream or adhesive gel) and finely distributed until a homogeneous mixture is achieved. The adhesive composition and the active composition are mixed together in a weight ratio of between 99:1 and 80:20, preferably between 98:2 and 88:12.

[0052] The adhesive compositions, which serve as a carrier material for the active composition (ie for the cosmetically and / or pharmaceutically active components or composition), absorb the active composition or its components, e.g. by mixing the adhesive compositions with the active composition to form a mixture of substances, whereby the adhesive properties of the adhesive composition are combined with the cosmetic and pharmaceutical effect of the active composition.

[0053] A particularly preferred adhesion composition is presented below. The particularly preferred adhesion composition is based on at least one oil or fat or a mixture thereof, preferably of vegetable origin, and at least one alkyl vinyl ether / maleic anhydride copolymer. The alkyl vinyl ether / maleic anhydride copolymer is advantageously present partly as an acid, ester and / or salt. Typically, the cations of the salts are selected from the group consisting of calcium, potassium, sodium, magnesium, aluminum, zinc salts and mixtures thereof, in particular from the group consisting of Ca 2+< , K +< , Na +< , Mg 2+< , Al 3+< and / or Zn 2+< . In particular, a methyl vinyl ether / maleic anhydride copolymer is used as the alkyl vinyl ether / maleic anhydride copolymer. The methyl vinyl ether / maleic anhydride copolymer is present, for example as a salt, ester and / or acid, in an amount of 20-45 wt.-% based on the total amount of the adhesion compositions. Advantageously, the methyl vinyl ether / maleic anhydride copolymer, for example as a salt and / or acid, is present in an amount of 25-45 wt.%, preferably 25-40 wt.%, and more preferably 28-40 wt.%, based on the total amount of the adhesion compositions.

[0054] In particular, the adhesive composition further comprises at least one water-soluble polymer selected from the group of cellulose derivatives. The cellulose derivatives are water-soluble polymers, preferably selected from the group consisting of methylcellulose, carboxymethylcellulose, sodium carboxymethylcellulose, hydroxypropylmethylcellulose, and mixtures thereof. Carboxymethylcellulose, in particular sodium carboxymethylcellulose, is preferably used. The water-soluble polymer selected from the group of cellulose derivatives is present, for example, in an amount of up to 45 wt.% based on the total amount of the adhesive compositions. Preferably, the water-soluble polymer selected from the group of cellulose derivatives is present in an amount of 2 to 40 wt.% based on the total amount of the adhesive compositions.

[0055] The adhesive composition preferably further comprises at least one water-soluble or swellable polymer selected from the group of polysaccharides, in particular xanthan. This serves as a thickener. Polysaccharides that exhibit structural viscosity, i.e., have a shear-thinning effect, are preferred. Xanthan, in particular, swells in aqueous solution, thereby increasing the viscosity of the medium and exerting a shear-thinning effect. Polysaccharides, in particular xanthan, are preferably present in an amount of 0 to 20 wt.%, more preferably 2 to 20 wt.%, based on the total amount of the adhesive composition.

[0056] Advantageously, the sum of the amounts of cellulose derivatives and polysaccharides is in the range of 2 to 40 wt.%, based on the total amount of the adhesive compositions. A weight ratio of polysaccharides, such as xanthan gum, to cellulose derivatives of at least 1:1, more preferably at least 2:1, has proven particularly advantageous. Insofar as cellulose derivatives are compared with polysaccharides in this application, the cellulose derivatives are not counted as part of the group of polysaccharides.

[0057] Optionally, the adhesive composition further comprises a liquid alcohol, especially ethanol. The alcohol is preferably present in an amount of 0 to 10 wt.%, and more preferably 2 to 8 wt.%, and more preferably 3 to 7 wt.%, based on the total amount of the adhesive composition. The addition of said alcohol results in improved bonding of the adhesive composition.

