[0002] Disclosed herein is a method for weight management in a subject in need thereof or a method for treating or preventing overweight, obesity or an obesity related disease or a symptom of an obesity related disease or ameliorating or reducing the pathology or severity of an obesity related disease or a symptom of an obesity related disease in a subject in need thereof, said method comprising administering to the subject a therapeutically effective amount of a GLP-1 receptor agonist in combination with a therapeutically effective amount of an FTO inhibitor. Said methods have produced a synergistic effect on weight management, in particular preserving the muscle during weight loss and showing better efficacy in long-term weight management.BACKGROUND OF THE INVENTION

[0003] Glucagon-like peptide-1 (GLP-1) receptor agonists, e.g., Semaglutide, Liraglutide, Albiglutide, Exenatide, Lixisenatide, Dulaglutide, Tirzepatide, Retatrutide, Mazdutide, Amycretin, Survodutide, Efinopegdutide, VK2735, Dapiglutide, AMG133, HRS9531 work by activating the GLP-1 receptor to improve glycemic control and promote weight loss. Semaglutide and Tirzepatide have been approved for the treatment of type 2 diabetes and weight management. Clinical trials have demonstrated that Semaglutide and Tirzepatide lead to significant weight loss in individuals with obesity or overweight.

[0004] Administration of GLP-1 receptor agonists, like Semaglutide, either subcutaneous or oral, has been reported to cause unwanted side effects, including, for example, nausea, constipation, diarrhea, vomiting, anxiety, darkened urine, headaches, swelling of facial parts, nightmares, abdominal pain, stomach pain, side pain radiating to the back, skin rashes, unusual tiredness, and / or unusual weakness. Importantly, these psychological and gastrointestinal side effects have been found to contribute to treatment discontinuation. Discontinuation of Semaglutide therapy has been shown to result in substantial weight regain and loss of the associated health benefits. Additionally, concerns have been raised regarding the effects of Semaglutide on body composition. Studies have indicated that up to 40%of the weight loss achieved with Semaglutide may be attributed to a reduction in lean body mass. There are also potential safety concerns regarding the impact of Semaglutide on pancreatic function. (Mark M Smits Front Endocrinol 2021, 12: 645563; John P H Wilding et al. N Engl J Med. 2021, 384 (11) : 989-1002; John et al. Diabetes Obes Metab 2022, 24 (8) : 1553-1564; and Matthew M. Clark Obesity (2021) 29, 662-671) .

[0005] FTO (fat mass and obesity-associated protein) is an enzyme that regulates energy homeostasis and fat mass. FTO is an α-ketoglutarate and iron (II) dependent nucleic acid demethylase, with N6-meA in mRNA as its preferred substrate. Inhibition of FTO leads to increased methylation can reduce food intake and body weight.

[0006] GLP-1 receptor agonists which directly target the GLP-1 receptor or insulin secretion pathways. There is a need to maintain or enhance the therapeutic effects of GLP-1 receptor agonists like Semaglutide or Tirzepatide for the management of obesity and related metabolic disorders. FTO inhibitors target the FTO enzyme to modulate gene expression and energy homeostasis. It’s possible to combine the two types of drugs to reduce and maintain body weight and / or body fat in a subject with or without diabetes.SUMMARY OF THE INVENTION

[0007] The inventors of the instant invention found that the combination of a GLP-1 receptor agonist and an FTO inhibitor has produced a synergistic effect on weight management, in particular preserving the muscle during weight loss and showing better efficacy in long-term weight management.
