Description

[0001] The invention relates to a surgical cleaning device and a non-surgical and non-therapeutic method for operating such a surgical cleaning device.

[0002] The invention particularly relates to a single-use surgical cleaning device that is designed and suitable for cleaning infected joint prostheses as well as infected bone and soft tissue areas. However, the surgical cleaning device can also be used to clean other used tools and instruments used during a medical operation (Surgery).

[0003] Unfortunately, in orthopedic surgery, septic revisions of joint prostheses infected with microorganisms are occasionally necessary. This involves explanting the infected joint prostheses and removing the infected or necrotic tissue. This removal of infected / necrotic tissue is called debridement. The explanted joint prostheses must also be cleaned.

[0004] In medicine, drive units powered by compressed air and electrical energy are currently known. Such external drive units are available in operating rooms and can be used for a variety of purposes. In septic bone surgery, the cleaning of debrided bone and soft tissue areas during the revision of infected joint endoprostheses and the cleaning of infected osteosynthesis plates by lavage are state of the art. Medical rinsing devices, so-called lavage systems, are used for this purpose. Physiological saline solution, Ringer's solution, and Ringer's lactate solution are commonly used as rinsing fluids. The goal of lavage is to remove remnants of infected tissue, biofilm residues, blood, and wound secretions. Lavage systems have been known for a long time.Examples of this are devices according to the documents DE 10 2011 018 708 A1, EP 2 939 699 B1, EP 3 187 208 B1, US 4 583 531 A, US 5 542 918 A, US 5 779 702 A, US 6 059 754 A, US 2005 / 0084395 A1 and US 2019 / 0151531 A1.

[0005] EP 3 574 852 A1 discloses a vacuum motor in which a plunger is periodically moved axially. US 2003 / 083681 A1 discloses a surgical handpiece with a rotating shaft protruding from the handpiece. US 2011 / 085845 A1 discloses a water-powered brush for washing dishes.

[0006] Debriding and cleaning can be performed by wound irrigation with so-called lavage systems, as well as by excising, rasping, sawing, and even brushing. A device for brushing and sawing is known, for example, from EP 3 741 316 A1. The devices used are contaminated with tissue residue and microbial germs after debridement. These instruments must be carefully cleaned and then sterilized before being reused. Medical personnel must protect themselves from contamination or infection through the transmission of microbial germs during cleaning. Therefore, it is desirable to provide a cost-effective surgical cleaning device that could be disposed of with standard surgical waste after a single use without complex and potentially hazardous cleaning steps. It would then make sense, particularly for reasons of resource and environmental protection, but also for cost reasons, if the drive system were not required to use batteries, accumulators, and electric motors, which contain non-ferrous metals and therefore potentially require complex recycling. After mechanical cleaning or processing, debridement is often required to remove potentially infectious residues such as chips created during mechanical processing. This requires an additional work step, and all devices used must then be laboriously disinfected or disposed of.

[0007] The object of the present invention is therefore to overcome the disadvantages of the prior art. In particular, the object of the invention is to develop a single-use surgical cleaning device intended for cleaning infected joint endoprostheses and infected bone and soft tissue areas. This device should be as simple and cost-effective to manufacture as possible. Furthermore, the device should be sterilizable by ethylene oxide or gamma sterilization. The cleaning device should be easy to use and require as few necessary steps as possible. It would be advantageous if the cleaning device could be hygienically disposed of by incineration after use. The surgical cleaning device must be sterile to be surgically applicable. The surgical cleaning device should, if possible, be disposed of by incineration along with other clinical waste after cleaning. The surgical cleaning device should be powered by a drive unit commonly used in medical operations.

[0008] The objects underlying the present invention are achieved by a surgical cleaning device according to claim 1 and by a non-surgical and non-therapeutic method according to claim 12. A surgical cleaning device according to the invention comprises a housing, a drive shaft, wherein the drive shaft is rotatably mounted in the housing, wherein the drive shaft has a rear end for connection to a drive unit and wherein the drive shaft has a front end arranged opposite the rear end, a plurality of elastically deformable bristles, wherein the elastically deformable bristles are arranged at the front end of the drive shaft and are connected to the drive shaft, wherein the elastically deformable bristles protrude at least partially from the housing and wherein the elastically deformable bristles extend mostly or all away from the axis of rotation of the drive shaft,a pump for pumping a liquid, wherein the pump is connected to the drive shaft and is to be driven by the drive shaft, a liquid supply line, wherein the liquid supply line is connected to the pump in such a way that a liquid can be sucked into the pump through the liquid supply line from the pump, a liquid discharge line, wherein the liquid discharge line is connected to the pump in such a way that a liquid can be pressed from the pump out of the pump into the liquid supply line, wherein the liquid discharge line has at least one liquid outlet opening in the region of the elastically deformable bristles, wherein the liquid discharge line opens through the at least one liquid outlet opening in the direction of the elastically deformable bristles,

[0009] The front end of the drive shaft extends over a length of the drive shaft and is therefore not to be understood as a point or location without spatial extension.

[0010] The fact that the elastically deformable bristles extend at least partially away from the rotational axis of the drive shaft means that the direction in which the elastically deformable bristles extend away from the rotational axis of the drive shaft has a radial component relative to the rotational axis of the drive shaft. The elastically deformable bristles do not have to extend perpendicularly away from the rotational axis of the drive shaft. However, it can be provided that the elastically deformable bristles extend perpendicularly away from the rotational axis of the drive shaft.

