MUSCLE ATROPHYTECHNICAL FIELD

[0001] The present disclosure concerns L-lactic acid and / or a polymer thereof for use in the treatment of a muscle injury and / or a disease, a disorder and / or a condition characterized by muscle atrophy. Compositions comprising L-lactic acid and / or a polymer thereof are also disclosed for said use.BACKGROUND

[0002] Muscle atrophy and decreased muscle mass in the human body may result from an injury; a condition, such as disuse, aging, lack of exercise; a disease and / or a disorder leading to adverse consequences such as muscle imbalance, weakness, falls and even death. Research on pharmacological and therapeutic solutions for addressing muscle atrophy or muscle mass loss are of great significance for improving human quality of life and health. However, so far, apart from exercise programs and nutritional supplementation, there are still no effective treatment options available to prevent or treat muscle injuries, diseases, disorders and / or conditions characterized by muscle atrophy and / or muscle mass loss / weakening. Exercise programs can be complex and time-consuming, and may not always yield the desired results. Nutritional supplementation, while helpful to some extent, has limited effectiveness.

[0003] Skeletal muscle atrophy is a debilitating consequence of many multiple chronic diseases and conditions that involve starvation. It reduces treatment options and positive clinical outcomes as well as compromising quality of life and increasing morbidity and mortality. Though considerable research has been undertaken to find the drug target and the molecular mechanisms that improve skeletal muscle atrophy, no drug was approved to treat skeletal or other muscle atrophy. Currently, the medications used in clinical practice for muscle injuries and diseases, disorders and conditions characterized by muscle atrophy and / or muscle mass loss / weakening do not directly promote muscle tissue regeneration and thus are unsatisfactory.

[0004] Muscle injury refers to the condition in which the muscle tissue is damaged or injured. It can involve partial damage to the muscle tissue or complete tearing or  rupture. Severe muscle injuries may lead to loss of muscle function, requiring appropriate treatment and rehabilitation measures to facilitate muscle repair and recovery.

[0005] Traditional beliefs suggest that poly-lactic acid (PLA) for such purposes can induce an inflammatory response while gradually generating collagen protein, which has a limited repair function. However, during this limited repair process, inflammation and / or excessive fibrosis may occur, leading to the formation of scar tissue that can affect movement and tissue functionality. Therefore, PLA has not been advised in the treatment of the aforementioned muscle injuries, and diseases, disorders and conditions characterized by muscle atrophy and / or muscle mass loss / weakening.

[0006] Despite of great efforts in this field, it still remains a challenge to provide a solution for the effective treatment of muscle injuries and diseases, disorders and conditions characterized by muscle atrophy.SUMMARY OF INVENTION

[0007] It is an object of the present disclosure to provide an agent and / or composition, that is able to reduce, or at least partially overcome, challenges in the prior art relating to the treatment of a muscle injury and / or a disease, a disorder and / or a condition characterized by muscle atrophy.

[0008] It is an object to provide an agent and / or composition for use in the treatment of a muscle injury and / or a disease, a disorder and / or a condition characterized by muscle atrophy. It is an object to provide an agent and / or composition for use in the preventive treatment of a muscle injury and / or a disease, a disorder and / or a condition characterized by muscle atrophy. It is an object to provide an agent and / or composition for use in the therapeutic treatment of a muscle injury and / or a disease, a disorder and / or a condition characterized by muscle atrophy. In particular, it is an object of the present invention to provide means for the treatment of muscle injury and / or a disease, a disorder and / or a condition characterized by muscle atrophy, wherein said treatment does not induce a significant inflammatory response.

[0009] The present inventors have surprisingly found that L-lactic acid and / or a polymer thereof, as well as compositions thereof, have an unexpected therapeutic potential in the treatment of muscle injury and / or a disease, a disorder and / or a condition  characterized by muscle atrophy. Additionally, the present inventor have found that said L-lactic acid and / or a polymer thereof, as well as compositions thereof, as disclosed herein, for use according to the present disclosure do not lead to excessive inflammation and / or fibrosis. Thus, one or more of the above mentioned objects, and other objects that are apparent to the skilled person from reading the entire disclosure, are met by the various aspects disclosed herein.

[0010] L-lactic acid and / or a polymer thereof

[0011] Thus, in a first aspect of the present disclosure there is provided L-lactic acid and / or a polymer thereof for use in the treatment of a muscle injury and / or a disease, a disorder and / or a condition characterized by muscle atrophy.

