Abstract

This item is a care management sheet designed to organize the user's medical and treatment history and ensure they receive appropriate care in nursing and medical settings. As shown in the front diagram illustrating the use of this item, the right half of the item contains sections for the user's specific information, emergency contact information, a visual representation of their treatment history, allergy information regarding food and drugs, regular medication information, other precautions, information on care management-related organizations, and the name of the issuing organization. The left half of the item also contains sections for the user's specific information, emergency contact information, medical and treatment history, regular medication information, information on care management-related organizations, and the name of the issuing organization.