[0011] Preferably, the front end of the drive shaft can protrude at least partially from the housing. The part of the drive shaft protruding from the housing preferably comprises the front end of the drive shaft.

[0012] Preferably, it can further be provided that the liquid drainage opens into the at least one liquid outlet opening in the region of the elastically deformable bristles.

[0013] It can also be provided that the liquid drainage ends at at least one liquid outlet opening.

[0014] It is preferred according to the invention if the elastically deformable bristles or at least three-quarters of the elastically deformable bristles extend perpendicularly (at a right angle) away from the axis of rotation of the drive shaft and / or extend at an angle of at least 45° away from the axis of rotation of the drive shaft.

[0015] The drive shaft is preferably designed entirely or at least at the front end of the drive shaft as a straight rod, wherein the axis of rotation of the drive shaft runs within the straight rod and parallel to the axis of the straight rod.

[0016] The surgical cleaning device according to the invention is characterized by the fact that it has no motor or energy storage device. This allows the surgical cleaning device to be manufactured cost-effectively as a hygienic disposable product for medical applications and, in particular, to be made of highly combustible plastic.

[0017] To be suitable for surgical use, it is essential that the surgical cleaning device is sterile or at least that all surfaces of the cleaning device are sterilizable, such as by gamma-ray sterilization or sterilization with ethylene oxide. For this purpose, the surgical cleaning device can preferably be contained in a package permeable to ethylene oxide.

[0018] The rear end of the drive shaft can be designed as a connection for connecting to an external drive unit, in particular as a square bar, hexagon bar or generally as an edge bar.

[0019] It can be provided that the housing has a shaft housing and a handle for holding the surgical cleaning device with one hand, wherein the shaft housing encloses the drive shaft at least in a region between the front end of the drive shaft and the handle, and wherein the shaft housing is connected to the handle, and the handle has an opening for the rear end of the drive shaft.

[0020] This allows the surgical cleaning device to be used safely and conveniently by hand. In addition to protecting the components inside, such as the drive shaft and fluid drain, the shaft housing can also be used to create a protected cavity for other parts of the surgical cleaning device.

[0021] Furthermore, it can be provided that the drive shaft is rotatable and driven by a separate drive unit, wherein the separate drive unit is connected or connectable to the rear end of the drive shaft, in particular is detachably connected or connectable.

[0022] The separate drive unit can be an external drive unit.

[0023] This eliminates the need for a dedicated motor or energy storage device. This allows the surgical cleaning device to be manufactured for single use and disposed of as a hygienic disposable product by incineration.

[0024] Furthermore, it can be provided that the elastically deformable bristles extend axially and radially away from the drive shaft.

[0025] This allows the elastically deformable bristles to perform both radial mechanical cleaning and frontal mechanical cleaning at the tip of the front end of the drive shaft, and thus at the front end of the surgical cleaning device. This makes it possible to clean both flat and tubular areas, such as the proximal femur or tubular cavities in implants after septic revisions.

[0026] It can also be provided that the elastically deformable bristles project beyond the outer diameter of the housing in the region of the elastically deformable bristles by at least 1 mm, preferably 2 mm to 4 mm and particularly preferably by 5 mm to 10 mm.

[0027] This allows the surgical cleaning device to reach and mechanically clean undercuts, and the elastically deformable bristles can also reach deeper areas of the area to be cleaned. The handle of the housing can also have a larger outer diameter, which the elastically deformable bristles do not extend beyond.

[0028] According to a preferred development, it can be provided that the pump is a self-priming pump which is directly connected to the drive shaft, wherein the self-priming pump is arranged in the housing.

[0029] This results in a particularly simple design. It also ensures that fluid is ejected simultaneously with the rotation of the elastically deformable bristles, thus combining both cleaning effects. A self-priming pump in the surgical cleaning device has the advantage that the surgical cleaning device can itself aspirate irrigating fluids, such as physiological saline solution, Ringer's solution, or Ringer's lactate solution, which are then ejected from the surgical cleaning device in the form of jets of irrigating fluid.

[0030] Preferably, it can also be provided that the pump is connected to the drive shaft in such a way that, at least when the drive shaft is at a standstill, the liquid supply line in the pump is separated from the liquid discharge line in a liquid-tight manner or is sealed off, wherein preferably during rotation of the drive shaft the liquid supply line is connected to the liquid discharge line in a liquid-conducting manner.

[0031] The separation or isolation can be achieved by mechanical blocking within the pump. This prevents contaminated irrigating fluid from penetrating the fluid drain and pump into the fluid supply line and contaminating a reservoir for the irrigating fluid there, without the need for a separate or additional valve system. This allows the surgical cleaning device to be designed as a cost-effective and hygienic disposable product. The pump is connected to the drive shaft, which, when the drive shaft is not rotating, seals off the flow of irrigating fluid from the fluid supply line by mechanically blocking the connection between the fluid supply line and the fluid drain. When the shaft rotates, it opens the flow of fluid from the fluid supply line to the fluid drain. This makes it possible to keep the device simple and cost-effective by omitting valves and check valves.

[0032] According to a particularly preferred embodiment of the present invention, it can be provided that the pump is an impeller pump which is arranged on the drive shaft, wherein the impeller pump has a rotor with impeller blades, wherein the rotor with the impeller blades is connected to the drive shaft in a force-fitting and / or form-fitting manner.

