Furthermore, the invention relates to a method for providing image data for visualizing a patient's eye and a computer program.
[0002] An eye surgery visualization system of the type mentioned above, comprising an image sensor and a computer unit containing an image processing routine for an image of the object area captured by means of the image sensor, is known from DE 10 2009 030 504 A1.
[0003] German patent DE 10 2016 203 473 A1 describes an ophthalmic surgery visualization system with a left and a right observation beam path, each fed to an image sensor. The ophthalmic surgery visualization system has eyepieces for a left and a right viewing eye, in which the image of the object area, captured by the image sensor in the left and right observation beam paths, is displayed.
[0004] Ophthalmic surgery visualization systems are used in ophthalmic surgery, for example, during surgical procedures on the posterior segment of a patient's eye.
[0005] German patent DE 41 14 646 A1 describes an ophthalmic surgical visualization system comprising an operating microscope with an ophthalmoscopy attachment module positioned below the main microscope objective as an extension of the microscope tube. This ophthalmoscopy attachment module has one or more ophthalmoscopic magnifying lenses that serve to generate an inverted image (i.e., vertically and laterally reversed) of the fundus of a patient's eye in a first intermediate image plane. An optical system for image erection and pupil reversal then erases the image from this first intermediate image plane and projects it laterally into a second intermediate image plane. The image of this second intermediate image plane can be seen by an observer in the ophthalmic surgery visualization system through the main microscope objective and a movable lens located between the main microscope objective and the image erection and pupil reversal system in the ophthalmoscopy attachment module. The ophthalmoscopy attachment module allows an observer to view a section of interest inside a patient's eye.
[0006] An observer examining an object area using an ophthalmic magnifying lens with an ophthalmic surgical visualization system will see an image of the object area that is oriented differently in the sections inside and outside the magnifying lens. Particularly in vitreoretinal surgery, this means that sections of the object area seen through the magnifying lens are displayed laterally correct, while sections located outside the magnifying lens are not displayed laterally correct.
[0007] It is desirable, however, that an observer can observe both the areas of an object region located on the side of the ophthalmic magnifier facing away from the microscope tube and areas not visible through the magnifier with the same image orientation. This ensures that, during ophthalmic surgery in which an endoscopic light guide is inserted into the patient's eye through a trocar, the observer does not have to move the light guide in reverse relative to the observed image. If the ophthalmic magnifier does not display sections of the patient's eye correctly, the observer—that is, typically the surgeon—perceives the insertion of instruments into the surgical field from the outside into the patient's eye during a surgical procedure using the ophthalmic visualization system. The movement from the edge of the observation image towards its center is perceived differently than corresponds to the actual movement of the instruments.
[0008] Against this background, the object of the invention is to create an eye surgery visualization system which, regardless of a set magnification, enables the observation of an object area with sections of a patient's eye that are arranged inside the patient's eye, without visualizing an observation image with image sections that have a different image orientation.
[0009] This problem is solved by the ophthalmic surgery visualization system specified in claims 1 and 2 and by the method specified in claim 8 for providing image data for visualizing a patient's eye. Advantageous embodiments of the invention are specified in the dependent claims.
[0010] The invention proposes providing an ophthalmic surgery visualization system comprising an image sensor and an imaging system for generating an image of an object area with an optical imaging beam path in an imaging plane on the image sensor, as well as a computer unit containing an image processing routine for processing an image of the object area captured by the image sensor, which has an image plane. The ophthalmic surgery visualization system includes an image display device for visualizing image data of an image processed in the computer unit. The ophthalmic surgery visualization system includes an ophthalmoscopic magnifier for imaging a section located within a patient's eye in an intermediate image plane conjugate to the imaging plane on the image sensor. The image processing routine serves to separate the first sections of the image of the object area, captured by the image sensor and generated by an imaging beam path passing through the ophthalmoscope lens, from the second sections of the image of the object area, which are complementary to the first sections and captured by the image sensor and generated by an imaging beam path running outside the ophthalmoscope lens. The first sections of the image of the object area are then transformed into mirrored first sections using an automorphism corresponding to a reflection of the first sections about two mutually perpendicular mirror axes lying in the image plane. Finally, the mirrored first sections are combined with the second sections complementary to the first sections to form a composite object area image that is displayed on the screen.
