Description

[Technical field]

[0001] cross reference This application claims priority from U.S. Provisional Patent Application No. 62 / 820,587, filed March 19, 2019, which is incorporated by reference in its entirety.

[0002] The present disclosure relates to the field of medical informatics. More specifically, the present disclosure relates to methods and systems for identifying subjects likely to be affected by a medical condition. [Background technology]

[0003] Physicians and other practitioners in the medical field collect and analyze medical information, such as, for example, test results provided by use of medical devices or laboratory test results, in order to establish a diagnosis for a patient. Traditionally, medical information has been stored on paper in clinicians' offices, and there have been significant concerns that computerized medicine could lead to information theft and dissemination of sensitive personal information.

[0004] In recent years, medical information is increasingly stored on computers and communicated over networks. Although the accuracy and availability of medical information has improved, progress in computer analysis of medical information has been fairly limited. The wealth of data on patients' medical histories is still underutilized. [Prior art documents] [Patent documents]

DETAILED DESCRIPTION OF THE PREFERRED EMBODIMENTS

[0014] Like numbers represent like features in the various drawings.

[0015] Various aspects of the present disclosure generally address one or more of the problems associated with underutilization of medical information related to subjects in the general population.

[0016] In general, the present disclosure introduces techniques for training an artificial intelligence (AI) system to build a classifier using information obtained by direct observation of specific criteria associated with a medical condition in a large number of subjects. The classifier is then applied to medical profiles of the general population of subjects to identify target groups of subjects likely to be affected by the same medical condition. This direct observation of the specific criteria provides so-called "ground truth" content, or a "gold standard," for reliably training the AI system.

[0017] In addition to the specific criteria, information relating to at least one secondary characteristic of the subject is also used to train the AI system to build the classifier.

[0018] An example of a positive indication that a particular criterion is directly observed in a given subject may comprise the direct detection of a particular biomarker in a given subject. A classifier may then be constructed to identify a target group of subjects based on the likelihood of the presence of a particular biomarker in the target group of subjects. A non-limiting and illustrative example of the present technology relates to the identification of a target group of subjects that may be affected by Alzheimer's Disease (AD). AD may be diagnosed in a given subject through direct observation using a positron emission tomography (PET) scan to detect and quantify the presence of amyloid in the subject's brain, amyloid being a particular biomarker associated with AD. In one embodiment, if amyloid is detected in the brain of a given subject, the particular criterion (the presence of amyloid) is said to be positive. In this embodiment, if amyloid is not detected in the brain of a given subject, the particular criterion is negative for a given subject. In another embodiment, the amount of amyloid may be quantified based on the PET scan results, and if the amount of amyloid exceeds a predefined threshold, a positive indication may be provided, and if the amount of amyloid is less than the predefined threshold, a negative indication. Whether positive or negative, the results of the PET scan are considered reliable and thus form the "ground truth" diagnosis for a given subject.

[0019] Continuing with the same non-limiting illustrative example, a multispectral retinal scan is a data-rich image in which certain features invisible to the human eye can enable the detection of certain health problems, including AD. By analyzing the features in the subject's retina, secondary characteristics may be revealed that provide indications of AD in the subject, and this analysis provides evidence of the presence of amyloid in the subject's brain. Abnormalities may be detected, for example, by using texture analysis of the retinal image. Related imaging techniques are described in International Patent Application Publication No. WO2016 / 041062A1, published March 23, 2016, and International Patent Application Publication No. WO2018 / 073784A1, published April 26, 2018, to Sylvestre et al., the disclosures of which are incorporated herein by reference in their entireties.

[0020] In the above example, subjects who receive a positive value for the specific criterion that amyloid is detected by a PET scan of the brain are placed in an experimental group, which is a subset of the general population. Other subjects who receive a negative value for the specific criterion that amyloid is not detected by a PET scan of the brain are placed in a control group, which is another subset of the general population. Images of the retina of the subjects in the experimental and control groups are acquired and processed to define secondary characteristics of these subjects. These information elements are used to train an AI system. Although not absolutely true in all cases, members of the experimental group in which amyloid is positively detected generally exhibit a specific texture in the retina. Conversely, members of the control group in which amyloid is not detected generally do not exhibit such a specific texture in the retina. The classifier built by the trained AI system can then classify other subjects from the general population as being part of a target group of subjects likely to be affected by Alzheimer's disease based on the texture of their retina. Table 1 summarizes the results for a very small population of exemplary subjects.