[0012] As presented in the appended Examples using an in vivo experimental model, the present inventors have found that L-lactic acid and / or polymers thereof is surprisingly beneficial in the treatment of a muscle injury and / or a disease, a disorder and / or a condition characterized by muscle atrophy. As demonstrated by the inventors, for example in Example 3, both L-lactic acid and a polymer thereof achieve the disclosed treatment effect. Accordingly, in one embodiment, there is provided L-lactic acid for use in the treatment of a muscle injury and / or a disease, a disorder and / or a condition characterized by muscle atrophy.

[0013] As exemplified below, polymers of L-lactic acid may be particularly beneficial in the treatment of a muscle injury and / or a disease, a disorder and / or a condition characterized by muscle atrophy. For example, the inventors have found that such benefit is that polymers of L-lactic acid degrade slower than L-lactic acid upon administration to a subject in need thereof. As demonstrated in the Examples, this beneficial character of polymers of L-lactic acid allows for achievement of the treatment effect as disclosed herein upon at least one administration of a polymer of L-lactic acid to a subject in need thereof. Thus in one embodiment, there is provided a polymer of L-lactic acid for use in the treatment of a muscle injury and / or a disease, a disorder and / or a condition characterized by muscle atrophy. The inventors envision that any polymer which comprises L-lactic acid is beneficial in said treatment. In one embodiment, said polymer is thus selected from the group consisting of poly-L-lactic acid, poly-D, L-lactic acid and any copolymer which comprises L-lactic acid but does not comprise D-lactic acid.

[0014] The inventors have shown, for example in Examples 1 and 2, that poly-L-lactic acid as well as poly-D, L-lactic acid, which is a copolymer of L-lactic acid and D-lactic acid, are advantageous in said treatment. Thus, in one embodiment, said polymer may be selected from the group consisting of poly-L-lactic acid and poly-D, L-lactic acid. Without being bound by theory, the inventors envision that the concentration of said L-lactic acid in said poly-D, L-lactic acid may vary and may be lower than the concentration of D-lactic acid in said poly-D, L-lactic acid. In such cases, the inventors envision, without being bound by theory, that treatment conditions, such as administration frequency and / or dose, may be adjusted for achieving the disclosed treatment effect. Thus in certain embodiments, the concentration of said L-lactic acid in said poly-D, L-lactic acid is at least about 25%, such as at least about 30%, such as at least about 35%, such as at least about 40%, such as at least about 45%, such as at least about 50%, such as at least about 55%, such as at least about 60%, such as at least about 65%, such as at least about 70%, such as at least about 75%, such as at least about 80%, such as at least about 85%, such as at least about 90%, such as at least about 95%. In another embodiment, the ratio of L-lactic acid and D-lactic acid in said poly-D, L-lactic acid is at least about 1: 5, such as wherein said ratio is about 1: 1. In a particular embodiment the concentration of said L-lactic acid in said poly-D, L-lactic acid is at least about 50%, such as about 50%. Thus, in a particular embodiment the ratio of L-lactic acid and D-lactic acid in said poly-D, L-lactic acid is about 1: 1. It is to be understood that said %corresponds to a concentration in a ratio of weight to volume (weight / weight; w / w) .

[0015] In another embodiment, said polymer is selected from the group consisting of poly-L-lactic acid and any copolymer which comprises L-lactic acid but does not comprise D-lactic acid. The inventors have demonstrated that in certain embodiments, poly-L-lactic acid and any copolymer which comprises L-lactic acid but does not comprise D-lactic acid may be advantageous for said treatment, since D-lactic acid and polymers thereof which do not comprise L-lactic acid are not proven to be effective in said treatment. Instead, as shown in the present examples D-lactic acid and polymers thereof which do not comprise L-lactic acid induce an inflammatory response, without any beneficial therapeutic effect in terms for example cross section of muscle fibers (see Figures 3 and 4, and Table 3) . Without being bound by theory, the  inventors envision that the concentration of said L-lactic acid in said copolymer which comprises L-lactic acid but does not comprise D-lactic acid may vary and may be lower than the concentration of the other, one or more, component (s) in said copolymer. In such cases, the inventors envision, without being bound by theory, that treatment conditions, such as administration frequency and / or dose, may be adjusted for achieving the disclosed treatment effect. Thus in one embodiment, the concentration of said L-lactic acid in said copolymer is at least about 5%, such as at least about 10%, such as at least about 15%, such as at least about 20%, such as at least about 25%, such as at least about 30%, such as at least about 35%, such as at least about 40%, such as at least about 45%, such as at least about 50%, such as at least about 55%, such as at least about 60%, such as at least about 65%, such as at least about 70%, such as at least about 75%, such as at least about 80%, such as at least about 85%, such as at least about 90%, such as at least about 95%. In a particular embodiment, said concentration of said L-lactic acid in said copolymer which comprises L-lactic acid but does not comprise D-lactic acid is at least about 20%, such as about 20%. It is to be understood that said %corresponds to a concentration in a ratio of weight to volume (weight / weight; w / w) .