[0011] Complementary sections of the image of the object domain are understood to be separate image sections which, when combined, form the image of the object domain.
[0012] An ophthalmic surgery visualization system according to the invention can also include a first image sensor and an imaging system for generating an image of an object area with an optical imaging beam path in an imaging plane on the first image sensor, a second image sensor and a second imaging system for generating an image of an object area with an optical imaging beam path in an imaging plane on the second image sensor, and a computer unit with an image processing routine for processing an image of the object area with an imaging plane captured by the first image sensor and for processing an image of the object area with an imaging plane captured by the second image sensor. This ophthalmic surgery visualization system then includes an image display device for visualizing image data from the first and second image sensors processed in the computer unit. The ophthalmic surgery visualization system according to the invention also has an ophthalmoscopic magnifying lens for imaging a section located within a patient's eye in an intermediate image plane conjugate to the imaging plane on the first image sensor, and for imaging a section located within a patient's eye in an intermediate image plane conjugate to the imaging plane on the second image sensor. In the ophthalmic surgery visualization system, the image processing routine serves to convert the first sections of the respective image of the object area in the imaging plane on the first and second image sensors, each generated by an imaging beam path passing through the ophthalmoscopic magnifying lens, into second sections that are complementary to the first sections. These second sections are generated by the image sensors with an imaging beam path extending outside the ophthalmoscopic magnifying lens.to separate the respective image of the object area, to transform the first sections of each image of the object area into mirrored first sections in an automorphism corresponding to a reflection of the first sections about two mutually perpendicular axes of reflection lying in the image plane of the image, and to combine the mirrored first sections with the complementary second sections to form a composite object area image supplied to the image display device.
[0013] The magnification of the first imaging system and the magnification of the second imaging system can be the same or different.
[0014] According to the invention, it can be provided, for example, that the first imaging system images the object area at a higher magnification than the second imaging system. In this way, it is possible to image the retina of a patient's eye in full format on the first image sensor and to supply the second image sensor with the image of the sclera of the patient's eye. It should be noted that in an ophthalmic surgery visualization system according to the invention, it can be provided that a first stereoscopic partial image of the object area is acquired by means of several first image sensors and a second stereoscopic partial image of the object area is acquired by means of several second image sensors, wherein the first stereoscopic partial image and the second stereoscopic partial image have different magnifications and wherein the first stereoscopic partial image and the second stereoscopic partial image are combined to form a complete stereoscopic image supplied to the image display device.
[0015] In a preferred embodiment of the invention, the image processing routine includes an evaluation stage for image data which, depending on image information in the form of a section of the patient's retina, triggers the exclusive processing of image data relating to the first sections of the image in a retina visualization routine. In this routine, the first sections of the object area image are transformed into mirrored first sections using an automorphism corresponding to a reflection of the first sections about two mutually perpendicular mirror axes lying in the image plane. These mirrored first sections are then output to the image display device as a single retina visualization object area image. This avoids the unnecessary processing of image data in two video streams when, for example, only the image of the retina of one patient's eye is to be displayed as the object area image.
[0016] The image processing routine in the ophthalmic surgery visualization system can include an algorithm for detecting a lens edge of the ophthalmic magnifier using image processing, which defines the area enclosed by the lens edge of the ophthalmic magnifier in the image of the object area as first sections of the image of the object area generated with an imaging beam path passing through the ophthalmic magnifier.
[0017] The ophthalmoscopic magnifier in the ophthalmic surgery visualization system is preferably held on an ophthalmoscopic magnifier carrier which has at least one section with a characteristic color, wherein the algorithm for detecting an edge of the ophthalmoscopic magnifier includes a color evaluation routine adapted to the characteristic color of the at least one section of the ophthalmoscopic magnifier carrier.
[0018] A preferred embodiment of the invention provides that the image processing routine includes an algorithm for detecting the pupil of the patient's eye by means of image processing and defines the area enclosed by the lens edge of the ophthalmoscopic magnifier in the image of the object area as first sections of the image of the object area generated with an imaging beam path passing through the ophthalmoscopic magnifier.
