Description

Technical Field

[0001] The present invention relates to a diabetic kidney index reading disc, and in particular to a diabetic kidney index reading disc for predicting the clinical susceptibility of diabetic nephropathy. Background Art

[0002] In my country, the incidence of diabetes is increasing year by year, and it has become one of the biggest threats to the health of the Chinese population. The greatest harm of diabetes is not only manifested in blood sugar itself and its related acute glucose and lipid metabolism disorders, but also in its serious chronic complications that affect multiple systems, including kidney disease, eye disease, foot disease, neuropathy and other complications. Among them, diabetic nephropathy (DN) is the most harmful microvascular complication, which manifests as proteinuria in the early stage, renal dysfunction, nephrotic syndrome, hypertension and other chronic kidney disease manifestations in the middle and late stages, and finally most of them develop into end-stage renal disease (ESRD), requiring dialysis and other replacement therapies.

[0003] Clinically, once DN develops, it will progress rapidly if not effectively and promptly intervened. Early detection and timely intervention can effectively delay and slow the progression of DN's kidney lesions. Therefore, the current focus is on early prediction and early detection. Currently, basic research focuses on the study of early biomarkers, but it still lacks clinical application value.

[0004] Long-term clinical practice has found that many diabetic patients often have positive urine sugar when they are detected in the early and middle stages of DN (positive urine protein). In addition, it is found that in many DN populations, the higher the urine sugar, the higher the urine protein. In view of the above defects, it is necessary to design a diabetes kidney index reading for predicting the susceptibility to clinical onset of diabetic nephropathy. Summary of the invention

[0012] Furthermore, the second marking card is fixedly arranged at the bottom of the second mounting pin, and the second circular plate is fixedly arranged at the front end of the second mounting pin.

[0013] Furthermore, a plurality of clearance holes are integrally provided at the upper and lower ends of the shell, and the clearance holes are through holes.

[0014] Compared with the existing technology, the diabetes renal index reading disc used to predict the clinical susceptibility of diabetic nephropathy first screens diabetic patients who have visited the endocrinology department and have been hospitalized at least twice (with an interval of no less than 3 months) within two years, and those with positive urine sugar (1+ or more) in urine tests, and excludes those who have been clearly diagnosed with primary glomerular disease or secondary glomerular disease due to other non-diabetes reasons before the diagnosis of diabetes, and excludes hospitalizations with urinary tract infection (positive urine white blood cells, one "+" or more) and urine occult blood (positive urine routine BLD, one "+" or more) at admission. The urine routine, blood biochemistry (mainly renal function, blood sugar, glycosylated hemoglobin) and other data of the morning examination on the second day of hospitalization, as well as the course of disease (months), age, weight, blood pressure and other data are collected and entered into an excel sheet for subsequent retrospective analysis. Positive urine protein is defined as ± and above. Urine sugar values include urine protein and urine sugar. For the convenience of research, the qualitative values (±~++++) need to be converted into numerical quantitative mode according to the semi-quantitative linear range. The corresponding values are ±=0.5, +=1,