[0016] The inventors have found that any copolymer which comprises L-lactic acid but does not comprise D-lactic acid is advantageous for said treatment due to the presence of said L-lactic acid in said copolymer. Accordingly, it is not needed that the other, one or more, component (s) in said copolymer has / have any treatment effect. In an embodiment, said copolymer thus does not comprise glycolic acid. In certain embodiments, said polymer is selected from the group consisting of poly-L-lactic acid, any copolymer comprising L-lactic acid and ethylene glycol, any copolymer comprising L-lactic acid, ethylene glycol and hydroxyacetic acid, any copolymer comprising L-lactic acid and taurine, any copolymer comprising L-lactic acid and chitosan and any copolymer comprising L-lactic acid and ε-caprolactone; such as the group consisting of poly-L-lactic acid and any copolymer consisting of L-lactic acid and ethylene glycol. In one embodiment, said polymer is a copolymer consisting of L-lactic acid and ethylene glycol. As demonstrated in Example 4, a copolymer consisting of L-lactic acid and ethylene glycol is beneficial in the treatment as disclosed herein. In certain embodiments, the concentration of said L-lactic acid in  said copolymer is at least about 5%, such as at least about 10%, such as at least about 15%, such as at least about 20%, such as at least about 25%, such as at least about 30%, such as at least about 35%, such as at least about 40%, such as at least about 45%, such as at least about 50%, such as at least about 55%, such as at least about 60%, such as at least about 65%, such as at least about 70%, such as at least about 75%, such as at least about 80%, such as at least about 85%, such as at least about 90%, such as at least about 95%. In a particular embodiment, said concentration is at least about 30%, such as about 30%. As demonstrated in Example 4, in one embodiment, the concentration of said L-lactic acid in said copolymer is at least about 50%, such as at least about 55%, such as at least about 60%, such as at least about 65%, such as at least about 70%, such as at least about 75%, such as at least about 80%, such as at least about 85%, such as at least about 86%. In one particular embodiment, said concentration of said L-lactic acid in said copolymer is about 86%. It is to be understood that said %corresponds to a concentration in a ratio of weight to volume (weight / weight; w / w) .

[0017] In a particular embodiment said polymer is poly-L-lactic acid. As demonstrated by the inventors in the Examples, poly-L-lactic acid is particularly beneficial in said treatment. It is to be understood that poly-L-lactic acid is a homopolymer of L-lactic acid, i.e. the concentration of L-lactic acid in said polymer is 100%. The inventors have concluded that the higher the concentration of said L-lactic acid in said polymer the better treatment effects may be attained.

[0112] The biceps brachii muscle tissue was fixed using fresh 4%paraformaldehyde solution and subsequently embedded in paraffin. The paraffin-embedded tissue was sectioned into 4-8μm thick slices, and the tissue sections were mounted on glass slides for staining. Hematoxylin and Eosin (H&E) staining or Masson's staining were performed according to standard protocols.

[0193] 8-week-old Sprague-Dawley rats were kept in temperature-controlled room (22±2 ℃) with a 12-hour dark-light cycle for 7 days of acclimatization. After 7 days of acclimatization, on day 8, the left hind limbs of the animals were immobilized with plaster casts to maintain relaxation of the muscles in the left hind limbs, thereby inducing muscle atrophy of these muscles. After 18 days of such physical fixation (on day 25) , the immobilization was stopped and the plaster was removed. After removal of the plaster each animal received a treatment of the gastrocnemius and brachialis muscles of the left hind limb as indicated in Table 5. Accordingly, group A2 received one intramuscular injection of a single dose of physiological saline solution in a volume equivalent to the volume of a single dose of compositions in groups B2, D2 and E2. Group B2 received one intramuscular injection of a single dose of a composition comprising microspheres of a PLLA homopolymer and group C2 received a subcutaneously implanted scaffold (acuboid) comprising a PLLA homopolymer. Single dose administrations and scaffold implantation were performed after the removal of the physical fixation (on day 25) . Groups D2 and E2 received intramuscular injection of a composition comprising either L-LA (group D2) or D-LA (group E2) repeatedly, i.e. daily one injection of a dose of 3 mg for 14 days.