[0058] The adhesive composition may contain at least one further substance selected from the group consisting of trihydroxystearin, silicon dioxide and phosphoglycerides.

[0059] The adhesive composition preferably contains trihydroxystearin in an amount of 0.001-5.0 wt.%, and more preferably 0.1-4.5 wt.%, and even more preferably 0.5-4.0 wt.%, based on the total amount of the adhesive composition. The addition of trihydroxystearin improves the adhesive properties of the adhesive composition and thus of the entire agent (mixture of substances) according to the invention.

[0060] The adhesive composition optionally contains silicon dioxide in an amount of 0.001-5 wt.%, more preferably 0.1-4 wt.%, based on the total amount of the adhesive composition. The flowability and consistency of the adhesive composition can be positively influenced by the addition of silicon dioxide. Silicon dioxide can effectively prevent a tendency to liquefy.

[0061] The adhesive composition optionally contains phosphoglycerides, preferably in an amount of 0.001-4 wt.%, more preferably 0.01-3 wt.%, and even more preferably 0.1-2 wt.%, based on the total amount of the adhesive composition. The phosphoglycerides are selected in particular from the group of lecithins, expediently, for example, soy lecithin.

[0062] A preferred adhesive composition is characterized in that at least one lecithin and trihydroxystearin are present in combination therein, optionally lecithin, trihydroxystearin, and silicon dioxide are present in combination. Advantageously, lecithin is present in an amount of 0.001-4 wt.%, more preferably 0.001-3 wt.%, particularly preferably 0.01-2 wt.%, and trihydroxystearin is present in an amount of 0.001-5 wt.%, more preferably 0.1-4.5 wt.%, particularly preferably 0.5-4 wt.%, based on the total amount of the adhesive composition. If vegetable oil or fat, in particular with a certain minimal unsaturated fatty acid content, is used for the adhesive composition, advantageous adhesive properties are achieved, especially when the adhesive composition contains substances selected from the group consisting of phosphoglycerides and trihydroxystearin.

[0063] Alternatively, silicon dioxide is present, or optionally in combination with trihydroxystearin and / or a phosphoglyceride, especially lecithin. In addition to silicon dioxide, trihydroxystearin and polyethylene glycol also lead to stable adhesive compositions. These substances deliver good results for compositions based on vegetable oils or fats as well as mineral oils or fats.

[0064] It is preferred that the total amount of trihydroxystearin, silicon dioxide and phosphoglycerides contained does not exceed a maximum of 15 wt.%, preferably a maximum of 12 wt.%, more preferably a maximum of 10 wt.%, based on the total amount of the adhesive composition.

[0065] A particularly preferred adhesion composition is characterized in that a vegetable oil or fat is contained to at least 25 wt.%, preferably to at least 30 wt.%, based on the total amount of the adhesion composition, wherein the fatty acid content of the vegetable oil or fat preferably consists of at least 20 wt.%, more preferably at least 30 wt.%, more preferably mostly, more preferably at least 65 wt.%, more preferably at least 80 wt.%, of higher fatty acids with a chain length of 16-18 C atoms.

[0066] A particularly preferred adhesive composition is further characterized by the use of vegetable oil or fat whose fatty acid content consists, in particular, of at least 20 wt.%, preferably 40 wt.%, more preferably 50 wt.%, more preferably 60 wt.%, more preferably 70 wt.%, more preferably 80 wt.% unsaturated fatty acids. This has the advantage that the adhesive composition is particularly well tolerated when swallowed, which can be particularly advantageous when used on children.

[0067] The amount of vegetable oil or fat influences the consistency of the adhesive compositions. If too little is added, the adhesive composition and thus the entire mixture (including the active ingredient and the adhesive composition) can take on a dry, grainy consistency. The addition of silicon dioxide, trihydroxystearin, and phosphoglycerides can counteract this.