[0033] This results in a particularly simple and cost-effective design. At the same time, the fluid supply and discharge lines are separated by the impeller vanes in the impeller pump, thus preventing contamination of the flushing fluid by contaminated used flushing fluid in a simple manner and without the need for a valve system.

[0034] Furthermore, it can be provided that the liquid drainage is formed at least in part by a part of the housing which delimits an intermediate space between the drive shaft and the housing, wherein the at least one liquid outlet opening is realized by at least one bore in the housing or in a front seal of the housing in the region of the elastically deformable bristles.

[0035] This makes the construction particularly simple and cost-effective.

[0036] It can therefore also be provided that the intermediate space between the part of the housing surrounding the drive shaft and the drive shaft is designed as the liquid discharge, wherein one or more bores are arranged in the part of the housing surrounding the drive shaft as the at least one liquid outlet opening.

[0037] In order to generate a pulsed liquid jet of the rinsing liquid, it can be provided that at least one diaphragm is mounted on the drive shaft in such a way that the at least one diaphragm covers the at least one liquid outlet opening or at least one of several liquid outlet openings at least once and uncovers it at least once during a complete rotation of the drive shaft about its axis of rotation.

[0038] This ensures that a fluid flow exiting through the at least one fluid outlet opening is interrupted at least once per rotation of the drive shaft, creating pulsating fluid jets. This improves the cleaning effect of the surgical cleaning device.

[0039] Preferably, the at least one cover is mounted on the drive shaft in a form-fitting and / or force-fitting manner.

[0040] In order to remove the used rinsing fluid, it can be provided that the surgical cleaning device has a suction device with at least one suction opening in the region of the elastically deformable bristles, wherein the suction device has a suction line which is connectable or connected to a vacuum source, wherein the suction line is preferably realized by a tube, wherein the tube encloses the housing at least in sections.

[0041] This allows the used rinsing fluid to be sucked out and removed from the object to be cleaned.

[0042] It can be provided that the suction device is connected or can be connected to a separator for separating solid components from the used rinsing liquid and / or the negative pressure source has a vacuum pump which is part of the surgical cleaning device and which can be driven by the drive shaft.

[0043] This means that the used rinsing fluid does not have to be removed in any other way using a separate device.

[0044] It can further be provided that the surgical cleaning device has a splash guard, wherein the splash guard partially covers the elastically deformable bristles, wherein preferably the splash guard is fastened to the housing and is either rotatably mounted against the housing or is rigidly connected to the housing.

[0045] This reduces the amount of used rinsing fluid sprayed into the environment. This allows for more hygienic work with the surgical cleaning device. The elastically deformable bristles can brush against the inside of the splash guard when the drive shaft rotates. This allows the rinsing fluid and any residue adhering to the splash guard to be brushed off.

[0046] Preferably, it can also be provided that the surgical cleaning device is made of plastic, wherein preferably the entire surgical cleaning device can be disposed of without residue by incineration.

[0047] This allows the surgical cleaning device to be used as a hygienic disposable product in the medical field and also to be manufactured cost-effectively.

[0048] It can be provided that the at least one liquid outlet opening is shaped and arranged relative to the elastically deformable bristles in such a way that a jet of rinsing liquid ejected through the at least one liquid outlet opening hits the effective area of the rotating elastically deformable bristles.

[0049] The effective range of the rotating elastically deformable bristles is the area covered by the free ends of the elastically deformable bristles, which are opposite a connection of the elastically deformable bristles to the drive shaft, when the elastically deformable bristles rotate. This is the area of the surgical cleaning device intended for mechanical cleaning using the elastically deformable bristles.

[0050] By the jet of rinsing liquid hitting the effective area of the rotating elastically deformable bristles, a particularly good cleaning effect is achieved, since the particles mechanically detached from the elastically deformable bristles can be carried away by the flow of rinsing liquid.

[0051] Furthermore, it can be provided that the surgical cleaning device has a flexible drive shaft extension, wherein the flexible drive shaft extension is connected or connectable to the rear end of the drive shaft and wherein the flexible drive shaft extension is connectable to an external drive unit, so that the drive shaft with the external drive unit can be rotated relative to the housing via the flexible drive shaft extension.

[0052] This allows the external drive unit to be placed away from the elastically deformable bristles of the surgical cleaning device and thus from the cleaning location, so that the external drive unit, which contains a motor, does not become contaminated and therefore does not require complex cleaning.

[0053] The objects underlying the present invention are also achieved by a non-surgical and non-therapeutic method for operating a surgical cleaning device, the method comprising the following steps: A) Rotating a drive shaft of the surgical cleaning device; B) Rotating a plurality of elastically deformable bristles arranged at a front end of the drive shaft by rotating the drive shaft; C) Driving a pump with the rotating drive shaft; and D) Pumping a rinsing liquid with the pump through at least one liquid outlet opening, wherein the rinsing liquid is sprayed in a pulsed or unpulsed manner into the region of the rotating elastically deformable bristles, in particular is sprayed into the effective region of the rotating elastically deformable bristles.

[0054] The process can be used, for example, to clean a used, explanted prosthesis so that it can be reused. However, it is also possible to use the process to clean other new and used implants or new or used medical instruments before use.

[0055] It can be provided that the method is carried out with a surgical cleaning device according to the invention above.

