Description

The invention relates to an arrangement comprising a patient positioning device with a positioning table and a transfer plate for positioning a patient, wherein the transfer plate is arranged on the positioning table so as to be movable in a longitudinal direction relative to the positioning table, and a support device with a support area. For examinations using a medical imaging device, the patient is positioned on a patient positioning device. The patient rests on a transfer plate that can be inserted into the imaging area of the medical imaging device along its longitudinal axis. Often, the imaging area is spatially limited, so the dimensions of the transfer plate perpendicular to its longitudinal axis are also small. This severely restricts the available space for placing items needed during the examination. Another consequence is that the patient must actively keep their arms close to their body, especially during preparation for the examination. This can, for example, lead to tension in the patient and / or make injections more difficult. The following are considered state of the art: EP 1 238 685 A1, US 2004 / 0 158 926 A1, WO 02 / 30 285 A2 and DD 1 55 584 A1. The invention aims to provide an improved support area near the patient positioning device. This problem is solved by the arrangement according to claim 1 and the system according to claim 10. Further advantageous features of the invention are contained in the dependent claims. In particular, the invention relates to an arrangement comprising: - a patient positioning device with a positioning table and a transfer plate for positioning a patient, wherein the transfer plate is arranged on the positioning table so as to be movable in a longitudinal direction relative to the positioning table, and - a support device with a support area, - wherein the patient positioning device has at least one connecting element for connection with the support device, - wherein in an operating state of the arrangement in which the support device is connected to the patient positioning device, the support area can be arranged at least temporarily offset next to the transfer plate in a transverse direction that is horizontal and perpendicular to the longitudinal direction. According to the invention, the transfer plate is suitable for positioning a patient, in particular such that the longitudinal direction of the transfer plate is essentially parallel to a longitudinal axis of the patient's body. The longitudinal axis of the body is oriented from a foot area to a head area of the patient. The transverse direction can, for example, be parallel to a transverse axis of the body of a patient positioned supine or prone. The transverse axis of the body is oriented from the left side to the right side of the patient. Thus, the support area can be positioned, in particular, laterally to the left or right of the patient. According to the invention, the support area can be arranged offset from the transfer plate with respect to the transverse direction. This avoids an overlap of the support area and the transfer plate in the vertical direction. In this way, the support area is particularly accessible from above. This increases the total available support surface formed by the transfer plate and the support area. As a result, a comparatively large support surface can be achieved with simultaneously small dimensions of the transfer plate, especially with respect to the transverse direction. In particular, the support device can have a support area encompassing the support area, wherein at least a part of the support area is arranged with respect to the transverse direction in an area defined by the transfer plate. According to an advantageous embodiment of the invention, the support device is particularly suitable for supporting a part of the patient's body, for example, part of the arm, in the contact area and / or for receiving an object that can be used during the examination. In particular, the support device is designed to transfer a load acting on the contact area to the patient positioning device. The support device has sufficiently high stiffness and strength. This is achieved by selecting an appropriate shape and materials, e.g., fiber-reinforced composite, plastic, metal, or combinations thereof. The contact area can have a shape adapted to the anatomy of the body part to be supported or the shape of the object to be received. In an embodiment that can be implemented in a structurally simple manner, the contact area has a flat surface. In particular, the support area can be arranged, at least temporarily, offset in the transverse direction next to the transfer plate in such a way that the surface is essentially horizontally oriented. According to an advantageous embodiment of the invention, the support area can be arranged within reach of the patient and / or of an operator located directly next to the patient positioning device who operates the medical imaging device. Preferably, the support area can be arranged in a space that is 20 cm to 100 cm, particularly preferably 30 cm to 80 cm, away from a centerline of the transfer plate in the transverse direction, the centerline being parallel to the longitudinal direction of the transfer plate. In particular, the support area can be arranged next to the transfer plate such that an arm of the patient positioned on the transfer plate can be at least partially accommodated within the support area. In particular, the support device can be arranged in a region defined by the transfer plate in the longitudinal direction. This