[0348] The sustained-release component is a part of a drug delivery system, which allows the drug to keep a period of continuous time in vivo, for example, far exceeds the time that the drug is metabolized or excreted from the body, such as at least about 1 week, at least about 2 weeks, at least about 3 weeks, at least about 1 month, at least about 2 months, at least about 3 months, at least about 4 months, at least about 5 months, at least about 6 months, at least about 9 months, at least about 12 months, at least about 18 months, at least about 2 years, at least about 3 years, at least about 5 years or longer. Usually, the sustained-release component is an implant, such as a solid implant, which plays a role by encapsulating the drug or otherwise embedding the drug in the implant. If the implant is biodegradable or bioerodible, the drug can be released along with the biodegradation or bioerosion of the implant. The implant can also be porous, so that after a period of time, the drug can diffuse out of the implant. The biodegradable or bioerodible implant can be porous or non-porous. Usually, non-biodegradable or non-bioerodible implants are porous, and the drug is released by diffusion. However, other mechanisms may also operate, such as osmotic pumps.
[0349] In some embodiments, the drug delivery system can be implanted in a mammal, such as a human, at intervals of about 1 week to about 10 years, about 1-12 weeks, about 3-6 months, about 6-9 months, about 9-12 months, about 12-18 months, about 18-24 months, about 0.1-3 years, about 3-5 years, or about 5-10 years.

This type of delivery system is an injectable all-silica-based microparticle-silica hydrogel controlled-release system that uses different types of encapsulated therapeutic agents and bioactive agents to significantly reduce burst. A detailed description of such silica-based sustained-release components and their preparation methods can be found in U.S. Patent No. 9,949,922, issued to Jokinen et al. on April 24, 2018, which is incorporated herein by reference in its entirety.
[0355] Another type of silica-based sustained-release component includes a flowing silica composition and a gel containing a drug, which can be obtained by a method for preparing a flowing silica composition, including a sol-gel conversion, wherein redispersion is performed. The redispersion includes: after reaching the gel point of the sol-gel conversion, a liquid is added to the gel formed by the sol-gel conversion, and added within a sufficiently short time period after reaching the gel point, so as to produce a rheologically uniform flowing silica composition after the gel and liquid are mixed, which is injectable and remains injectable through a 22G fine needle in this way or by stirring for a short time <30 seconds. These flowing and injectable sustained-release silica compositions can increase the stability of the encapsulated therapeutic agent and maintain its activity. A detailed description of such a silica-based sustained-release component and its preparation method can be found in U.S. Patent Application No. 20140057996 of Jokinen et al., published on February 27, 2014, which is incorporated herein by reference in its entirety.

These delivery systems include loading the biomolecule into the pores of the silica carrier material, thereby stabilizing the biomolecule. However, these systems can also be used for small molecule therapeutic compounds. A detailed description of such silica-based sustained-release components and methods for preparing them can be found in U.S. Patent No. 9,603,801, issued to Barnett et al. on March 24, 2017, which is incorporated herein by reference in its entirety.

[0413] Example 2: Solid Phase Synthesis of PG-L-CNTF Characterized by Ester Bonds:
[0414] Peptide 6 was attached to the resin at the NH2 terminus, protected with a CBz group on the free amine group and a benzyl group on the free acid group, and coupled with polyethylene glycol at the carboxylic acid terminus to provide resin-LK(Cbz)-K(Cbz)-E(Bn)-FLGGD(Bn)-GV-CO2(CH2-CH2-O) using methods known in the art. n -CH2-CH2-OH (SEQ ID NO: 19). The properties of the starting materials and the reaction sequence can be modified to improve selectivity, and all reactions are carried out using methods known in the art. Bimatoprost free acid is prepared by methods known in the art, and its three hydroxyl groups are protected by benzyl (or other hydrogenation-unstable protecting groups, as the case may be). The free alcohol end of the protected peptide 6 compound and the free acid of the protected prostaglandin compound can be coupled by any suitable ester coupling method known in the art. After the coupling reaction, the protected ester can be completely deprotected using a hydrogenation method or other debenzylation methods known in the art to release the peptide 6-linker-bimatoprost ester derivative. As known in the art, different orthogonal protecting group strategies can be adopted as needed to optimize the efficiency of the entire process.
[0415] Embodiment 3:
[0416] Eight African green monkeys were screened to identify four animals with high normal IOP. Animals were anesthetized with ketamine 10 mg / kg and xylazine 0.5 mg / kg. 50 μl of 100 ng of bimatoprost acid was injected into the vitreous of one eye using a shelved technique through a 30-gauge syringe, and 50 μl of phosphate buffered saline (PBS) was injected into the contralateral eye at 8:30 p.m. on the 0th, 1st, and 2nd days as a concurrent control. On the 3rd day (8:30 pm) and the 4th day (5:30 pm), the dose of bimatoprost acid was gradually increased to 200 ng in 100 μl, and the same volume of PBS was given in the contralateral eye. IOP was measured with a Tonovet tonometer at 8:30am, 11:30am, and 8:30pm on day 1; at 8:30am, 11:30am, and 5:30pm on day 4; and at 8:30am on days 2, 3, and 5. The person who measured IOP was unaware of treatment assignment. Slit-lamp examination was performed on days 0, 1, and 5. OCT was performed at baseline and on day 5.
[0417] One of four animals (B392) responded to the 100 ng dose with a ≥30% reduction in IOP at 1 time point compared to the fellow eye (a typical effect of a prostaglandin analog on IOP). Three of four animals responded to the 200 ng dose with a ≥30% reduction in IOP at 1 or more time points compared to the fellow eye receiving PBS, and one animal (B392) had a 52% reduction in IOP in the treated eye on Day 5. Notably, this was the same animal that responded to the 100 ng dose, suggesting that this animal was more responsive to bimatoprost acid than the other animals. The time course of IOP reduction is consistent with the short half-life of bimatoprost when injected into the eye by bolus injection. One animal (B300) did not respond to either dose. The actual IOP measurements and the percent reduction in IOP are shown in Figures 1 and , respectively. Figure 2 middle.
[0418] Clinical examination revealed no ocular inflammation, but one animal experienced corneal edema, which was thought to be related to the multiple surgeries and not medication. OCT showed no cystoid macular edema in any animal.
[0419] These results show the obvious IOP lowering effect of bimatoprost acid in the vitreous, with dose response between the dosage of 100ng and 200ng.Due to no observation of ocular inflammation, this is unlikely to be the reason for the IOP reduction of the eye for treatment.Further, this research is covered and controlled, and the same volume is used to carry out vehicle injection in the contralateral eye. Therefore, the IOP result is unlikely to be related to surgery (for example, wound leakage), because only in the eye for treatment, significant IOP reduction is seen.It is expected that further dosage increase will have the effect of even greater IOP reduction. The test article has good tolerance.
[0420] Based on these results, an extended-release form of intravitreal bimatoprost acid is expected to safely achieve clinically meaningful IOP reductions and be well tolerated by patients with the known caveats of prostaglandin or prostaglandin agonist exposure to the retina.