[0021] [Table 1]

[0022] The examples in Table 1 are simplified for ease of illustration. The general population may include a larger number of subjects, as well as the experimental, control, and target groups. Additionally, while Table 1 shows a single secondary feature, more secondary features may be used to train the AI system and / or identify subjects in the target group.

[0023] Table 1 shows the simple case where there is a one-to-one correspondence between the specific criterion (positive or negative for amyloid in the PET scan) and the secondary characteristic (a specific texture that indicates whether amyloid is present in the brain). However, other examples can be contemplated. In the case of a specific criterion of positivity associated with another pathology that generally corresponds to positive values of the secondary characteristics "W", "X", "Y" and "Z", this positive criterion may correspond in a particular subject to positive values of a subset of the secondary characteristics including "W", "X", and "Y", and in another particular subject to positive values of another subset of the secondary characteristics including "W", "Y", "Z". The training of the AI system improves as the number of subjects in the experimental and control groups increases and as the number of secondary characteristics of the subjects increases.

[0024] It has been observed that PET scans may also reveal the presence of tau protein in the subject's brain. Detection of tau protein in the subject's brain may also indicate that the subject has AD. Table 1 can be redrawn by replacing the term "amyloid" with "tau" or "amyloid and / or tau."

[0025] In one embodiment, instead of assigning (or not assigning) subjects from the general population to a target group as likely (or not) affected by AD, the classifier may assess the amyloid status of one or more subjects or all subjects in the general population. The assessed amyloid status may be indicated as positive or negative in the global database 104 in the medical profile of the subject undergoing the assessment. It is also contemplated to provide a numerical value of the assessed amyloid status for each subject in the general population, similar to the quantified value obtained from a PET scan.

[0026] While AD and texture analysis of retinal images are described in the examples above, other examples presented below relate to other sources of ground truth information for defining positive and negative information for specific criteria of other pathologies, and further examples relate to other sources of information relating to other secondary characteristics of subjects in the general population.

[0027] Referring now to the drawings, Figure 1 is a general architecture diagram of a network 100 adapted to obtain and process medical information, according to one embodiment. The network 100 includes a patient management platform 102 comprising a global database 104. The network 100 also includes an artificial intelligence system 106, a biomarker repository 108, and multiple medical modalities. The global database 104 comprises medical profiles of a population of subjects known by the patient management platform 102. The AI system 106 may comprise a machine learning system or a deep learning system.

[0028] In one embodiment, the network 100 uses selectable and specific features present in the multispectral retinal eye scan. To this end, one of the medical modalities includes a retinal eye scanning system 110 located on-site, for example in an ophthalmology clinic. The retinal eye scanning system 110 provides image features and anatomical features of the patient's eye. The retinal eye scanning system 110 is used by medical professionals assisted by ophthalmic technicians to obtain images of the patient's retina. Other medical modalities include a pool of sources of various types of auxiliary data, such as, for example, a source of digital medical survey (DMS) results 112, a clinical research information repository 114, an electronic medical record (EMR) 116, and a portal 118 (e.g., an intelligent mobile terminal) that transmits signals, measurements, and other information elements from other devices, including sensors and wearable devices. For example, but not limited to, DMS results may include cognitive test results, general health survey information, etc. Clinical research information may include a list of specific medications prescribed to the patient, results of specific tests, etc. The EMR may include patient information such as gender, age, lifestyle habits, medications consumed by the patient, genetic information, etc. Information obtained from wearable devices may include signals from heart monitors, sleep monitors, sensors attached to shoes to detect balance problems of the patient, etc. Inclusion of additional auxiliary data sources into the network 100 is also contemplated.

[0029] The patient management platform 102 defines cohorts 120, 122, and 124 of subjects who have undergone tests that provide "ground truth" information in the form of positive or negative values for a particular criterion for one or more particular medical conditions. Although all three cohorts 120, 122, and 124 are shown, the actual number of cohorts may be fewer or more. In this context, the positive value may be based on a binary detection of the particular criterion or on a measurement value exceeding a predefined threshold value of the particular criterion. When a measurement value is available for a particular criterion corresponding to a given medical condition, a gradation of severity of that medical condition may be determined for subjects in the cohort of subjects.

[0030] Information related to these cohorts is transmitted to AI system 106. AI system 106 is trained using this information and secondary characteristics of subjects in cohorts 120, 122, and 124 to learn to identify biomarkers that, when present in some subjects, reveal the likelihood that the subjects are affected by the medical conditions associated with those biomarkers. AI system 106 stores information about the identified biomarkers in biomarker repository 108.

