Abstract
Description
Technical Field
[0001] The present invention relates to a tricuspid valve regurgitation surgical instrument for pulmonary artery insertion, and more particularly to a tricuspid valve regurgitation surgical instrument for pulmonary artery insertion, which is experimentally inserted into the patient's tricuspid valve in an obliquely penetrating manner and removed after a predetermined period of time, for confirming whether right ventricular failure, which may occur when tricuspid valve regurgitation is treated by surgery or other permanent treatment methods, has occurred. Background Art
[0002] Tricuspid regurgitation is also called tricuspid regurgitation. As a tricuspid valve disease, tricuspid regurgitation is a symptom in which the tricuspid valve cannot be completely closed when it should be closed, and a space (orifice) is formed. When the right ventricle contracts, part of the blood that should be discharged from the right ventricle to the pulmonary artery flows back to the right atrium through the space (orifice).
[0003] In the past, treatments for tricuspid regurgitation included a method of correcting the regurgitation surgically by opening the patient's chest and cutting open the heart, and a method of permanently inserting a tricuspid regurgitation surgical instrument invented in US Patent No. 8486136 B2, US Patent No. 7854762 B2, and US Patent No. 9474605 B2 into the patient's body to treat the regurgitation. However, when the tricuspid regurgitation is treated by the surgical method or permanent surgical method, the blood flow from the right ventricle to the pulmonary artery suddenly increases, causing overload of the right ventricle, resulting in symptoms of right ventricular failure. Summary of the invention

[0055] Preferred embodiments of the present invention will be described in detail below with reference to the accompanying drawings.
[0056] Figure 1 This is a perspective view showing a tricuspid regurgitation surgical instrument for insertion into the pulmonary artery according to a preferred embodiment of the present invention.
[0057] If reference Figure 1 The tricuspid valve regurgitation surgical instrument 100 for pulmonary artery insertion includes a blocking portion 10 and an insertion tube 20 having a guidewire guide lumen 22 formed therein.
[0058] The blocking portion 10 includes: a support wire 14, both ends of which are fixed to the insertion tube 20; and a blocking film 12, which connects the insertion tube 20 and the support wire 14. Of course, the support wire 14 may also be not fixed to the insertion tube 20 at both ends or at one end.
[0059] The blocking film 12 is made of a material that is flexible, not easy to tear, and suitable for the human body, and can be made of medical polyurethane, polyolefin, silicone resin, e-PTFE (expanded polytetrafluoroethylene), PTFE (polytetrafluoroethylene), etc.
[0060] The support wire 14 is used to maintain the shape of the blocking film 12, and can be made of synthetic resin such as nylon or metal (stainless steel, nylon coated on metal, etc.) wire. The support wire 14 can be a single wire or a plurality of wires twisted together. The blocking film 12 can be formed in double or in a form in which the support wire 14 is inserted into the blocking film 12.
[0061] The blocking film 12 is as follows Figure 1 As shown, the blocking film 12 may be in not only a semicircular shape but also a circular shape. In order to adjust the position of the circular blocking film 12 or fix its shape, a plurality of insertion tubes 20 may be installed.
[0062] The insertion tube 20 formed with the guidewire guiding lumen 22 may be made of a rubber material, a flexible plastic material, or the like, and a material having high flexibility, good softness and excellent resilience that can move with the heartbeat is used.
[0063] Figure 2 The present invention is a sequence diagram showing the steps of a tricuspid valve regurgitation surgery for pulmonary artery insertion using the tricuspid valve regurgitation surgery instrument for pulmonary artery insertion of the present invention. Figure 3 This is a three-dimensional cross-sectional view showing the process of inserting a tricuspid valve regurgitation surgical instrument for pulmonary artery insertion into the body.
[0064] If reference Figure 2 and Figure 3 The tricuspid regurgitation surgery procedure for pulmonary artery insertion includes a guide wire moving step S10, a tricuspid regurgitation surgery instrument moving step S20, a tricuspid valve space blocking step S30, and a tricuspid regurgitation surgery instrument removing step S40.
[0065] The guide wire moving step S10 is a step for easily moving the instrument 100 for tricuspid regurgitation surgery for pulmonary artery insertion of the present invention by moving the guide wire 50 along the femoral vein-lower vena cava-tricuspid valve-pulmonary artery. The movement of the wire and the catheter in the body can be observed by X-ray. In order to safely move the guide wire 50 into the body, a guide wire guide tube (not shown in the figure) with a lumen formed inside is required. The guide wire guide tube (not shown in the figure) includes a balloon or pigtail-shaped hooking device at the upper end in order to pass through the safe zone of the tricuspid valve. Around the tricuspid valve, there are valve lower structures such as tricuspid leaflets, tricuspid chordae and papillary muscles and moderator bands, and this space is called an unsafe zone. The safe zone refers to a safe place outside the unsafe zone where the wire or catheter will not cause damage to the human body even if it passes through. When the hook device moves through the unsafe zone, it gets stuck in the lower structure of the valve and the control cable and cannot move forward, so it can only move through the safe zone. Therefore, after the guide wire guide tube (not shown in the figure) is inserted along the femoral vein-inferior vena cava-tricuspid valve safe zone-pulmonary artery, the guide wire 50 is inserted into the guide wire guide tube (not shown in the figure) and safely moved into the body. If the guide wire 50 reaches the pulmonary artery, the movement is stopped and the guide wire guide tube (not shown in the figure) is removed from the body. At this time, the tricuspid valve regurgitation surgery instrument 100 for pulmonary artery insertion is prepared so that it can be inserted into the sheath tube 40 with a lumen formed inside and moved into the body.