avoids protruding parts at the head end and / or the foot end of the transfer plate. Thus, the invention enables an improved support area near the patient. This support area can, for example, hold items needed by the operating personnel during the examination. It can also, for example, support the patient's arm. An injection, such as of a contrast agent, into the patient's arm can therefore be easily performed. By supporting the arm, the support area allows for improved patient relaxation. According to a further aspect of the invention, in the operating state of the arrangement in which the support device is connected to the patient positioning device, the support area can be arranged, at least temporarily, offset vertically next to the transfer plate. In particular, the support area can be arranged higher or lower than the transfer plate. This allows for a height of the support area relative to the transfer plate that is adapted to the needs of the patient and / or the operating personnel. Furthermore, it allows for a height of the support area relative to the transfer plate that is adapted to the geometric conditions of the patient positioning device. A support area that can be positioned vertically relative to the transfer plate would also be conceivable. Preferably, the support area can be arranged at a vertical distance of less than 30 centimeters from the transfer plate.Preferably, the support area can be arranged at a vertical distance of less than 10 centimeters from the transfer plate. In particular, the support area can be arranged next to the transfer plate in such a way that an essentially horizontal arm of the patient positioned on the transfer plate can be at least partially supported on the support area. According to the invention, at least one connecting element is arranged on the storage table. This prevents the transfer plate's longitudinal movement from being restricted by the support area. For example, the transfer plate can be inserted into an image capture area that is spatially limited in the transverse direction. According to a preferred aspect of the invention, at least one connecting element is designed for a positive-locking connection with the support device. This allows the transfer of the load acting on the support area to the patient positioning device to be achieved in a structurally simple manner. According to a preferred aspect of the invention, at least one connecting element is designed for a detachable connection with the support device. This allows the support device to be removed if necessary. Thus, the freedom of movement of the transfer plate is not restricted. This also facilitates, for example, the patient's entry and exit from the transfer plate. In particular, it prevents the patient from bearing a large portion of their body weight on the support surface. This reduces the risk of damage to the support device and / or injury to the patient. Furthermore, this increases the area available near the transfer plate for operating personnel and / or other medical equipment. In particular, the support device can have a support body corresponding to the connecting element such that the support body can be inserted into the connecting element and connected to the connecting element in a form-fitting and / or detachable manner. For example, the connecting element can have a recess corresponding to the support body. According to a preferred aspect of the invention, the support device can be selectively connected to the patient positioning device in either a first or a second orientation relative to the transfer plate. In particular, the first and second orientations can be mapped onto each other by means of a fictitious rotation of the support device about a vertical axis of rotation. This can be achieved, for example, by a corresponding structure, such as a recess, of the connecting element, which can be mapped onto itself by a fictitious rotation by certain angles with respect to a vertical axis of rotation. The support device can thus form different angles with the longitudinal direction of the transfer plate in the first and second orientations. This allows, in particular, the distance of the contact area to the centerline of the transfer plate, which is parallel to the longitudinal direction of the transfer plate, to be changed. This allows the support area to be positioned in different areas of the room. In particular, the support area can be positioned at varying distances from the transfer plate in the transverse direction. For example, the support area can be positioned so that it overlaps the transfer plate vertically. This prevents, for instance, any restriction of the transfer plate's freedom of movement. It also facilitates, for example, the patient's entry and exit from the transfer plate. Specifically, it prevents the patient from bearing a large portion of their body weight on the support area, thus reducing the risk of damage to the support device and / or patient injury. In this way, the support area can be positioned according to the needs of the patient and / or the operating personnel.Furthermore, this increases the area available near the transfer plate for operating personnel and / or other medical equipment. According to a preferred aspect of the invention, the patient positioning device has several connecting elements arranged along the longitudinal direction of the transfer plate. In this way, the support device can be positioned at different locations along the longitudinal direction of the transfer plate, for example, depending on the needs of the patient