[0068] A preferred adhesion composition includes, for example, based on the total amount of the adhesion composition a) 25-60 wt.% of at least one oil and / or fat, preferably of vegetable origin, b) 2-40 wt.% of at least one water-soluble polymer selected from the group of cellulose derivatives and polysaccharides, c) 25-45 wt.% of at least one alkyl vinyl ether / maleic anhydride copolymer, d) 0-10 wt.% or optionally up to 10 wt.% of an alcohol, e) 0-5 wt.% or optionally up to 5 wt.% trihydroxystearin, f) 0-4 wt.% or optionally up to 4 wt.% phosphoglycerides, g) 0-5 wt.% or optionally up to 5 wt.% silicon dioxide, and h) 0-10 wt.% further additives.

[0069] Other additives include, in particular, flavorings (such as menthol, citrus lemon, peppermint flavoring, and menthyl lactate), lactose monohydrate, zinc oxide (ZnO), petrolatum, paraffin, and petrolatum. Other possible additives include antioxidants, colorants, stabilizers, thickeners, emulsifiers, etc. A single substance can belong to several different groups of effects or have multiple effects.

[0070] Zinc oxide can influence, and in particular increase, the bond strength and duration. Zinc oxide should preferably be avoided for applications in the mouth and throat. It is generally preferable to limit the zinc content to a maximum of 1% by weight of the total adhesive composition, or preferably to avoid using any zinc additives.

[0071] The addition of paraffins and petroleum jelly is optional. The total content of paraffins and petroleum jelly is preferably limited to a maximum of 3% by weight, more preferably to a maximum of 1% by weight, of the total amount of the adhesive composition. Advantageously, paraffin and petroleum jelly are not added, particularly for applications in the mouth and throat.

[0072] The addition of petrolatum is optional. The petrolatum content is preferably limited to a maximum of 2% by weight, more preferably to a maximum of 0.5% by weight, of the total amount of the adhesive composition. Advantageously, no petrolatum is added, especially for applications in the mouth and throat.

[0073] The addition of lactose monohydrate is optional. It can serve as a carrier for flavoring and / or the active ingredient. It improves the consistency of the mixture and, in particular, leads to a creamier consistency. Preferably, the total lactose monohydrate content is limited to a maximum of 1 wt.%, more preferably to a maximum of 0.5 wt.%, of the total amount of the adhesive composition.

[0074] For application by the user / patient, a plug of the viscous agent is applied to the area to be treated (especially a wound and, where applicable, inflammation) and spread over the area to ensure that the peripheral zone of the area to be treated is also well covered. The agent forms a barrier or protective layer on the area to be treated. At the same time, the pharmaceutically active ingredients contained in the protective layer unfold their effect. The moisture from the skin, mucous membranes, and / or wound, as well as any moisture present in the environment, dissolves the powdered components of the agent, allowing them to react with each other. Because these components dissolve only slowly, a good long-term effect can be achieved.

[0075] A topical treatment agent advantageously comprises a flowable, pasty adhesive composition suitable for adhering to the area to be treated after application, and a pharmaceutical active composition comprising a saline chloride compound selected from the group of alkali or alkaline earth metal salts and at least one oxidizing agent selected from the group of persulfate compounds. When the organic or inorganic chlorine compound and the oxidizing agent dissolve, chlorine or chlorine oxide is formed, which has surprisingly proven particularly effective in the treatment of wounds and, where appropriate, inflammations of various types. The active composition advantageously contains an organic carboxylic acid to adjust the pH to <6. The invention is explained below using examples. BRIEF DESCRIPTION OF THE CHARACTERS

[0088] Study to evaluate the biofilm-inhibiting and biofilm-destroying activity using the example of application composition A.