[0031] The AI system 106 may extract secondary features from relevant information, for example, multispectral medical images such as retinal scans. In one embodiment, the deep learning system uses convolutional neural networks or equivalent techniques. Multispectral medical images have a high-dimensional structure that includes a combination of spatial and spectral information. The deep learning system is trained, typically using a large number of multispectral medical images as training samples to build a classifier. The AI system 106 first extracts key features from the multispectral medical images. Key features that are significantly correlated with a particular condition are identified. If the classifier is built as a gradient boosted tree, multivariate modeling may be performed.

[0032] The AI system 106 builds a classifier 126 that can be applied to the global database 104 to identify specific subjects within a population of subjects that may be affected by a given medical condition based on the presence of a given biomarker. In a non-limiting embodiment, the AI system 106 uses key features extracted from the multispectral medical images to build the classifier 126, which can learn to detect the given biomarker. The classifier 126 can be scaled up as new features are extracted and provided to the AI system 106. As a result, a medical report 128 is issued by the AI system 106 for the specific subject. The medical report 128 can confirm the presence of the given biomarker in the specific subject, and the subject is then marked in the global database 104 as a member of a target group of subjects that may be affected by the given medical condition.

[0033] The following presents non-limiting examples of applications of the network 100. Some inputs of data into the patient management platform 102 may relate to subjects (patients) participating in clinical studies. Other data inputs into the patient management platform 102 may relate to subjects in the general population who would benefit from advances in the detection of various medical conditions, e.g., specific measures of detection of various biomarkers.

[0034] This embodiment involves the transfer of a subject's retinal eye scan from a retinal eye scanning system 110 to a patient management platform 102. The patient management platform 102 handles management of the subject's identity. Data related to this subject, and other subjects who are authorized to access their data, can be transferred to the patient management platform 102 from a source of DMS results 112, from a clinical research information repository 114, from an EMR 116, and from a portal 118, for example with the aid of an intelligent mobile terminal.

[0035] Controlled cohorts 120, 122, 124 of subjects with known specific health conditions (e.g., subjects with various retinal conditions) are provided to build and train the classifier 126 using algorithms executed by the AI system 106. The AI system 106 may fetch information about specific subjects from the global database 104 through queries, allowing the AI system 106 to identify and separate subjects that share a specific condition to create ground truth information for building the classifier 126. Training the AI system 106 may be an iterative process, such that the performance of the classifier 126 is improved as new and revised data of current and new subjects is added to the global database 104.

[0036] Once the trained AI system 106 has constructed the classifier 126, retinal eye scans of new subjects may be obtained and processed by the classifier 126 to detect eventual indications of the presence of different biomarkers in the retinas of these subjects. Subjects whose information was not used to train the system 106 for a particular biomarker may benefit from the present technique to detect evidence of the presence of that particular biomarker. As the global database 104 grows, new biomarkers can be added to the biomarker repository 108 and become available to the general population of subjects, so the present technique is scalable. The presence or absence of a particular biomarker provides valuable information in the diagnosis of various ocular or systemic diseases that manifest in the eye, such as, for example, Alzheimer's disease, Parkinson's disease, multiple sclerosis, diabetes, vascular dementia, dementia with Lewy bodies, amyotrophic lateral sclerosis (ALS), and some specific cardiovascular diseases. Subjects identified as likely to be affected by a given condition are identified as members of the target group for the given condition. These subject medical reports may be sent for storage in the global database 104 of the patient management platform 102, the medical reports carrying information about the eventual presence of certain biomarkers. Medical professionals may access these medical reports through logging into the patient management platform 102.

[0037] The information contained in the network 100 is evolutionary in the sense that the amount and accuracy of the information stored in the global database 104 increases as new subjects are added to the population stored in the global database 104 and new information about existing subjects in the population is added. In light of this growth, the classifier 126 may be updated and improved via retraining of the AI system 106 as the growth provides additional information of the type used to initially build the classifier 126 and / or as the growth provides additional information related to the subjects whose information was used to build the classifier 126. New specific criteria (e.g., previously undetected biomarkers) may be identified and new cohorts may be defined for the new condition. The AI system 106 may be trained using this further information to build new classifiers 130, 132 for the new condition. As new classifiers 130, 132 are built for the new condition, they may be applied to the existing population of the global database 104 in light of identifying new target groups of subjects from among the population that may be affected by the new condition. Although FIG. 1 shows three classifiers 126, 130, and 132, the actual number of classifiers may be fewer or more.