[0056] The method then has the advantages described for the surgical cleaning device. Furthermore, it can be provided that, prior to step A), the surgical cleaning device is connected to a drive unit, wherein the rotation of the drive shaft in steps A) to D) is driven by the drive unit. Preferably, the drive unit is connected to a rear end of the drive shaft opposite the front end.

[0057] This clarifies that an external drive unit is used to drive and rotate the drive shaft of the surgical cleaning device.

[0058] Furthermore, it can be provided that the used rinsing liquid is at least partially sucked off with a suction device during steps A) to D), wherein preferably after the suction, solid particles of the used rinsing liquid are separated from the liquid components with a separator.

[0059] This can increase the cleaning effect of the process and reduce contamination of the environment.

[0060] According to the invention, the method is not used for medical or therapeutic treatment of a patient.

[0061] The method according to the invention excludes the use of non-patentable medical methods. As already explained, the method can be used to clean surgical instruments and explanted implants that are not connected to a patient's body, so that in this case, there is no medical application that directly affects the body.

[0062] The invention is based on the surprising discovery that a surgical cleaning device can be operated by a single drive shaft both for operating a brush with elastically deformable bristles and simultaneously for generating a pulsed or non-pulsed stream of rinsing fluid in the effective range of the elastically deformable bristles. The drive shaft can be driven by a drive unit commonly used in operating rooms, and the surgical cleaning device can be designed so simply and cost-effectively that the surgical cleaning device can be manufactured as a cost-effective disposable product that allows hygienic disposal after a single use. The surgical cleaning device can be designed as a simple attachment for attachment to an external drive unit. The construction according to the invention enables the surgical cleaning device to be manufactured largely or entirely from plastic, thereby enabling hygienic disposal by incineration with other surgical waste.

[0063] At the same time, the workflow when using the surgical cleaning device is simplified because the steps of mechanically removing residues and rinsing the cleaned surface with rinsing fluid can be carried out at the same time. This increases the cleaning effect. In particular, when using a flexible drive shaft extension, the external drive unit can be positioned far enough away from the area to be cleaned that contamination of the drive unit can be prevented. These measures can eliminate the need for time-consuming cleaning of the surgical cleaning device and the external drive unit, thereby reducing or even eliminating the risk of insufficient cleaning and thus the risk of a dangerous infection when using the surgical cleaning device and the drive unit again after cleaning.

[0064] According to the invention, tissue areas are cleaned mechanically by the action of the elastically deformable bristles and simultaneously by one or more non-pulsed or pulsed jets of irrigating fluid, whereby the irrigating fluid can be fed into the device from the outside via a hose supply. According to the invention, the irrigating fluid is sucked in by the pump contained in the surgical cleaning device and then expelled again in the form of spray jets. These irrigating fluid jets flow out of the surgical cleaning device either non-pulsed or pulsed. Furthermore, it is possible to configure the surgical cleaning device with the aid of a suction device in such a way that the used irrigating fluid can be at least partially removed from the cleaned tissue area after cleaning with the aid of the surgical cleaning device.

[0065] The simultaneous use of rotating, elastically deformable bristles and at least one spray jet of rinsing fluid, and the inventive design of the surgical cleaning device, allow rinsing fluid to be sprayed out synchronously with the drive of the rotating, elastically deformable bristles, and, if necessary, the used rinsing fluid to be re-suctioned, so that a cleaning effect can be achieved through the simultaneous action of the rotating, elastically deformable bristles together with rinsing fluid jets. Contaminants mechanically removed by the moving, elastically deformable bristles are washed away by the simultaneously acting rinsing fluid jets. This prevents the contaminants from re-adhering to the cleaned surfaces.

[0066] Electric drive units for surgical tools are common worldwide in surgery and orthopedics. These are used to drive oscillating saws and for drilling. Rechargeable batteries are typically used as the energy source for such drive units. The surgical cleaning device according to the invention can be easily connected to the external drive unit via the rear end of the drive shaft and a drill chuck. The medical user holds the drive unit with one hand and operates the drive device with the other hand. By holding the surgical cleaning device firmly, rotation of the surgical cleaning device with the drive shaft is prevented. When using a drive shaft extension, the external drive unit can also be easily set up and secured in a remote location, and the user only has to hold the surgical cleaning device.

[0067] An exemplary surgical cleaning device according to the invention is composed of a) a handle designed as a housing, b) a freely rotatable drive shaft, which is partially arranged in the housing, c) a rear shaft end of the drive shaft protruding from the housing and intended for connection to a drive unit, d) a front shaft end of the drive shaft protruding from the housing and to which elastically deformable bristles are attached, e) a shaft housing connected to the handle, wherein the shaft housing surrounds the drive shaft at least partially and in which the front shaft end is mounted, wherein the elastically deformable bristles of the front shaft end protrude at least partially from the shaft housing, f) a pump connected to the drive shaft, g) a liquid supply line connected to the pump, h) a liquid discharge line extending from the pump and having one or more liquid outlet openings, which are aligned with the area of the elastically deformable bristles, i) wherein the surgical cleaning device is driven by an external drive unit which is connected or connectable to the rear shaft end of the surgical cleaning device, and j) wherein, during the rotational movement of the drive shaft, the elastically deformable bristles at the front shaft end rotate about a longitudinal axis of the drive shaft and the pump is simultaneously driven by the rotational movement of the drive shaft.

