Abstract
Description
Technical Field
[0001] The present invention relates to compositions and methods that can be used for the treatment of parasitic infestations, particularly to cutaneous ectoparasite infections.
Background Art
[0002] The term "parasitism" refers to parasitic diseases caused by animals such as arthropods (so-called mites, ticks, and lice) and parasites. Ectoparasite infections are parasitic diseases caused by organisms that mainly inhabit on the surface of the host. Examples of ectoparasite infections are scabies, body lice, head lice, ticks, and fleas.
[0003] Mites are one of the smallest arthropods. Most mite species are harmless to humans or livestock, but several species can settle on mammals and cause allergic diseases. Many types of rashes accompanied by itching are caused by mites. Among these, scabies is caused by parasitic mites such as Sarcoptes scabiei. Scabies is a contagious disease that spreads by skin-to-skin contact and sometimes by contact with contaminants. Scabies is transmitted by female scabies mites that burrow into the uppermost layer of the epidermis to lay eggs and die after 30 to 60 days. When infestation occurs, the first papules appear within 2 to 5 weeks. The main symptom of scabies is severe itching, which is particularly severe at night. Preferred areas of infestation are areas where the skin is thin (Deutsches Arzteblatt International 2016;113:757-62 / Supplementary Material).
[0004] Because these mites are localized in the stratum corneum, topical treatment with anti-scabies drugs is usually sufficient to kill the mites, mite larvae, and eggs. Permethrin is one of the drugs chosen to treat scabies because it is effective against different types of scabies regardless of the patient's age. A 5% permethrin cream is available on the market. The 5% permethrin cream contains 0.45 g of benzyl alcohol per tube, which can cause allergic reactions such as allergic contact dermatitis. Permethrin is also available on the market as InfectoPedicul® (430 mg / 100 mL), a solution for temporary treatment of hair in case of head lice. This solution contains, among other things, ethanol (96%), 2-propanol, and water.
[0005] Crotamiton is a scabies and antipruritic available as a cream or lotion for topical use only. It exists in a colorless to slightly yellowish oily form and has a faint amine-like odor. It is miscible with alcohol and methanol. Crotamiton is toxic to scabies mites. Crotamiton is available as a 10% lotion, cream, and ointment, as well as a 5% gel. Treatment takes 3–5 days to be effective.
[0006] Benzyl benzoate is another option for treating scabies. This substance exhibits good acaricidal and ovicidal activity. Commercial products containing benzyl benzoate also contain propylene glycol and cetyl stearyl alcohol, which are potential contact allergens. Benzyl benzoate emulsions are available in two concentrations, 25% and 10% (Antiscabiosum®). The emulsion is applied to the skin for three consecutive days and can only be removed by showering on the fourth day.
[0007] Demodex mites are a genus of small mites that live in or near the hair follicles of mammals. Two species of Demodex mites, Demodex folliculorum and Demodex brevis, inhabit humans and are both often called eyelash mites. Both species are mainly found on the face, near the nose, on the eyelashes and eyebrows, but can also occur in other parts of the body. Mites can be eradicated with topical insecticides such as crotamiton cream, permethrin cream, and ivermectin cream, as well as with topical or systemic metronidazole.
[0008] Lice are wingless, almost invisible insects that parasitize the head, body, or genital area, surviving by feeding on blood. They spread easily from person to person through close contact and sharing of clothing and other personal belongings. Head lice and pubic lice live directly on humans, while body lice live in clothing and bedding.
[0009] To treat head lice, non-medical shampoos and creams containing pyrethrin and piperonyl butoxide are applied for 10 minutes and then rinsed off. Medical-grade permethrin (synthetic pyrethrin), applied as a liquid or cream, is also effective.
[0010] As a medical drug, Lindan can be applied as a lotion or shampoo. This also treats lice infestations, but it is not as effective as other formulations.
[0011] All of these lice treatments are repeated for 7-10 days to kill any newly hatched lice. Lice may begin to develop resistance to the medication, making them difficult to eradicate. If lice become resistant to standard treatment, a single dose of the medication ivermectin is usually administered orally. Lice that negatively affect the eyelashes can be treated with petrolatum applied for 8-10 days, fluorescein eye drops, orally administered ivermectin, petrolatum ointment, or physostigmine ointment, or by carefully removing each louse using instruments.
[0012] One drawback of commercially available anthelmintic formulations is that they contain alcohol, which can cause further irritation and pain to the skin, especially in areas where lesions are present.
[0013] Most commercially available anthelmintic compositions are semi-solid formulations in the form of gels, ointments, or creams. However, these are difficult to distribute evenly over large areas of skin or body surface, and they do not naturally spread to hard-to-reach areas of the body, such as the back or skin folds. For example, patients with scabies often need to be hospitalized. This is because it is impossible for them to treat large areas of skin, such as the entire body surface, themselves using existing products.