[0019] An inventive method for providing image data for visualizing a patient's eye comprises the steps of supplying an image of the patient's eye to an image processing routine and processing the image into image data within the image processing routine. The image of the patient's eye is captured using an imaging beam path that at least partially passes through an ophthalmic magnifying lens. Here, the first sections of the image of the object area, captured by the image sensor and generated by an imaging beam path passing through the ophthalmoscopic magnifier, are separated from the second sections of the image of the object area, which are complementary to the first sections and are captured by the image sensor and generated by an imaging beam path running outside the ophthalmoscopic magnifier. The first sections of the image of the object area are transformed into mirrored first sections by an automorphism corresponding to a reflection of the first sections about two mutually perpendicular mirror axes lying in the image plane. The mirrored first sections are then combined with the second sections complementary to the first sections to form image data of a composite object area image.
[0020] A computer program according to the invention includes program code means for executing the methods specified above on a computer unit.
[0021] Further details of the invention will become apparent from the following description of exemplary embodiments with reference to the figures.
[0022] They show: Fig. 1. A first ophthalmic surgery visualization system with one patient eye; Fig. 2 an image of the patient's eye captured by an image sensor in the ophthalmic surgery visualization system; Fig. 3 a flowchart of an image processing routine of a computer unit of the eye surgery visualization system; Fig. 4 a second ophthalmic surgery visualization system; and Fig. 5 a third ophthalmic surgery visualization system.
[0023] The one in Fig. The first ophthalmic surgery visualization system 10 shown in Figure 1 includes a surgical microscope 12, which serves for the stereoscopic viewing of an object area 14. The surgical microscope 12 has an imaging optic 16 with a main microscope objective system 18, which is housed in a base body 20. The surgical microscope 12 includes an illumination device 22, which enables the illumination of the object area 14 by means of an illumination beam path 23 that passes through the main microscope objective system 18. The surgical microscope 12 has an afocal magnification system 24 through which a first stereoscopic partial observation beam path 26 and a second stereoscopic partial observation beam path 28 are guided. The operating microscope 12 has a binocular tube 30 connected to an interface 29 of the base body 20. This tube provides a first eyepiece 32 and a second eyepiece 34 for the left and right eyes 35a, 35b of an observer. The main objective system 18 of the operating microscope 12 is intersected by the first stereoscopic partial observation beam path 26 and the second stereoscopic partial observation beam path 28. The operating microscope 12 includes a first image acquisition unit 36 with a first objective lens system 38 and a first image sensor 40. The image acquisition unit 36 is used to acquire image information from the first stereoscopic partial observation beam path 26. A second image acquisition unit 42 allows the operating microscope 12 to acquire image information from the second stereoscopic partial observation beam path 28.The second image acquisition device 42 has a second lens system 44 and contains a second image sensor 46.
[0024] An ophthalmoscopy attachment module 48 with an ophthalmoscopy lens 50 mounted in an ophthalmoscopy lens holder 49 is connected to the operating microscope 12. The ophthalmoscopy lens 50 serves to image a section 52, which lies inside the patient's eye 54, through its natural lens and cornea into an intermediate image plane 56, which is conjugate to the image plane 58 on the first image sensor 40 and to the image plane 60 on the second image sensor 46.
[0025] The ophthalmoscopic magnifying lens holder 49 is colored green so that it can be easily distinguished from the color tones of a patient's eye 54 and the colors of the surgical instruments used in ophthalmic surgery, and so that it is clearly visible in the object area 14. It should be noted that in an alternative embodiment of the invention, the ophthalmoscopic magnifying lens holder 49 can also be colored yellow. The ophthalmoscopic magnifying lens holder 49 can also have another suitable color that allows it to be clearly visible in the object area 14 during ophthalmic surgery against a background image containing bodily fluids such as blood and tissue, as well as surgical instruments.
[0026] The ophthalmic surgery visualization system 10 has a computer unit 62 which contains an image processing routine for an image of the object area 14 acquired by the first image sensor 40 and the second image sensor 46. A monitor 64, designed for visualizing 3D image information, is connected to the computer unit 62 as an image display device. The computer unit 62 of the ophthalmic surgery visualization system 10 can be controlled via a keyboard 66 as an input interface.
[0027] It should be noted that in a modified embodiment of the ophthalmic surgery visualization system, a first and a second monitor can also be provided for visualizing 3D image information. For example, an overview image of the object area can be displayed on the first monitor, and a magnified section of the patient's eye on the second monitor.
