Abstract

A device comprising a series of telescopic sleeves, each with a guide sleeve at one end that fits into a suitable bone socket, called a guide socket, and a tube at the other end. A second configuration, called a sliding guide, consists of a support (vertical) perpendicular to another support connecting it to a guide sleeve, and a sliding piece with a double tube. One tube is slid into the shank of the drill, and the other is slid into the support (vertical). Regardless of its configuration, the device can, while remaining aligned with an implant, be attached to a guide rail or any other oral template to allow for precise drilling of the implant well. Description

Title of the invention: Telescopic and / or Sliding Surgical Guide in Dental Implantology.

[0001] The present invention relates to the field of dental implantology and describes a device acting as a surgical guide. It allows for precise drilling of the implant well intended to receive a specific implant. In a first configuration, it consists of a series of telescopic sleeves (1) of variable heights and widths, and in a second configuration, of a sliding system.

[0002] They each have at one end a guide sleeve, called a guide sleeve (2) and at the other end a tube (4), [Fig.1]. The latter is placed between the edge of the head of a contra-angle (dentist's drill), on the side of insertion of a drill and the boundary between the drilling section and the shank of this drill.

[0003] In an endosseous configuration, the Guide Sleeve (2) is housed in a suitable intraosseous recess made at the location intended for the implant. The tube (4) is slid into the shank or tail of a dental drill. It is inserted into the head of a contra-angle handpiece (dentist's drill), which clamps the tube (4) between the head of the contra-angle and the drill's cutting edge. The tube (4) has a height equal to or less than the distance between the edge of the contra-angle handpiece, on the side where the drill is inserted, and the boundary between the drill's cutting edge and its shank. This height is the distance between the two dashed lines shown in [Fig. 2]. The internal diameter of this tube (4) is such that the drill's shank has no lateral play during rotation. Its height is such that it prevents or limits vertical play.

[0004] All the fixing rods or shanks of commercially available drills have the same diameter so that they can be inserted into the head of all commercially available contra-angles.

[0005] The telescoping may incorporate a spring mechanism.

[0006] Some stop mechanism(s) allows the telescoping stroke and therefore the drill stroke to be limited.

[0007] The operating mode is as follows:

[0008] A drill bit, from any commercially available implant system, is inserted shank-first into the telescopic sleeves (1) up to the tube (4) through which it passes to be fixed in the head of the contra-angle handpiece. (This procedure is performed with all drill bits necessary for creating the implant well. Regardless of the configurations described in this patent, the fixing of the drill bits in the head of the contra-angle handpiece is always done by passing the shank of the drill bit through the tube (4) in the direction of drill bit insertion, so that the tube (4) is wedged between the head of the contra-angle handpiece and the boundary between the drilling section of the drill bit and the shank of the same drill bit).

[0009] The Guide Sleeve (2) of the device is inserted into a bone housing (3) adapted to the morphology of the guide sleeve and whose axis is that of the implant, determined by the scanner by the practitioner.

[0010] After this installation, drilling is carried out.

[0011] In an extraosseous configuration, the bone cavity is replaced by an identical cavity in a buccal template. The device can be free or fixed to the buccal template. The drilling procedure is the same as for the endoosseous configuration.

[0012] In a second configuration, the device is called a sliding surgical guide and it consists of a double-tube sliding piece [Fig.4] and a support (6) of any shape allowing the sliding piece to slide in this support (6).

[0013] The support (6) is positioned perpendicular to a support (8) connecting it to the guide sleeve (2). The support (6) is parallel to the axis of the implant drilling and offset laterally from the drilling zone; its free end is oriented in the opposite direction to the drilling direction. The support (6) is shown in [Fig. 3] as a vertical rod of arbitrary cross-section.

[0014] The sliding part [Fig.4] consists of a support (7), of arbitrary shape, connecting two tubes arranged perpendicularly to it. One is identical to the tube (4) and the other tube (5) is of such shape and dimensions that it slides without friction in the support (6) [Fig.3].

[0015] The slide guide may have more than one support (6), the sliding part then having more than one tube (5).

[0016] In a third configuration the telescopic system and the sliding system are combined and form a single assembly.

[0017] Regardless of its configuration, the device can be endo-osseous or extra-osseous. In the latter case, it is juxta-osseous or juxta-gingival.

[0018] Regardless of its configuration, when the device is juxta-osseous or juxta-gingival, it is fixed to a splint or any other oral template.

[0019] Operating mode of the second configuration:

[0020] The guide sleeve (2) is placed in a bone guide slot (3) or in an oral template. The shank of a drill bit is passed through the tube (4) to fix it to the contra-angle handpiece, and then the tube (5) is placed in the holder (6). The tube (5) slides smoothly within the holder (6), guiding the drilling along the correct implant axis, as determined by the CT scan. The tube (5) can have a variable height to adjust the drill stroke to the desired drilling depth, again using the same CT scan.

[0021] The Guide Sleeve (2) can be an elastic pin possibly equipped with wings for better gripping.
