The foregoing applications are incorporated by reference in their entireties into this application.
[0002] The present invention relates generally to injection systems, devices and processes for facilitating various levels of control over fluid injection, and more specifically to systems and methods relating to safety syringes in medical environments. [Background technology]
[0003] Countless syringes, such as the one shown in FIG. 1A (2), are consumed daily in medical environments. A typical syringe (2) comprises a tubular body (4), a plunger (6), and a needle (8). As shown in FIG. 1B, such syringes (2) may be utilized not only to inject fluids into a patient, but also to draw or expel fluids from or into a container, such as a medicine bottle, vial, bag, or other drug containment system (10). In fact, due to regulatory constraints in some countries, such as the United States, and concerns about maintaining sterility, when using a medicine bottle (10) with a syringe (2) as found in certain patient environments, such medicine bottles may have to be discarded after only a single patient use, generating medical waste from the bottle and residual medicine waste, and further contributing to periodic shortages of certain important medications. Referring to FIG. 2A, three luer type syringes (12) are shown, each having a distally disposed luer fitting geometry (14) that allows them to couple with other devices having similar mating geometry, such as the luer manifold assembly (16) shown in FIG. 2B. The luer fitting (14) of the syringe in FIG. 2A may be referred to as a "male" luer fitting, while that of FIG. 2B (18) may be referred to as a "female" luer fitting. One of the luer connections may be threaded (in which case this configuration may be referred to as a "luer lock" configuration), allowing the two sides to couple by relative rotation, which may be combined with a compressive load. In other words, one embodiment of a luer lock is configured to utilize rotation, possibly in conjunction with compression, to engage threads into a male fitting, which engages a flange onto the female fitting, bringing the devices together into a fluid-tight connection. In another embodiment, a tapered mating geometry may be utilized to provide a Luer engagement using compression, without threads or rotation (such configurations may be referred to as "slip-on" or "cone" Luer configurations). Although such Luer connections are said to be relatively safe for the operator, there is a risk of spillage / leakage of medication and of parts breaking during loading to provide the Luer connection. On the other hand, the use of needle injection configurations carries the risk of the sharp needle contacting or piercing a person or unwanted structures. For this reason, so-called "safety syringes" were developed.
[0004] One embodiment of a safety syringe (20) is shown in FIG. 3, in which a tubular shielding member (22) is spring-biased to cover the needle (8) when released from a locked position relative to the syringe body (4). Another embodiment of a safety syringe (24) is shown in FIGS. 4A-4B. In such a configuration, after full insertion of the plunger (6) into the syringe body (4), the retractable needle (26) is configured to retract (28, 26) back to a safe position within the tubular body (4) as shown in FIG. 4B. Such a configuration configured to collapse upon itself may be associated with issues of blood splashing / aerosolization, safe storage of preload energy that may cause malfunction and premature start, reduced accuracy in delivering the total dose injection due to residual dead space in the spring compression volume, and / or reduced retraction speed control that may be associated with pain and patient discomfort.
[0005] Further complicating the syringe market is the increasing demand for pre-filled syringe assemblies, such as those shown in FIGS. 5A and 5B, which generally comprise a syringe body or “drug containment delivery system” (34), a plunger tip, plug or stopper (36), and a distal seal or cap (35) that may fit over a luer-type fitting (FIG. 5A shows the cap 35 in place, while FIG. 5B removes the cap to illustrate the luer fitting 14). Drug fluid may reside in a volume or drug reservoir (40) between the distal seal and the distal end (37) of the plunger tip (36). The plunger tip (36) may comprise a standard butyl rubber material and may be coated with a biocompatible lubricious coating or the like to promote a preferred seal and relative movement to the associated syringe body structure and material. The proximal end of the syringe body (34) in FIG. 5B includes a conventional syringe flange (38), such as what is known as a “Gerresheimer” flange configuration. Syringe body (34) preferably comprises a translucent material, such as glass or a polymer. A plunger tip (36) may be positioned within syringe body (34) to form a containment volume within drug chamber or reservoir (40) and to assist in the ejection of associated fluid through the needle. The syringe body may define a substantially cylindrical shape (i.e., allowing a plunger tip 36 having a circular cross-sectional shape to establish a seal with the syringe body) or may be configured to have other cross-sectional shapes, such as an ellipse.
[0006] Such assemblies are desirable because they can be standardized and manufactured in precise volumes by a small number of manufacturers around the world who can meet all of the world's ever-changing regulations for filling, packaging, and drug / drug bond material selection and component use. However, such simple configurations will generally not meet the new global standards for single-use, safety, auto-disable, and needle-stick resistance. This has led certain suppliers to move to more "vertical" solutions, such as (41) featured in FIG. 5C, which attempt to meet all or at least part of the standards with one solution, and as a result of trying to meet these standards for many different scenarios, such products may have significant limitations (including some of those described above with reference to FIGS. 3-4B), relatively high inventory and utilization costs.
[0007] There is a need for improved injection systems that address the shortcomings of currently available configurations, and in particular a need for a safety syringe solution that can take advantage of the existing and relatively well-controlled supply chain of conventionally delivered pre-filled syringe assemblies such as those described with reference to Figures 5A-5B.