[0068] An exemplary method according to the invention for cleaning with a surgical cleaning device according to the invention can be characterized in that the cleaning is carried out by elastically deformable bristles rotating about a longitudinal axis of a drive shaft, wherein, simultaneously with the rotational movement of the elastically deformable bristles, unpulsed or pulsed jets of rinsing liquid are sprayed into the area of the elastically deformable bristles, and wherein the drive of the rotating elastically deformable bristles and the pump drive of the rinsing liquid are carried out by an external drive unit connected to the surgical cleaning device.

[0070] In the figures and in the following description of the exemplary embodiments of the present invention explained with reference to the figures, the same reference numerals are sometimes used for different exemplary embodiments and for different individual parts for the same or similar parts in order to simplify the comparability of the exemplary embodiments and readability.

[0071] Figure 1 shows a schematic cross-sectional view of an exemplary first surgical cleaning device 1 according to the invention. Figures 2 to 7Further views, cross-sectional views, and enlarged detail views of the surgical cleaning device 1 and its parts are shown. The surgical cleaning device 1 can have a housing 2 that seals off the surgical cleaning device 1 to the outside and can be used in a rear region as a handle for holding the surgical cleaning device 1.

[0072] Inside the housing 2 there is a drive shaft 3 which is rotatably mounted therein. The drive shaft 3 has a rear end 4 which is connected directly or via a drive shaft extension 200 (see Figures 16 and 17) is connected or connectable to an external drive unit (not shown). The external drive unit can, for example, be a multi-purpose drive commonly used in operating rooms, or a drill or similar. Opposite the rear end 4, the drive shaft 3 has a front end 5, to which a plurality of elastically deformable bristles 6 are attached. The elastically deformable bristles 6 can protrude radially from the drive shaft 3 and, together with the front end 5 of the drive shaft 3, form a brush mounted on a rotatable base relative to the housing 2.

[0073] The surgical cleaning device 1 further comprises a pump 7, which can be connected to the drive shaft 3 in the region of the rear end 4 of the drive shaft 3. The pump 7 can be driven via the drive shaft 3 and can be connected to the drive shaft 3 for this purpose. The pump 7 can preferably be designed as an impeller pump and is in the Figures 6 and7 shown in detail. The pump 7 is connected to a liquid supply line 8 for supplying a rinsing liquid to the pump 7 and to a liquid discharge line 9 for discharging the rinsing liquid from the pump 7. Instead of the impeller pump, another pump can also be used. However, the impeller pump is particularly preferred due to its simple design and the fact that the impeller pump physically separates the liquid supply line 8 from the liquid discharge line 9 when the impeller pump is at a standstill.

[0074] The drive shaft 3 can be formed at the rear end 4 as a hexagonal connection 10. A chuck or a suitable mating connection of the external drive device can be connected to the connection 10. The drive shaft extension 200 can be connected between the connection 10 and the mating connection of the external drive device. The connection 10 can be mounted in a pump housing 12 of the pump 7 via a circular disk-shaped counterbearing 11.

[0075] A connection 14 for connecting the fluid supply line 8 to a supply line from a rinsing fluid source (not shown) can be formed on the pump housing 12. The pump housing 12 can be closed on its front side with a pump cover 18. The drive shaft 3 can be guided into the pump 7 via a bearing 16.

[0076] The housing 2 can have a thicker rear section that functions as a handle, and a shaft housing 22 as the thinner front section. The handle of the housing 2 can have a disc-shaped bearing 20 on its front side. A passage can be arranged in the disc-shaped bearing 20, into which the liquid drain 9 opens in the handle of the housing 2. From there, the liquid drain 9 can be guided through the shaft housing 22 to several liquid outlet openings 28. The liquid outlet openings 28 open in the area of the elastically deformable bristles 6 in their effective area, i.e., in the area covered by the elastically deformable bristles 6 when the drive shaft 3 rotates.

[0077] The shaft housing 22 can be formed on its front side as a splash guard 24 that partially encloses the elastically deformable bristles 6. A cap 26 can be arranged on the front side, covering the drive shaft 3 at its front tip.

[0078] An impeller with several impeller blades 27 can be arranged in the pump 7. The impeller can be fastened to the drive shaft 3 and rotate with the drive shaft 3. For this purpose, a receptacle 36 can be fastened to the drive shaft 3, which is frictionally connected to the impeller. The pump 7 can then be operated by rotating the impeller with the drive shaft 3. In this way, the pumping of the rinsing liquid 30 with the pump 7 takes place simultaneously with the rotation of the elastically deformable bristles 6, and thus the ejection of a liquid jet of the rinsing liquid 30 into the effective area of the elastically deformable bristles 6 (see Figure 5 and analogously the Figures 10and 11 ).

[0079] In order to generate a pulsating flow of the rinsing liquid 30, a diaphragm 32 can be arranged on the drive shaft 3 in the partial area of the rinsing liquid discharge 9 formed by the shaft housing 20. The diaphragm 32 can have areas with different radii, so that upon a full rotation of the drive shaft 3, the liquid outlet openings 28 or the liquid discharge 9 in front of them are temporarily open (see Figure 5 below) and are temporarily closed (see Figure 5(above). This creates a pulsating discharge of the rinsing fluid 30, which is synchronized with the rotational speed of the drive shaft 3. The drive shaft 3 can be guided through the front of the shaft housing 22 via a seal 34. Simultaneously with the rotation of the elastically deformable bristles 6 being driven and the discharge of the rinsing fluid 30 being driven, a pulsating operation of the rinsing fluid flow can also be generated via the rotation of the drive shaft 3.