[0016] [Non-Patent Document 1] Deutsches Arzteblatt International 2016;113:757-62 / Supplementary materials [Overview of the Initiative] [Problems that the invention aims to solve]
[0017] The object of the present invention is to provide an anthelmintic composition for topical application to the target skin, which can be used to treat parasitic infestations such as scabies, lice, and ticks. [Means for solving the problem]
[0018] The object of the present invention is achieved by the claims. The present invention provides an anthelmintic composition containing an anthelmintic and a semi-fluorinated alkane.
[0019] In a second aspect, the present invention provides an anthelmintic composition containing an anthelmintic and a semi-fluorinated alkane for use as a medicament, particularly for use in a method of treating parasitic infestations.
[0020] In a third aspect, the present invention also provides a method for topical application of an anthelmintic composition, which comprises applying an anthelmintic composition containing an anthelmintic and a semi-fluorinated alkane using a wipe, roll-on dispenser or spray dispenser.
[0021] In a fourth aspect, there is provided a method of treating parasitic infestations, which comprises topically applying to the skin of a subject in need thereof an anthelmintic composition containing an anthelmintic and a semi-fluorinated alkane.
[0022] In a fifth aspect, the present invention provides a kit comprising: a) an anthelmintic composition containing an anthelmintic and a semi-fluorinated alkane; and b) a dispenser selected from a roll-on dispenser, a spray dispenser and a wipe.
[0023] The inventors of the present invention have found that the anthelmintic composition can be obtained by mixing an anthelmintic and a semi-fluorinated alkane. The composition does not contain alcohol and is easily spread thinly on the skin by non-contact application using, for example, a roll-on dispenser, a wet-type wipe, or a spray dispenser, thereby simplifying the application process to the skin of the subject. The composition of the present invention further has the advantage of not leaving a residue containing fat on the skin.
[0024] When the composition of the present invention is applied by a roll-on dispenser, a spray dispenser, or a wet-type wipe, it has the further advantage that it can be easily and quickly distributed over a large or small area of the skin. The composition of the present invention is characterized in that when applied by non-contact application using a spray dispenser, a roll-on dispenser, or a wet-type wipe, it spreads naturally and can be easily and quickly distributed over areas that are difficult to reach, such as the back or skin folds.
[0025] The composition of the present invention does not contain water and does not require a preservative, a surfactant, or an additional penetration enhancer, all of which can cause pain in inflamed skin and local tolerance problems during short-term and long-term use.
[0026] The composition of the present invention can be distinguished from any cream, ointment, or foam by its ease of application and improved skin safety, especially when applied to sensitive or difficult-to-treat areas such as the scalp, face, intertriginous areas, genital area, nails, hands, and feet. This composition is designed for non-contact application using a special container and is delivered as a liquid. There is no need to rub or massage the composition into the skin, and it is possible to wear clothing a short time after applying the composition without waiting a long time for absorption into the skin. It does not stain clothing or fabrics such as bed sheets / pillows.
Mode for Carrying Out the Invention
[0027] The present invention provides an anthelmintic composition containing an anthelmintic and a semi-fluorinated alkane.
[0028] Anthelmintics are drugs used to treat parasitic diseases, such as those caused by ectoparasites and parasitic fungi. In preferred embodiments of the present invention, the anthelmintic is an ectoparasite anthelmintic. An ectoparasite anthelmintic is an anthelmintic used to treat ectoparasitic infections, i.e., a drug used to kill parasites that inhabit the body surface. Preferably, the anthelmintic is an ectoparasite anthelmintic selected from the group consisting of pyrethroids, pyrethrins, anilides, benzoates, more preferably pyrethroids, benzoates, or anilides, and most preferably pyrethroids or anilides.
[0029] Anilides (or phenylamides) are a class of chemical compounds that are acyl derivatives of aniline. Pyrethroids are synthetic chemical insecticides whose chemical structure matches that of pyrethrin, and are plant-derived insecticides derived from chrysanthemums.
[0030] In preferred embodiments, the anthelmintic is selected from permethrin, benzyl benzoate, and crotamiton. More preferably, the anthelmintic is selected from permethrin and crotamiton, and most preferably permethrin.
[0031] In a more preferred embodiment, the anthelmintic is selected from permethrin, benzyl benzoate, crotamiton, ivermectin, and lindan.