[0015] ++=2, +++=4, ++++=8. Urine protein value: The qualitative to quantitative conversion of urine protein is basically the same as urine sugar, "±~+++" corresponds to 0.5~4 respectively, and the above is 0 if negative. The diabetic renal index refers to the urine protein sensitivity index, which is the ratio of the urine protein value to the urine sugar value. In order to eliminate the statistical inconvenience caused by the conversion of urine protein (molecule) and urine sugar (denominator) to 0 when they are negative in some cases, the urine sugar value and urine protein value will be uniformly calculated by adding a fixed constant of 0.25 (corrected value) according to the convention of A+0.25, B+0.25 for statistical calculation. Therefore, the corrected diabetic renal index should be (A+0.25) / (B+0.25), see below for details. Figure 6According to the level of the diabetic renal index, the diabetic renal index is divided into five risk stratifications, namely, low, moderate, moderately high, high, and extremely high, according to the five degree ranges of x<0.1, 0.1≤x<0.2, 0.2≤x<0.5, 0.5≤x<1, and 1≤x. By using the device in conjunction with the device, after the medical staff has calculated the diabetic renal index statistically, they push the placement plate by hand, so that the placement plate drives the connecting pin linkage top block and the sliding pin to move left and right along the direction of the guide groove. At this time, the top block applies a thrust to the first circular plate and the second circular plate, so that the first circular plate and the second circular plate drive the corresponding first mounting pin and the second mounting pin together with the first marking card and the second marking card to move out of the clearance hole. At the same time, the first spring and the second spring are deformed, and each time the first marking card and the second marking card that are moved out of the clearance hole form a pair, and only one first marking card and the second marking card are moved out of the clearance hole at a time. The content marked on each first marking card in the device corresponds to the value of one of the diabetic renal indexes. , and the content marked on each second marking card is the severity of the corresponding glycemic renal index value on the first marking card, until the glycemic renal index calculated by the medical staff belongs to the range of the value on the first marking card moved out of the makeshift hole, the medical staff puts down the placement plate, at this time, the first marking card moved out of the makeshift hole and the first circular plate and the second circular plate corresponding to the second marking card are stuck in the groove, so that the state of the first marking card and the second marking card being moved out of the makeshift hole is fixed, and the medical staff checks the second marking card corresponding to the first marking card to know the severity of the patient's glycemic renal index. At the same time, the guide hole is to ensure the normal left and right movement of the connecting pin, and the first spring and the second spring are to allow the first installation pin to drive the first marking card and the second installation pin to drive the second marking card to quickly reset. In addition, according to whether the urine protein is positive during the first hospitalization within the time period, it is divided into a urine protein positive combined with urine sugar positive (UP+ / G+) group and a urine protein negative combined with urine sugar positive (UP- / G+) group. The statistical method is that quantitative data are expressed as mean ± standard deviation, and the means between the two groups are compared by the t-test of independent samples; qualitative variables are described by constituent ratios or rates, and constituent ratios or rates between different groups are compared by chi-square test; the trend chi-square test is used to analyze the occurrence of urine protein in the later stage of patients with different diabetic renal index; Spearman correlation analysis is used to analyze the factors related to the occurrence of urine protein. P≤0.05 is considered statistically significant, and the statistical analysis is completed in SAS 9.4. The case screening situation is that 197 cases are selected through conditional screening, including 48 cases of urine sugar positive urine protein positive group in the first hospitalization within the time period, including 27 cases hospitalized twice within the time period, 13 cases hospitalized three times, 5 cases hospitalized four times, and 2 cases hospitalized more than five times; and there are 149 cases in the urine sugar positive urine protein negative group, including 126 cases hospitalized twice, 18 cases hospitalized three times, 3 cases hospitalized four times, and 2 cases hospitalized five times or more, see below for details. Figure 7The analysis of the general clinical data of the two groups of patients, including urine protein, urine sugar, diabetic renal index, renal function, blood sugar biochemistry, disease course, age and other multi-indicators showed that in biochemical indicators, the two groups of patients had statistically significant differences in urine sugar (3.19±2.63 vs 3.97±2.99), diabetic renal index (1.11±1.84 vs 0.35±2.99), BUN (7.77±3.99 vs 6.17±5.79), and UA (348.56±109.46 vs 310.63±128.13), which were manifested in that the above biochemical indicators in the UP+ / G+ group were significantly higher than those in the UP- / G+ group; while in Cr (110.66±93.45 vs 82.28±313.98), blood sugar (12.31±7.03 vs There was no statistical difference in biochemical indicators. Analysis of non-biochemical indicators such as age and disease course showed that there were significant differences between the two groups of patients in disease course (138.30±88.73 vs 105.90±75.96), systolic blood pressure (139.73±21.81 vs 129.91±18.36), and diastolic blood pressure (84.75±13.35 vs 79.67±11.46) (all P < 0.0001). The disease course and blood pressure (systolic blood pressure / diastolic blood pressure) of the UP+ / G+ group were significantly higher than those of the UP- / G+ group; however, there was no statistical difference between the two groups in age (55.18±16.53 vs 51.43±15.50) and weight (59.00±29.60 vs 63.29±16.66) (all P > 0.05). For details, see Figure 8 . Regarding the occurrence of urine protein in the later stage of the two groups of patients, among the patients who were hospitalized for more than 2 times, the occurrence of urine protein in the later stage was observed between the UP+ / G+ group and the UP- / G+ group. Statistics showed that in the UP+ / G+ group, among the 48 patients, 34 patients showed positive urine protein in the second or more hospitalizations, with a positive rate of 72.35%. Among the 149 patients in the UP- / G+ group, only 19 patients showed positive urine protein in the later stage (any second or more hospitalizations), with a rate of 12.75%. The positive rate of urine protein in the later stage of the UP+ / G+ group was significantly higher than that of the UP- / G+ group, and the difference was statistically significant. For details, see Fig. 9. For the analysis of the occurrence of urine protein in the later stage in patients with different diabetic renal indexes, in order to further explore and analyze the correlation between different diabetic renal indexes and the occurrence of urine protein, we merged the data of UP+ / G+ and UP- / G+, and divided the diabetic renal index into five corresponding risk strata of low, moderate, medium-high, high, and very high according to x<0.1, 0.1≤x<0.2, 0.2≤x<0.5, 0.5≤x<1, and 1≤x, respectively. The incidence of urine protein in the later stage in the five risk strata from low to high was 13.08%, 26.09%, 40.00%, 58.82%, and 60.00%, respectively. The results showed that with the increase of diabetic renal index and risk stratification, the rate of positive urine protein in the later stage became higher and higher. For details, see Fig.10 . The above results were tested by Cochran-Armitage trend test, and the upward