[0080] An internal thread 38 for attaching a supply line for the rinsing liquid 30 can be arranged in the connection 14 for the liquid supply line 8. The pump 7 can then be connected to the connection 14 with the rinsing liquid source (not shown). The surgical cleaning device 1 can be operated by connecting the surgical cleaning device 1 to the connection 10 of the drive shaft 3 directly or via a drive shaft extension 200 with the external drive unit. The external drive unit drives the drive shaft 3. The rotation of the drive shaft 3 operates the pump 7 and simultaneously rotates the brush constructed with the elastically deformable bristles 6. The pump 7 generates a stream of rinsing liquid 30, which is ejected via the rinsing liquid outlet openings 28 in the direction of the elastically deformable bristles 6. A pulsating stream of rinsing liquid 30 can be generated with the aid of the aperture 32.With the surgical cleaning device 1, mechanical cleaning can be carried out with simultaneous action of the liquid jet of the rinsing liquid 30.

[0081] Figure 8 shows a schematic cross-sectional view of an exemplary second surgical cleaning device 51 according to the invention. The surgical cleaning device 51 can have a housing 52 that seals off the surgical cleaning device 51 from the outside and can be used in a rear region as a handle for holding the surgical cleaning device 51. The internal structure of the surgical cleaning device 51 is similar to that of the first embodiment.

[0082] Inside the housing 52 there is a drive shaft 53 which is rotatably mounted (in Figure 8 not visible inside the housing 52 but analogous to the drive shaft 3 according to the Figures 2 , 3 and 5). The drive shaft 53 has a rear end (in Figure 8 not visible but analogous to the first embodiment according to Figure 1 ), with the rear end directly or via a drive shaft extension 200 (see Figures 16 and 17) is or can be connected to an external drive unit (not shown). The external drive unit can be, for example, a multi-purpose drive commonly used in the operating room, or a drill or similar. Opposite the rear end, the drive shaft 53 has a front end 55 to which a plurality of elastically deformable bristles 56 are attached. The front end 55 of the drive shaft 53 with the bristles 56 projects out of the housing 52. The elastically deformable bristles 56 can project radially from the drive shaft 53 and also axially from the tip of the front end 55 of the drive shaft 53 and, together with the front end 55 of the drive shaft 53, form a brush mounted on a bearing that can rotate relative to the housing 52.

[0083] The surgical cleaning device 51 further comprises a pump 7, which can be connected to the drive shaft 53 in the region of the rear end of the drive shaft 53. The pump 7 can be driven via the drive shaft 53 and can be connected to the drive shaft 53 for this purpose. The pump 7 can preferably be designed as an impeller pump and is in the Figures 6 and 7 shown in detail. The pump 7 is connected to a liquid supply line 8 for supplying a rinsing liquid into the pump 7 and to a liquid discharge line 59 for discharging the rinsing liquid from the pump 7. The part of the liquid discharge line 59, which is located in the part of the housing 52 forming the handle, is analogous to the liquid discharge line 9 according to the first embodiment (see Figure 2 ) is trained in Figure 8not visible. Instead of the impeller pump, another pump can also be used. However, the impeller pump is particularly preferred due to its simple design and the fact that the impeller pump physically separates the liquid supply line 8 from the liquid discharge line 59 when the impeller pump is at a standstill.

[0084] The drive shaft 53 can have a connection at the rear end. A chuck or a suitable mating connection of the external drive device can be connected to the connection. The drive shaft extension 200 can be connected between the connection and the mating connection of the external drive device.

[0085] The pump 7 may have a pump housing 12, on which a connection 14 is formed for connecting the liquid supply line 8 to a supply line from a rinsing liquid source (not shown). The pump housing 12 may be closed on its front side with a pump cover 18. The drive shaft 53 may be guided into the pump 7 via a bearing 16.

[0086] The housing 52 may have a rear, thicker portion that functions as a handle, and a shaft housing 72 as a front, thinner portion. The handle of the housing 52 may have a disc-shaped bearing on its front side. A projection 74 for attaching a suction device 90, 140 may be arranged on the top side of the handle of the housing 52 (see the Figures 12 to 15). A passage can be arranged in the disc-shaped bearing, into which the portion of the fluid discharge line 59 opens in the handle of the housing 2. From there, the fluid discharge line 59 can be guided through the shaft housing 22 to several fluid outlet openings 78. The fluid outlet openings 78 open in the region of the elastically deformable bristles 56 in their effective area, i.e., in the area covered by the elastically deformable bristles 56 when the drive shaft 53 rotates.

[0087] An impeller with multiple impeller blades 27 can be arranged in the pump 7. The impeller can be mounted on the drive shaft 53 and rotate with the drive shaft 53. For this purpose, a mount 36 can be mounted on the drive shaft 53 and is frictionally connected to the impeller. The rotation of the impeller with the drive shaft 53 then drives the pump 7. In this way, the pumping of the rinsing fluid by the pump 7 occurs simultaneously with the rotation of the elastically deformable bristles 56, thus ejecting a jet of the rinsing fluid into the effective area of the elastically deformable bristles 56.

[0088] The pump 7 can be connected to a source of irrigation fluid (not shown) via the port 14. The surgical cleaning device 51 can be operated by connecting the surgical cleaning device 51 to the external drive unit via the drive shaft 53 port directly or via a drive shaft extension 200.