[0066] The pulmonary artery insertion tricuspid valve regurgitation surgical instrument moving step S20 is a step of inserting the guide wire 50 into the guide wire guide lumen 22 formed in the insertion tube 20 to move the pulmonary artery insertion tricuspid valve regurgitation surgical instrument 100 into the patient's body. The pulmonary artery insertion tricuspid valve regurgitation surgical instrument 100 inserted into the sheath tube 40 moves along the path (femoral vein-inferior vena cava-tricuspid valve-pulmonary artery) along which the guide wire 50 moves in the body.
[0067] The tricuspid valve space blocking step S30 is a step of inserting the pulmonary artery insertion tricuspid valve regurgitation surgical instrument 100 of the present invention into the patient's body, and blocking the tricuspid valve space (orifice) by the blocking portion 10. After the blocking portion 10 obliquely penetrates the tricuspid valve, the guide wire 50 and the sheath tube 40 are removed from the body, and the blocking portion 10 blocks the tricuspid valve space for a predetermined time.
[0068] The pulmonary artery insertion tricuspid valve regurgitation surgical instrument removal step S40 is a step of removing the pulmonary artery insertion tricuspid valve regurgitation surgical instrument 100 from the patient's body after confirming whether the patient has abnormal symptoms such as right ventricular failure and judging whether permanent surgery can be performed. After blocking the tricuspid valve space, if no abnormal symptoms appear after a predetermined period of time, the sheath 40 is reinserted into the body. The pulmonary artery insertion tricuspid valve regurgitation surgical instrument 100 is inserted into the sheath 40, and the pulmonary artery insertion tricuspid valve regurgitation surgical instrument 100 inserted in the sheath 40 is removed from the body. When an abnormal situation occurs, the pulmonary artery insertion tricuspid valve regurgitation surgical instrument 100 is immediately removed from the body.
[0069] According to the steps, before permanently treating tricuspid regurgitation in a patient with tricuspid regurgitation, the tricuspid valve space can be temporarily blocked to confirm whether permanent tricuspid regurgitation treatment can be performed.
[0070] Figure 4 The diagram shows Figure 1 A pulmonary artery fixing component is installed at one end of an instrument for tricuspid regurgitation surgery for pulmonary artery insertion, (a) is a stereoscopic view showing the pulmonary artery fixing component of the first embodiment, (b) is a stereoscopic view showing the pulmonary artery fixing component of the second embodiment, (c) is a stereoscopic view showing the pulmonary artery fixing component of the third embodiment, and (d) is a stereoscopic view showing the pulmonary artery fixing component of the fourth embodiment.
[0071] If reference Figure 4 The tricuspid regurgitation surgical instrument 100 for pulmonary artery insertion of the present invention may further include a pulmonary artery fixing member 30 at the upper end.

[0076] The pulmonary artery fixing body 32 of the third embodiment, which is formed of a band-shaped silk thread with a bulging center portion, can be composed of an elastomer, a shape memory alloy, or a self-expandable stent. When inserted into the sheath tube 40, in order to be moved into the body, it is inserted in a state where the center portion is pressed, and when the sheath tube 40 is removed, it is restored to the bulging state and fixed to the pulmonary artery.
[0077] If reference Figure 4 (d) The pulmonary artery fixing member 30 of the fourth embodiment includes: a pulmonary artery fixing body 32, which is formed of a silk thread having a belt shape; a pulmonary artery fixing member body 34, which is connected to the lower end of the pulmonary artery fixing body 32 and is formed in a ring shape; and a fixing member connecting wire 38, one end of which is connected to the pulmonary artery fixing body 32, passes through the central axis of the pulmonary artery fixing member body 34 and is inserted into the insertion tube 20.
[0078] The pulmonary artery fixing body 32 of the fourth embodiment, which is formed in the form of a belt, is inserted into the sheath tube 40 for insertion into the body and moved into the body. When the sheath tube 40 is removed, the pulmonary artery fixing body 32 or the pulmonary artery fixing member body 34 is pushed forward, and the pulmonary artery fixing body 32, which is formed in the form of a belt with a convex center, is fixed to the pulmonary artery.
[0079] Of course, other forms other than the above-mentioned pulmonary artery fixing member 30 may be used. In addition, the pulmonary artery fixing member 30 may be coupled to the lower end of the insertion tube 20 and fixed to the inferior vena cava.
[0080] When removing the pulmonary artery fixing member 30, there are the following removal methods: reinserting the sheath 40 into the body, inserting the pulmonary artery insertion tricuspid valve regurgitation surgical instrument 100 into the sheath 40 and removing it, or inserting a silk thread with a hook-shaped upper end (not shown in the figure) into the body, hooking it on the pulmonary artery protruding ring 36 and pulling it out of the body.
[0081] Of course, the tricuspid regurgitation surgical instrument 100 for pulmonary artery insertion does not necessarily need to be equipped with the pulmonary artery fixing member 30 .