[0032] The anthelmintic may be present at a concentration of 10 w / v% or less, preferably 5 w / v% or less, relative to the total volume of the composition. In a preferred embodiment, the anthelmintic may be present at a concentration of 0.5 w / v% to 10 w / v%, preferably 0.5 w / v% to 5 w / v%, relative to the total volume of the composition. In a further preferred embodiment, the anthelmintic may be present at a concentration of 0.5 w / v% to 1.5 w / v%, preferably about 1 w / v%, relative to the total volume of the composition.
[0033] Unless otherwise specified, when used throughout this specification in relation to this composition, the term "w / v%" indicates the amount of a component of the composition (e.g., an anthelmintic) as a weight percentage of the total volume of the composition (where "w" is weight and "v" is volume). For example, 0.05% (w / v) may be understood as relating to 0.5 mg of the component in 1 mL of the composition, and 0.1% (w / v) corresponds to 1.0 mg of the component in 1 mL of the composition.
[0034] The term “semifluorinated alkane,” also referred to as “SFA” throughout this document, refers, as used herein, to a linear or branched compound comprising at least one perfluorinated segment (F segment) and at least one nonfluorinated hydrocarbon segment (H segment). Preferably, the semifluorinated alkane is a linear or branched compound comprising one perfluorinated segment (F segment) and one nonfluorinated hydrocarbon segment (H segment). Preferably, the semifluorinated alkane is a compound that exists in a liquid state within a temperature range of 4 to 40°C.
[0035] The F segment and H segment of a linear or branched hemifluorinated alkane preferably contain 2 to 10 carbon atoms independently of each other. According to a preferred embodiment of the present invention, the hemifluorinated alkane is a linear compound of the formula (F(CF2)n(CH2)mH), where n and m are integers independently selected from 2 to 10.
[0036] According to another nomenclature, a linear hemifluorinated alkane may also be called FnHm, where F represents the perfluorinated hydrocarbon segment, H represents the nonfluorinated hydrocarbon segment, and n and m are the number of carbon atoms in each segment. For example, F4H5 is used in 1-perfluorobutyl-pentane. In a preferred embodiment of the present invention, the hemifluorinated alkane is a hemifluorinated alkane of the formula F(CF2)n(CH2)mH, where n is selected from 4 to 6 and m is selected from 5 to 10. Hemifluorinated alkanes selected from the group consisting of F4H5, F4H6, F4H8, F4H10, F6H6, F6H8, F8H8, and F6H10 are more preferred. Hemifluorinated alkanes selected from F4H8, F4H5, F6H6, and F6H8 are most preferred, or the hemifluorinated alkane is F6H8. Semifluorinated alkanes selected from the group consisting of F4H4, F4H5, F4H6, F4H8, F4H10, F6H2, F6H4, F6H6, F6H8, F8H8, and F6H10 are also preferred.
[0037] In the present invention, the composition may contain a hemifluorinated alkane in an amount of about 75% (w / w) to about 99% (w / w), more preferably about 90% (w / w) to about 99% (w / w), based on the total weight of the composition. In the most preferred embodiment of the present invention, the composition contains a hemifluorinated alkane in an amount of at least 75% w / w, preferably at least 85% w / w, and more preferably at least 90% w / w, based on the total weight of the composition.
[0038] As used herein, the term “spreadability” or “spreadability” refers to the property of a composition that allows it to be distributed onto the skin. Spreadability is a measure of smoothness directly related to the coefficient of friction with respect to the contact angle of droplets of a liquid or semi-solid composition on a standardized substrate. The effectiveness of topical treatment varies depending on the patient to whom the drug composition is spread in a uniform layer in order to administer a standard dose. Therefore, spreadability is an important property of these compositions, which is related to the precise delivery of the dose to the target site, the ease of application on the substrate, and, most importantly, to consumer choice. The anthelmintic composition of the present invention exhibits outstanding spreadability. Therefore, it is particularly suitable for administration to sensitive or hard-to-reach areas of the body or skin, and / or for non-contact application to the skin.
[0039] In the present invention, the composition may contain two or more hemifluorinated alkanes, preferably a mixture of at least two hemifluorinated alkanes, more preferably a mixture of at least two hemifluorinated alkanes, and at least one of the hemifluorinated alkanes is characterized by a vapor pressure of at least 2 mmHg, more preferably at least 4 mmHg, and even more preferably at least 10 mmHg at 25°C, so that the hemifluorinated alkane can be rapidly evaporated upon application to the skin. Preferably the anthelmintic composition may contain a mixture of at least two hemifluorinated alkanes, and the hemifluorinated alkane that can be rapidly evaporated upon application to the skin is selected from F6H2, F4H4, or F4H5, preferably F6H2.
[0040] In preferred embodiments, the composition of the present invention is a liquid or semi-solid composition; that is, the anthelmintic is dissolved or suspended in a semi-fluorinated alkane. In most preferred embodiments, the composition of the present invention is a solution, such as a solution or a semi-solid solution.