trend was statistically significant (Z=5.48, P<0.001). The stratified analysis of the factors related to the occurrence of urine protein and the correlation with urine glucose was further analyzed by Spearman correlation test. The results showed that blood uric acid (rs=0.29), creatinine (rs=0.44), course of disease (rs=0.22), blood pressure (SBP rs=0.23; DBP rs=0.16), body weight (rs=0.13) and urine glucose (rs=-0.07) were all important factors related to the occurrence of urine protein, and the correlation was statistically significant, as shown in Table 5. Further stratified analysis of the correlation of urine sugar according to the diabetic renal index found that in the moderate, medium-high, and high stratification ranges, that is, when the diabetic renal index was between 0.2 and 1, urine sugar was highly correlated with the occurrence of urine protein, and the difference was statistically significant (P < 0.01), while in the low and very high stratifications, urine sugar and urine protein were not significantly correlated (no statistical difference, P > 0.05), see Fig.11 as well as Fig.12 . In summary, urine sugar is another important factor that is highly correlated with urine protein in addition to relevant factors such as disease course and blood pressure. However, the stratified analysis of different diabetic renal indexes shows that the correlation of urine sugar has a limited range, including medium to high stratification intervals in addition to low and very high stratifications, which has an important predictive value for urine protein. By using the device in this application, urine protein and urine sugar are statistically converted, and the converted diabetic renal value can be used to know the severity of the patient's diabetic renal index, which is convenient for medical staff to accurately diagnose the patient's diabetic renal index. [0032] The Diabetes Kidney Index for predicting the clinical susceptibility of diabetic nephropathy first screens diabetic patients who have visited the endocrinology department and have been hospitalized at least twice (with an interval of no less than 3 months) within two years, and those with positive urine sugar (1+ or more) in urine tests, and excludes those who have been diagnosed with primary glomerular disease or secondary glomerular disease due to other non-diabetes reasons before the diagnosis of diabetes, and excludes hospitalizations with urinary tract infection (positive urine white blood cells, one "+" or more) and urine occult blood (positive urine routine BLD, one "+" or more) at admission. Positive urine protein is defined as ± and above. Urine glucose value includes urine protein and urine sugar. For the convenience of research, the qualitative value (±~++++) needs to be converted into a numerical quantitative mode according to the semi-quantitative linear range. The corresponding values are ±=0.5, +=1, ++=2, +++=4, ++++=8. Urine protein value: The qualitative to quantitative conversion of urine protein is basically the same as urine sugar. "±~+++" corresponds to 0.5~4 respectively. If the above is negative, it is 0. The diabetic renal index refers to the urine protein sensitivity index, which is the ratio of the urine protein value to the urine glucose value. In order to eliminate the statistical inconvenience caused by the conversion of urine protein (numerator) and urine sugar (denominator) to 0 in some cases where urine protein (numerator) and urine sugar (denominator) are negative, the urine glucose value and urine protein value will be uniformly calculated according to the convention of A+0.25, B+0.25, and a fixed constant of 0.25 (corrected value) will be added for statistical calculation. Therefore, the corrected diabetic renal index should be (A+0.25) / (B+0.25). See below for details. Figure 6According to the level of the diabetic renal index, the diabetic renal index is divided into five risk stratifications, namely, low, moderate, moderately high, high, and extremely high, according to the five ranges of x<0.1, 0.1≤x<0.2, 0.2≤x<0.5, 0.5≤x<1, and 1≤x. By using the device in conjunction with the medical staff, after calculating the diabetic renal index by statistics, the medical staff pushes the placement plate 9 by hand, so that the placement plate 9 drives the connecting pin 8 to link the top block 5 and the sliding pin 4 to move left and right along the direction of the guide groove 3. At this time, the top block 5 performs the first circular plate 13 and the second circular plate 17. The first circular plate 13 and the second circular plate 17 drive the corresponding first mounting pin 10 and the second mounting pin 14 together with the first marking card 12 and the second marking card 16 to move out of the clearance hole 18. At the same time, the first spring 11 and the second spring 15 are deformed. Each time the first marking card 12 and the second marking card 16 are moved out of the clearance hole 18, they form a pair, and only one first marking card 12 and the second marking card 16 are moved out of the clearance hole 18 each time. The content marked on each first marking card 12 in the device corresponds to the value of one of the diabetic renal indexes. , and the content marked on each second label card 16 is the severity of the corresponding glycemic renal index value on the first label card 12, until the glycemic renal index calculated by the medical staff belongs to the range of the value on the first label card 12 removed from the clearance hole 18, the medical staff puts down the placement plate 9, at this time, the first label card 12 removed from the clearance hole 18 and the first circular plate 13 and the second circular plate 17 corresponding to the second label card 16 are inserted into the groove 6, so that the state of the first label card 12 and the second label card 16 removed from the clearance hole 18 is fixed, and the medical staff By checking the second marking card 16 corresponding to the first marking card 12, the severity of the patient's diabetic renal index can be known. At the same time, the guide hole 7 is to ensure the normal movement of the connecting pin 8 left and right. The first spring 11 and the second spring 15 are to allow the first mounting pin 10 to drive the first marking card 12 and the second mounting pin 14 to drive the second marking card 16 to quickly reset. In addition, according to whether the urine protein is positive during the first hospitalization within the time period, it is divided into a urine protein positive combined with urine sugar positive (UP+ / G+) group and a urine protein negative combined with urine sugar positive (UP- / G+) group. The statistical method is that quantitative data are expressed as mean ± standard deviation, and the mean comparison between the two groups is performed by the independent sample t test; qualitative variables are described by constituent ratio or rate, and the comparison of constituent ratios or rates between different groups is performed by chi-square test; the analysis of the occurrence of urine protein in the later stage of patients with different diabetic renal indexes adopts trend chi-square test; Spearman correlation analysis is used to analyze the factors related to the occurrence of urine protein. P≤0.05 is statistically significant, and the statistical analysis is completed in SAS 9.4.The case screening was carried out according to the conditions, and a total of 197 cases were selected, including 48 cases in the group with positive urine sugar and positive urine protein in the first hospitalization during the time period, 27 cases in the group with two hospitalizations, 13 cases in the group with three hospitalizations, 5 cases in the group with four hospitalizations, and 2 cases in the group with more than five hospitalizations; and a total of 149 cases in the group with positive urine sugar and negative urine protein, including 126 cases in the group with two hospitalizations, 18 cases in the group with three hospitalizations, 3 cases in the group with four hospitalizations, and 2 cases in the group with five or more hospitalizations, see below for details.