[0089] The aim of this study was to evaluate the biofilm inhibiting and biofilm destroying activity of an ointment against Staphylococcus aureus To achieve this goal, a In vitro model system that uses the wounds in vivo imitates the conditions found. a) Materials and methods: 1. Strains and culture conditions

[0090] Staphylococcus aureus Mu50 was grown aerobically on Luria Bertani (LB) agar (BD, Sparks, MD) at 37°C. S. aureus Mu50 is a methicillin-resistant S. aureus strain. 2. Tested product

[0091] Ointment A (i.e., application composition A) was assessed by spreading approximately 250 mg aliquots on the artificial dermis. 3. Production of the artificial dermis

[0092] The artificial dermis consists of an upper layer and a lower layer. The upper layer is a chemically cross-linked, spongy hyaluronic acid (HA) layer and was prepared as previously described with minor modifications (Mineo et al., 2012). Sodium hyaluronate powder (1.20–1.80 MDA, Lifecore biomedical, Minneapolis, MN, US) was dissolved in distilled water (DW) at a concentration of 1.5% (pH 6.8). This high-molecular-weight HA (HMWA) solution was adjusted to a pH of 3.5 with a dilute HCl solution. Ethylene glycol diglycidyl ether (EX810; Sigma Aldrich, Bornem, Belgium) was used as a cross-linking agent for HA molecules. An aqueous EX810 solution was added dropwise to the HMW HA solution with vigorous stirring (weight ratio of HA to EX810: 5:1). This mixed solution was poured into a freeze-drying container, stored at 4°C overnight, frozen at -80°C, and then freeze-dried.This sponge was thoroughly rinsed with DW to remove any free cross-linking agent, then frozen at -80°C and freeze-dried to obtain the spongy cross-linked HMW-HA layer (cHMW-HA). The bottom layer is a spongy layer composed of HA and collagen (Col). HA powder was dissolved in DW at 1% concentration to prepare HMW-HA solution (pH 6.8). Separately, HA (1%) was autoclaved at 120°C for 1 h to obtain a partially hydrolyzed low-molecular-weight HA (LMW-HA) solution. A collagen solution containing 0.1% Col was prepared and heated at 50°C for 10 min to obtain a heat-denatured collagen solution. The above three solutions were mixed and then adjusted to pH 7.5 with NaOH. This mixed solution was poured into a freeze-drying container in which the spongy cHMW-HA layer was immersed. The combined product was stored at 4°C overnight, then frozen at -80°C and freeze-dried to obtain a bilayered spongy sheet. Both sides of the spongy sheet were irradiated with a UV lamp for 20 minutes to cross-link the collagen molecules. This spongy sheet was sterilized at 100°C for 1 hour to obtain a sterile artificial dermis (AD). 4. Chronic in vitro wound model to assess the inhibitory and biofilm-destroying effect of the ointment

[0093] Biofilms were cultured in a chronic pathogenic wound biofilm model. An overnight culture of the tested strain was pelleted, washed, resuspended in physiological saline (PS), and diluted to 106 CFU ml -1 (colony forming units per milliliter). The AD was soaked in 0.5 ml of chronic wound biofilm medium (Bolton Broth with 50% plasma and 5% hemolyzed freeze-thaw horse blood and 10 U / ml heparin) and plated into the wells (English wells ) of a 24-well microtiter plate (TPP), and 10 µl aliquots of the cell suspension were spotted onto each AD. Each well (ie, each well ) 0.5 ml of chronic wound biofilm medium was added. To assess the biofilm-inhibiting effect of the ointment, 250 mg aliquots were applied to the AD immediately after inoculation, and the biofilm model was incubated for 24 h at 37°C to allow biofilm formation on the AD.