and / or the operating personnel. Furthermore, the support device can be connected to several connecting elements simultaneously. This makes it possible, in particular, to create a support device with a comparatively large extension along the longitudinal direction of the patient bed. Specifically, the connecting elements can be arranged at regular intervals along the longitudinal direction of the transfer plate. According to a preferred aspect of the invention, in the operating state of the arrangement in which the support device is connected to the patient positioning device, the support area is rotatably mounted about a substantially vertical axis of rotation. This is achieved by a suitable rotary device for rotating the support area. The rotary device can include a pivot bearing. The pivot bearing can be formed by the patient positioning device and / or by the support device. This allows the support area to be arranged in different spatial regions. In particular, the support area can be arranged at different distances from the transfer plate with respect to the transverse direction. For example, the support area can be arranged in such a way that it overlaps the transfer plate in the vertical direction. This avoids restricting the freedom of movement of the transfer plate. This also facilitates, for example, the patient's entry and exit from the transfer plate. In particular, it prevents the patient from bearing a large portion of their body weight on the support surface. This reduces the risk of damage to the support device and / or patient injury. The support surface can thus be positioned according to the needs of the patient and / or the operating personnel. According to a preferred aspect of the invention, the support device has a folding mechanism for folding the support area such that, in the operating state of the arrangement in which the support device is connected to the patient positioning device, the support area can be folded about a folding axis that is substantially parallel to the longitudinal direction of the transfer plate. This prevents any restriction of the transfer plate's freedom of movement. This also facilitates, for example, the patient's entry and exit from the transfer plate. In particular, it prevents the patient from bearing a large portion of their body weight on the support area. This reduces the risk of damage to the support device and / or injury to the patient. According to a preferred aspect of the invention, the support device has a recess and a positioning device for positioning the support area relative to the recess such that, in the operating state of the arrangement in which the support device is connected to the patient positioning device, the support area can be inserted into and removed from the recess, at least partially. This reduces the space required, particularly for storing the support device. The support device can, for example, be stored in the recess and removed as needed. This avoids restricting the transfer plate's freedom of movement. It also facilitates, for example, the patient's entry and exit from the transfer plate. In particular, it prevents the patient from bearing a large portion of their body weight on the support area. This reduces the risk of damage to the support device and / or injury to the patient. According to an advantageous embodiment of the invention, the patient positioning device is designed to position the patient within an image acquisition area of a medical imaging device. In particular, the image acquisition area can be spatially limited with respect to the transverse direction. Specifically, the image acquisition area can be formed by a tunnel-shaped opening in the medical imaging device. In particular, the patient positioning device is designed such that the transfer plate can be inserted longitudinally into the image acquisition area. Such a transfer plate advantageously has small dimensions with respect to the transverse direction and thus, in particular, a small contact area. The invention further relates to a system comprising a medical imaging device and an arrangement according to the invention. According to a preferred aspect of the invention, the medical imaging device has an image acquisition area, wherein the patient positioning device is designed for positioning the patient within the image acquisition area. In particular, the image acquisition area can be spatially limited with respect to the transverse direction. Specifically, the transfer plate can be inserted at least partially into the image acquisition area in the longitudinal direction. In particular, the image acquisition area can be formed by a tunnel-shaped opening in the medical imaging device. Due to these design requirements, the extent of the transfer plate with respect to the transverse direction can be spatially limited. The invention thus allows for the selective provision of a support surface that is wider with respect to the transverse direction. According to a preferred aspect of the invention, the medical imaging device is selected from the group comprising a computed tomography device, a PET-CT device, a SPECT-CT device, a SPECT device, an X-ray device, a C-arm X-ray device, a magnetic resonance imaging device and combinations thereof. Embodiments of the invention according to the dependent claims are shown in the following drawings. The representation in the drawings is schematic and highly simplified, and not necessarily to scale. Fig. 1 shows in a perspective view an exemplary system 3 with a medical imaging device 5 and a patient positioning device 