[0038] 2a and 2b are sequence diagrams illustrating operations of a method for identifying a subject likely to be affected by a medical condition, according to one embodiment. In Fig. 2a and 2b, a sequence 200 comprises a number of operations, some of which may be performed in a variable order, some of which may be performed simultaneously, and some of which are optional.

[0039] Starting with FIG. 2a, the global database 104 stores medical profiles of a number of subjects in an operation 210. In an operation 220, test results are received at the patient management platform 102 for each of a plurality of subjects forming a subset of the population of subjects. The test results are received from one of the medical modalities of the network 100. For example, the test results may be obtained via a PET scan, a magnetic resonance imaging (MRI) scan, a computed tomography (CT) scan, an angiography scan, an x-ray, an ultrasound test, an optical coherence tomography test, an endoscopy test, a confirmed clinical diagnosis by a medical practitioner, a blood test, a visual test from a medical device, or a test providing an electrical signal measurement from a medical sensor. An experimental group including one or more subjects having a positive indication for a particular criterion related to a medical condition in the medical profile is defined in the global database 104 in an operation 230. The operation 230 may comprise a suboperation 235 in which a given subject is assigned as part of the experimental group if the test results of the given subject provide a positive indication for the particular criterion. A control group including one or more subjects having a negative indication for a particular criterion related to a medical condition in their medical profile is defined in the global database 104 in operation 240. If the test results of a given subject provide a negative indication for the particular criterion, operation 240 may comprise a suboperation 245 in which the given subject is assigned as part of the control group.

[0040] 2b, the AI system 106 is trained in operation 250 using the specific criteria and one or more secondary characteristics of the subjects in the experimental and control groups to build a classifier of the medical condition. The classifier is then used in operation 260 to extract a target group including one or more subjects from the population of subjects in the global database 104. In operation 270, the medical profile of each subject in the target group of subjects in database 104 is marked as potentially affected by the medical condition.

[0041] In one embodiment, the target group may be created to exclude subjects in the experimental group, since the subjects in the experimental group are known to be affected by a medical condition. Similarly, the classifier is not expected to designate subjects in the control group as part of the target group, since the subjects in the control group are known to be free of a medical condition. For members of the experimental group, a mark that these members are "likely to be affected by a medical condition" may be interpreted as redundant and ignored. For members of the control group, such a mark may be inaccurate and may result from a corner case or insufficient training of the AI system 106, in which case it may simply be ignored.

[0042] The sequence 200 may be executed multiple times in the network 100. In a non-limiting example, the medical condition described in the above description of the sequence 200 may be AD, the specific criteria may relate to positive or negative detection of amyloid via PET scans, and at least one secondary characteristic may relate to texture analysis of the retina of various subjects. The same sequence 200 may be executed with a view to identifying subjects who may suffer from other medical conditions, such as Parkinson's disease, multiple sclerosis, diabetes, or certain cardiovascular diseases, all of which have symptoms that may be detected in the subject's eyes. Diabetes may be diagnosed as positive through evaluating blood glucose levels with a view to a predefined criteria, such as a fasting plasma glucose level above 7.0 mmol / L. Currently, there are no known clear tests for diagnosing conditions such as Parkinson's disease, multiple sclerosis, dementia with Lewy bodies, vascular dementia, and amyotrophic lateral sclerosis (ALS). Clinical diagnosis by practitioners based on findings regarding a number of clinical factors may be used to form a positive or negative indication (ground truth) for Parkinson's disease or multiple sclerosis. The current state of research suggests that detection of alpha-synuclein symptoms in a subject's blood may ultimately be used to positively identify Parkinson's disease and dementia with Lewy bodies. Diabetes, Parkinson's disease, multiple sclerosis, vascular dementia, dementia with Lewy bodies, and amyotrophic lateral sclerosis (ALS) are all known to cause artifacts that can be detected through the use of retinal image analysis, providing information related to secondary characteristics used to train AI systems106.

[0043] Other sources of ground truth information for defining positive and negative information for specific criteria for other medical conditions may include, for example, blood test results, diagnoses from medical practitioners, MRI scans, angiography scans, x-rays, ultrasound, optical coherence tomography, endoscopy, visual inspection from medical devices, and electrical signal measurements from medical sensors, such as wearable sensors. Depending on the specific medical condition, different medical modalities may provide positive and negative values for different types of specific criteria and / or provide different types of secondary features. The sequence 200 may be executed at least once for each distinct specific criterion to allow the AI system 106 to build corresponding distinct classifiers and identify distinct target groups for distinct medical conditions.

