Description
[0001] The present invention relates to a device for identifying the correct positioning of the femoral component in prosthetic knee surgery. More specifically, the invention relates to a device that makes it possible to identify the correct positioning of the femoral component in prosthetic knee surgery according to the flexion-extension axis of the patella, an axis that by convention is parallel to the tibiofemoral flexion-extension axis.
[0002] As is known, in prosthetic knee surgery, restoring a neutral alignment of the knee is considered one of the main characteristics of why knee surgery is successful, in order to obtain a neutral mechanical axis that passes from the center to the femoral head through the center of activation of the knee down to the center of the ankle.
[0003] The method generally used is mechanical alignment. It requires an initial femoral cut that needs to be perpendicular to the mechanical axis of the femur. The tibial resection then needs to be executed perpendicular to the mechanical axis of the tibia. The result is a knee that is generally aligned with a valgus deformity angle of 5 degrees.
[0004] The objective of the desired alignment is to create a uniform distribution of load on the new joint that is to be implanted, in order to ensure a satisfactory clinical outcome and an adequate lifetime of the implant.
[0005] Notwithstanding the above, the alignment of the knee is not a condition that is always sufficient to ensure the success of the implant for the patient.
[0006] This is also in consideration of the fact that the desired alignment often tends to alter the tension of the ligaments and so cause unwanted stresses and, in the end, pain for the patient when moving the knee.
[0007] Nowadays alternative methods of alignment have been devised (purely kinematic, restricted, reverse, functional) as solutions to the above-mentioned problems.

[0011] Within this aim, an object of the present invention is to provide a device for identifying the correct positioning of the femoral component in prosthetic knee surgery that can be used during surgery, following the distal resection of the femur.
[0012] Another object of the present invention is to provide a device for identifying the correct positioning of the femoral component in prosthetic knee surgery, which makes it possible to considerably simplify the burden of work for the surgeon, by reducing positioning errors.
[0013] Another object of the present invention is to provide a device for identifying the correct positioning of the femoral component in prosthetic knee surgery that is highly reliable, easily and practically implemented and low cost.
[0014] This aim and these and other objects which will become better apparent hereinafter are achieved by a device for identifying the correct positioning of the femoral component in prosthetic knee surgery, characterized in that it comprises an anatomical template adapted to be applied after distal resection of the femur, said template having a concave upper portion and two lateral portions separated by a cavity, each one of said lateral portions being provided with at least one hole, a stem being insertable in said cavity and fixable therein by way of a fixing element which can slide along said cavity.

[0016] With reference to the figures, the device according to the present invention, generally designated by the reference numeral 1, comprises a contoured anatomical template 2 with a concave, i.e. hollowed-out, upper portion 3, which reproduces the shape of the patellofemoral prosthetic joint; the femoral template is adapted to be attached to the distal femoral resection surface (it is made in different sizes corresponding to the prosthetic implant) and is oriented on the femoral profile corresponding to the trochlear profile and to the internal femoral condyle using a supporting tab 13 on the posterior medial (otherwise called internal) condyle.
[0017] The anatomical template 2 is further provided with a central cavity 4 through which a stem 5 passes which is adapted to be inserted in the medullary canal of the femur 20 of the patient, and the stem 5 is coupled to a fixing element 6 which is accommodated so it can slide in the cavity 4 and fixed therein.
[0018] The fixing element is provided with a through hole 15 in which the stem 5 is inserted and locked therein using a grub screw 16.

[0024] The femoral component of the prosthesis is then aligned according to the following procedure.
[0025] First an anatomical template 2 is chosen that has dimensions that are closest to those of the knee of the patient being operated on, and this template 2 is applied after the distal resection of the femur.
[0026] It is necessary first of all to reconstruct the dimensions of the internal femoral condyle where the main flexion-extension movement of the knee occurs, in order to respect, release or tension the internal collateral ligament (depending on the individual case) using a common rear reference with perfect adherence of the template 2 to the resection surface and to the corresponding rear portion of the internal femoral condyle.
[0027] The template 2 is positioned by inserting the stem 5 in the medullary canal of the femur and immobilizing the template around that medullary stem using the fixing element 6.
[0028] In substance, the stem 5 supplies a fixed and non-modifiable point (dictated by the position of the medullary canal of the femur), around which the template 2 is positioned, with the fixing element being inserted on the stem 5 and protruding from the template 2, from the end opposite to the direction of extension of the femur 20.
[0029] The fixing element 6 can slide inside and along the cavity 4 of the template 2 and therefore the template 2 is positioned around such fixing element 6 until its concave upper portion 3 is adapted to and matches up as closely as possible with the shape of the joint portion of the patellofemoral prosthetic implant on the anterior femoral plane of the femur.

Claims
1. A device (1) for identifying the correct positioning of the femoral component in prosthetic knee surgery, characterized in that it comprises an anatomical template (2) adapted to be applied after distal resection of the femur, said template (2) having a concave upper portion (3) and two lateral portions (9, 10) separated by a cavity (4), each one of said lateral portions being provided with at least one hole (7, 8), a stem (5) being insertable in said cavity (4), a fixing element (6) being also provided, said stem (5) being fixable in said cavity (4) by way of the fixing element (6) which can slide along said cavity (4).
2. The device according to claim 1, characterized in that it comprises a pair of fins (11, 12) which are adapted to be fixed to said template (2) in an upper position, laterally to said concave upper portion (3).
3. The device according to claim 1 or 2, characterized in that it comprises a positioning indication tab (13) adapted to be applied below one or the other of said lateral portions (9, 10) of said template (2).
4. The device according to one or more of the preceding claims, characterized in that said fixing element (6) is provided with a through hole (14) for the passage of said stem (5), said fixing element (6) being configured to be positioned within said cavity (4) of the template (2).
5. The device according to one or more of the preceding claims, characterized in that said stem (5) is adapted to be inserted into the medullary canal of the femur (20).
6. The device according to one or more of the preceding claims, characterized in that said template (2) has a shape that reproduces the shape of the femur (20) after it has undergone distal resection.
7. The device according to one or more of the preceding claims, characterized in that said fins (11, 12) provide a visual reference for the surgeon to assess the chosen size of the template (2) and thus of the femoral component of the prosthesis.