9. In the example shown here, the medical imaging device 5 is, without limiting the general concept of the invention, a computed tomography device 5. The computed tomography system 5 has a gantry 11. The gantry 11 has a stationary frame 13 and a rotor 17 rotatably mounted about a system axis 15 via a rotary bearing device. A projection device 19, 21 for receiving projections of an area to be imaged is arranged on the rotor 17. The projection device 19, 21 has a radiation source 19, e.g., an X-ray source, and a radiation detector 21, e.g., an X-ray detector. Radiation can travel along a primary beam path 20 from the radiation source 19 to the area to be imaged and then on to the radiation detector 21. A reconstruction of a tomographic image of the area to be imaged can be performed on the basis of projections obtained from different projection directions by rotating the projection device 19, 21. For the reconstruction of a tomographic image, the computed tomography device 5 has a correspondingly designed data processing unit 23. The computed tomography device 5 has a control device 25 for controlling the computed tomography device 5. The control device 25 has a storage medium 27 on which computer programs for controlling the computed tomography device 5 are stored in an executable form. The computed tomography device 5 also has an input device 29 for inputting control information, e.g., imaging parameters, and examination parameters, as well as an output device 31 for outputting control information and reconstructed images. The computed tomography scanner 5 has a patient positioning device 9 with a positioning table 35 and a transfer plate 37 for positioning a patient 7. Figure 1 shows three examples of typical positions that the arms of the patient 7 can assume depending on the position of the area of the patient 7 to be imaged, in particular for imaging the abdomen, thorax, head, arms or legs. The transfer plate 37 is arranged on the support table 35 so that it can be moved along a longitudinal direction 41 relative to the support table 35. In the example shown here, the longitudinal direction 41 of the transfer plate 37 is parallel to the system axis 15. A center line of the transfer plate 37, parallel to the longitudinal direction 41 of the transfer plate 37, runs in a vertical plane with the system axis 15. The movement of the transfer plate 37 can be continuous or sequential and can be driven, for example, by a motor. The motor can have a connection to the control device 25, for example, either wireless or fixed. The computed tomography system 5 has an image acquisition area 39 formed by a tunnel-shaped opening. Projections are acquired in a portion of the image acquisition area 39. The image acquisition area 39 is spatially limited, particularly radially to the system axis 15, by the gantry 11. Specifically, the image acquisition area 39 is spatially limited with respect to the transverse direction 43, which is horizontal and perpendicular to the longitudinal direction 41 of the transfer plate 37. The transfer plate 37 can be inserted into the image acquisition area 39 at least partially along its longitudinal direction 41. In particular, the transfer plate 37 is dimensioned such that it can be inserted into the image acquisition area 39 at least partially, simultaneously with the patient 7 positioned on the transfer plate 37. Fig. 2 shows a perspective view of the first embodiment of the arrangement 1 according to the invention, comprising a patient positioning device 9 and a support device 47. In the present embodiment, the positioning table 35 is arranged on a positioning base 33 by means of a lifting device 34. In this way, the positioning table 34 together with the transfer plate 37 can be positioned with respect to a vertical direction 45. However, a positioning table 34 arranged at a fixed predetermined height would also be conceivable. The support device 47 has a bearing area 49. The patient positioning device 9 has a connecting element 51, 53, 55, 57 for connection to the support device 47. In the operating state of the present embodiment shown in Fig. 2, the support device 47 is connected to the patient positioning device 9. The bearing area 49 is arranged offset in the transverse direction 43, which is horizontal and perpendicular to the longitudinal direction 41 of the transfer plate 37, next to the transfer plate 37. The support area 49 optionally has a surface. This surface is essentially horizontally oriented. In the operating state of the present embodiment shown in Fig. 2, the support area 49 is arranged at least temporarily offset in a vertical direction 45 next to the transfer plate 37. In the present and in the following embodiments, the support device 47 is arranged with respect to the longitudinal direction 41 in an area defined by the transfer plate 37. In the present embodiment, the connecting element 51, 53, 55, 57 is arranged on the support table 35. This prevents the transfer plate 37 from being restricted in its longitudinal direction 41 by the support area 49. The transfer plate 37 can thus be inserted into an image acquisition area 39 that is spatially limited with respect to the transverse direction 43. The connecting element 51, 53, 55, 57 is designed for a positive-locking connection with the support device 47. Optionally, the connecting element 51, 53, 55, 57 is designed for a detachable connection with the support device 47. The connecting element 51, 53, 55, 57 has a recess. The support device 47 has a support body. The recess and the support body are designed to correspond