[0094] To assess the biofilm-destroying activity of the ointment, the biofilm model was incubated for 24 h at 37°C to allow biofilm formation on the AD. After biofilm formation, the medium was removed, the biofilms were washed with PS, fresh medium was added, and the ointment was applied to the biofilm. 5. Plating and data analysis

[0095] In both types of experiments, biofilms were rinsed after PS treatment to remove the treatment. The AD was placed in 10 ml of PS. Sessile cells were removed from the AD with three cycles of vortexing (30 s) and sonication (30 s; Branson 3510; Branson Ultrasonics Corp., Danbury, CT), and the number of CFU / AD ( / ml) was determined by plating the resulting suspensions. The normal distribution of the data was tested using the Shapiro-Wilk test. The data were analyzed by an independent sample t-test. Statistical analysis and linear regression analysis were performed using SPSS software, version 22.0 (SPSS, Chicago, IL, USA). b) Results:

[0096] The biofilm inhibiting and biofilm destroying effect of the ointment against S. aureus Mu50 is in Fig. 1, Table 1 and Table 2. The ointment showed a low, yet significant (p<0.01) biofilm-inhibiting effect. Treatment with the ointment during biofilm formation resulted in a 40% reduction in the number of CFU / AD. In addition, the ointment showed moderate biofilm-disrupting activity. Treatment of a mature biofilm with the ointment resulted in an 82.8% reduction in the number of CFU / AD. Table 1. In the biofilm inhibition assay, the product resulted in a significant (p<0.05) reduction in biofilm formation compared to the untreated control (labeled CTRL) (* in Fig. 1 ). In the biofilm disruption assay, the product resulted in a significant (p>0.005) reduction of a mature biofilm compared to the untreated control (labeled CTRL) (** in Fig. 1 ). Example 9: Multicenter, open-label, randomized clinical trial

[0100] A multicenter, open, randomized clinical study was conducted to evaluate and compare the efficacy and safety of the agent according to the invention in the form of a gingival paste dressing according to application composition A during a test phase I and in the form of a gingival brushing solution prepared from application composition B during a test phase II, whereby both forms of the agent according to the invention were used in subjects suffering from chronic periodontitis as an adjunct application after prior scaling and root planing (SRP). 9.1 . Subject descriptions:

[0101] A total of 29 male and female subjects or patients (ages 30-50) suffering from chronic periodontitis and requiring SRP treatment were examined and enrolled in the study. Of these subjects, 20 subjects were randomly assigned to a treatment group and 19 subjects to a control group. 9.2. Treatment: Treatment of the treatment group:

[0102] Treated with scaling and root planing (SRP), followed by Phase I and Phase II: Phase I: Immediately after SRP treatment on subjects suffering from chronic periodontitis, gingival paste (application composition A) was applied once over the gums and gingival sulcus for 2 to 3 hours. Phase II: Phase I was followed by brushing the teeth and gums with the gingival brushing solution once daily every evening after the last meal of the day for 10 consecutive days, beginning the day after SRP treatment. The gingival brushing solution was freshly prepared every day from application composition B: one tablet (approximately 2 g) of application composition B was dissolved in approximately 15 ml of water immediately before use.

[0103] The duration of treatment, including Phase I and Phase II, was 11 days. Treatment of the control group: Treated only with scaling and root planing (SRP). 9.3. Investigation plan:

[0104] Both treatment groups were assessed on day 0, day 5 and day 11 as follows: Before SRP treatment on day 0 (or up to 5 days earlier): demographic data, physical examination, vital signs, performance parameters, on day 0 after SRP treatment (control group) or after phase I treatment (treatment group): physical examination, vital signs, performance parameters, on day 5: physical examination, vital signs, clinical periodontal parameters and performance parameters, on day 11: physical examination, vital signs, clinical periodontal parameters and performance parameters, total bacterial count and analysis, efficacy was assessed by detecting subgingival plaque.

[0105] Parameters measured in each assessment (performance parameters): Plaque Index (PI) Gingival Index (GI) Periodontal Bleeding Index (PBI)

[0106] Bacterial count of periodontophtogenic bacteria: Prophyrompnas gingivalis (Pg) Prevotella intermedoa (Pi) Aggregatibacer actinomycetemcomitans (Aa) Fusobacterium nucleatum (Fn) 9.4 Test results 9.4.1. Microbial gingival infection during the treatment period:

[0107] Control group: 66.7% were infected at the start of treatment. Of these 66.7%, 100% were still infected after 11 days.