[0089] The external drive unit drives the drive shaft 53. The rotation of the drive shaft 53 operates the pump 7 and simultaneously rotates the brush constructed with the elastically deformable bristles 56. The pump 7 generates a stream of rinsing fluid, which is expelled through the rinsing fluid outlet openings 78 toward the elastically deformable bristles 56. The surgical cleaning device 51 thus allows for mechanical cleaning with simultaneous exposure to the jet of rinsing fluid.

[0090] Figure 9shows a schematic cross-sectional view of an exemplary third surgical cleaning device 101 according to the invention. Figures 10 and 11 Cross-sectional views are shown as enlarged detail of the surgical cleaning device 101. The surgical cleaning device 101 may include a housing 102 that seals off the surgical cleaning device 101 to the outside and can be used in a rear region as a handle for holding the surgical cleaning device 101.

[0091] Inside the housing 102 there is a drive shaft 103 which is rotatably mounted therein. The drive shaft 103 has a rear end 104 which is connected directly or via a drive shaft extension 200 (see Figures 16 and 17) is connected or connectable to an external drive unit (not shown). The external drive unit can, for example, be a multi-purpose drive commonly used in operating rooms, or a drill or similar. Opposite the rear end 104, the drive shaft 103 has a front end 105 to which a plurality of elastically deformable bristles 106 are attached. The elastically deformable bristles 106 can protrude radially from the drive shaft 103 and, together with the front end 105 of the drive shaft 103, form a brush mounted on a rotatable base relative to the housing 102.

[0092] The surgical cleaning device 101 further comprises a pump 7, which can be connected to the drive shaft 103 in the region of the rear end 104 of the drive shaft 103. The pump 7 can be driven via the drive shaft 103 and can be connected to the drive shaft 103 for this purpose. The pump 7 can preferably be designed as an impeller pump and is in the Figures 6 and 7 shown in detail. The pump 7 is connected to a liquid supply line 8 for supplying a rinsing liquid to the pump 7 and to a liquid discharge line 109 for discharging the rinsing liquid from the pump 7. Instead of the impeller pump, another pump can also be used. However, the impeller pump is particularly preferred due to its simple design and the fact that the impeller pump physically separates the liquid supply line 8 from the liquid discharge line 109 when the impeller pump is at a standstill.

[0093] The drive shaft 103 can be formed at the rear end 104 as a hexagonal connection 110. A chuck or a suitable mating connection of the external drive device can be connected to the connection 110. The drive shaft extension 200 can be connected between the connection 110 and the mating connection of the external drive device. The connection 110 can be mounted in a pump housing 12 of the pump 7 via a circular disk-shaped counterbearing 11.

[0094] A connection 14 for connecting the fluid supply line 8 to a supply line from a rinsing fluid source (not shown) can be formed on the pump housing 12. The pump housing 12 can be closed on its front side with a pump cover 18. The drive shaft 103 can be guided into the pump 7 via a bearing 16.

[0095] The housing 102 can have a thicker rear portion that functions as a handle, and a shaft housing 122 as a slimmer front portion. The handle of the housing 102 can have a disc-shaped bearing 120 on its front side. A passage can be arranged in the disc-shaped bearing 120, into which the liquid drain 109 opens in the handle of the housing 102. From there, the liquid drain 109 can be guided through the shaft housing 122 to several liquid outlet openings 128. The liquid outlet openings 128 open in the area of the elastically deformable bristles 106 in their effective area, i.e., in the area that the elastically deformable bristles 106 sweep over when the drive shaft 103 rotates. A cap 126 can be arranged on the front side, covering the drive shaft 103 at its front tip.

[0096] An impeller with several impeller blades 27 can be arranged in the pump 7. The impeller can be fastened to the drive shaft 103 and rotate with the drive shaft 103. For this purpose, a receptacle 36 can be fastened to the drive shaft 103, which is frictionally connected to the impeller. The pump 7 can then be operated by rotating the impeller with the drive shaft 103. In this way, the pumping of the rinsing liquid 30 with the pump 7 takes place simultaneously with the rotation of the elastically deformable bristles 106, and thus the ejection of a liquid jet of the rinsing liquid 30 into the effective area of the elastically deformable bristles 106 (see Figures 10 and 11 ).

[0097] In order to generate a pulsating flow of the rinsing liquid 30, a diaphragm 132 can be arranged on the drive shaft 103 in the partial area of the rinsing liquid discharge line 109 formed by the shaft housing 120. The diaphragm 132 can have areas with different radii, so that upon a full rotation of the drive shaft 103, the liquid outlet openings 128 or the liquid discharge line 109 in front of them are temporarily open (see Figure 10 ) and are temporarily closed (see Figure 11This creates a pulsating discharge of the rinsing liquid 30, which is synchronized with the rotational speed of the drive shaft 103. The drive shaft 103 can be guided through the front of the shaft housing 122 via an insert 134. The liquid outlet openings 128 can be formed in the insert 134. Simultaneously with the rotation of the elastically deformable bristles 106 being driven and the discharge of the rinsing liquid 30, a pulsating operation of the rinsing liquid flow can also be created via the rotation of the drive shaft 103.