[0044] As mentioned above, certain secondary characteristics may be identified through imaging of biological tissue, for example, the subject's retina. FIG. 3 is a sequence diagram illustrating operations of a method for processing images of biological tissue, according to one embodiment. In FIG. 3, a sequence 300 comprises a number of operations, some of which may be performed in a variable order, some of which may possibly be performed simultaneously, and some of which are optional. In operation 310, the patient management platform 102 receives an image of the biological tissue of the particular subject from one of the medical modalities of FIG. 1 and stores the image of the biological tissue of the particular subject in the medical profile of the particular subject in the global database 104. In a non-limiting example, the biological tissue is the retina of the particular subject, and the image is received at the patient management platform 102 from a retinal eye scanning system 110.

[0045] A texture analysis of the image of the biological tissue of the particular subject may be performed in operation 320 to define one or more of the secondary characteristics of the particular subject in operation 340. Alternatively, or in addition to the texture analysis performed in operation 320, various anatomical features of the image of the biological tissue may be analyzed in operation 330 in consideration of defining one or more of the secondary characteristics of the particular subject in operation 340. Non-limiting examples of such anatomical features that may be identified in operation 340 include one or more of optic disc morphology, vascular morphology, arterial-venous ratio, vascular diameter, vascular fractal dimension, vascular tortuosity, drusen, exudate, branching coefficient, branching angle, branching asymmetry, arteriovenous nicking, retinal structure thickness, area of foveal avascular zone, vascular capillary density, blood perfusion density, optic disc disk diameter, optic disc disk area, optic disc cup diameter, optic disc cup area, eye movement, and hemorrhage.

[0046] In one embodiment, one or more of the operations of sequence 300 may be performed by a deep learning system included in AI system 106.

[0047] Other secondary characteristics that are part of the medical profile of a particular subject may be provided to the patient management platform 102 and further to the AI system 106 by one of the medical modalities shown in FIG. 1. In a non-limiting example, such secondary characteristics of a particular subject may include one or more of an indication of the presence or absence of a particular gene or group of genes in the genotype of the particular subject, the age of the particular subject, the sex of the particular subject, the height of the particular subject, the weight of the particular subject, a list of medications prescribed for the particular subject, the blood pressure level of the particular subject, the cholesterol level of the particular subject, the blood glucose level of the particular subject, images obtained by structural neuroimaging, lifestyle factors, body mass index, a set of administrative claims data, and a cognitive function assessment, e.g., an assessment obtained for the particular subject from a cognitive test. It is contemplated that additional types of medical information that may be expressed in a logical form (true or false, or positive or negative) or that may be expressed in a numerical form may be used to define the secondary characteristics of subjects within the population of the global database 104.

[0048] As mentioned above, the AI system 106 may be trained with further information to build additional classifiers 130, 132. For example, FIG. 4 is a sequence diagram illustrating the operations of a method for identifying good responders to a drug of interest, according to one embodiment. In FIG. 4, the sequence 400 comprises a number of operations, some of which may be performed in a variable order, some of which may possibly be performed simultaneously, and some of which are optional. The sequence 400 may include members of an experimental group defined in operation 230 (FIG. 2a) based on their positive indications for a particular criterion of a pathology, and members of a target group identified in operation 260 (FIG. 2b) and marked as likely to be affected by the pathology in operation 270 (FIG. 2b). In a non-limiting example, subjects in the experimental group are affected by AD, and their diagnosis is obtained via a PET scan revealing the presence of amyloid in the brain, and textures indicative of the presence of amyloid in the brain are detected in the retinas of subjects in the target group. Several of these subjects have been prescribed the drug of interest, leading to an improvement in the condition of some subjects, but not others.