such that the support body can be inserted into the recess and positively locked to the connecting element 51, 53, 55, 57. In the present embodiment, the patient positioning device 9 has several connecting elements 51, 53, 55, 57 which are arranged along the longitudinal direction 41 of the transfer plate 37. In this way, the support device 47 can be arranged at different positions along the longitudinal direction 41 of the transfer plate 37, for example depending on the needs of the patient 7 and / or the operating personnel. In particular, the connecting elements 51, 53, 55, 57 are arranged in a row. The connecting elements 51, 53, 55, 57 can have a regular distance between them. Optionally, the support device 47 can be connected to the patient positioning device 9 in either a first or a second orientation relative to the transfer plate 37. The recess can have a structure, e.g., in the form of a cross slot, which can be mapped onto itself by a fictitious rotation by a predetermined angle, for example, 90 degrees, with respect to a vertical axis of rotation. The first and second angles that the support device 47 can thus form with the longitudinal axis of the transfer plate 37 in the first and second orientations, respectively, therefore differ by the predetermined angle. Thus, the distance of the support area 49 to the system axis 15 is, for example, increased in the first orientation and decreased in the second orientation. In the operating state of the present embodiment shown in Fig. 2, the support area 49 is rotatably mounted about a substantially vertical axis of rotation 63. Optionally, the support device includes a rotary device 61 with a rotary bearing for rotating the support area. The rotary bearing can have a vertical axis of rotation 63, which is located in the area of the connecting element 51, 53, 55, 57. The support area 49 is arranged in the vertical direction 45 below the transfer plate 37. This improves the freedom of movement for rotating the support device 47. By rotating the support device 47, an overlap of the support area 49 and the transfer plate 37 in the vertical direction and / or a distance of the support area 49 with respect to the transverse direction 43 from the transfer plate 37 can be adjusted. In Fig.2 the support device 47 is rotated such that the distance of the support area 49 with respect to the transverse direction 43 to the transfer plate 37 is increased. Fig. 3 shows in a perspective view the first embodiment of the arrangement 1 according to the invention, wherein the support device 47 is rotated such that the distance of the support area 49 with respect to the transverse direction 43 to the transfer plate 37 is reduced. Fig. 4 shows a perspective view of a second embodiment of the arrangement 1 according to the invention, wherein several connecting elements 51, 53, 55, 57 are arranged in a row at regular intervals along the longitudinal direction 41 of the transfer plate 37 on the support table 35. The support device 47 has several support bodies arranged at regular intervals. The support device 47 can thus be connected to several of the connecting elements 51, 53, 55, 57 simultaneously. In particular, the support device 47 can be connected to several of the connecting elements 51, 53, 55, 57 simultaneously in different positions along the longitudinal direction 41 of the transfer plate 37.Thus, depending on the needs of the operating personnel and the patient 7 and / or the requirements of the examination, different support devices 47, which differ in particular by an extension along the longitudinal direction 41 of the transfer plate 37, can be easily and quickly interchanged. Fig. 5 shows a perspective view of a third embodiment of the arrangement 1 according to the invention. The support device 47 shown in Fig. 5 has a greater extent along the longitudinal direction 41 of the transfer plate 37 than the support device 47 shown in Fig. 4. Fig. 6 shows a perspective view of a fourth embodiment of the arrangement 1 according to the invention. The support device 47 has a folding device 71 for folding over the support area 49 such that, in the operating state of the arrangement 1 in which the support device 47 is connected to the patient positioning device 9, the support area 49 can be folded over about a folding axis 73 that is substantially parallel to the longitudinal direction 41 of the transfer plate 37. The support device 47 has a locking device for locking the support area 49 such that a load acting on the support area 49 can be transmitted to the patient positioning device 9. Fig. 7 shows in a perspective view the fourth embodiment of the arrangement 1 according to the invention, wherein the support area 49 is folded over. Fig. 8 shows a perspective view of a fifth embodiment of the arrangement 1 according to the invention, wherein the support device 47 has a recess 81 and a positioning device, for example a rail guide, for positioning the support area 49 relative to the recess 81. In the operating state of the arrangement 1 shown in Fig. 8, the support device 47 is connected to the patient positioning device 9, and the support area 49 can be inserted into and removed from the recess 81, at least partially. The support device 47 can have a locking device for locking the support area 49 such that a load acting on the support area 49 can be transmitted to the patient positioning device 9. Fig. 9 shows in a perspective view the fifth embodiment of the arrangement 1 according to the invention, wherein the support area 49 is inserted section by section into the recess 81.