[0028] The Fig. Figure 2 shows an image 68 of the patient's eye in the imaging plane 58, captured by the image acquisition device 36, with the ophthalmoscopic magnifying lens 50 and surgical instruments 59. The image 68 has first sections 70, which are generated by means of an imaging beam path passing through the ophthalmoscopic magnifying lens 50, and has complementary second sections 72, which are caused by an imaging beam path coming from the object area 14 and passing by the ophthalmoscopic magnifying lens 50.
[0029] The technical function of the image processing routine in the computer unit 62 consists, firstly, in separating the first sections of the image 68 of the object area 14 in the imaging plane 58 on the first image sensor 40, which are captured by the image acquisition device 36 and produced by an imaging beam path passing through the ophthalmoscopy magnifying lens 50, from the second sections 72 of the image 68 of the object area 14, which are complementary to the first sections 70 and are projected onto the first image sensor 40 and produced by an imaging beam path running outside the ophthalmoscopy magnifying lens 50.In a corresponding manner, the image processing routine in the computer unit 62 is designed to separate, i.e., to separate, the first sections of an image corresponding to image 68 of the object area 14 in the imaging plane 60 on the second image sensor 46, which are acquired by the second image acquisition device 42 and generated by an imaging beam path passing through the ophthalmoscope lens 50, from the second sections 72 of an image corresponding to image 68, which are complementary to the first sections 70 and acquired by the first image sensor 40 and generated by an imaging beam path running outside the ophthalmoscope lens 50.
[0030] Furthermore, a function of the image processing routine in the computer unit 62 is to subject the first sections 70 of the image of object area 14 to an image inversion operation, since the image of section 52 located inside the patient's eye 54, generated in the intermediate image plane 56 by means of the ophthalmoscopic lens 50, is vertically and laterally inverted in the intermediate image plane 56. In addition, it is a function of the image processing routine in the computer unit 62 to combine the first sections 70 of the image of object area 14 and the second sections 72 of the image of object area 14 into a composite object image that is laterally and vertically correct, which can then be displayed on the monitor 64 of the ophthalmic surgery visualization system 10.
[0031] The Fig. Figure 3 shows a flowchart 74 for the image processing routine. The image processing routine in the computer unit 62 has an image section separation stage 76, to which the image from an image sensor 40, 46 is fed. The image section separation stage 76 of the image processing routine contains an algorithm for detecting a lens edge of the ophthalmic magnifier 50, which recognizes the structure of the ophthalmic magnifier carrier 49 in the image fed to an image sensor 40, 46 by means of image processing. The algorithm of the image section separation stage 76 includes a color evaluation function for this purpose, which is adapted to a characteristic color of the ophthalmic magnifier carrier 49.
[0032] The image processing routine includes a filter stage 78. In filter stage 78, the image sections of the image captured by an image sensor 40, 46 are convolved using ring filters 80. The filter center 81 and the filter radii r1 and r2 in the captured image are varied to determine a set of pixels in the image that corresponds to the section of the ophthalmic magnifying lens carrier 49 surrounding the ophthalmic magnifying lens 50. The inner filter radius r1 of the ring filter 80 at which the convolution function assumes an extreme value is then determined as a boundary 82 of the ophthalmic magnifying lens 50. Based on the boundary 82 of the ophthalmic magnifying lens 50 determined in this way, the area of the Fig. The first sections 70 of the image of object area 14 shown in 2 were determined in order to then, in a further step, delineate these first sections 70 from the area of the in the Fig. 2 shown second sections 72 in the image of object area 14 to separate.
[0033] The first sections 70 of the image of object area 14, separated from the second sections 72 of the image by filter stage 78, are then fed to a reflection stage 84 in the image processing routine. In the reflection stage 84, the first sections 70 of the image from an image sensor 40, 46 are transformed into reflected first sections 70' by an automorphism through reflection about two mutually perpendicular mirror axes 86, 88 lying in the image plane of the image 68. It should be noted that in an alternative embodiment, the automorphism of the reflection routine can also be implemented as a rotation of the first sections 70 about a rotation axis perpendicular to the image plane. Furthermore, it is possible, for example, to provide a point reflection as such an automorphism.