[0041] In one embodiment, the present invention relates to an anthelmintic composition comprising an anthelmintic dissolved in a vehicle containing a hemifluorinated alkane. Preferably, the anthelmintic composition comprises an anthelmintic dissolved in a vehicle containing a hemifluorinated alkane, and optionally one or more excipients. More preferably, the anthelmintic composition comprises an anthelmintic dissolved in a vehicle containing a hemifluorinated alkane, and optionally one or more excipients selected from the group consisting of oily excipients and cosolvents.
[0042] The anthelmintic composition of the present invention is preferably formulated as a solution and can therefore be applied to the skin using, for example, a wet wipe, a roll-on dispenser, or a spray dispenser. The wet wipe may contain a base material such as a nonwoven fabric impregnated with the anthelmintic composition. The wet wipe is particularly useful when the area to be treated is small, such as the face, scalp, or the areas around the eyes, eyebrows, and eyelashes.
[0043] Alternatively, the anthelmintic composition of the present invention may be placed in a dispenser such as a roll-on dispenser, spray dispenser, or drip dispenser. Application of the anthelmintic composition of the present invention by a roll-on dispenser or spray dispenser may yield useful results when the entire body or a large area of the body needs to be treated, as well as when local administration to a small area of skin is required, for example. A drip dispenser may be used for local administration to an even smaller area of skin.
[0044] Preferably, the composition of the present invention is in the form of a wet wipe, roll-on, or spray composition. Therefore, the composition in the form of a wet wipe, roll-on, or spray relates to a composition formulated to be suitable for use in or contained in devices such as wet wipes, roll-on dispensers, or spray dispensers. More preferably, the composition of the present invention is in the form of a roll-on or spray composition. Even more preferably, the composition of the present invention is in the form of a spray composition.
[0045] Preferably, the compositions of the present invention are provided in a form that enables non-contact application. That is, application to the skin and administration to the skin are carried out without contact with the target skin area. Accordingly, the compositions of the present invention are provided in a form that enables non-contact application. This is in the form of a spray, roll-on, dripper bottle, or wet wipe. Accordingly, compositions in the form of a spray, roll-on, dripper, or wet wipe relate to compositions formulated to be suitable for non-contact application and used in or contained in devices such as spray dispensers, roll-on dispensers, dripper bottles, or wet wipes packaged in pouches. More preferably, the anthelmintic composition for non-contact application of the present invention is in the form of a spray composition or a roll-on. Even more preferably, the anthelmintic composition for non-contact application of the present invention is in the form of a spray composition.
[0046] The anthelmintic composition of the present invention is water-free and does not require a pharmaceutical component selected from preservatives, surfactants, or penetration enhancers. Therefore, the anthelmintic composition of the present invention is water-free and preferably does not contain preservatives, surfactants, and / or penetration enhancers.
[0047] In preferred embodiments, the anthelmintic composition is subject to topical administration, preferably a liquid composition that can be administered topically by non-contact application to a large area of skin using only a small volume of composition, in order to uniformly distribute the anthelmintic agent to the target site or region. Therefore, the present invention utilizes the excellent spreading ability of semi-fluorinated alkanes, which, when applied as droplets, enables uniform distribution of a target area of skin larger than 0.5 cm² per microliter of composition. By applying the composition non-contact to the target skin area using a roll-on dispenser or spray dispenser, the anthelmintic composition of the present invention enables uniform distribution to an even larger target area. For example, uniform distribution of a target skin area larger than 0.5, 1, 2, 3, 4, 5, 10, and 15 cm² is possible per microliter of composition.
[0048] The anthelmintic composition of the present invention is preferably alcohol-free. Since alcohol can cause further irritation or allergic reactions to the skin, the absence of alcohol is a further advantage of the composition of the present invention. In a preferred embodiment, the composition of the present invention is alcohol-free and / or preservative and / or water-free. In a preferred embodiment, the anthelmintic composition of the present invention is water-free, alcohol-free, and preservative-free.
[0049] The compositions of the present invention may also contain one or more additional excipients as additional components. As used herein, the term “excipient” means any pharmaceutically acceptable natural or synthetic substance that may be added to the compositions of the present invention. The compositions may contain one or more excipients, such as antioxidants, preservatives, liquid or oily excipients, cosolvents, waxes, surfactants, or lubricants, or combinations thereof.
[0050] Suitable antioxidants that may be used include, for example, butylated hydroxytoluene (BHT), butylated hydroxyanisole (BHA), tertiary butylhydroquinone (TBHQ), vitamin E, vitamin E derivatives (i.e., α-tocopherol acetate), and / or ascorbic acid or its derivatives.