[0108] Treatment group: 60% were infected at the start of treatment. Of these 60%, only 25% were still infected after 11 days.

[0109] When comparing the control group and the treatment group, Fig. 2on the left side of the graph, the percentage of subjects in each group who were infected at the start of treatment, and on the right side of the graph, the percentage of subjects in each group who were still infected on day 11.

[0110] After full treatment on day 11, a large number of subjects in the treatment group were free of infection, while the control group continued to show the same number of infected subjects as at the start of treatment. 9.4.2. Summary of plaque index progression over the treatment period:

[0111] When comparing the control group and the treatment group, Fig. 3 the plaque index average for each group on day 0 before SRP, on day 0 after SRP and phase I treatment, on day 5, and on day 11.

[0112] Plaque levels were assessed using the Turkesy modification of the Quiglet-Hein Plaque Index (* in Fig. 3 ) assessed.

[0113] During the 11-day treatment period, plaque levels decreased in both groups, but to a significantly lower level in the treatment group than in the control group. 9.4.3. Summary of gingival index progression over the treatment period

[0114] When comparing the control group and the treatment group, Fig. 4 the gingival index average for each group on day 0 before SRP, on day 0 after SRP and phase I treatment, on day 5, and on day 11.

[0115] The gingival index was assessed using the Loe and Silness 1963 index system for assessing the gingival condition, qualitative changes in the gingiva were recorded (** in Fig. 4 ). Marginal and interproximal tissues were scored separately on a scale of 0 to 3.

[0116] During the 11-day treatment period, the gingival level decreased in both groups, but to a significantly lower level in the treatment group than in the control group. 9.4.4. Summary of periodontal bleeding index progression over the treatment period

[0117] When comparing the control group and the treatment group, Fig. 5 the periodontal bleeding index average for each group on day 0 before SRP, on day 0 after SRP and phase I treatment, on day 5, and on day 11.

[0118] The periodontal bleeding index was measured using the Muhleman and Son 1971 score chart, which describes the sulcus bleeding (*** in Fig. 5 ).

[0119] During the 11-day treatment period, periodontal bleeding levels decreased in both groups, but to a significantly lower level in the treatment group than in the control group. 9.4.5. Halitosis sensation after 11 days

[0120] When comparing the control group and the treatment group, Fig. 6 the percentage of subjects who experienced a reduction in the sensation of halitosis for each group on day 11.

[0121] 100% of subjects in the treatment group experienced a reduction in the sensation of halitosis, while in the control group only about 40% of subjects experienced a reduction. 9.5 Safety results:

[0122] No serious adverse events were reported during the study period. No subjects were withdrawn from the study due to an adverse event. The overall test treatment was well tolerated by all study subjects. 9.6 Conclusion:

[0123] None of the subjects in the treatment group complained of irritation after applying the periodontal paste dressing. Intragroup comparisons showed that subjects in the treatment group experienced statistically significantly greater reductions in plaque index, gingival index, and periodontal bleeding index after 11 days.

[0124] The application of the periodontal paste dressing significantly reduced the bacterial load in the gums compared to the control group on day 11. A total of 9 of 11 subjects (82%) in the treatment group were successfully cured of bacterial infection after 11 days of treatment, whereas none of the subjects in the control group were cured after day 11.

[0125] After treatment, all subjects in the treatment group experienced a reduction in gingival pain, dryness, gingival or sulcus bleeding, and symptoms of infection. In summary, the use of periodontal paste dressing and gingival brushing solution as an adjunct to SRP treatment is extremely beneficial for subjects compared to those receiving SRP treatment alone.

[0126] Overall, these results from Example 9 demonstrate that the gingival paste dressing and gingival brushing solution are effective, safe, and well tolerated as an adjunct to SRP in the management of chronic periodontitis.