The first movable mechanism is composed of a first mounting pin, a first spring, a first marking card and a first circular plate. The first mounting pin is movably arranged at the top of the mounting bar, the first spring is fixedly arranged on the left and right sides of the first mounting pin, and the first spring is fixedly connected to the mounting bar, the first marking card is fixedly arranged on the top of the first mounting pin, and the first circular plate is fixedly arranged at the front end of the first mounting pin. The second movable mechanism is composed of a second mounting pin, a second spring, a second marking card and a second circular plate. The second mounting pin is movably arranged at the bottom of the mounting bar, the second spring is fixedly arranged on the left and right sides of the second mounting pin, and the second spring is fixedly connected to the mounting bar, and the second marking card is fixedly arranged at the bottom of the second mounting pin The second circular plate is fixedly arranged at the front end of the second mounting pin, and is used in conjunction with the diabetic renal index reading disk. When the medical staff calculates the diabetic renal index statistically, they push the placement plate by hand, so that the placement plate drives the connecting pin, the linkage top block and the sliding pin to move left and right along the direction of the guide groove. At this time, the top block applies a thrust to the first circular plate and the second circular plate, so that the first circular plate and the second circular plate drive the corresponding first mounting pin and the second mounting pin together with the first marking card and the second marking card to move out of the clearance hole. Synchronously, the first spring and the second spring are deformed, and the first marking card and the second marking card moved out of the clearance hole each time form a pair, and only one first marking card and the second marking card are moved out of the clearance hole each time. The content marked on each first marking card in the diabetic renal index reading disk corresponds to a numerical range of the diabetic renal index, and the content marked on each second marking card is the severity of the diabetic renal index value on the corresponding first marking card, until the diabetic renal index calculated by the medical staff belongs to the numerical range of the first marking card moved out of the makeshift hole, the medical staff releases the placement plate, at this time, the first marking card moved out of the makeshift hole and the first circular plate and the second circular plate corresponding to the second marking card are inserted into the groove, so that the state of the first marking card and the second marking card being moved out of the makeshift hole is fixed, and the medical staff then checks the second marking card corresponding to the first marking card to know the severity of the patient's diabetic renal index.