[0049] A first subset of the experimental and target groups of subjects who received the drug of interest and responded positively to the drug of interest is identified in operation 410. Conversely, a second subset of the experimental and target groups of subjects who received the drug of interest and responded negatively to the drug of interest is identified in operation 420. The AI system 106 is further trained in operation 430 using one or more secondary characteristics of the subjects in the first and second subsets to construct a drug-related classifier for the drug of interest. The drug-related classifier can be one of the classifiers 130 or 132, rather than the classifier 126 constructed in operation 250 (FIG. 2b). In operation 440, the drug-related classifier is used to extract a drug-related target group from the population of subjects in the global database 104, the drug-related target group including one or more subjects, optionally excluding the subjects in the first and second subsets. Alternatively, in operation 440, the drug-associated classifier may be applied to subjects in the experimental and target groups only because subjects in these groups are likely to benefit from receiving the drug of interest for the treatment of a medical condition applicable to sequence 200 (FIGS. 2a and 2b). Then, in operation 450, each subject in the drug-associated target group of subjects is marked in the global database 104 as a potential candidate for responding positively to the drug of interest.

[0050] It has been observed that people affected by a given medical condition are often affected by another medical condition. The relationship between medical conditions may be observed empirically by physicians, but may still be difficult to predict. Another example of further information that may be used to train the AI system 106 is shown in FIG. 5, which is a sequence diagram illustrating the operations of a method for identifying a subject group that may be affected by a new medical condition, according to one embodiment. In FIG. 5, the sequence 500 comprises a number of operations, some of which may be performed in a variable order, some of which may be performed simultaneously, and some of which are optional. As in FIG. 4, the sequence 500 may include members of the experimental group defined in operation 230 (FIG. 2a) based on their positive indications for a particular criterion of the medical condition, and members of the target group identified in operation 260 (FIG. 2b) and marked as likely to be affected by the medical condition in operation 270 (FIG. 2b).

[0051] In operation 510, a new medical condition is detected in a third subset of the experimental group of subjects. The new medical condition is associated with a new specific criterion. As a non-limiting example, the new medical condition may be cardiovascular disease, where the given subject has suffered a heart attack, and the new specific criterion may be related to arteriosclerosis. The AI system 106 is further trained in operation 520 using one or more secondary characteristics of the subjects in the third subset, one or more secondary characteristics of the subjects in the control group, and the new specific criterion to build a new classifier. In one embodiment, if a large number of subjects in the third subset of the experimental group of subjects with the new medical condition is deemed sufficient to further train the AI system 106, a new classifier may be built.

[0052] The new classifier constructed in operation 520 is different from the classifier 126 (FIG. 1) constructed in operation 250 (FIG. 2b). In the arteriosclerosis example, the new classifier is constructed to identify subjects in the experimental group who are at risk for cardiovascular disease. The new classifier is used in operation 530 to extract a new target group including one or more subjects from the population of subjects in the global database 104. Alternatively, in operation 530, the new classifier may be applied only to subjects in the experimental group, since it may identify subjects in this group who are likely to be affected by both the pathology applicable to sequence 200 (FIGS. 2a and 2b) and the new pathology. In operation 540, each subject in the new target group of subjects is marked in the global database 104 as potentially affected by the new pathology.

[0053] Returning to Figures 2a and 2b, once the sequence 200 has been executed for a given medical condition and each subject's medical profile for a target group of subjects has been marked as potentially affected by the given medical condition, other events may lead to retraining of the AI system 106 and updating of the classifier for the given medical condition. As described below in connection with Figures 6 and 7, the AI system 106 may be retrained as new information is received at the patient management platform 102 and the AI system 106. In one embodiment, the operations described in connection with Figures 6 and / or 7 may be executed each time new information is received. In another embodiment, these operations may be executed at regular intervals or based on an external trigger.

[0054] Figure 6 is a sequence diagram illustrating operations of a first method for retraining an artificial intelligence system, according to one embodiment. In Figure 6, sequence 600 comprises a number of operations, some of which may be performed in a variable order, some of which are possibly performed simultaneously, and some of which are optional. Sequence 600 concerns adding a given subject to one of the experimental and control groups for a given medical condition addressed in sequence 200 (Figures 2a and 2b).

[0055] In operation 610, the patient management platform 102, and more specifically, the global database 104, receives a new positive or negative indication for a particular criterion for a given subject of the population of subjects. The value of the indication is verified in operation 620. If the indication is positive, the given subject is added to a list of subjects in the experimental group in operation 630. If the indication is negative, the given subject is added to a list of subjects in the control group in operation 640. In either case, the artificial intelligence system is retrained in operation 650 using the particular criterion and one or more secondary characteristics of the subjects in the experimental and control groups to update the disease state classifier 126. Note that at this point, either the experimental group or the control group has been modified by operations 610 to 640. Thus, there should be at least some new or modified information that may affect the retraining of the AI system 106.