[0034] The mirrored first sections 70' are then passed to a combination stage 90 in the image processing routine, where the mirrored first sections 70' and the second sections 72 of an image of object area 14 captured by the image sensors 40, 46 are recombined to form a composite object area image 68'. The image data of the composite image is then visualized on the monitor 64 as a stereoscopic image of object area 14.
[0035] It should be noted that the image processing routine in the computer unit 62 may contain an evaluation stage for image data acquired by means of the first image sensor 40 or the second image sensor 46, which, depending on the acquired image information, e.g. image information in the form of a section of the retina of the patient's eye 54, prevents the processing of image data provided by the first or the second image sensor 40, 46.
[0036] It should also be noted that by including a retina visualization routine in the image processing routine, which is triggered depending on captured image information and in which the first sections 70 of the respective image of the object area 14 are transformed into mirrored first sections in an automorphism corresponding to a reflection of the first sections about two mutually perpendicular mirror axes lying in the image plane of the image and are then output as a retina visualization routine object area image to the image display device in the form of a monitor 64, the unnecessary processing of image data can be avoided if, for example, only the image of the retina of a patient's eye 54 is to be displayed as the object area image.
[0037] The Fig. Figure 4 shows a second ophthalmic surgery visualization system 10'. Where assemblies and elements in the ophthalmic surgery visualization system 10' correspond to assemblies and elements in the ophthalmic surgery visualization system 10 described above, they are identified by the same numbers as reference symbols. The ophthalmic surgery visualization system 10' includes a data display device 92, which is connected to the computer unit 62 and enables the display of information in the first and second stereoscopic partial observation beam paths 26, 28, superimposed on an image of the object area 14 visible in the binocular tube 30.
[0038] The Fig. Figure 5 shows a third ophthalmic surgery visualization system 10". Where assemblies and elements in the ophthalmic surgery visualization system 10" correspond to assemblies and elements in the ophthalmic surgery visualization system 10 described above, they are identified by the same numbers as reference symbols. Unlike the operating microscope 12 of the ophthalmic surgery visualization systems 10 and 10', the operating microscope 12 of the ophthalmic surgery visualization system 10" is a purely digital operating microscope for the stereoscopic acquisition of the object area 14 by means of a first image sensor 40 and a second image sensor 46.
[0039] The computer unit 62 of the second and third ophthalmic surgery visualization systems 10', 10" contains, like the computer unit 62 of the first ophthalmic surgery visualization system 10, an image processing routine for processing the images of the object area captured by the image sensors 40, 46 with the above based on the Fig. 3 described functionality.
[0040] In summary, the following preferred features of the invention are particularly noteworthy: An eye surgery visualization system 10 has an image sensor 40, 46 and has an imaging system for generating an image 68 of an object area 14 with an optical imaging beam path in an imaging plane 58, 60 on the image sensor 40, 46. The ophthalmic surgery visualization system 10 comprises a computer unit 62, which includes an image processing routine for an image 68 of the object area 14 with an image plane, captured by means of the image sensor 46, and an image display device 64 for visualizing image data of an image 68 processed in the computer unit 62. According to the invention, the ophthalmic surgery visualization system 10, 10', 10" has an ophthalmoscopic magnifying lens 50 for imaging a section 52 located within a patient's eye 54 in a plane 58, 60 relative to the image plane 58, 60 on the image sensor 40.46 conjugate intermediate image plane 56. The image processing routine serves to separate the first sections 70 of the image 68 of the object area in the image plane 60 on the image sensor 40, 46, which are captured by means of the image sensor 40, 46 and produced with an imaging beam path passing through the ophthalmoscopic magnifying lens 50, from the second sections 72 of the image 68 of the object area 14, which are complementary to the first sections and are captured by means of the image sensor 40, 46 and produced with an imaging beam path running outside the ophthalmoscopic magnifying lens 50, and to separate the first sections 70 of the image 68 of the object area 14 in a reflection of the first sections 70 about two mutually perpendicular mirror axes 86 lying in the image plane of the image 68.88 to convert the corresponding automorphism into mirrored first sections 70' and to combine the mirrored first sections 70' with the second sections 72 of image 68, which are complementary to the first sections 70, to form a composite object area image supplied to the image display.