[0098] An internal thread 38 for attaching a supply line for the rinsing liquid 30 can be arranged in the connection 14 for the liquid supply line 8. The pump 7 can then be connected to the connection 14 with the rinsing liquid source (not shown). The surgical cleaning device 101 can be operated by connecting the surgical cleaning device 101 to the connection 110 directly or via a drive shaft extension 200 (see Figure 17) is connected to the external drive unit. The external drive unit drives the drive shaft 103 directly or via the drive shaft extension 200. The rotation of the drive shaft 103 operates the pump 7 and simultaneously rotates the brush constructed with the elastically deformable bristles 106. The pump 7 generates a stream of rinsing fluid 30, which is ejected via the rinsing fluid outlet openings 128 in the direction of the elastically deformable bristles 106. A pulsating stream of rinsing fluid 30 can be generated with the aid of the aperture 132. With the surgical cleaning device 101, mechanical cleaning can thus be carried out with the simultaneous action of the liquid jet of the rinsing fluid 30.

[0099] In Figure 12 is a first suction device 90 in a schematic perspective view ( Figure 12 Middle) and in two schematic perspective enlargements ( Figure 12top left and bottom right). The suction device 90 can be used with the second or third exemplary surgical cleaning device 51, 101. This is exemplified in the Figure 14 (Figure top left) for the second exemplary surgical cleaning device 51.

[0100] The suction device 90 can have a suction line 91 for removing used rinsing fluid from the area of the brush of the surgical cleaning device 51, 101. On the rear side of the suction device 90, this can be formed as a plug-in socket 92 for connection to the handle of the housing 52, 102 of one of the surgical cleaning devices 51, 101. The plug-in socket 92 can have an opening 93 for receiving the projection 74 of the surgical cleaning device 51 (see Figure 14 (top left). This achieves a more stable connection between the suction device 90 and the housing 52, 102.

[0101] The suction device 90 can also have a splash guard 94 on its front side, which partially covers the bristles 56 of the surgical cleaning device 51 (see Figure 14 (top left). A drainage connection 95 with a rinsing fluid drain 96 can be arranged in the area of the plug-in socket 92. The rinsing fluid drain 96 is connected to the suction line 91 in a fluid-permeable manner. The rinsing fluid drain 96 is connected or connectable to a vacuum source (not shown) and serves to remove the used rinsing fluid. Such vacuum sources are commonly found in hospitals. A separator (not shown) for separating solid components from the suctioned used rinsing fluid can be arranged between the vacuum source and the rinsing fluid drain 96.

[0102] A suction opening 97 is provided at the front of the suction line 91, through which the used rinsing fluid can be sucked into the suction line 91. The suction opening 97 is preferably funnel-shaped. For a tight and secure connection of the discharge connection 95 to the vacuum source, an internal thread 98 can be provided in the discharge connection 95.

[0103] In Figure 13 A second suction device 140 is shown in a schematic perspective view (center) and in two schematic perspective detail enlargements (top left and bottom right). As part of a surgical cleaning device according to the invention, the suction device 140 is shown in Figure 14 (second and third picture from top left) and in Figure 15 (as a detail enlargement of the second image from the top left in Figure 14). The suction device 140 can be used with the second or third exemplary surgical cleaning devices 51, 101. This is shown by way of example in the Figures 15 and 14 (there the second figure from the top left) for the second exemplary surgical cleaning device 51 and in Figure 14 (the middle figure there) for the third exemplary surgical cleaning device 101.

[0104] The suction device 140 can have a suction line 141 for removing used rinsing fluid from the area of the brush of the surgical cleaning device 51, 101. On the rear side of the suction device 140, this can be formed as a plug-in socket 142 for connection to the handle of the housing 52, 102 of one of the surgical cleaning devices 51, 101. The plug-in socket 142 can have an opening 143 for receiving the projection 74 of the surgical cleaning device 51 (see Figure 14 second from top left and Figure 15 ). This results in a more stable connection.

[0105] A drainage connection 145 with a rinsing fluid drain 146 can be arranged in the area of the plug-in socket 142. The rinsing fluid drain 146 is connected to the suction line 141 in a fluid-permeable manner. The rinsing fluid drain 146 is connected or connectable to a vacuum source (not shown) and serves to remove the used rinsing fluid. Such vacuum sources are commonly found in hospitals. A separator (not shown) for separating solid components from the suctioned used rinsing fluid can be arranged between the vacuum source and the rinsing fluid drain 146.

[0106] A suction opening 147 is provided at the front of the suction line 141, through which the used rinsing fluid can be sucked into the suction line 141. Preferably, the suction opening 147 is arranged within a funnel 144. For a tight and secure connection of the discharge connection 145 to the vacuum source, an internal thread 148 can be provided in the discharge connection 145.

[0107] It is also possible to generate the negative pressure applied to the rinsing fluid outlet 96, 146 of one of the suction devices 90, 140 with a second pump (not shown) driven by the drive shaft 3, 53, 103 of the surgical cleaning devices 1, 51, 101. For this purpose, the second pump can be connected to the drive shaft 3, 53, 103. The second pump can be part of the device according to the invention.

[0110] The drive shaft extension 200 can have a movable tube 202 in which a movable shaft 204, such as a cable, can be arranged. The movable tube 202 and the movable shaft 204 therein can preferably be bent and / or curved in the manner of a tube. For connection to the connection 10, 110 of the drive shaft 3, 53, 103, a matching edge hole 206 can be provided, into which the connection 10, 110 can be positively inserted.