[0056] In operation 660, the updated classifier is used to extract an updated target group that includes one or more subjects, optionally excluding subjects in the experimental and control groups, from the population of subjects in the global database 104. The medical profile of each subject in the updated target group of subjects is marked in the global database 104 as potentially affected by the medical condition in operation 670.

[0057] In one embodiment, the sequence 600 may be performed once at operations 610 through 670. In another embodiment, the sequence 600 may be temporarily paused after operations 630 or 640, so that the AI system 106 is retrained at operation 650 periodically, or when a sufficient number of subjects have been added to one of the experimental or control groups by repeating operations 610 through 640. Other modifications of the sequence 600 that result in retraining the AI system 106 are also contemplated. For example, the sequence 600 may be adjusted such that the AI system 106 is retrained more frequently when the number of subjects in the experimental and / or control groups is less than a predetermined number of subjects.

[0058] FIG. 7 is a sequence diagram illustrating the operations of a second method for retraining an artificial intelligence system, according to one embodiment. In FIG. 7, sequence 700 comprises a number of operations, some of which may be performed in a variable order, some of which may possibly be performed simultaneously, and some of which are optional. As in the case of sequence 600 (FIG. 6), sequence 700 follows the execution of sequence 200 (FIGS. 2a and 2b). Sequence 700 concerns the change in secondary characteristics of subjects in the experimental and control groups for a given medical condition addressed in sequence 200 (FIGS. 2a and 2b).

[0059] In operation 710, the patient management platform 102, more specifically the global database 104, is notified of the detected new or modified value of one of the secondary characteristics of one of the subjects of the experimental or control group. The AI system 106 is retrained in operation 720 using the specific criteria and the new or modified value of one of the secondary characteristics of one of the subjects of the experimental or control group to update the classifier 126 of the medical condition. The updated classifier is used in operation 730 to extract an updated target group including one or more subjects from the population of subjects in the global database 104. The medical profile of each subject of the updated target group of subjects is marked in the global database 104 as possibly affected by the medical condition in operation 740.

[0060] 6, the sequence 700 is not necessarily executed all at once after operation 710. The AI system 106 may be retrained in operation 720 at various intervals or based on various conditions.

[0061] Each of the operations of sequences 200, 300, 400, 500, 600, and 700 may be configured to be processed by one or more processors, the one or more processors being coupled to a memory device. For example, FIG. 8 is a simplified block diagram of a system 800 for identifying subjects likely to be affected by a medical condition, according to one embodiment. The system 800 includes a patient management platform 102 and its global database 104, an AI system 106, a biomarker repository 108, a retinal eye scan system 110, a source of DMS results 112, a clinical research information repository 114, an EMR 116, and a portal 118. The system may also include additional medical modalities 802, 804, and 806, as well as an external database 808. The medical modalities 802, 804, and 806 may provide various types of medical information, such as, for example, PET scans, blood test results, electrical signal measurements from medical sensors, and the like. The patient management platform 102 communicates with the AI system 106, the biomarker repository 108, the retinal eye scanning system 110, the source of DMS results 112, the clinical research information repository 114, the EMR 116, the portal 118, additional medical modalities 802, 804, and 806, and an external database 808 via a communication interface 810.

[0062] In addition to the communication interface 810, the patient management platform 102 includes a processor 812 and a memory device 814. The processor 812 is the controller for the patient management platform 102 and the entire system 800. The processor 812 is operatively connected to the memory device 814, the communication interface 810, and other components of the system 800 through the communication interface 810. It is contemplated that the memory device 814 may be divided into multiple memory devices and / or storage units. Similarly, it is contemplated that the processor 812 may include multiple cooperating processors. The communication interface 810 may include multiple communication devices adapted to communicate with components of the system 800 external to the patient management platform 102, separate communication devices using separate network interface technologies, and separate communication protocols, as necessary for compatibility with these components of the system 800.

[0063] In the non-limiting embodiment shown in FIG. 8, the global database 104 is integrated into the memory device 814. The global database 104 includes partitions 816, 818, and 820 for storing a list of subjects of the experimental group, the control group, and the target group, respectively, along with their medical information. The memory 814 also includes a non-transitory computer-readable medium 822 having machine-executable instructions stored thereon for performing at least some of the operations of the sequences 200, 300, 400, 500, 600, and 700 when executed by the processor 812. Some or all of these operations may be performed by the processor 812. Alternatively, some of these operations may be performed by other components of the system 800, including, but not limited to, by the AI system 106.

[0064] In operation, the system 800 for identifying subjects likely to be affected by a medical condition may function as follows: The global database 104 stores medical profiles of a population of subjects. The communication interface 810 receives positive and negative indications of a particular criterion associated with a medical condition of a subset of the population of subjects from one or more of the medical modalities 802, 804, or 806. The communication interface 810 receives one or more secondary characteristics of the population of subjects from the retinal eye scanning system 110, from one or more of the medical modalities 802, 804, or 806, and / or from one or more of the sources of DMS results 112, the clinical research information repository 114, the EMR 116, and the portal 118.

[0065] Non-limiting examples of the one or more secondary characteristics provided by the one or more second medical modalities include, for a particular subject, results of a texture analysis of an image of the particular subject's biological tissue, results of a vascular morphology analysis of an image of the particular subject's biological tissue, an indication of the presence or absence of a particular gene or group of genes in the genotype of the particular subject, the age of the particular subject, the sex of the particular subject, the height of the particular subject, the weight of the particular subject, a list of medications prescribed for the particular subject, the blood pressure level of the particular subject, the cholesterol level of the particular subject, images obtained by structural neuroimaging, lifestyle factors, body mass index, a set of administrative claims data, the blood glucose level of the particular subject, and an indication of the presence or absence of a particular gene or group of genes in an assessment obtained for the particular subject from a cognitive function assessment, e.g., a cognitive test.

[0066] The processor 812 defines an experimental group 816 in the global database 104 that includes one or more subjects who have a positive indication for the particular criteria in their medical profile. The processor 812 also defines a control group 818 in the global database 104 that includes one or more subjects who have a negative indication for the particular criteria in their medical profile. The processor 812 provides the particular criteria and one or more secondary characteristics of the subjects in the experimental and control groups to the AI system 106 via the communication interface 810.

[0067] The AI system 106 builds a classifier 126 (FIG. 1) of the medical condition based on the provided criteria and the list of provided characteristics. The AI system 106 may store information about the provided criteria, which generally identify biomarkers, in the biomarker repository 108. The AI system 106 uses the classifier to extract a target group, including one or more subjects, from the population of subjects in the global database 104. To this end, the AI system 106 may fetch information about the subjects in the global database 104 via the communication interface 810 and the processor 812. The processor 812 receives the identification of the subjects of the target group from the AI system 106 via the communication interface 810. The processor 812 may remove any subjects of the experimental group 816 and the control group 818 from the target group before storing the remaining content in the global database 104 as the target group 820. The processor 812 also marks the medical profile of each subject of the target group 820 of subjects in the global database 104 as potentially affected by the medical condition.

The system of claim 15, wherein the one or more secondary characteristics include, for a particular subject, elements selected from results of a texture analysis of the multispectral image of the retina of the particular subject, results of a vascular morphology analysis of the multispectral image of the retina of the particular subject, and combinations thereof.

17. The method described in claim 1, wherein the systemic disease exhibiting symptoms in the subject's eyes is one of Alzheimer's disease, Parkinson's disease, multiple sclerosis, diabetes, vascular dementia, dementia with Lewy bodies, amyotrophic lateral sclerosis (ALS), and cardiovascular disease.

18. The method described in claim 7, wherein the systemic disease exhibiting symptoms in the subject's eyes is one of Alzheimer's disease, Parkinson's disease, multiple sclerosis, diabetes, vascular dementia, dementia with Lewy bodies, amyotrophic lateral sclerosis (ALS), and cardiovascular disease.

19. The system described in claim 15, wherein the systemic disease exhibiting symptoms in the subject's eyes is one of Alzheimer's disease, Parkinson's disease, multiple sclerosis, diabetes, vascular dementia, dementia with Lewy bodies, amyotrophic lateral sclerosis (ALS), and cardiovascular disease.

20. The system of claim 16, wherein the one or more secondary characteristics further include elements selected from, for a particular subject, an indication of the presence or absence of a particular gene or group of genes in the particular subject's genotype, the age of the particular subject, the sex of the particular subject, the height of the particular subject, the weight of the particular subject, a list of medications prescribed for the particular subject, the blood pressure level of the particular subject, the cholesterol level of the particular subject, the blood glucose level of the particular subject, images obtained by structural neuroimaging, lifestyle factors, body mass index, a set of administrative claims data, an assessment obtained for the particular subject from cognitive testing, and combinations thereof.
